Complaint: Phantom Braking -- My wife and I took ownership of our 2021 Tesla Model 3 at the beginning of July 2021. ... k to be reliable and hassle-free to operate. On a few long stretches of road in Nevada, Wyoming, and Utah we engaged model 3's adaptive cruise control. Three times during that trip, between July 2nd and July 8th, 2021, the car braked hard, without warning, while traveling at highway speeds. From what we could determine, shadows cast on the road at the approaching side of gentle hills, with the sun in front of us, may have been interpreted by the car's camera's as an approaching or stationary object on the road. This was very unnerving and caused us to lose faith in the adaptive cruise control system. This was both disappointing and a bother since it meant that we were driving a 50 thousand dollar car without cruise control, making driving a more tiresome experience. This past labor day weekend, 2022, we drove to Las Vegas on the same Tesla model 3 and again activated the adaptive cruise control while driving in Nevada. We thought that by now the periodic software updates would have fixed the problem. This time the Phantom Braking occurred more frequently and on seemingly level sections of road. After about four attempts at using the adaptive cruise control we stopped. This was, again, disappointing that we were driving a car whose adaptive cruise control failed to perform as it was designed to. We were disappointed because for what we paid for the car, this feature should be more mature than it is. Luckily we did not have other cars close behind us at the times the car's computer together with the sensors and / or cameras decided it needed to apply the brakes.Read more

... tor. The same thing happened in my 2015 Mazda CX5. In that case I had it checked at the dealer. The technician found nothing wrong but was able to reproduce the problem when his boot pressed both the brake pedal and accelerator because both pedals were so close together. Since this would probably be an inexpensive software change, it would be prudent for a safety recall by manufacturers that have pedals that are close to one another.Read more

In 2018 a recall for the dual powershift clutch was repaired, now the TCM faulted out in the car while i was driving, i had an infant in the car and it died in the middle of a busy highway almost causing us to crash! ... currently for these problems. I work 2 jobs, Latham ford refused to even look at my vehicle for 2 months, i work, i cannot go without a car. The tcm is currently on a national backorder for thousands of people. I had no means of transportation for an entire month, it cost me hundreds of dollars for taxis and ubers. I found a replacement part on my own and had it installed yesterday. the manufacturer stated in the description, that the part was already programmed, it was not. It was misrepresented, and it cost me an extra $400.00 to have it programmed, and also the software needed to be updated to clear error codes in the tcm for it to be relearned. So far im out about 2 thousand dollars for a known manufacturing issue that could have ended FATAL. I want to be reimbursed for every dime i spent, and the inconvienience! I have reciepts for the part and labor! I will never buy a FORD product again after this. I could have died, along with my passenger. something needs to be done!Read more

At which point I was maybe 20 ft behind the 18-wheeler and all of a sudden the car is adaptive cruise control tried to accelerate the car back up to the setting of 65. It didn't register the 18-wheeler at all It just registered that the car in front of me had cleared the space and if I had not been paying attention and immediately applied the brakes to stop the car manually It absolutely would have accelerated directly into the back of the 18-wheeler. I was maybe 5 ft and it was still accelerating when I slammed on the brakes. I've been able to since reproduce the effects, it doesn't register 18 wheelers correctly in this specific situation as vehicles. I've even had the camera recalibrated in between tests. The contact stated that medical attention was required for being hit in the chest by the air bag. The contact stated she was unaware if the occupants of the other vehicles needed medical attention. A police report was filed. The vehicle was taken to the dealer however a failure cause was not diagnosed. The manufacturer was made aware of the failure however, no assistance was offered. We are taking the vehicle to the dealership again today 9/22/22 because it started again around 9/1/22 and we have been waiting for a loaner car again from Dealership for same problems. Meawhile, being forced to drive it due to only owning 1vehicle. Hyundai Manufacturer has continued to have dealership change out parts and this keeps happening. The vehicle has just over 25K miles on it to date. The first time we had it in the dealership for this problem was on 4/6/22. We have almost rearended several vehicles and are scared to death to drive this vehicle. It is a safety hazard. We purchased the 2021 Sonata vehicle brand new on 11/24/21 along with the lifetime parts and labor warranty. It had 15 miles on it. We also traded our 2018 Hyundai ELantra. We paid off the 2021 Sonata completely in 2021. The total cost of this transaction was $34,294.74.We have also followed all the Maintenance requirements. It is urgent that this complaint is investigated as soon as possible and Hyundai Manufacturer take responsibility now before someone gets killed. The lane keep assist of the car is supposed to keep the car centered when enabled. It does this job well most of the time however a few times, the car keep wanting to hug the right lane line to the point where lane departure alert starts beeping. ... ve to actively hold the steering to keep it centered since the system will keep fighting you to gravitate towards the right lane line. This issue occurred twice as of this date with no specific pattern observed that might trigger it. The abnormal behavior is observed when the system is engaged for a while in its normal operation. This can pose a safety issue if the car suddenly drifts to the right rather keep it centered in the lane since it occurs without warning. No warning lights or alerts come in the car when this abnormal behavior occurs. However the dashboard (sport touring digital cluster) visualization also shows the car deviating towards the right lane line during this behavior indicating the car's camera also sees this abnormal behavior but the lane keeping logic somehow is issuing the wrong control commands. When traversing from one hwy to another on an off ramp at 39 mph the truck suddenly accelerated like the pedal was pushed to the floor, speed jumped to 55mph in a flash. I hit the brakes hard once and then again to stop the sudden acceleration. Truck had a mind of its own. Dangerous situation, fortunately no one was in front of me to run into.

