This is my 10th report; it relates to 2 issues I have reported before 1. The "phantom breaking" on cruise control that I have reported a few times previously has apparently been fixed on a software update, but we were not notified. ... entioned my past problems, including the phantom breaking. The man was dismissive and a bit defensive and asked if I'd made an appointment. I said I'd been told several times they hadn't figured out a software fix (and even Tesla employees were simply refraining from using cruise control, which is what I did). He said they "fixed it a while ago" and he's not had complaints since then. I tried cruise control on my 45 minute freeway drive and indeed, the problem did not occur. Interesting that they knew it was an ongoing problem, didn't deal with it, and didn't announce when they finally did fix it. I do not yet know if it is an ongoing safety problem for me but I'm planning to gradually increase my use of cruise control, since most of my driving is high-speed freeway driving. 2. I asked about getting warranty rebate for my original tires that have failed at 38K miles. He said I would have to pursue that through the distributor (American Tire?) and that he'd only had 2-3 customers ever do that. He said most customers just pay for better tires. He offered to sell me another Continental tire in the $400 range and implied that I could find tires through a tire store. I went to Discount tire who can get me a rebate on only 1 of the 4 tires because it's down to 4/32" tread. The other 3 can't be warrantied until they reach 4/32"...and I've already hydroplaned/slid on the freeway in a sleet storm with wet roads. The company knowingly provides inadequate tires and does not stand behind them. This is an unacceptable safety risk and a moral outrage.Read more

I am writing to file a formal safety complaint against Tesla, Inc. regarding a software update failure that has disabled legally required vehicle functions on my Tesla for over two years. For more than 26 months, my vehicle has been unable to complete a software update through Tesla's over-the-air (OTA) update system. As a direct result of this failure, my vehicle's display screen does not functi

This prevents me from accessing my speedometer, turn signals, and other legally required safety features, rendering the vehicle non-compliant with federal and state motor vehicle safety standards during this period. I have made multiple documented attempts to resolve this issue through Tesla's customer support channels over the course of these two years. Despite these efforts, the issue remains unresolved. Tesla has now informed me that resolving the defect will require a paid service visit — despite the fact that the failure originates within Tesla's own software update infrastructure, not from any action or negligence on my part. I am requesting that NHTSA investigate this matter on the following grounds: 1. The software defect disables legally required vehicle safety functions, including the speedometer, turn signals, hazard lights, and more. 2. The defect has persisted for over two years despite repeated good-faith efforts by the vehicle owner to resolve it through the manufacturer. 3. Tesla's OTA update system is manufacturer-controlled infrastructure. A failure within that system constitutes a manufacturer defect, not a maintenance issue. 4. Tesla is requiring payment from the consumer to correct a defect in their own systems. I believe this issue may not be isolated to my vehicle and could affect other Tesla owners experiencing similar OTA update failures. I contacted the dealership who directed me to Subaru of America to report the incident. Their customer advocacy and quality assurance department reviewed telematics data and have refused to physically inspect my vehicle even though I have requested it as a customer because I do not feel safe driving this vehicle. Last Friday, I was driving with my family on Interstate 95 when a tractor trailer truck did not see me beside it and started to switch into my lane. I tried to quickly accelerate to get out of the way before it hit us but my car went into "turtle mode" and the speed suddenly dropped by 15 or 20mph and would no longer drive correctly. Luckily, I was in the right lane and able to swerve onto the shoulder of the highway and narrowly avoid the large truck. Otherwise, if I had been in a middle lane, I would have either been hit by the truck or likely hit another car next to me. This issue has been sporadically occurring with my car for about 5 months now. I have had it into the Mercedes dealer three times and, each time, they say they think they fixed the issue. But, each time the same issue has returned. They said that Mercedes USA knows it is a big issue with their new hybrid SUVs but Mercedes doesn't yet know how to fix the problem and has been unwilling to publicly acknowledge the problem or offer it's customers any assistance. Doing a quick Google search online for "Mercedes hybrid performance extremely limited" also quickly brings up different online forums where other owners of this same car have had the exact same issue that Mercedes has been unable to fix or resolve. While driving the truck will shift into 1st gear. It will also shift back and forth between gears randomly. When this happens, the wheels may lock up or just skid. At the end of a downhill cloverleaf coming coming on and off a highway, the car slowed to a crawl but would not stop. It was going slowly as if "creeping" like when in B mode but was not in B mode. The brakes would not stop it. I was able to shift gears from Drive to N to P but no response, car continued to creep forward until it gently bumped into the back of an 18 wheeler. I submitted my claim to Volvo, esp since there are similar recalls so I thought it was a known thing, but apparently not reported on my exact make and model (there were not very many made). I did not know there was damage to my vehicle, and the truck driver and myself did not even get out, he didn't feel me, and I assumed the bumper just bumped him so no police report. This was an intermittant failure and after I re booted the vehicle I slowly drove home after brakes seemed to be working after power on/off. I did not know that it was my HOOD that bumped the truck. I did not know there was damage until I got home. If this car's make and model were NOT included in the recall, they should be. TERRIFYING. It first happened around 1 year (early 2026) after the new car purchase (Nov 2025). There are no warning lights or messages. The dealer claimed they were unable to reproduce the issue during a brief test drive without me present and incorrectly attributed it to a bulletin without performing a detailed diagnosis or repair or looking at the video we recorded on the issue. The issue persists and affects safe operation of the vehicle. No inspection has been done by police or insurance. In one instance, this occurred while other vehicles were approaching from behind, creating a dangerous situation where I could not accelerate to match traffic flow. This resulted in a high risk of being rear-ended due to the unexpected loss of acceleration. I am aware of other consumer reports describing similar transmission-related symptoms such as slipping, hard shifting, and hesitation in this model. I have requested that the dealership further review and escalate the diagnosis to determine whether the issue aligns with any known concerns, Technical Service Bulletins, or manufacturer-supported programs. The vehicle and any related components are available for inspection upon request. At this time, the vehicle experiences reduced engine performance, including stalling, and inability to accelerator as needed, which pose a safety risk while driving, particularly in traffic or at highway speeds. The vehicle has been inspected and confirmed by the dealership. I have also contacted the manufacturer, who advised me to work directly with the dealership to have the issue reviewed and potentially escalated. I have since requested that they further evaluate the condition and determine whether it corresponds to any known issues or manufacturer-supported repair programs. At this time, the vehicle has not been inspected by police, insurance representatives, or other third parties. This check engine light and fluctuating RPM directly impacting the vehicles acceleration was the primary indicator of the issue. The manufacturer previously issued two Customer Satisfaction Program notices to address this issue for other model year vehicles. I have attached both notices to this complaint. This demonstrates a manufacturer defect which is a safety hazard for drivers.Read more

