We just completed a 6500 mile round trip road trip from So Cal to Toronto Canada and back. 5/24/22-6/12/22 During the trip we used the "Auto Pilot" with and with out "Auto Steering" Not Full Self Driving. My deduction on why this is happening is that the Tesla cameras are "Infrared" sensitive and it seemed that the car was applying the brakes when it would see the heat waves off the hot road surface. Also in early morning one day the sun was directly behind us and the cars shadow was directly in front of us on the road surface and it kept applying the brakes thinking it was another car. In slow moving bumper to bumper traffic going 30MPH or less does not seem to initiate the "Phantom Braking". But at highway speeds it is super scary and dangerous when this happens. ;(Read more

... asked for it to be enabled. Then I left my house on June 9th for a 12 day, 2500 mile round trip to Seattle and back. During the trip I cautiously began to use autopilot functions, and found them to be very useful, particularly the TACC cruise control feature. As the trip progressed, I began to use the TACC more and more because I was beginning to conclude it might be safer than my own driving in specific situations). On the 6th day I began to return home and increased my use of both autopilot functions (TACC and auto-steer). The further I drove, the more certain I became what was happening, but not why. But first you have to understand the constraints. TACC controls the accelerator and the brake pedal and therefore also controls the regenerative braking capability. The other half of autopilot is the auto-steering capability, which controls only the steering wheel. But the important part is -- you can operate the car with only TACC engaged but you can't engage auto-steer unless you activate TACC first, or at the same time. During my trip I experienced some degree of PB something like 25 times. The 3-5 worst were similar to stepping on the brake pedal firmly. On one occasion I had no traffic near me so I didn't immediately override the stop, to see what it would do. The car continued to rapidly brake from about 75 mph down to about 25 when I stepped on the accelerator to break the PB episode. My conclusions -- PB is a TACC problem. Auto-steer does not change the behavior and PB doesn't happen unless TACC is engaged.Read more

A Tesla car in front of us suddenly braked for no apparent reason and shortly thereafter pulled over to the shoulder and stopped. A few minutes later we experienced a sudden braking action in our Tesla car. In May/2022 we observed two separate incidences of a phantom van displayed on the Tesla central console while parking our car into our garage. A phantom van appeared once on the right side and once on the left side when the Tesla car was half-way into the garage. We have just moved from the Midwest to Phoenix(car only has 30k miles), and the hesitation and stalling is worse than ever. Even with a 3/4 depress on the accelerator, the notion of movement correlating with the amount of gas given is an eon apart. I must also mention that there are times that the car releases like a race car as well, but with minimal pressure on the accelerator. It seems like it’s an obvious programming issue or maybe just a few bad relays. I’m hoping there is a light at the end of the tunnel for this one. 2. Suspension: Our vehicle has sounded like it has had major issues since the beginning, but was told it was normal. The car has always made a loud clunk when reversing after a first of the day start. It is then followed up after warming up with a clunk in the front seats center below us, and while turning in either direction. Hitting any minor bump on a highway while on any mild curve also causes the vehicle to jump a few feet over by itself. It’s frightening judging just how the car will grip after it’s roll. The only remedy is to decelerate under the speed limit during any curve to avoid the situation all together. I’m not sure I’d ever purchase a Mitsubishi again, as I’m only scratching the surface with mechanical defects. I would be happy to finally get some answers on how this was left in the dark so long. ... 6/15/2022 the day after my engine failed my car was diagnosed by All 4 U Auto and he confirmed that the engine had blown and along with the gasoline tank both needed replacements. No, there were no warning lamps nor messages prompting alerts of my vehicle. There were several symptoms of the problem prior to the failure that occurred before and after the Ford recall; 18S32. I completed the recall fuel tank deformation 1/3/2019. I am still experiencing issues with my gas tank, which are listed on the official Ford website. I can only pump approximately 80 miles worth of gas or 1/4 full worth of gas. If I pump anymore the vehicle will stall and require for me to restart it multiple times 2 - 10, the more I "over pump" the more I will have to restart and longer it will stall. During pumping even 1/4 worth of gas my entire vehicle will shake and make a loud sound, and with re cranking causing engine malfunction. The website mentions; "Owners are advised to maintain at least a half tank of fuel until their vehicle is serviced for this issue." although I can only pump 1/4 full worth of gas at a time. Another issue is I cannot leave the vehicle idle over 5 minutes, even when in traffic if I do not keep driving up 6 inches every five minutes it will turn off. This problem has negatively affected my battery, on a few occasions leaving the vehicle idle has caused my battery to need to be recharged and replaced. Lastly the vehicle does exactly what the recall mentions; "inaccurate fuel gauge readings". While pumping gas the mileage on the dashboard sometimes does not increase.Read more

