... mediate, simultaneous electronic and mechanical malfunction. The dashboard abruptly displayed an Adaptive Cruise Control (ACC) error/failure warning, along with auto start/stop failure. Immediately following this alert, the vehicle experienced a severe, sudden loss of mechanical power and propulsion. 3. Vehicle Behavior & Safety Hazard Pressing the accelerator pedal failed to yield normal engine response or acceleration. The drop in power felt as though the vehicle entered an unprompted "limp mode" or suffered a critical powertrain/software communication failure, severely limiting my ability to maintain highway speeds. Because this occurred inside a highway tunnel, the sudden deceleration presented an immediate and extreme safety hazard, nearly causing a rear-end collision with the traffic behind me. I was forced to navigate the underpowered vehicle out of the active lanes of traffic under highly dangerous conditions. After pulling over to a safe area and cycling the ignition (turning the vehicle off and back on), the mechanical power partially/fully returned, suggesting a transient or software-driven electronic control unit (ECU) fault. The historical error codes should be stored in the vehicle's modules. 4. Post-Incident State The vehicle was presented to McKenna Porsche (Norwalk, CA) the following day for diagnosis of the power loss and ACC failure. I have been informed that replacement ABS modules for this vehicle have been unavailable for approximately nine months and remain unavailable as of today. This appears to be a widespread issue affecting Jeep Wrangler JK vehicles, with many owners reportedly unable to obtain replacement safety-critical ABS modules through Mopar or dealership networks. Because the ABS and traction/stability systems are malfunctioning, the vehicle cannot pass inspection and is unsafe for my family of four, especially during rain or emergency braking situations. This issue affects federally mandated vehicle safety systems, including anti-lock braking and electronic stability control. The technician also noted the need for additional diagnostics involving the Output Speed Sensor (OSS) circuit within the transmission. The vehicle remains available for inspection at the dealership. This condition created a safety concern because the transmission was shifting erratically and the vehicle could not be relied upon for normal operation. Unpredictable transmission behavior creates a risk during acceleration, merging, highway travel, and other driving situations where consistent vehicle response is required. At the time, I was preparing for a cross-country military PCS move and did not believe the vehicle was safe or reliable to drive long distance. The problem was confirmed by an authorized Ford dealership who documented the condition, diagnostic codes, and repair recommendation. The findings were provided to Ford Motor Company through Corporate Case Number CXH-08096574-L9M6T4. Ford representatives advised that the Automotive Safety Office was notified regarding the technician findings and the open recall. On XXX, while turning left from [XXX] onto [XXX], I had sufficient time to complete the turn and pressed the accelerator to proceed. However, the accelerator became completely unresponsive and the vehicle would not accelerate. My vehicle was left partially in the intersection while an oncoming vehicle approached. The other driver could have easily struck the driver’s side of my vehicle, creating the risk of a serious or fatal accident. Fortunately, a collision was avoided, but this incident demonstrated a significant safety hazard. In addition to this loss of acceleration, the vehicle has experienced recurring starting problems and electrical malfunctions. Pensacola Honda found that the battery tests normally when removed from the vehicle, but does not function properly when installed. Despite numerous repair attempts and escalation to American Honda, the problem remains unresolved. I believe this defect poses a serious safety risk to myself, my passengers, and others on the road due to the potential for sudden loss of acceleration while driving. COMPONENT FAILURE: The mechatronic unit, the electro-hydraulic controller that manages all gear selection and clutch engagement inside the DSG transmission has failed. ... intermittent failure to engage gear after stopping at intersections and stop signs. The transmission would slip out of gear or refuse to engage, leaving the vehicle momentarily unable to accelerate from a dead stop in traffic, a dangerous condition with cross-traffic approaching. The condition progressed to include an audible whining and slipping noise during downshifts, consistent with inadequate hydraulic clamping pressure on the clutch pack. The transmission then lost the ability to engage reverse gear entirely. The vehicle was driven home in forward gears only and is now inoperable and parked. SAFETY RISK: On multiple occasions, the vehicle was unable to accelerate from a complete stop at intersections with active cross-traffic. Loss of reverse gear eliminates the ability to maneuver out of dangerous situations in parking lots, driveways, and streets. This vehicle is unsafe to operate. WARNING SIGNS: No warning lamps or dashboard messages appeared prior to or during the failure. The first symptom was intermittent hesitation and slipping when attempting to engage from a stop. The condition worsened rapidly over a short period until reverse was lost completely. DEALER/MANUFACTURER CONTACT: The manufacturer was contacted. A callback was promised within two business days and was never received. These symptoms are identical to NHTSA Recall 24V-267 (Ford 24S24) — loss of motive power from the BCM/PCM failing to detect 12V battery degradation — and the follow-on battery recall 25V-158 (Ford 25S26). My vehicle is a 2024 Maverick and is NOT included in 24V-267, which Ford limited to 2022-2023 Mavericks built Feb 2021–Oct 2022. I am reporting because my 2024 shows the identical failure mode, including the total absence of warning before power loss, matching the owner reports that prompted Recall Query RQ24014 into the adequacy of the 24V-267 remedy. I request NHTSA consider whether the affected population should include model year 2024 Mavericks. These events are recurring and have occurred at highway speed and while merging, creating a serious crash risk. The car was not drivable, so I brought the car to the dealership and the service manager told me that this error was noted by them as they were working on the PCM and should be fixable. However, after about 50 days of the vehicle in the dealership( April 9 2026- now), they have been unable to find a resolution to clear this error and I have been without the vehicle. I am being told Porsche Germany and Porsche North America engineering are unable to identify or unable to repair the chassis computer and there are more vehicles in a similar situation around the country. I have called Porsche North America for support or help with explaining the reason why this brand new vehicle has this defect. I have made close to 20 phone calls after countless hours of being on hold and waiting, with 0 calls returned. I got one email back after 40 days saying the local dealership will handle my case and Porsche North America will close my case even though my car was still being repaired. I believe this may be a manufacturing/ design error and may involve several other cars. It was considered a total loss due to the frame and passenger side as well as both air bags deployments. The driver sustained major lacerations on his forearm and was bleeding profusely from his arm during the time of the accident.

