2017 Ford Escape 65,000 miles,1.5l coolant intrusion.#2 cylinder misfire code. First thing upon raising the hood noticed and acknowledged there was no coolant in resevoir. Attempting to replace spark plugs. Removed/replaced #4 plug. Attempting to replace #3 spark plug, plug broke off. Researching and collaborating with reputable mechanic concluded that short block coolant intrusion has occurred.
... Upon disassembly have encountered turbo boost and catalytic converter full of oil, also needing replaced. Head has been sent to machine shop for inspection and repair. Presently awaiting for repairs to be completed and costs established. Present estimate over $5000. I paid cash for this vehicle in 2019. Vehicle had been leased for 2 years previous. Vehicle had 20,000 miles on it. Ford was well aware of the coolant intrusion (bad engineering) is before they sold me the vehicle in November 2019. They never mentioned or offered any warranty specific to this issue. They sold me a a vehicle knowing that if enough things went wrong while driving this vehicle, catastrophic situation was possible. They should have corrected this issue before they sold it to the next customer.Read more

Fuel tanks shields have rotted. I have researched this and it appears that the reason this problem occurs is because of a process called galvanic corrosion. This vehicle has died 4 times in one week. when this happens, multiple lights have flashed in the dash board, including the "eye sight" warning. Then, it would not restart but continued to "turn over" when the ignition button was pushed. One instance was on a busy street at a stop light, another at a busy intersection in the inner left turn lane, the other 2 instances were in parking lots. The fir
... st 2 times the vehicle was able to be jump started and was not taken to the dealership. The 3rd, it was jump started and taken to the dealership. They said the battery was slightly low, but had sufficient charge. They changed the fob battery, and topped off the car battery. That was all. The next day was the episode where it died in the turn lane. I had it towed to the dealership. This time they changed the battery to a more high capacity battery, and admitted that these vehicles have a battery put in them at the manufacturer that is not strong enough for the systems it runs. I even found a service bulletin about it online, #TSB 07-218-23R. This should be a recall item, and they should have changed this battery at my first visit! A vehicle suddenly dying on a busy roadway puts those inside at risk, as well as everyone around them. Very disappointed in this 2023 Subaru Forester Premium.Read more

2018 Volvo XC90, VIN [XXX] , 54,618 miles. Original owner. All service performed exclusively at Volvo Cars West Houston (same dealer, all three failures). Volvo Technical Journal TJ 34648.1.2, titled "Prior Approval - Lack of airflow causing poor cabin cooling," is classified as a Product Fix and explicitly lists the 2018 XC90 as an affected vehicle. This TJ required the dealer to submit a Prior
... Approval report to Volvo Corporate when installing the resistor fix. At 12,017 miles (August 2020), the dealer installed the evaporator resistor per TJ 34648.1.2, Part No. 32241228, under VST Operation 98734-2 — meaning Volvo Corporate has documentation of this vehicle's evaporator vulnerability on file since 2020. At 15,930 miles (March 2021), the AC compressor failed and was replaced under warranty, consistent with refrigerant starvation from the same underlying evaporator leak. The evaporator has now failed completely at 54,618 miles despite Volvo's own corrective measure being applied at 12,017 miles. The current failure (April 2026) includes a confirmed front evaporator refrigerant leak and AC discharge hose leak, documented by dealer borescope inspection showing corrosion on the evaporator fins and visual evidence of refrigerant oil residue at the discharge hose fitting. Dealer repair estimate is $4,406. This is a documented pattern failure across 2016-2018 Volvo XC90 models. My 2013 Toyota Prius with only 99,000 miles is experiencing a brake system failure. The brake warning light, ABS light, and traction control light all illuminate. This is a supplemental report to my previous filing regarding a 2023 VW ID.4 battery fire. On May 28, 2025, while being operated, the vehicle suffered a catastrophic thermal event. White smoke emanated from the front compartment followed by active flames. The vehicle was a total loss. Critical Safety Finding: A subsequent forensic examination conducted on October 29, 2025 (File 27739-01) by a cer
... tified fire investigator confirmed that after the initial fire was suppressed, the vehicle reportedly caught fire an additional time while being held at a tow/storage facility. Component Failure: The fire originated in the front compartment area. The failure is consistent with the "thermal runaway" risks identified in Recall 26V030, involving defective battery cell electrodes. Safety Risk: The multiple ignition nature of this failure poses a lethal risk to occupants, emergency responders, and tow/salvage facility operators. The fact that the vehicle reignited twice in storage, long after the initial suppression, suggests that current safety protocols or the scope of the existing recall may not fully address the volatility of this specific battery defect. Inspection Status: The vehicle was inspected by the manufacturer’s engineers and an independent forensic investigator. However, the manufacturer and the insurer are currently withholding the Photographic Report and Technical Field Report from the consumer. These records are vital to understanding the severity of the "multiple -ignition" event and should be subpoenaed or requested by NHTSA investigators to ensure the recall remedy is sufficient to prevent secondary fires in storage environments.Read more

