While driving the vehicle at 55 mph, it suddenly lost the ability to accelerate even when I pressed down on the accelerator. The RPMs went up to 4 or 5 but I could not go over 30-35 mph. Fortunately, I was able to move into a left turn lane to get away from the flow of traffic and avoid an accident. A semi-truck was behind us when I was unable to accelerate. We had to have the vehicle towed to a l
... ocal dealership in the area. I had been driving the vehicle for about 2 hours when this occurred. The service department at the dealer was able to recreate the issue. My vehicle has been out of service and remains at the dealership. They told me that a new transmission would be installed due to loss of transmission fluid. They had a difficult time identifying the issue because there were no warning lights or codes registered.Read more

COMPONENT/SYSTEM FAILURE: Timing chain and timing chain tensioner/guide assembly failure. The timing chain has stretched prematurely, causing the vehicle to fail to start, loss of acceleration response, check engine light illumination, and audible rattling noise from the engine compartment. The failed components are available for inspection upon request at the dealership. SAFETY RISK: The vehicle
... would not accelerate when the gas pedal was pressed, creating a dangerous situation where the vehicle could not respond to driver input. This poses a serious safety risk in traffic, highway merging, or emergency evasive situations. Additionally, the failure to start leaves occupants stranded without warning. DEALER CONFIRMATION: The problem has been diagnosed and confirmed by an authorized Infiniti dealer. The dealer determined the cause is timing chain stretching and has quoted $6,500 for repair, which includes timing chain, tensioners, and guides replacement. PRIOR WARNINGS: Approximately one week prior to complete failure, the vehicle began exhibiting a rattling noise from the engine. The check engine light illuminated. Shortly after, the vehicle would not start and would not accelerate when the gas pedal was pressed. KNOWN DEFECT: This is a documented defect. Infiniti Technical Service Bulletin ITB19-027a addresses timing chain-related diagnostic trouble codes on 2017-2024 QX60 vehicles with the VQ35DD engine. The timing chain tensioner and guide failure on VQ-series engines is a well-documented premature wear issue affecting vehicles well before expected service life. At 80,000 miles, this failure is premature and indicative of a design or manufacturing defect, not normal wear. REQUEST: I am requesting NHTSA investigate this safety defect and consider requiring a recall for affected Infiniti QX60 vehicles. Nissan/Infiniti corporate has declined goodwill assistance despite the existence of TSBs acknowledging the issue.Read more

My vehicle lost reverse. It can not reverse what so ever. I was in traffic when it happened thankfully someone where there willing to help. I was told I needed a valve body, after spending 3k plus I was then told the transmission has to be replaced. And its a common thing with these vehicle losing your ability to reverse while driving is very very dangerous.

I was stopped at Starbucks in a small business area. I needed to make a sharp left turn to get out of there to get back on the highway, I would typically go very slow as to not hit any parked cars etc. As soon as I went forward the car took off so fast I put both feet on the brakes. The car usually sets alarms off for everything! I looked for somewhere I could go to each side so as not to go on th

I then hit a cement planted area for bushes in the parking lot. It was about 6" high. My car hit two sides of the planter. That is the point it stopped. Still no warning lights or alarms! The Insurance reported the car is totaled.Read more

Back on October 10th, 2025, I took the software update related to the A7C recall. On October 11th, my vehicle became totally bricked and undriveable due to this update. I had it towed to a dealership where they supposedly "fixed" it by rolling the software back. However, my recall letter stated that this was not a permanent fix and there currently is none. It has been 4 months since this began and

Component or system that malfunctioned: The braking system on my 2022 Toyota Highlander Hybrid, specifically the transition between regenerative braking and hydraulic friction braking, appears to malfunction intermittently. During braking, the vehicle sometimes lurches or surges forward instead of slowing smoothly, especially at low speeds or when braking downhill. The vehicle is available for ins
... pection upon request. How safety was put at risk: The unexpected forward surge while braking increases stopping distance and creates uncertainty about braking performance, particularly in traffic or parking situations. This creates a risk of rear-end or low-speed collisions and reduces my confidence in the braking system. Problem reproduced or confirmed by dealer or service center: The vehicle was inspected by a Toyota dealership, and a Technical Service Bulletin (TSB) software update intended to address braking behavior was performed at approximately 50,000 miles. However, the braking problem continues unchanged after the repair. I had to pay the dealer to perform this TSB update. The dealer was initially reluctant to apply the TSB and did not associate my braking complaint with the known issue. I had to research the TSB myself and request that it be applied. The invoice notes that the dealer reproduced the issue. Vehicle or component inspected by manufacturer or others: The vehicle has been inspected by a Toyota dealership service department. It has not been inspected by police or insurance representatives, as no accident has occurred. Warning lamps or prior symptoms: No warning lights or messages appear when the problem occurs. The issue happens intermittently without warning and has occurred since the vehicle was new. The condition appears to be getting progressively worse. The issue seems more likely to occur when braking while the hybrid battery is near or at full charge, suggesting a possible problem in the transition between regenerative and friction braking.Read more

