The failure appears to be a false-positive defect in the eye-tracking software logic and its integration with the braking system. ... incident the vehicle continued to propel forward instead of slowing. Braking was extremely difficult. The brake pedal felt abnormally stiff (“thick”) and required significant effort to depress. Even while actively braking, it felt as though the vehicle was still attempting to accelerate forward and did not slow normally. This occurred for a couple hundred yards before I was able to pull over due to safety concerns. At the same time, the vehicle was in All-Purpose drive mode, but the ride height was stuck at kneel height and would not adjust, which is abnormal. After pulling over, the vehicle would not shift into Park. I had to press the brake pedal with both feet while fully depressing it and lifting myself off the seat to keep the vehicle from moving. I then had to open the driver door in order to force the vehicle into Park. There were no warning lights or messages displayed on the dashboard. The vehicle was towed to the Rivian Service Center but has not yet been inspected because the service center is currently closed. This malfunction created a serious safety risk due to unintended acceleration, loss of normal deceleration, difficulty braking, abnormal suspension behavior, and inability to shift into Park.Read more

I haven't had any brake failures in my truck or scares, but whoever has this issue has to understand how annoying the constant chime is. Can't use cruise control and worst thing is if you are considering selling your truck outside of a Ram dealer, they won't take it in because of the open recall and privately, who would really consider buying a truck with this issue. My fear is that Mopar (Stellantis) is just putting this in the backburner with no plans on fixing it.Read more

The loss of propulsion occurred in traffic, creating a collision hazard. After stopping, the vehicle had no heat and limited power while unable to start leaving us stranded on the shoulder of a heavily traveled highway in 10-degree weather. My two toddlers were in the vehicle, creating additional risk from traffic exposure and cold. This was a repeat incident. Approximately two weeks earlier, the vehicle experienced a similar power loss. A Cadillac dealership replaced the 12V battery and stated the issue was resolved. However, on January 22, 2026, I contacted the dealership and provided a video showing abnormal starting behavior, indicating the problem may not have been fixed. I followed up by phone the next day but received no response. Despite this prior repair and warning, the same failure occurred again. There were no clear warning lights or messages immediately prior to the power loss. After the failure, the vehicle was unable to restart. I also attempted to use OnStar, a paid emergency service, but was placed on hold for approximately 30 minutes. Due to unsafe conditions, my wife had to come pick us up to remove our children from danger. This incident involves a repeat loss of power while driving, prior warning signs, and failure of emergency support systems, raising serious safety concerns.Read more

I would like to formally file a safety complaint regarding my Chevrolet vehicle and to express concern over Chevrolet’s handling of a known defect. I was able to pull over to the side of the road and bring the vehicle to a stop. I immediately contacted my local Chevrolet service department and was advised to bring the vehicle in six days later, the following Friday. Due to the safety risk, we did not use the vehicle during this time out of concern for a potential accident. When the vehicle was inspected by the Chevrolet dealership, we were informed that Chevrolet had identified this issue as being related to cold weather operation and that a custom shield had been designed to address the problem. We were also told that Chevrolet vehicles were not adequately designed for cold weather conditions, which led to this failure. Despite this acknowledgment and the fact that Chevrolet has engineered a corrective part, we were informed that the repair—costing approximately $1,600—is not covered under our extended warranty, as the shield was not installed on the vehicle at the time of purchase. The vehicle remains unsafe to drive. When accelerating, engine RPMs surge from approximately 2,500 to 4,000 RPM, and the vehicle can suddenly decelerate, creating a serious risk of rear-end collisions if a following driver is forced to brake abruptly. This issue represents a significant safety hazard. Chevrolet has acknowledged the defect by creating a corrective part, yet is declining to cover the repair cost, leaving the responsibility with the customer for a known design issue. A few hundred yards into the construction my check engine light came on, and then after a minute or so it started blinking but there was no place to pull over safely. I reduced speed and semi traffic was lining up behind me. Then the engine and rest of car shut off, losing power steering. I ended up weaving into the construction part and was finally able to stop but not without danger to myself and others. Motor would restart but sounded terrible and I did not feel safe driving further and did not have faith it would make it far, especially since I'd have to re-enter the highway construction traffic. I had it towed to local mechanic, who diagnosed the problem as a piston rod bearing spin/stuck, or in other words a spun/slipped rod/crank bearing.Read more

The contact stated that upon depressing the brake pedal, the brake pedal was stiff and failed to respond, causing the vehicle to crash into an unoccupied parked vehicle. The contact stated that the distance between the two vehicles could fit two parked vehicles. The contact stated that her teenager was occupying the front passenger's side seat during the incident. The contact stated that the front passenger's side and driver's side seat belts failed to restrain the passengers as needed, causing both passengers to use their arms to protect themselves. The police officers arrived at the scene but did not file a report. Both passengers sustained arm, neck, and head pain but declined to be transported to the hospital by ambulance. The contact stated that the gear shifter was locked after the crash and required two hands to move to park(P). The rental company was informed of the failure. The contact was able to drive the vehicle back to the residence. The vehicle was driven to the Rental Company the following day. No further information was available. On one occasion, the vehicle experienced a loss of power while driving at highway speed, requiring immediate driver intervention to avoid a dangerous situation. These incidents make the vehicle feel unpredictable and unsafe to operate as intended. I regularly transport family members, and these failures place my safety and the safety of others at significant risk of serious injury or death. The vehicle has been inspected by an authorized Genesis dealership on two separate occasions. Despite extended road testing, the dealership has stated they were unable to reproduce the issue and no permanent repair has been performed. The manufacturer has been notified, and the defect remains unresolved. No warning lights or messages were present prior to the incidents. I did not intend to accelerate. The vehicle rapidly increased speed, left the roadway, struck a street sign and a planter, and the airbags deployed. Event Data Recorder (EDR) data retrieved by the insurer (Mercury Insurance) shows the vehicle traveling at low speed immediately prior to the event, followed by a recorded 100% accelerator pedal position. This data does not reflect my driving intent and appears inconsistent with the low-speed conditions. The EDR documentation notes that accelerator pedal position is sampled asynchronously and reflects values transmitted to the SRS Control Module, not continuous pedal input or driver intent. No brake input was recorded prior to the impact. The sudden acceleration occurred too quickly to react. Mercedes-Benz inspected the vehicle and released only the EDR data, declining to provide further clarification regarding throttle control logic, pedal sensor correlation, or torque command behavior. I am reporting this incident due to concern that low-speed throttle control behavior, software logic, or transmission engagement may result in unintended acceleration without corresponding driver intent, posing a safety risk during parking-lot or low-speed driving conditions.Read more

