The contact owns a 2015 Tesla Model S. The contact stated that while he, his wife, and his son were in the vehicle he had begun veering right pulling into the driveway, at approximately 5 MPH, he had begun slightly releasing the brakes when the vehicle suddenly accelerated at accelerated high speeds. ... e. As a response, the contact attempted to maneuver the vehicle to avoid crashing into the garage. The contact crashed into a nearby tree, jumped the curb, and became airborne crashing into the side of the home. The driver's side of the vehicle crashed into the side of the home where the laundry room was located ricocheting hitting the kitchen and bedroom windows and eventually coming to a stop. Due to the home being on a slope, the vehicle ended up falling into a nearby embankment after coming to a stop. All air bags deployed. The vehicle was towed. The contact was informed by insurance the vehicle would more than likely be totaled however this had not been confirmed. A police report was filed. The contact received emotional distress, cuts, and bruising to their hands. arms and forehead. The contact also suffered from back pain. The contact's wife sat in the rear on the driver's side and received emotional distress, trouble breathing, bruising on their face, and bleeding in the mouth. Their wife also received neck, back, and head pain. The contact stated that their wife ended up falling to the floor of the vehicle as a result of the crash. The contact's son sat in the rear on the passenger's side and received emotional distress, and back and neck pain. All persons received medical attention on-site, such as cleaning the wounds and icing any bruising. They were advised to seek additional care, however, have not been able to do so as of yet. I have called RAM cares who is nothing more the a shell of the company to appear as though they care but of are little to no help. So recap, I have a ($60,000) truck that is the equivalent of a base model s10 ($15,000)(basic model truck) because one part has made it impossible to use the truck in its fully designed capacity. No four wheel drive, no cruise control, and a dinging and flashing light that happens every 2 seconds and a message that say service electronic braking system. I have been driving this car for 11 years and never ran out of gas until running out 3 times in the past 2 months. A complaint regarding this concern was provided to NTSA and I have not had a response. The car has subsequently repeatedly moved backward when in Drive. I also have videos of these occurrences. I returned the car to the service center for these behaviors and was told that there was nothing that could be done. The service center said that I "canceled" my request for servicing, and noted that in the Tesla service files. I did not cancel my appointment. They refused to examine and service the car. We recently experienced the car steering wheel locking and inableing the wheel to be turned right, during a 3 point parking exit. I took the car in and was told that the problem was a "firmware" update problem. The problem of driving in reverse when the car is placed in Drive has repeated itself in the past 3 weeks, and I again have videos of this occurrence. The contact sustained soreness on the neck and the buttocks, and a swollen left knee and left ankle. The contact's daughter sustained a laceration on the neck and neck pain. Both the contact and her daughter received medical assistance. The contact was unsure whether the other driver sustained any injuries. The vehicle was towed to a tow lot, where it was totaled. The issue has since been isolated as a coolant leak in my cylinders and Ford is very aware of the issue and has a TSB 22-2229 discussing some of the symptoms. The failure of the vehicle to meet the power requirements asked of it to merge and the sudden loss of power nearly left my vehicle rear ended with both my toddler and my teenager present. The check engine light illuminated showing a code of P0301 for a cylinder 1 miss-fire. All 4 spark plugs were replaced in an attempt to fix the issue. The check engine light came on again within 48 hours after the repair was made. During the next inspection the coolant level was noted as low. Coolant was added at that time and cylinder 1 coil pack was replaced to fix the issue. The check engine light came on again and coolant was once again low within 48 hours of the 2nd repair. Ford’s service bulletin TSB 22-2229 addressing the known coolant intrusion issue for the 2015-mid 2019 Ford Edge was presented as the needed repair. This engine failure poses a risk for the car owner, occupants, and surrounding bystanders. It has a fire risk, power failure risk, and quite possible a loss of life risk. ... e vehicle in for service of this issue because my wife had experienced this while driving on Interstate 95, in Maryland. The vehicle stopped responding to the accelerator, so she pulled onto the shoulder, put the vehicle in park and turned it off. When she turned it back on, it was back in automatic mode, and she was able to resume driving. The technician and service manager advised that when this occurs, the vehicle can be returned to automatic mode by pulling the right paddle forward. That information does not appear in the vehicle manual, or on the Jeep website. The technician and service manager also advised that there is no way to disable the paddles to prevent the problem from repeating, Other drivers have experienced this. See [XXX] . I believe it is a serious safety problem. To that point, there is a pending safety recall -- NHTSA 23V- 412 (copy attached) -- that addresses loss of motive power for another reason. Here is the description in the recall notice: The transmission control module and the hybrid control processor on your vehicle [1] may experience loss of communication caused by diagnostic reactions to faults resulting in an engine shut down condition. Engine shut down may result in an unexpected loss of motive power while driving, which can cause a vehicle crash without prior warning. The sudden enabling of the paddle shifter and resulting loss of motive power is also a safety issue that needs prompt attention. I had the vehicle back to the dealer numerous times before they replaced this item. 14000 miles later the same problems are happening again. I was on the way home from work when out of no where the engine light and a indicator saying low engine oil pressure came on I totally lost power driving on a highway and the car came to a stop. Mechanic told me it was the oil belt tensioner caused the engine failure and now i have to pay 4000 dollars on top of my monthly car and insurance company for a vehicle that just quite on me.

Yesterday traveling to Katy, it happened again but only this time the vehicle didn't shift back into gear for a longer period of time and again almost causing us to get rear ended. I have taken my vehicle to Ford only to tell me that they test drive it and nothing happened so they were not able to fix anything. I have read several complaints about this issue with the 10 speed transmission but nothing is being done about it. This is supposed to be a safe family vehicle on the road but instead it's a DEATH trap waiting to happen. I was headed straight but stopped so that I could move into a left turn lane. As I got into the left turn lane and was straightening up behind the person in front of me at almost a dead stop the R1S lunged forward and accelerated at a high rate of speed hitting the car in front of me (speed control issue already known on the R1T). Of course now all my side sensors don’t work. I am still waiting on diagnostics from Rivian. Additionally, my seatbelt didn’t lock throwing me forward. I have only had this SUV two months with 848 miles. The car in front of me had substantial damage. If anyone had been in her backseat especially a child they would have been injured. My safety was in danger with the seatbelt not locking. I am waiting on the diagnostics still from Rivian. Insurance is waiting on Rivian.