When running the vehicle in electric mode (and sometimes hybrid), there seems to be an issue with acceleration. When pressing the pedal, the vehicle begins to accelerate until it reaches around 50% power. Once that happens, it either barely accelerates or doesn't at all - even if you are pushing the gas pedal completely to the floor. After a short period of time, the engine kicks on to compensate,
... but there is of course a delay there and many times causes the vehicle to lurch forward. After taking my vehicle to the dealership twice for this, I was told that although Stellantis is keeping quiet about this, during the 68C recall software flash fix Stellantis installed something into the vehicles to essentially throttle the battery's power. They claim this is to avoid the battery from getting too hot. I was never notified that they would be completely altering the functionality of my vehicle and how it drives - which is inappropriate - and I believe this also poses a safety risk as the loss of propulsion/delay in acceleration and the lurching of the vehicle could cause an accident you're pulling out onto a road or are in traffic. Stellantis is claiming nothing was done to the vehicle and my dealership is now claiming it is functioning as intended, which makes sense because if they DID make this change, it is technically running how they want it to. However, I have had this vehicle for nearly 2 years and it NEVER drove like this before. I have also seen numerous posts and comments from other owners on the NHTSA website, Reddit, Facebook, and various Jeep forums mentioning they are having the EXACT same issues I am having, so I know it is not just my vehicle. This, along with the other recalls that STILL have no resolution is completely unacceptable and Stellantis would rather gaslight us into believing our vehicles are fine instead of owning up to the truth - that they are not.Read more

I am reporting a persistent safety defect involving loss of intended drivability in my 2017 Ram 1500 with an 8-speed automatic transmission. For the past 3 years, on almost every use of the vehicle, the transmission enters an abnormal operating state where it becomes stuck in a single gear (most often 3rd or 4th) and will not upshift or downshift. When this occurs, the vehicle experiences reduced
... ability to accelerate and loss of normal drivability. The condition does not resolve while driving and requires me to pull over, shut the vehicle off, and restart it to restore operation. This defect has occurred multiple times at freeway speeds, requiring me to pull over onto the highway shoulder due to loss of vehicle function. A vehicle that cannot reliably shift or accelerate at highway speed presents a clear safety hazard to the driver and surrounding traffic. Requiring a stop-and-restart is not a safe or reasonable mitigation strategy in real-world traffic conditions. The issue occurs more frequently when the vehicle is cold, indicating a predictable and repeatable failure mode, not driver error or misuse. I have had to alter normal driving behavior, including extended warm-up periods and avoiding highways, to reduce risk. I was informed by repair facilities that this is a known issue related to transmission control module (TCM) software, and that Chrysler has issued Technical Service Bulletin(s) describing TCM software updates to address abnormal shifting and drivability behavior. The existence of these TSBs demonstrates manufacturer knowledge of the failure mode and a defined corrective action, yet the issue has not been addressed as a safety recall. I am submitting this complaint so the issue can be evaluated based on objective safety criteria. A vehicle that routinely loses drivability and requires emergency stops on freeways warrants formal safety investigation.Read more

I bought the car in November 2025. I was on a highway, going at a comfortable, 65 mph speed (a few weeks after purchasing this happened the first time ). Suddenly, the car stopped accelerating, of course I panicked, slowly pulled off to the shoulder, and shut the car off. Luckily, it was not rush hour . I restarted the car, and it was normal driving. I texted my salesperson and asked if he had he

