In the 5 months I have owned this vehicle, the windshield has been chipped at least six time by debris while driving. As I am retired, I do not do a lot of freeway travel and 700 of the 3400 miles were from a trip to visit my mom. I had fewer windshield chips on my previous Honda CRV in 8 years and 90000 miles than I have had with this 2026 CRV. My dealer ignored my request to even look at the iss
... ue, saying glass was not covered by their warranty and when I called Honda USA, they took the information and casually told me to 'have a blessed day'. My first big chip was repaired through my insurance in December 2025 and I will have 5 more chips looked at today by their preferred shop, Safelite. I have taken photos, as best I could, of the chips I discovered while washing the car in the bright sunlight. Having a clear field of vision is so critical to safe driving and the manufacturer should provide a durable windshield for passenger safety.Read more

The issue involves a malfunction of the auto start/stop system in my 2025 Honda Odyssey, where the engine fails to automatically restart after coming to a stop. The vehicle instead requires a manual push‑button start to regain engine function. The vehicle and all related components are available for inspection upon request. This failure created a significant safety risk, as the vehicle became u
... nexpectedly inoperative in active traffic. Losing propulsion at intersections or stop‑and‑go conditions exposes occupants and surrounding drivers to the risk of a rear‑end collision or being stranded in a hazardous location. The problem has not yet been reproduced or confirmed by a dealer or independent service center, but it has occurred during normal operation. The vehicle has not been inspected by the manufacturer, police, insurance representatives, or other third parties regarding this specific failure. There were no warning lamps, messages, or prior symptoms before the failure occurred. The issue first appeared without any advance indication.Read more

A motorcycle fire occurred involving a Royal Enfield Himalayan 450 while the vehicle was parked and not in operation. The specific component that failed is UNKNOWN. However, based on the observed damage pattern, the fire appears to have originated in the rear-mid section of the motorcycle, in the area near the battery, wiring harness, or fuel-related components. The incident presents a significa
... nt safety concern because the fire occurred without active operation of the vehicle. This raises the possibility of an electrical short, battery-related issue, or fuel system malfunction that could lead to spontaneous ignition. If such a condition were to occur in a confined space (such as a garage) or near structures, it could pose a serious risk of property damage, injury, or loss of life. There were no known warning lights, error messages, or prior symptoms observed before the incident. The failure occurred suddenly. The vehicle has not been inspected by the manufacturer. No technical inspection has been conducted to determine the root cause of the fire. A local fire department investigated the incident and determined that the cause of the fire is “undetermined.” No external ignition source or confirmed arson was identified. The damaged motorcycle remains available for inspection upon request. Given the severity of the incident and the lack of a determined cause, further investigation into potential electrical or fuel system defects is warranted to ensure there is no broader safety risk affecting similar vehicles.Read more

The USB port in the center console of my 2022 Honda Accord (VIN: [XXX] ) is physically loose and wobbly, causing intermittent disconnection of Apple CarPlay. The vehicle is available for inspection upon request. When CarPlay disconnects while driving, I lose access to my navigation (GPS), requiring me to manually interact with my phone or the infotainment screen to restore the connection. This cr
... eates a significant distracted driving hazard. The disconnection occurs unpredictably with minor road vibrations or bumps, meaning it frequently happens at highway speeds. Not yet. The vehicle has not yet been brought in for this specific issue. The problem is consistently reproducible — any slight movement of the USB cable causes disconnection. This is a known and widely reported issue No. The vehicle has not yet been inspected by any of these parties regarding this specific defect. No warning lamps illuminate. The only symptom is the CarPlay interface disconnecting on the screen when the vehicle hits a bump or the cable is slightly moved, indicating the USB port itself is physically loose and failing to maintain a stable connection. The issue began and has persisted since I first noticed it. No prior warning was given before the port became loose. The USB port used for wired Apple CarPlay is physically loose and wobbly, causing the connection to drop with the slightest vibration or road bump. I have tried multiple cables and positions without improvement. Losing CarPlay while driving means losing active navigation, forcing dangerous driver distraction to reconnect. This is a known defect reported by numerous 2021 Accord owners. I am requesting this be investigated for a potential recall or technical service bulletin. Vehicle was parked, engine off, and unattended. An internal electrical fire originated in the overhead/sunroof wiring area. Damage includes melted wiring harness near the sunroof, burned center console, and soot throughout the interior. The fire originated in a constant-power electrical circuit and spread downward into the cabin. There were no aftermarket modifications, no external heat sources, a
... nd no objects in the vehicle that could have caused ignition. Ambient temperature conditions were normal and insufficient to cause this damage. This represents a serious safety defect because the fire originated spontaneously within the vehicle’s electrical system and could have resulted in a full vehicle fire or injury if occupants were present. Vehicle has multiple ongoing defects, most critically a malfunctioning occupant detection system causing the alarm to activate randomly, including overnight when the vehicle is empty. This creates a safety concern and repeated public disturbances. The issue has not been resolved despite multiple service attempts. Additional issues include persistent propulsion system fault messages, repeated majo

