While driving, my 2017 Ford F‑150 experienced a sudden loss of the Output Shaft Speed (OSS) sensor signal, causing the transmission to stay stuck in a higher gear and triggering the wrench light. The service advisor did not disclose any known issues and never called me back after saying he would check parts availability. These symptoms match the known failure pattern of the molded lead frame in the 6R80 transmission. My truck exhibited intermittent behavior, harsh shifting, and unsafe drivability. I had to pay $1,250 out of pocket to replace the lead frame to restore normal operation. I later learned that NHTSA has opened an Engineering Analysis into 2015–2017 F‑150 trucks for this exact issue (OSS signal loss due to internal electrical degradation of the lead frame). My vehicle’s symptoms, diagnostic codes, and required repair are consistent with the defect under investigation. This failure creates a safety hazard because the truck can suddenly lose the ability to accelerate or shift correctly, especially in traffic or at highway speeds. I am requesting that NHTSA include my vehicle in the investigation and that Ford be required to address this defect.Read more

... his occurred in an active school pickup area with parents and children nearby. There were no cars or pedestrians directly in front of me at that moment. I still fear what the outcome might have been if anyone had been present. Immediately after stopping, I observed that the adaptive cruise control speed indicator on the instrument cluster was set to 15 and was crossed out, indicating it had recently been active and disengaged. However, I did not engage adaptive cruise control during that trip. Based on the car’s behavior and the display state, my best assessment is that adaptive cruise control was unintentionally engaged during the parking maneuver causing the car to attempt to accelerate toward a previously set cruise speed despite being in a low-speed, turning scenario. Again, I did not use ACC on this trip. A sudden acceleration in a school parking lot presents an extreme hazard. The only reason this incident did not result in injury or death was immediate braking and luck. There were no warning lamps or messages prior to the incident. The car has not yet been inspected by a dealer or manufacturer. I am aware of multiple publicly shared videos from other Mach-E owners documenting similar incidents, some resulting in crashes. This appears to be a systemic adaptive cruise control design or software issue that warrants immediate investigation and correction to avoid serious injury or death.Read more