A few weeks later, it happened again, this time it stayed at around 36 mph. I pulled over and shut the car off, restarted, and no issues. This time I told my salesperson I wanted to bring it in, but they had a one-month wait and offered no help, didn't seem phased or concerned. I took the appointment for February 17, which was approximately a 4-week wait. Again, no lights or warning signals. The third time this happened I was merging on to a very busy ramp to enter a busy highway, and the speed stopped at 11 mph, and I panicked, pulled over restarted the car and that was enough for me to call and insist they take the car as soon as possible, as we were expecting a major snowstorm which did happen the next day. I did not take the car out and brought it in to the dealer on Monday. Again, they seemed unconcerned but took the car after I insisted. The car has been there for 2-weeks now and they claim they cannot find a problem. I called Hyundai corporate. After investigating, they too said there was nothing they could do since they could not find a problem. I may add that while this is happening I am stepping on the gas and nothing happens.Read more

The vehicle has repeatedly experienced a sudden loss of acceleration, entering a reduced-power state while driving. Most recently, this occurred less than one week after I was advised that the issue had been fully resolved through a relay replacement. Critically, these incidents have occurred while my [XXX] child was in the vehicle, creating an unsafe and unacceptable situation. Below is a summ
... ary of the work that was done to resolve the safety concern. October: Drivetrain/Powertrain control unit internal fault Replacement of gateway/powertrain control module Wiring, grounding, and software inspections November: Active Brake Assist, ESP, and voltage supply faults Failure of both rear wheel speed sensors Replacement of left and right rear wheel speed sensors December: Dealership identified a faulty relay as the root cause Relay replaced and vehicle returned to service January, the vehicle again lost acceleration. And now, again in February. The same loss in acceleration incident. At this point, the vehicle has demonstrated a pattern of unresolved, safety-critical electrical failures. Given the repeated nature of the issue, the prior replacement of major components, and the involvement of braking and drivetrain systems, I no longer feel confident that additional part replacement without corporate oversight will result in a safe or permanent resolution. I am an automotive engineer. I have been involved with vehicle development programs in the disabled vehicle conversion community for over two decades and have numerous patents in that space. I have been involved with vehicle testing and calibration as well. My vehicle demonstrates a SIGNIFICANT HESITATION when the accelerator is pressed that is not consistent. I live in a roundabout dense l
... ocation where timing an entry into a roundabout is easy using a normal vehicle. The 1 to 2 second hesitation where there is throttle input and no action at all from the vehicle (engine does not rev, no motive power observed) is very dangerous. I took this up with Toyota and my vehicle was checked out and "learning transmission" memory was wiped clean with me being the only driver was the remedy. The issue got better for a short time but is back and this morning I pulled out thinking there was plenty of time in front of a bus a good distance away and the lack of motive power for a full second or two resulted in a near miss. This vehicle has about 20,000 miles and was purchased by me new. There are NO WARNING LAMPS, and no DTCs thrown when this happens. I have seen several videos where people cite the same experience and a website where there is a class action lawsuit in place regarding this issue. 1. What component or system failed or malfunctioned, and is it available for inspection upon request? The failed component is the drive motor inverter cooling system, specifically the drive motor inverter cooler (OEM part number 87821986). This component is responsible for regulating temperature within the high-voltage electric drive system. The vehicle is currently at the dealership with the re
... pair order open, and the part has not yet been replaced due to national backorder status. The vehicle and failed component are available for inspection upon request. 2. How was your safety or the safety of others put at risk? The failure triggered a “High Voltage System Warning” message. This warning indicates a malfunction within the electric propulsion system, which controls vehicle movement and power delivery. A malfunction in the inverter cooling system can lead to: Loss of propulsion Sudden power reduction Vehicle shutdown while driving Potential overheating of high-voltage components Because this is a $75,000 electric performance vehicle with only approximately 5,100 miles and four months of ownership, a critical high-voltage system failure creates a serious safety concern. I do not feel the vehicle is safe to operate, and I have refused to drive it under these conditions. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The issue was diagnosed and confirmed by an authorized Acura dealership service department on January 29, 2026. The dealership determined that the drive motor inverter cooler requires replacement. The repair order remains open because the required OEM part is on national backorder with no confirmed ETA. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The vehicle has been inspected by the authorized Acura dealership service department. 1. Loss of motive power at freeway speeds (drops to ~40 mph) despite >40% battery charge 2. Uncommanded switching between electric and gas propulsion; driver cannot control the transition 3. Unintended acceleration in reverse / tire spin even after releasing accelerator 4. Steering wheel shaking at highway speeds → potential loss of control ***All of these items create a high risk
... of rear-end crash, parking-lot collision, and pedestrian/property hazard This is not “normal hybrid operation.” It is an intermittent propulsion control failure (loss of power + uncommanded acceleration behavior) that materially affects safe operation, so it should be handled as a warrantable defect and a safety defect, regardless of recall status. And because the vehicle cannot reliably maintain electric propulsion and instead experiences unpredictable loss of power and forced transitions to the gasoline engine—even with significant battery charge remaining—it is not functioning as a hybrid/electric vehicle as advertised. I paid an additional premium for hybrid/electric capability, but due to these propulsion-control failures the vehicle cannot be safely or consistently operated in electric mode and therefore is not providing the advertised hybrid/electric functionality.Read more