Dear Toyota Japan Customer Experience Team, I am the owner of a 2016 Toyota Tacoma currently registered in Guam, U.S.A. I am writing to request your assistance regarding a water leak issue from the rear brake light area, which has caused the carpets and seats to become wet and develop mold. I originally purchased this vehicle in Saipan and recently relocated to Guam. When I visited the Guam Toyo
... ta Service Center, they kindly inspected my vehicle and informed me that this issue is related to the 3rd brake light gasket recall; however, the recall period has already expired. Unfortunately, I was never notified of this recall while the vehicle was in Saipan. Continuous exposure to mold has seriously affected both my health and my daughter’s health. Because of this, I had the vehicle professionally cleaned and sanitized by a specialist company. The Tacoma has always been our family’s favorite car, but now I drive it only because I have no choice. Every time I get in, I feel anxious and uneasy. The water leak itself is concerning, but the mold issue worries me even more. I truly hope this water leak and the mold problem can both be resolved so that I can once again enjoy driving my Tacoma with peace of mind. The blue circles in the attached photos indicate areas suspected of mold. These photos were taken right before I sent the car for cleaning. According to the cleaning company, when they removed the seats, they found water underneath the seats and carpets. Whenever I drive the vehicle, I experience persistent coughing and headaches due to mold exposure. This has become a serious health and safety concern. Since this issue is directly related to a previously recalled safety defect, I kindly request Toyota Japan to consider a Goodwill Repair or Safety Exception to address the water leak and the resulting interior damage.Read more

Vehicle: 2020 Jeep Grand Cherokee 4WD Purchase Date: [XXX] Mileage at Purchase: ~54,900 miles Dealer: Cherry Hill Jeep Chrysler Dodge, Cherry Hill, NJ I am filing this complaint because I am currently being required to drive a vehicle with a severe wheel misalignment that is visually observable, while the selling dealership has deemed it 'drivable' and has not completed the necessary repairs. On
... [XXX], I was involved in an accident in which I struck a curb after sliding on black ice. The damage included a bent lower control arm and a cracked subframe. I brought the vehicle to Cherry Hill Jeep Chrysler Dodge for repairs, which took approximately three weeks. I took delivery of the repaired vehicle on March 13, 2026. Upon taking delivery, I immediately noticed a severe misalignment. The wheel misalignment is so pronounced it is visually apparent while driving. I returned the vehicle to the dealership and they acknowledged the alignment issue. They have since told me there are additional parts needed to complete the repair, and that those parts have not yet arrived. As of today, April 10, 2026, the dealership has confirmed they still do not know when the parts will arrive. Despite the unresolved alignment issue, the dealership has deemed the vehicle 'drivable' and I have been left with no choice but to continue driving it. My daily commute is approximately 60 miles per day, 6 days per week. I have driven hundreds of miles in this condition. Driving a vehicle with severe wheel misalignment poses serious safety risks, including unpredictable handling, accelerated and uneven tire wear leading to potential tire failure, and reduced ability to maintain control in emergency situations — particularly concerning given that this vehicle's incident originated on icy roads. I am requesting that the NHTSA investigate the dealership's decision to return this vehicle to me in an unsafe condition and require me to continue operating it without a timeline INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA)Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The Powertrain/Engine Control System. The vehicle suffers from a total loss of motive power while driving in traffic. It is available for inspection. How was your safety or the safety of others put at risk? The vehicle unexpectedly enters "Limp Mode" while driving at various speeds in active traffic.
... When this happens, the accelerator pedal becomes completely unresponsive, forcing the vehicle to a crawl or a dead stop regardless of the flow of traffic around it. This creates a life-threatening risk of rear-end collisions and high-speed accidents, as the car cannot maintain speed or move to safety. I am forced to operate a vehicle that effectively "shuts down" while I am in the middle of the road. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. Multiple independent mechanics have confirmed the stalling and electrical instability. Despite replacing the crankshaft sensor and both batteries, the sudden loss of power in traffic persists. Shops have stated the issue is a manufacturer-level failure they cannot fix. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? It has been inspected by independent service centers. The manufacturer has not inspected it yet, as it is extremely hard to duplicate unless it is driven for a certain time at certain temperatures. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? The failure is temperature-dependent, occurring almost exclusively when ambient temperatures reach 71°F or higher. When the failure happens, the dashboard displays flashes the drive sign(limp mode). There are no other lights or anything triggered to display what the issue is related to. These symptoms began roughly one year ago, and has returned daily now that it is getting hot again.Read more