The vehicle was returned the same day. On May 31, one day later, the error occurred again. The defect remains unresolved after four service visits. Out-of-pocket towing and rental car costs have been incurred each time. I believe this defect is related to the hydraulic clutch actuator issue covered under Kia recall SC276 and the active class action lawsuit (FeganScott v. Kia/Hyundai). Despite these efforts, I have not been provided with a fix that addresses the root cause of the problem. Ford has long been aware of widespread TCM issues affecting certain Ford Focus models, making the continued failure of this vehicle especially concerning. My greatest concern is safety. During a recent incident, the vehicle unexpectedly slammed into gear with significant force while my son and I were inside. The sudden engagement nearly caused me to lose control of the vehicle and almost sent us into a ditch. Had road conditions or traffic been different, the outcome could have been much worse. I have dash camera footage documenting the incident and am willing to provide it upon request. A vehicle that has already undergone a transmission replacement should not continue experiencing severe transmission-related failures, particularly failures that create a risk of loss of vehicle control. This ongoing issue places not only my family at risk but also other motorists sharing the road. I respectfully request a thorough investigation into the recurring transmission and TCM failures affecting my vehicle, including a review of the October 2022 transmission replacement and Ford's response to these repeated issues. I also request that Ford provide a permanent solution and appropriate assistance given the history of repairs and the known defects associated with this model. I am requesting that NHTSA review this as a potential unintended movement / unintended acceleration / park-to-drive activation / rollaway safety defect. I tried cutting the wheel and the tires seemed unresponsive and the vehicle didn't stop until it hit a small tree on the right side of the parking area. Upon trying to move the vehicle back to the original parking space the vehicle seemed to only allow steering to the right and hit a tree on the right passenger front door. The airbags, part of the service work previously completed from another recall, didn't deploy despite having an 85lb passenger in the vehicle. The dash flashed with many sensors when I initially tried to stop the vehicle when it was in drive. The vehicle is available for inspection. The safety of myself and my passenger were at risk. I was transported to the ER. The problem was not confirmed by anyone yet. The vehicle was inspected by police visually and damages documented in the police report. I have not spoken to the dealer as I'm now weary of their service center practices since this occurred the first time I drove the vehicle after they made the repairs for the passenger airbag and brake recalls.Read more

It would appear that Chevrolet is aware that this is happening in its turbo models. I have an extended warranty that was purchased through the dealership. This issue caused them to replace the fuel sealant, the fuel caps, and the fuel pump. However, 2 weeks later my vehicle is still leaking a significant amount of oil. When I took the vehicle to be serviced, I was told that the sealant is broken again and gaskets have to be replaced. Upon researching this online other owners have experienced the same issues. This unpredictable behavior creates an immediate and severe safety risk to the occupants of my vehicle and others on the road: 1. Intersection Collisions: The sudden lag leaves the vehicle stranded in oncoming traffic during turns, while the subsequent lunge threatens to overshoot the intended lane. 2. Pedestrian Hazards: The sudden, uncontrollable lunging makes low-speed maneuvers in school zones, crosswalks, and parking lots incredibly hazardous. 3. I then heard engine RPM increase audibly, consistent with an acceleration command, while my foot remained fully on the brake. I felt forward thrust working against my braking. Every trip in 87°< temperatures, a 2-3 time limp mode was in store. I realized in June of 2025 that this problem doesn't throw any DTCs when it happens and because sometimes he didn't wait long enough to clear the limp mode, I had him disconnecting the negative battery cable then reattaching it in order to clear the resident memory to get him going right away. I went a step further and installed gate switch on the negative battery post so he could just lift the hood, break the circuit with the gate arm and put it back down. I write this today because the car's 1st episodes for 2026 was on a similar mileage trip home, however it was only 82°. The CVT fluid has been changed, the level is correct and his driving habits appear to be in line with GPS ETAs vs. his actual arrival times. I went down the rabbit hole regarding the 2013 Kizashi TCM recall and I truly believe the defective TCMs they found have been around for some time before the 2013 Model year. He loves his car, but the car is putting him at risk as well as the driving public around him EVERY TIME THIS HAPPENS.