On other trips they may occur 2-3 times over several miles/minutes. driving/stopping on wet pavement could be problematic because the ABS, Cruise control, and built-in trailer brake controller ALL stop working when the warning lights come on. If hauling a very heavy trailer, and the trailer brake controller fails, then this could cause a fatal condition for not only the driver of the truck, but all those in the vicinity should a sudden slow down or stop be needed.Read more

I investigated and Unintended Acceleration crash that occurred on 8-9-2021. The vehicle had just left a body shop where it had repairs to the left front of the vehicle near the ECM. The driver reported the vehicle would not slow down. He face timed a witness and showed her his foot was on the brake and not the accelerator pedal collaborating his statement. Upon impact the fuel tank of the Cadillac exploded igniting both vehicles on fire and burning a fed-x truck that was folded in half from the sever impact. The EDR report stated the brakes were applied fully for a full 5 seconds prior to the crash while the vehicle was still accelerating. The accelerator was reading a constant 57% during the lead up to the crash and the brake rotors showed heat related glazing from excessive braking. The engine torque was also at 1023NM leading up to impact collaborating Unintended Acceleration further. Based on the evidence gathered my opinion is there was a short in the components in the left front of the vehicle most likely the ECM harness that caused the crash. It is unknown if the damage was caused by the previous crash, the body repairs or a manufacture defect at this time. The radiator had also just been replaced so moisture could have contributed to the malfunction. ... L. I was told by several people at these dealerships the following reasons why they haven't gotten to me - or quite frankly, simply refuse to do the work. That is, unless it's winter, when they have nothing better to do - and good luck getting a bike to a dealership in Illinois in the middle of winter. The first excuse is that, "it's just too much work". They have to remove the engine to repair the bike. They're "short staffed" and "Yamaha doesn't pay them enough to perform the recall work". They're opting to take in bikes for which they can charge full price while my bike and thousands of others like mine sit unused and unsellable, because no one wants an unridable motorcycle. I've spoken with and written Yamaha customer service. Their reasoning for not doing more to make sure the work gets done, is that "every dealership is privately owned" and they "can't tell their dealships how to behave or what to do." They do however, recommend that I don't ride the bike until it's repaired and that I can "feel free to look for another dealer to do the repairs". Wow. Thanks for absolutely nothing. I've literally called every Yamaha dealership in a 60 mile radius of where I live. Some will actually tell you that they're simply refusing to do any recall work. These recalls are based on manufacturer defects that could cause life threatening injuries or death. It seems clear that Yamaha has a responsibility to their customers and are refusing to do anything about it.Read more

During this situation, the “driver override “ message would appear although no accelerator pedal pressure was being applied. Additionally, the ETC warning light would randomly light up and then clear. When the vehicle was stopped and placed in park, the engine would rev to approximately 4000-5000 RPM. Especially in congested traffic conditions, this runaway engine RPM condition is a terrifying situation. This malfunction has occurred on this vehicle before, and to a similar extent. However, the Jeep dealer could find no underlying cause. The vehicle will also occasionally operate at a higher than normal RPM, but not to the extent described here. I do have an appointment with my local Jeep dealer for June 21. I will describe the issue with them in detail. My purpose for entering this report is to add my issue to any potential list of other Jeep owners who may have reported a similar issue.Read more

Problem occurred last night when attempting to accelerate and merge onto the highway, felt like the car was in neutral ('D' light was blinking as well as the 'Hybrid' light). What is concerning is that VW did know about this problem and they only did a recall for very small number of vehicles. We called VW and they said no recalls on our 2016 Jetta Hybrid for this problem. I'm reporting the problem here because we have experienced the same problem multiple times and the dealer said to replace the Mechatronic J743 unit, this was very cost prohibited for us at the time.Read more

As I was driving to church on highway 46W and then, it turned into 23N, I noticed my car was driving by itself. I pushed on the brake pedal because it's an intersection, and I noticed it slowed down, but as soon as I let my foot off the pedal the car was accelerating again. This time I lifted both my feet away from the pedals and had them in the air, and I checked if the mat or anything was touching the gas pedal, but nothing was on or pushing it. So, I decided to downshift, because my car is standard manual, and my car began revving. The RPMs went high and then, I shifted up and pushed the gas pedal and after I did that, that's when the gas pedal popped, it made a loud pop, and the car stopped accelerating on its own. If I had to stop immediately, I could have, DEFINITELY, got into an accident for something that was out of my control.