The 2025 Hyundai Santa Fe Hybrid has had multiple occasions, most recently on February 3, 2026 with 12,330 miles on the car, where the gas-powered engine fails to provide any power to the drivetrain. After turning the vehicle off and on several times, eventually, the gas engines re-engages. The first incident of this type for this vehicle was on August 7, 2025 with approximately 4,400 miles on the vehicle. It was taken to the dealer, Key Hyundai of Salem, who could not find any DTCs and could not replicate the problem. They said "bring it back if it happens again." They next time was on February 3, 2026 where it happened several times on a trip to and from a local restaurant. This time, at DTC code of P088000 POWERTRAIN / POWERLOSS was recorded and reported. However, by the time the car was seen by Key Hyundai of Salem on February 5, 2026, the code has cleared itself. Hyundai's corporate policy is that historical codes can not be used for Warranty claims. So, the dealer tried one potential fix, cleaning the TCM connectors under the driver's seat, and returned the vehicle stating that Hyundai corporate would not authorize warranty work unless/until the car was presented to the dealership with a current / live DTC or visible broken parts. It seems unsafe to put a vehicle back on the road that could lose power and drivability at any time for no apparent reason. I am aware that there have been several other reports of similar POWER LOSS / LIMP MODE situations by owners of 2024-25 Hyundai Santa Fe Hybrids. ... ndicating the underlying problem was not corrected. When the loss of power occurs, the vehicle does not respond to acceleration input and behaves as though propulsion is cut off completely. This happens suddenly and without warning while driving.This condition creates a dangerous situation, particularly when driving in traffic or at speed as was the case on 12/4/25 when driving 70 mph on the freeway, the second time the issue occurred for us. The car was immediately brought to the dealership on 12/4/2025 and remains there today. On the first occurrence the dealership attributed it to low battery charge and replaced both batteries. The dealership has performed inspections and diagnostics including:Battery testing and replacement,Wiring and harness inspections,Software updates to vehicle control modules,Extended road testing.Despite these efforts:The issue has not been permanently resolved,The dealership has stated the concern could not be duplicated,The dealership has indicated that fault codes were not always present. A STAR case has been opened, Jeep Customer Care has been contacted regarding this issue, and a field technician has been requested to come to the dealership to assess the vehicle. I am reporting a safety-related defect involving unintended vehicle movement on my Hyundai Ioniq 5 (Model Year 2023, SEL AWD). Observed behavior: With the vehicle in Drive and the Auto Hold function active (indicated by the white “AUTO HOLD” indicator in the instrument cluster), I brought the vehicle to a complete stop by pressing the brake pedal. ... le remained stationary, and I removed my foot from the brake. After approximately three minutes of remaining stopped, a driver-assistance system error message appeared on the instrument cluster. At that point, Auto Hold disengaged (the “AUTO HOLD” indicator disappeared), and the vehicle began to creep forward on its own. I did not command acceleration, did not press the accelerator pedal, and did not intentionally release the brake. The movement was unexpected and created a safety risk. The first occurrence happened in a school drop-off line with the vehicle stopped near other vehicles and pedestrians. I have since reproduced the behavior and captured it on video. Related observations: Similar driver-assistance system errors have occurred while the vehicle was in Park and in Drive, with or without Auto Hold active. In those cases, the vehicle did not move because it was either mechanically locked or I had my foot on the brake. In all cases, Auto Hold deactivated. Safety concern: This behavior raises concerns about the reliability of Auto Hold and its interaction with braking and driver-assistance systems, particularly after the vehicle has been stationary for an extended period. Unintended forward movement presents a significant safety hazard. I am reporting this issue for investigation of potential defects in software, sensors, or control logic. When this happened the first time (last year), I took it to the Honda dealer where I bought it, and they said they had never heard of this happening. When it happened the second time in January, I called Honda, spoke to an agent who gave me a case number (XXX), and said she has heard of something like this from other drivers, but the solutions were all different, so she couldn't say what would fix it for me. I did some internet research, and found other drivers who describe the exact same issue: [XXX] I've switched to premium gasoline ($$) and won't let it get lower than 1/4 full, and have an OBDII scanner in my glove compartment ready to go if it happens again. That said, this is a very dangerous defect, and I feel unsafe in my car. I am disturbed that a problem of with such potentially serious consequences has no solution that anyone can offer me. When this happens, the vehicle does not respond to the accelerator for several seconds even if I press the accelerator to the floor. This has put my safety and the safety of others at risk because the vehicle fails to accelerate when needed (e.g., when merging, turning, or avoiding traffic), creating multiple dangerous near-miss situations. The warning message appears at the time of the event; I did not notice a warning lamp or message consistently prior to the event. This issue has not yet been reproduced or confirmed by a dealer or independent service center. A dealer service visit is scheduled for next week. The vehicle has not yet been inspected by the manufacturer, police, or insurance representatives for this problem. 2. Seats (driver seat causes leg numbness/spasms impairing pedal control): The driver seat bottom is unusually firm, and after approximately 30 minutes of driving my legs become numb from the lower thighs down to my feet and I experience spasms to the point that I cannot safely operate the brake and accelerator pedals. This is a safety risk because it can prevent proper pedal control while driving. This condition has been consistent since shortly after purchase. ... While the vehicle was often still technically drivable, these issues regularly caused the car to operate at reduced functionality, with key safety and convenience features disabled. Although software updates have reduced the frequency of these issues, they still occur intermittently and often require a vehicle reset or time to resolve. Because of these persistent and unpredictable warnings, my family and I have avoided taking this vehicle on long trips out of concern that the car may become unsafe or inoperable while traveling. The vehicle was brought in for service in December 2024 in an attempt to address these software-related issues; however, the service did not resolve the problem at that time. Hardware Failure #1 – GHCA In January 2025, the vehicle experienced a failure of the GHCA component, rendering the all-electric vehicle unable to charge. This occurred shortly after purchase. Service appointment booked: January 27, 2025 Vehicle in shop: aprox 30 days The vehicle was out of service for approximately one full month. During this time, I shared my experience on Reddit due to the severity of the issue and the vehicle’s newness. That post received over 350,000 views, and numerous Volvo EX90 owners reported experiencing the same GHCA failure—many stating they had required multiple replacements of the same component. This strongly suggests a widespread defect rather than an isolated incident. Hardware Failure #2 – GHCA (Repeat Failure) On February 3, 2026, the GHCA failed again, and the vehicle was returned to the dealership for service with no ETA on a fix.Read more