While driving, the vehicle experiences intermittent electrical malfunctions affecting multiple systems. The door locks cycle repeatedly, windows roll down on their own, and when attempting to roll the windows back up, they immediately roll back down. The dashboard lights flicker, become extremely bright, and at times go out completely. The central display (including Apple CarPlay) also shuts off e
... ntirely. Multiple warning alerts appear, including emergency brake warnings. These failures occur while the vehicle is in motion and create a safety risk due to loss of system visibility, inability to control vehicle functions, and unexpected vehicle behavior. The vehicle was initially taken to an independent repair shop due to these issues, where I was informed of Recall 97GF (wiring harness issue), which is associated with the same electrical failures. The vehicle was then taken to a dealership for recall service. The dealership retained the vehicle for approximately one week and advised that the repair resolved the issue. The vehicle operated normally for approximately three weeks following the repair. However, on or about [XXX], the same electrical issues reoccurred. The vehicle was returned to the dealership on February 20th, where it remained in service for over 45 days. Despite multiple repair attempts and extended time in service, the issue was not resolved and continues to occur. The dealership has indicated that the recall procedure was completed, but the original condition persists. The issue was also escalated directly to Volkswagen of America through a customer care case. The manufacturer reviewed the situation and stated that the recall procedure had been completed and the condition was no longer considered related to the recall. The case was subsequently closed without resolution, and the vehicle continues to experience the same electrical failures. Emergency Call (SOS) system failure / internal battery defect. This is apparently a known issue on the VW Atlas (and other VW/Audi models with Car-Net/SOS systems). Owners regularly report SOS/Emergency Call errors associated with the backup battery or the telematics module. Some dealers even acknowledge it as a “known issue” with those systems. This isn’t rare, especially in the 3–5 y
... ear window of the vehicle's age. The backup battery itself is cheap (roughly $50–$100 in most cases); however, the labor is not, because it typically requires a glove box/dash teardown and access to the module behind panels or the console. SOS-related repair costs, due to labor, are generally quoted between $850 and $1,500, depending on the city, state, and dealership. Sadly, VW knows this is an issue and refuses to do anything about it, creating a potentially dangerous situation where, in an emergency, the call button (sold as a must-have feature) will not function without this expensive battery replacement. My research shows this is an ongoing issue even in new models, yet they refuse to remedy it.Read more