Infotainment system is extremely slow and unreliable, and both the center display and driver screen reset unexpectedly while driving or parked. Driver-side window intermittently reports as open or cracks open without input. Bluetooth and Apple CarPlay connections are unreliable. Driver profile settings do not consistently adjust seating and mirror positions. Mechanical issues include a glove compartment that does not open, premature wiper failure, and a front wheel center cap that detached while driving. Vehicle has required multiple tows and has been in service for an extended period (approximately one month) without resolution. Catastrophic structural failure of the motorcycle frame. The lower Left Hand Cradle Assembly (Triumph Part # T2071344), which connects the front engine mount to the rear swingarm pivot, sheared completely through. The fracture occurred precisely at the Heat-Affected Zone (HAZ) of the factory weld. This localized metallurgical defect causes a total loss of chassis integrity. A failure of this prim
... ary structural component while at speed would result in an immediate loss of vehicle control, leading to a severe or fatal crash. There were no warning lights or operational symptoms prior to discovering the completely severed steel tube during a visual inspection. The failed component is available for physical inspection. The manufacturer (Triumph) has been notified with high-resolution photographic evidence but refused to investigate the defect or authorize a safety replacement without forcing the owner to pay an out-of-pocket dealership diagnostic fee. This specific weld failure is a documented design flaw on this platform, and the manufacturer actively modified this frame component in subsequent model years, indicating prior knowledge of the structural weakness.Read more

While driving at approximately 70 mph on I-76 , a “CVT HOT” warning appeared on the dashboard. Immediately after, the vehicle lost power and would not accelerate normally, creating a safety hazard in traffic. I was forced to pull over multiple times. This issue began after a recall repair was performed on the vehicle. The condition resolved temporarily after shutting the vehicle off, but the f
... ailure was sudden and dangerous. While driving, multiple warning messages appeared including AWD system errors and check engine light. The vehicle’s performance became unstable and unreliable. This created concern about loss of control and drivability. The vehicle had recently undergone recall service, after which these issues began occurring intermittently. The vehicle was brought to an authorized Nissan dealership multiple for diagnosis of the CVT warning, AWD errors, and loss of power. The dealership kept the vehicle for several hours on multiple occasions but stated they were unable to replicate the issue. No repair was performed any of the times. The safety concern remains unresolved. This continually happens and they yell at me saying there's nothing they could do. To be direct: I am aware that “unable to replicate” is often used to avoid opening warranty claims or authorizing costly repairs. However, this does not resolve the issue, nor does it eliminate the safety risk. The vehicle is demonstrably unsafe when the failure occurs, and the burden should not be shifted onto the customer simply because the issue is intermittent.Read more