Myself and all family drivers experience this IMMEDIATELY after activation. Dealership has reported no trouble after 50 miles of driving (as stated). I have video of this issue and am adding them to youtube (2026 hyundai palisade cruise control fail) for others to see. This is UNSAFE and DISTRACTING! The alerts alone are enough to deactivate this (if available, but: it cannot be deactivated!) let alone decelerating from speed for "no good reason" outside of drivers DIRECT control. The issue has intensified from the initial symptoms, (which were as I described, but resolved by turning the car off, waiting 30 seconds to a minute and then turning the car on again) to how it is now: in addition to the check engine light, every other light and warning system has gone off when driving on the highway and the engine was unable to go faster than 40 miles an hour, and it does not resolve by restarting the car. The dealership and Honda America’s main phone line have as of yet been unable to give me a clear answer as to why this is happening or whether it can be fixed. Unplugging the 12v battery also had no effect! The vehicle kept driving as if it was in drive, and there as no way to stop it without placing the foot on the brake pedal the entire time or placing the blocks I referenced. The only way I was able to shut it off, was to pull what's known as the Fireman's Loop, which I'm guessing firemen pull in the case of an accident. I wasn't aware of this mechanism, and only learned of it after posting on numerous social media apps regarding this issue. This is a Major issue, as we are talking about a moving and unstoppable car! Tesla needs to build some mechanism that allows the car to be shut off by the driver or to override the screen and place the car in park. On 3 different occasions the car randomly accelerated at a high speed without stepping on the gas pedal. The first time was in a parking lot. The second time was on a busy road while traveling at 35 mph. The car accelerated to about 60 mph, fortunately once applying the brakes the vehicle slowed down to gain control. The third time it happened was recently on [XXX] while slowly backing down [XXX].

After the third incident I took the car to a dealer Planet Subaru in Hanover MA. After keeping the car for a few days, the dealer called and said that they could not find anything wrong with the car and there was nothing that they could do for me. QTY 15 units waiting. Low battery warnings & P0D33-11 code are related. Weak 12V battery = stress the DC/DC converter, fault codes, conversely, a failing DC/DC prevent the 12V battery charging. The car was able to clear the 2-3ft concrete base of the post and the metal beam crashed into the glass roof of the car (which was shattered and thankfully the surfboard racks prevented it from fully crashing into the vehicle - but the racks are completely crushed), the metal post then launched itself into two more vehicles as well as the car itself. Airbag was not deployed at any point, car was going over 30 mph when it finally stopped (the concrete base likely pierced the battery to make the car stop). Logs didn't even note that there was a crash and the vehicle is totaled. We have salvage holding off on destruction so that the EDR can be salvaged and retrieved.Read more

We followed this guidance exactly, disabling Bluetooth and wireless CarPlay and using only a USB connection. Despite this, the infotainment system continues to crash and reboot while driving. Because the vehicle relies on the center screen for climate controls, vehicle settings, and camera access, this failure creates driver distraction and a potential safety risk. Loss of the center display while driving is not acceptable in a new vehicle. This appears to be a software or system stability defect affecting the infotainment system and is not limited to wireless phone connectivity. The collision involved three vehicles: my VW, the main car I hit, and a rental car. My car spun and veered sideways, the parked SUV on the right was pushed sideways, forming a rough “H” pattern, and the rental car went up onto a rail ramp where children walk. My grandson observed the car’s tires spinning at impact, and he hit the seat behind me. Without the parked cars, the vehicle would have continued directly toward the school building, creating a very serious risk of fatal injuries to parents, staff, and children. While driving my 2017 Honda Pilot Elite, the vehicle has experienced four separate incidents of sudden and complete loss of power, including while traveling at highway speeds and during rush-hour traffic. ... terstate 275 in heavy traffic, creating an immediate risk of collision. In one incident, the vehicle died while traveling at highway speeds. In another, it shut off at a red light and blocked traffic during rush hour. The vehicle has been taken to an authorized Honda dealership multiple times. Diagnostic testing initially found no issues. Later visits attributed the problem to the battery, despite the battery being recently replaced and the condition recurring after replacement. The symptoms exactly match Honda Pilot start/stop engine failure recalls affecting nearby model years. However, because my model year is not included, the dealership refused to repair the vehicle in accordance with the recall, even when I offered to pay for the repair. I believe this represents a safety-related defect involving sudden engine shutdown while driving. The vehicle is unreliable and unsafe to operate. Furthermore, given the repeated failure of this vehicle, the dealership’s inability to permanently repair the condition after multiple attempts, and the ongoing safety risk, I am concerned this matter may meet the criteria for a nonconformity under Florida’s Motor Vehicle Warranty Enforcement Act. I am hopeful Honda will resolve this matter cooperatively and without the need for further regulatory or legal action.Read more