Since the release of V14, the Full Self-Driving (FSD) system automatically defaults to operating approximately 7 MPH above the posted speed limit. This occurs on all road types, including city streets and highways. When FSD is engaged, the vehicle sets the cruising speed above the detected speed limit without driver input. There is no setting that allows the driver to enforce strict compliance wi
... th the posted limit. Available speed settings only permit equal or greater deviation above the speed limit. The driver cannot manually lock or command the system to remain at or below the posted limit while FSD is active. The only way to prevent speeding is to fully disengage FSD. This is not a malfunction. The vehicle is operating as designed under V14. This creates a safety concern because: 1.The system is programmed to exceed legal speed limits by default. 2.The driver does not retain meaningful authority to ensure lawful operation while automation is engaged. 3.Automatic speeding increases crash risk in construction zones, school zones, and changing speed areas. 4.Drivers may receive citations despite not intentionally selecting an unlawful speed. Because this behavior was introduced via V14 software, it likely affects all Tesla vehicles running FSD V14, not just this vehicle.Read more

On multiple occasions, the transmission does not respond to gear changes such as moving from drive to reverse or park into drive. On those occasions, the D for drive would be blinking. More concerning and very dangerous is a lack of propulsion mid drive, the transmission would not go into gear. The engine would rev high, and it would not move forward, except by whatever momentum it originally had.
... This has happened to me on at least four occasions two of which I was in the left lane of three and four lane wide streets with cars passing me at high speed on the right. I needed to pull over turn the car off and then on again, which is the only way that the transmission would start working again. Sometimes the transmission would be jerky, especially after those incidents. crossing three and four lanes while people are going 50 and 60 miles an hour is a very dangerous situation when you have no power. To Whom It May Concern, I am submitting this complaint to report serious and ongoing safety concerns regarding my Mazda CX-90. I believe this vehicle presents a significant safety risk to occupants and other drivers on the road. Since the date of purchase, this vehicle has been subject to numerous recalls, repeated software updates, and multiple mechanical issues. The number of recalls alone rai
... ses concern about the safety and reliability of this model. In addition, the vehicle has been in service many times, and I have frequently been provided a loaner vehicle due to unresolved problems. On the day of purchase, the vehicle suffered a mechanical failure and had to be towed. While driving in my neighborhood with my children inside, the vehicle suddenly stopped moving and became stuck in reverse. A brand-new vehicle should not experience this type of failure. More recently, I experienced a dangerous steering event on the highway. While traveling in the left lane, the vehicle suddenly swerved into the right lane, then back to the left, and continued oscillating between lanes. The steering felt momentarily out of my control. If surrounding traffic had been heavier, this could have resulted in a serious collision. After inspection, the service department stated that no issue was found and offered no corrective action. Additional safety-related concerns include: The vehicle has unexpectedly stopped driving while in motion on multiple occasions. The steering system behaves unpredictably at times. The hybrid system has malfunctioned repeatedly, the vehicle has overheated and emitted a strong smoke odor. These repeated failures involving steering, powertrain, and hybrid systems raise serious safety concerns.Given the pattern of recalls, ongoing mechanical failures, and the inability of the service to diagnose or correct these issues, I believe this vehicle poses an ongoing safety hazard I respectfully request that NHTSA formally investigate these issues KimRead more

Cruise Control Significant safety. 2/13/2026. The first time I experienced my car made sudden high-speed acceleration, brain starts scrambling, what's going on? Ok cruise controls on, turned it off, tapped brake. Now with Cruise Control Off, I had to keep in contact with the accelerator to manually maintain speed. I had to pay much attention to my speed. Was towing a very small travel trailer, abo
... ut 750#. The above actions stopped the sudden acceleration. Got my speed back under control. Thought since the problem was resolved quickly along with me disbelieving what just happened, I turned Cruise Control back ON. So, I got my speed back to where it should be and put Cruise Control ON. And, again it didn't happen right away, but when it did, it was exactly the same sudden acceleration. Vehicle to a Honda dealer next week. No warning signs or sounds, either before or after the incident, both occurrences.Read more

