Within minutes of leaving the dealership, the vehicle stalled again in active traffic and had to be returned. The issue was escalated to the Ford Engineering hotline and a Field Service Engineer. February 17, 2026 – The dealership attempted to return the vehicle after performing a BECM reprogramming. Upon pickup, new issues were immediately observed including near-stall conditions and hybrid system malfunction, so the vehicle was left with the dealership again. February 18, 2026 - The owner brought forth concerns about Ford’s Technical Service Bulletin (TSB) 17-0015 which identifies potential damage to the traction battery interlock harness as a root cause for intermittent stalling and "Stop Safely Now" warnings. Although the dealership recorded Diagnostic Trouble Code B11D8—a fault directly related to the stall condition—the dealership later stated that no diagnostic codes relevant to the TSB were present and this component was not inspected. I was driving late night to a campsite in a very rural part of the Big Island of [XXX] , about 45 minutes from where I live. I was by myself except for my dog, in the passenger seat. I was on a country road with a 30 mph speed limit, going about 25 mph. I engaged the cruise control and it immediately went up to 40-45 mph, far faster than the limit and far faster than was safe given the wet condi

As I tried to slow the car down, using the steering while cruise control adjustment buttons, I steered through a curve and rather than turn with the road, as I had intended, the truck slid off the road to the left, effectively hydroplaning, into two trees, at about 40 mph. The airbags deployed as the truck came to a complete halt against the two trees. The car automatically called 911 as I sat there trying to assess what had just happened, whether I was hurt, and whether my dog was hurt. I told the 911 operator that the truck had called them automatically adn that I was ok and I would call in the morning after assessing what had happened. I am waiting for the police report and also waiting for the incident report from Rivian, which has an on-board log of events. I don't know why the cruise control engaged at such a high speed, rather than at the posted speed limit, but this appears to be a known flaw with Rivian's cruise control that I was not aware of. The cruise control and software are available for inspection by NHTSA. There is also an active recall on this vehicle that I was aware of but had been unable to schedule for service yet b/c it required shipping the truck to [XXX] on a barge. I had only had the truck on island for a little more than a month. From previous operations I had no reason to believe that the cruise control was faulty. It is possible that the recall issue may have been a contributing factor to the crash but I don't know b/c the vehicle has not been inspected yet by Rivian. A Ford dealership has diagnosed the issue as a failure of the transmission lead frame connector. Ford Motor Company previously issued a Customer Satisfaction Program extending warranty coverage for this specific defect. However, that coverage was limited by time rather than mileage or actual condition of the vehicle. Although my vehicle is outside the time-based coverage window, the extremely low mileage clearly indicates that this is not a normal wear-and-tear issue. Instead, it represents a premature failure of a known defective component. This defect presents a significant safety concern. Despite braking, the vehicle continued forward and struck the exterior of the building. I was able to avoid the window area where the children were standing. The airbag deployed, and the front passenger side of the vehicle was damaged. The building was not structurally damaged, and no one was injured. There were no obstructions such as floor mats interfering with the accelerator, and there was no unintended pedal input. My foot was on the brake at the time of the incident. This appears to be a case of sudden unintended acceleration. The issue has not been reproduced or confirmed by a dealer or service center, and there were no prior warning signs before the event. The vehicle is available for inspection upon request. It has been towed to the body shop. The problem has not been reproduced or confirmed by a dealer or independent service center at this time. The van has just approx 55000 miles. While driving on highway, it suddenly lost power and from 60mph it came down to 20 miles in a matter of few seconds after few violent jerks. My safety and the safety of others has been put at risk multiple times because the vehicle stalled at traffic lights in active traffic, leaving me unable to accelerate through an intersection. The problem has been confirmed by Honda Marysville dealership. The dealer stated they could not repair the issue and advised me to modify my driving behavior as a workaround. No repair was performed. The vehicle has been inspected by the selling dealership. A message appears during the failure telling me to restart the car. The problem first appeared within the first year of ownership on a brand new vehicle with basically zero miles at purchase. The loss of all warning indicators eliminated visibility into any safety-critical vehicle alerts that may have been active during the failure periods. The problem was confirmed by the dealership and is the subject of Safety Recall 25TA08 (Combination Meter May Go Blank). The vehicle was brought to a Toyota dealership where the recall was documented and the repair was eventually completed. The replacement part required VIN-specific production and programming by Toyota, resulting in an extended out-of-service period of nearly two months at the dealership, and approximately five months from the first occurrence of the failure. During the out-of-service period, the vehicle could not pass state inspection and was legally inoperable. The component has been replaced and is no longer available for inspection.Read more

Dealer could not replicate. No codes but repair shop had to recalibrate after 1st incident and may have lost code on 1st incident(wreck). Now that insurance knows what happened, I don't know if they will intervene. hopefully. No warnings went off or messages. Airbag did not deploy. I believe that this is an unintentional acceleration, especially occurring twice now. Dealership says car is safe and they will monitor. I guess it means when we have another incident. My vehicle is not currently included in Recall 30B, but the symptoms are consistent with that recall involving instrument cluster failure. I am concerned that this defect may affect additional vehicles not currently covered. I am requesting that NHTSA investigate this issue and consider expanding Recall 30B to include vehicles like mine. 6R80- Unexpected downshifting at highway speeds without driver input causing the vehicle to slow down. Harsh/rough when shifting and sounds like I'm driving on the rudders on the side of the highway. No warning lamps or messages. Occasionally downshifts hard when I'm slowing down and then potentially locking up my rear wheels.