The truck tires had seized up causing the driver to drag the truck into the flatbed. The emergency brake kept resetting itself. The following day 7/13 Chevrolet had the truck towed to the dealership. On Monday 7/16 we were informed the engine had catastrophic failure with less than 24k miles. This is the second major engine breakdown as the first was at 5k miles where one side of the engine had collapsed lifters. Engines are on back order but in reviewing other experiences the replacements experience the same issues. The truck is not a safe vehicle to drive.Read more

I was traveling in the number two lane and my speed was approximately 78 MPH when all of a sudden I had a loss acceleration power and my Kia EV9 began to decelerate rapidly and my dashboard began to flash a loss of power warning. ... the rapid deceleration I had to take evasive action as northbound highway traffic was flowing smoothly. My clearest path was to move to the lefthand shoulder, which required me to cross the number one lane and the carpool lane. During this time, my Kia EV9 continued to decelerate. Once I got my vehicle onto the shoulder I placed my vehicle in park at which time my dashboard highlighted the front storage compartment of my vehicle in red. I assumed that meant my frunk was open. I exited my vehicle and checked my frunk and indeed found that my frunk was open and the first stage of the two-stage latch system was disengaged. Had the second stage of the latch system been disengaged it could have resulted in catastrophic consequences due to me traveling at highway speeds on a multilane highway. To be clear, I did not touch the interior compartment frunk release button and my keys were sitting in my left cargo short pocket located on the side of my left leg. I had also been traveling for approximately five to seven minutes and had entered the highway via the northbound [XXX] . I came to a stop at approximately the [XXX] area, which is approximately two miles north of [XXX] . I am aware that electric model Kia vehicles have a safety measure in which if the frunk or trunk is open the vehicle computer system does not allow the vehicle to engage in drive or reverse mode. FAILURE INCIDENT: On March 15, 2026, at low speed (under 25 mph) on a dry surface, my Lyriq failed to activate its Automatic Emergency Braking system while approaching a stationary object. The vehicle did not decelerate or stop as designed. The ABS system appeared to release braking pressure rather than maintain it, allowing the vehicle to continue forward and strike the object. The front grille assembly sustained significant physical damage as a direct result. This failure is identical in nature to the defect described in Recall 24V589 — specifically, the ABS falsely activating and releasing braking pressure during low-speed operation on a dry surface — despite the recall remedy having been applied and certified complete ten months prior. SAFETY CONCERN: The software remedy applied under Recall 24V589 did not correct the defect on my vehicle. This raises serious concerns about the effectiveness of the EBCM software update as a remedy across all affected vehicles. If other vehicles received the same software update and the defect persists, there is a significant ongoing safety risk to the public. Diagnostic trouble codes were present related to the hybrid system and high-voltage battery, including P1D43, P1A21, U029A, and U0412. These codes indicate faults in the hybrid control system and battery communication. A Chrysler service department inspected the vehicle, confirmed the trouble codes, and returned it to me without repair because the issue was not immediately reproducible at the dealership. The vehicle has a history of similar intermittent issues, including prior loss-of-power events and repeated service visits for hybrid system faults. The condition is not consistently reproducible, but when it occurs it results in a sudden loss of motive power while driving. This creates a significant safety risk due to unexpected loss of propulsion in active traffic.Read more

Unfortunately I'm not sure if I'm going to be able to get a Ford dealership to do anything about it since there isn't an official recall, and I'm afraid to keep driving it as this is likely going to keep happening - but fixing it myself is going to be expensive I think. It is available for inspection upon request. I felt that my safety was and continues to be at risk - downshifting into 1st gear, or whatever that was, while driving high speeds is dangerous and could potentially significantly damage my vehicle and myself. Battery charging system cannot be used on fast chargers without potential overheating and fire, therefore, I cannot travel safely beyond my immediate area without incurring a risk to myself and others. The recall has been ongoing with empty promises of a fix since September of 2024, over 1 year and 6 months ago. So, that means this risk has been in place for much longer.