The system that failed was the acceleration. This how it happened the first time, because after that it has happened at least two more in one year. I was leaving my office parking area, and I was waiting for the cars in front of me to continue moving. Then I was ready to start moving and put my foot on the accelerator and just went to a high acceleration. Which I immediately slammed the brakes bec
... ause otherwise I would have smashed the rear part of the vehicle in front of me. My safety was in jeopardy because if I would have hit the car in front of me and my airbags would have deployed. And the speed of the acceleration was so, that the driver in front of me would have definitely felt it in his neck and God forbids what else!!! Yes, I reported to my dealer, and they said that nothing showed in their computer system, and mentioned something of a throttler not been wrong, (which I don't know what that is). There were no warning lamps or messages in the dashboard prior to the failure. This happened to me again in random occasions where I was just getting my car out of my parking in my condominium, also lucky, it was early in the morning and no cars around. AND the other instance also in my condominium parking area, but this time arriving, and also, lucky no cars near!!!! Your website describes a similar complaint for the same make and model of my car, thus implying that there is a problem with the system.Read more

Each time the repairs occur, I end up renting a car and the time it sits at the dealer or repair shop has varied from 45 days to several days. This car needs to go. Car took off after going over railroad track, and accelerated without driver being able to control it. The breaks and steering went out as well. It’s like car was being driven remotely. ... r 8 months having this car. First major incident was December 2024, but rocking and began one month after purchasing vehicle in August of 2024. Acceleration incident; December 2024: 4 months after purchase; vehicle stopping in traffic: April 2025 8 months after purchase. Apparently new tires were put on. Almost had a blow out within 1 year and 4 months of purchase and found out when getting oil changed this past December 2025. Lights and radio randomly shut off, and gas’s tank is often locked or will not close. This car is scary, and has had problems that someone has covered up for a very long time. I have papers claiming everything was checked upon delivery; However; two incidences alone could have ended in multiple fatalities. Someone is responsible for hiding something something here, and this needs to be reported before someone or multiple people die that is due to a defect in the car. My 2018 Ram 2500 has an active safety recall for the ABS/EBS system that has been open for over two years with no remedy or parts available. The issue was first noticed in August of 2023. I contacted CDJR dealership on how to repair this issue. This is a serious safety defect that directly affects braking stability, trailer brake communication, and emergency handling. Because the ABS module and
... electronic brake system are malfunctioning, the truck cannot safely perform the heavy towing it was purchased for. When towing a large trailer, the trailer’s electronic braking system does not communicate with the truck, forcing the truck to absorb the full braking load. This has resulted in unstable braking, increased stopping distance, and unsafe emergency handling at highway speeds. The ABS fault also disables cruise control, which significantly increases driver fatigue on long-distance trips. I regularly travel 1,500 miles each way to Florida, and without cruise control I must maintain throttle manually for hours at a time, requiring more frequent stops and reducing alertness. This defect has caused premature brake wear due to the trailer brakes not engaging properly. I was forced to replace the rear rotors and pads at only 32,000 miles, costing me $2,700. This is not normal for a heavy-duty truck and is directly related to the ABS/EBS failure. The unresolved recall has also severely reduced the resale and trade-in value of the vehicle. Dealers discount or refuse vehicles with open safety recalls and no available remedy. I am also paying $138 per month for a Mopar extended warranty that I cannot use to repair this defect because the manufacturer has not supplied the necessary parts for over two years. This is an unreasonable delay for a safety-critical recall. The vehicle is unsafe for its intended purpose, creates financial harm, and exposes me to ongoing risk every time I tow. I am requesting NHTSA intervention to compel Stellantis to provide a timely remedy.Read more

The truck has approximately 5,000 miles. When adaptive cruise control is engaged, the vehicle accelerates on its own to 85 mph whenever the posted speed limit changes, and there are no available settings to prevent this. For example, if the speed limit is 55 mph and adaptive cruise control is set to 55 mph, when the limit changes to 65 mph the truck automatically increases speed to 85 mph without