... oses forward power (because it has shifted itself to Neutral), until I come to a complete stop, shift to Park, and then shift back to Drive to move forward. Usually the engine stays on, but on at least one occasion the engine turned off and I had to restart. This has happened at least five times total, including three times within the past month. The events have occurred while decelerating to a stop, braking but not stopping, and accelerating from a slow down. Each time, I experienced a sudden and unexpected loss of propulsion, requiring me to stop the vehicle in traffic, shift fully into Park, and then reselect Drive before the vehicle would move again. These incidents have occurred in active traffic situations and have placed me and others at risk of a collision due to the sudden loss of power (SEE DETAILS IN PDF). I previously brought the vehicle to a dealer for this issue after it had just occurred (~1 year ago, before the recent increase in frequency); however, the dealer stated that the vehicle did not throw a code, or no code was stored. They could not replicate the problem and no repair was made. There are at least 55 NHTSA complaints filed that appear the same as my issue; none appear to have been resolved by dealers or RAM. This appears to be a systemic safety defect involving the powertrain and electronic shifting system. The unexpected loss of propulsion in traffic presents a serious risk of injury or death. I am submitting this complaint due to the ongoing and unresolved safety concern. I am scared to drive my family in my vehicle until it is known to be fixed.Read more

For an extended period, no remedy was available. Safety Recall 68C (NHTSA 25V-741) involves a High Voltage Battery defect that may lead to a vehicle fire. FCA instructed owners not to recharge the vehicle and to park away from structures or other vehicles. I have been unable to safely charge the vehicle for over six months and have been restricted from using my assigned covered parking due to fire risk. Despite multiple dealership visits for recall-related service, the vehicle has not been fully restored to safe, normal operation. No loaner or substitute transportation has been provided during safety-related repairs. These conditions significantly impair the safety, use, and value of the vehicle and create ongoing risk to occupants and surrounding property. There were no warning lights, no prior looseness, and no unusual resistance prior to the failure. This appears to be a mechanical/structural failure of the accelerator pedal assembly, not an electronic throttle or sensor malfunction. Due to the dangerous location and inability to move the vehicle, law enforcement (a sheriff) responded approximately 30 minutes later to assist with traffic safety. The vehicle remained immobilized in traffic for an extended period, and it took nearly two hours for a tow truck to arrive. This incident created a serious and prolonged safety hazard, exposing me and other motorists to a high risk of collision. The vehicle had approximately 9,700 miles and has been driven under normal conditions. The vehicle is currently not being driven due to safety concerns. The failed accelerator pedal assembly is available for inspection. This created an immediate hazard as I was unable to keep up with the flow of traffic, forcing me to make an emergency exit across multiple lanes while moving significantly slower than other vehicles. The loss of motive power increased the risk of a high-speed rear-end collision. 3. Has the problem been reproduced or confirmed by a dealer? Yes. The problem was confirmed by Heritage Toyota Service. The technicians confirmed an internal transmission failure. A secondary consultation with a transmission specialist (AAMCO) also identified the failure as a known mechanical defect common to this transmission model. 4. Has the vehicle been inspected by the manufacturer... or others? Yes, it has been inspected by the authorized Toyota Dealership listed above. I have also reported the issue to Toyota Brand Engagement (National), but a corporate field inspection (DSPM) has not yet been performed. 5. Were there any warning lamps... or other symptoms? Symptoms: A distinct whining noise from the engine/transmission area that increased with vehicle speed. Prior Events: The vehicle was taken to the dealer specifically for this noise. The dealer updated the vehicle's software but was unable to provide a remedy for the failures. The dealer informed the contact of NHTSA Campaign Numbers: 25V741000 (Electrical System) and 25V766000 (Engine); however, the parts to do the recall repair were unavailable. The contact stated that the manufacturer exceeded a reasonable amount of time for the recall repair. While driving approximately 25 MPH, after accelerating from being idle at a stop light, the vehicle suddenly jerked and stopped. The contact pressed the accelerator pedal, and when doing so, the vehicle accelerated at a high speed, immediately causing the contact to lose control of the vehicle. The vehicle ended up crashing into a fire hydrant, causing it to stop. The air bags did not deploy. The contact received injuries to their shoulders, lower back, neck, and left knee. Medical attention was needed. A police report had been filed. The vehicle was towed from the incident. The contact stated that while the police were there, the officer noticed the engine revving as if the accelerator pedal was being pressed; however, the pedal was not pressed at the time.