Despite significant effort, we were unable to secure a rental vehicle in the Dewey Beach area, even after contacting multiple rental agencies and the dealership directly. This was later resolved, and I am currently in a rental through Enterprise; however, the stress and uncertainty of potentially being stranded was significant and unacceptable. While at the dealership, my wife witnessed the tow operator unloading the vehicle without proper equipment to safely lower it. As a result, the vehicle’s hitch made forceful contact with the pavement and became lodged. Multiple dealership employees observed the incident. After considerable effort to free the vehicle, it was released abruptly and rolled into another vehicle on the lot, causing additional damage. An argument then occurred between the dealership owner and the tow operator, which my wife was forced to witness. Hi Dear My car doesn’t report crash.
Incident date: Feb 1, 2025
My vehicle's Maintenance Minder system—the sole official schedule for maintenance per the Owner's Manual (Pages 649-658)—was corrupted by an authorized Honda dealer. The Service Advisor stated that sometimes technicians reset all the Maintenance Minder codes rather than just the applicable ones. Honda Corporate admitted this is a possibility when I followed up after the service. They stated t
... hat resetting all the codes rather than the ones that apply, can affect when and how future Maintenance Minder Codes appear. Honda provided contradictory information: first stating that the Maintenance Minder state cannot be reconstructed to accurately alert the owner to future necessary maintenance, then subsequently stating that reconstruction is possible if 'deemed necessary.' This leaves the owner with a corrupted safety maintenance system, with the remedy apparently contingent on Honda's discretion rather than the owner's right to accurate maintenance alerts. Without accurate alerts, critical maintenance items (brake fluid, coolant, transmission fluid) may not be serviced at proper intervals, increasing risk of component failure. Attached: Service invoice (RO #189526, 04/03/26) showing dealer added unscheduled Coolant Flush (Item 5) to my B1 request and reset ALL codes. Manual (pp. 649-658) confirms Item 5 is a separate sub-item triggered only by the system, not mileage. Honda Corporate emails admit resetting all codes corrupts future alert accuracy but offered contradictory remedies (first 'cannot fix,' then 'if deemed necessary'). This systemic failure—dealers bypassing the algorithm and Honda failing to enforce corrections—leaves owners with corrupted safety maintenance data, risking missed critical service intervals.Read more

THE BACK DOOR ON THE DRIVER'S SIDE DOES NOT LATCH PROPERLY. THE DOOR WILL TRY TO CLOSE BUT YOU HAVE TO KEEP SLAMMING THE DOOR UNTIL IT WILL FINALLY LATCH. I ONCE DROVE A FEW FEET AND THE DOOR HAD TO BE HELD SHUT. IT FINALLY CLOSED. THE DEALERSHIP WAS CONTACTED LAST YEAR (2025) ONCE I READ ON REDDIT THAT A TSB/EXTENDED WARRANTY WAS ISSUED. BUT I WAS TOLD THAT IT DIDN'T COVER MY VEHICLE AND IF I
... BROUGHT IT IN, A DIAGNOSTIC FEE AND FEE TO FIX IT WOULD BE CHARGED. SO I DO NOT OPEN THAT DOOR ANYMORE, BUT THAT IS NOT SAFE. I''M AFRAID IT WILL OPEN AT SOME POINT WHILE I AM DRIVING.Read more

I am reporting a potential unintended automatic emergency braking event involving a 2025 Tesla Cybertruck. On 23Mar2026, at approximately 40 mph, I was traveling behind a Tesla Cybertruck on a public roadway. There were no visible obstacles, hazards, or debris in the roadway that I observed. The Cybertruck suddenly and aggressively braked without warning. The abrupt deceleration appeared inconsi
... stent with road conditions and is consistent with what is commonly described as “phantom braking.” As a result of the sudden stop, I was unable to avoid a collision with the rear of the vehicle despite maintaining a reasonable following distance. After the impact, the Cybertruck initially left the scene, stating they were unaware a collision had occurred. No debris or roadway hazard was documented at the scene in the police report (FVPD report #2026-00777). The other driver later indicated a plastic bag may have triggered the vehicle’s emergency braking system. I am submitting this report as a potential safety issue involving unintended or inappropriate automatic emergency braking behavior.Read more