On Tuesday September 13th 2022 morning around 10.55am EST, I had entered the Hindu temple of Florida premises driving my TESLA model X. I slowly progressed towards the parking spot and made a slow right turn to park the car. At that time the Tesla suddenly accelerated at full speed, took off like a rocket, jumped the curb, knocked down a sign post and jumped into the ditch which leads into a pond.

The car veered to the right, jumped out of the ditch back toward the parking spots, hit two vehicles ( a Honda CRV and a Lexus) with a significant force and hit a tree with a big impact and finally stopped. Surprisingly airbags did not deploy. I was wearing the seat belt and was the only one inside the vehicle. Fortunately with gods grace I escaped with minimal injuries. I get nightmares every day since then thinking about how close to death I was. Had the gone gone straight into the pond, I could have drown to death inside the vehicle, had it gone in a different direction and hit those concrete walls of the temple, I could have been crushed. More significant was it did not run over any of the visitors who frequent thaw temple! This sudden unintended acceleration not only shocked me but also shocked several people who were witness to this crazy event. The CCTV footage I obtained shows the car going towards the parking spot and caught the initial acceleration and take off and the big thuds twice that ensue within a few seconds. I have heard Tesla cars were involved in certain freak accidents but never imagined that I could be part of one. I’m uploading a few photos, a video clip and a cctv footage clip for your review. I read reports that Tesla blamed these type of unintended acceleration on driver errors pressing the gas pedal! I have seen many blogs, forums and actually know another Lancer owner who’s model is a 2012 and has the exact same issue. I don’t know why this hasn’t been addressed yet when it’s affecting more than just one Lancer here or there. Bad for the engine bad for the transmission bad for my travel. And the worst part is that any mechanic is unable to detect the issue unless the car is actively coding. I’ve had this car for 3 years now and I feel it’s a big safety concern and needs to looked into. First occurrence was on a trip down to a local lake car stalled on the side of a highway this was in 2018. Now any trip further than over 30 miles or more the car will throw this code it looks like a gear and it has a exclamation point in the middle. It’s progressively getting worse and effecting the over all performance more each time I wish to travel. Throttle tends to rev and stick at 4000rpm randomly. Not sure if this is caused by the system software or hardware failure or both. Once this happens the engine functions erratically and the exhaust pipes will heat up until they are red hot and will melt and cause a fire or explosion in the vehicle. As covid was present, the car wasn’t driven much during. However the knock/ jerk between 1-1.6 RPM’s has always been present, even returning after the TCM reset. Dealership was notified, and neglected to return calls or follow up. Fast forward to 9/15/2022, after 3-4 wks of calling and leaving voicemails to no avail from the dealership service department manager/ dealership. I set up an appointment for 9/15/2022. On 9/5 & night of 9/14/22, the car instrument cluster lighted up with all notifications, and dashes for the total mileage on the car, orange wrench light - indicating Ford Limp mode, and low fuel popup (car was just fuels 1 hr prior to driving from Costco for grocery) I left and drove up a hill when this occurred as I was going 40-45 miles per hour the speedometer dropped to 0 MPH & the car barely drove beyond 5-15 MPH. I pulled over at the nearest local street from The hill parked the car and powered off the engine. I had a OBDII Tool that indicates: Global OBD FORD. P0720(1 / 1) Output Shaft Speed Sensor Circuit. Vehicle happens to accelerate involuntarily when driving, especially on highway. You'll be driving, and then suddenly it starts to accelerate faster than usual.