What component or system failed: The entire Air Conditioning system, specifically the AC Compressor, Condenser, and Evaporator, failed catastrophically. The authorized Honda dealer diagnosed it as "Black Death," meaning the AC compressor internally shattered, sending metal shrapnel and contaminated oil throughout all AC lines and components. How was your safety put at risk: This failure is a SEVE
... RE SAFETY HAZARD. Because the AC compressor is dead, the windshield defroster/defogger system no longer functions. During rain or changes in temperature/humidity, the inside of the windshield fogs up instantly and cannot be cleared. This causes a sudden and complete loss of driver visibility, creating a high risk of a fatal crash on the highway. Has the problem been reproduced or confirmed: Yes. The vehicle was inspected by an authorized Honda dealership. The technician confirmed the catastrophic failure ("Black Death") and stated the entire system needs to be replaced. Previous history & Manufacturer response: Honda has known defects with the 2016 Civic AC system (Warranty Extensions 19-091 & 23-039). The condenser was previously replaced under warranty in 2023. Now, the defective compressor shaft seal has led to the complete destruction of the system. Despite this being a known defect that causes secondary damage to the entire system, Honda Corporate has refused to cover the repair costs for the collateral damage (evaporator and lines). The parts are currently on the vehicle and available for inspection. Were there warning lamps: There were no warning lights on the dashboard prior to the failure. The only symptom was the AC suddenly blowing hot air and the immediate loss of the defogging capability.Read more

Made appointment in March 2026 due to a planned trip to [XXX] mid-February to mid-March. Failed transmission poses critical safety risks including loss of acceleration, stalling in traffic and unexpected deceleration. Subaru of New England was able to install the transmission at about 50% of the initial estimate over $12,000 (final paid $5400). Subaru of America file was claimed (Case # XXX-XXX) for the balance, but any further reimbursement was rejected. Incident Date: 04/05/2026 Vehicle Speed: 65–70 MPH Location: [XXX] Component: Engine / Power Train / Fuel System Summary of Safety Defect: While traveling at highway speeds (65–70 MPH) with four passengers (including two children), the vehicle suffered a sudden, catastrophic engine failure without any prior warning or symptomatic noise. The Malfunction Indicator Light (MIL) began flashing r

Hazardous Conditions encountered: Upon the loss of power, I observed white smoke trailing from the exhaust and a strong burning odor entering the cabin from the engine compartment. To avoid a high-speed rear-end collision, I was forced to perform an emergency maneuver across active lanes of traffic to reach the shoulder. Due to the immediate fear of a vehicle fire (indicated by the smoke and burning smell), my family was forced to evacuate the vehicle and wait outside in a remote location for six hours. Correlation to Known Defects: This incident appears to be a direct manifestation of the defect described in Nissan Recall ID R25E3/R25E4 / NHTSA ID 26V0801 regarding Engine Main Bearing Seizure. The failure resulted in a complete loss of propulsion and a potential fire risk due to fluid ignition or a breached engine block. Post-Incident Status: The vehicle was towed to Nissan of Cumming and is currently awaiting a formal inspection for internal metal debris. Subject: Vehicle Fire Following ABS Recall Repair – Safety Complaint I am filing a safety complaint regarding a vehicle fire that occurred shortly after a recall repair was performed. Vehicle: Jeep (Grand Cherokee/2019) Mileage at time of incident: Approximate: 58,350 Approximately four weeks ago, my vehicle underwent a recall repair related to the ABS system at Westgate Jeep located in Rale
... igh, NC 27617. Following that repair, I experienced brake issues due to air in the brake lines, which required a return visit to the same service provider for correction. On April 5, 2026, the vehicle caught fire while parked in my driveway. The fire originated in the front driver-side engine compartment area, where the ABS and brake components are located. The fire caused extensive damage to the engine bay, wiring, and surrounding components, rendering the vehicle inoperable. Given the timing and location of the fire, I am concerned that the recall repair or subsequent brake system work was performed improperly leading to either a brake fluid leak or an electrical malfunction or defective parts that caused the fire. This incident presents a serious safety risk, as it could have resulted in injury or loss of life. I am requesting a full investigation into: - The quality and safety of the recall repair performed - Whether improper installation, damaged components, or fluid leaks contributed to the fire - Whether other vehicles repaired under this recall may be at similar risk The vehicle has not been altered or repaired since the fire and is available for inspection. Supporting documentation (repair invoices, photos, etc.) can be provided upon request. I respectfully request that this matter be reviewed urgently to prevent potential harm to other drivers.Read more
