Vehicle: 2005 Nissan Altima | VIN: [XXX] To the Executive Team and Legal Counsel, I am writing this under extreme duress following a catastrophic, life-threatening vehicle failure on April 23, 2026, in Ripley, WV. While transporting my [XXX] son, my 2005 Nissan Altima suffered a sudden engine stall, followed by the engine bay becoming saturated with oil and emitting heavy smoke. This event is
... the direct result of Nissan’s failure to remediate known federal safety defects, specifically NHTSA Recall 07V527000 (Crankshaft Sensor/Stalling) and NHTSA Recall 06V223000 (Oil Consumption/Fire Risk). The front camera lens cover — located in the grille below the Chevy bowtie logo — has developed a spiderwebbed/shattered crack pattern. The camera appears to still be working and parts of the image are crystal clear but the cracks in the cover distort the pict

On April 20, 2026, at approximately 3:30 PM, a 2025 Ram ProMaster operated as part of a commercial delivery fleet experienced a sudden vehicle fire while driving on a rural road near [XXX] [XXX] ). According to the driver, the vehicle drove over a small amount of corn husk debris on a low-maintenance gravel road, which is a normal and expected driving condition in this area. Immediately after,
... the vehicle became immobilized, and smoke and fire rapidly developed inside the vehicle and spread to the surrounding field. The driver exited the vehicle quickly and sustained second-degree burns and loss of facial hair. Emergency services responded, and approximately ten fire units were required to contain the fire. The driver was taken to the hospital by ambulance. The vehicle was a total loss. Notably, more than 24 hours after the initial incident, the vehicle reignited while being handled by a tow company. This incident does not appear consistent with normal operating risk. Driving over agricultural debris such as corn husks is a routine condition in rural Nebraska and should not result in catastrophic vehicle failure or fire. Additionally, another fleet operator in the same geographic market reported a nearly identical incident involving a 2025 Ram ProMaster less than one month prior. In that case, the vehicle also caught fire while being driven under normal rural road conditions. There is currently a known recall affecting 2025 Ram ProMaster vehicles. Given the similarity of incidents, severity of outcome, and potential pattern, this may indicate a broader safety defect. This event resulted in driver injury, complete vehicle loss, and damage to surrounding agricultural property. Immediate investigation into potential fire risk and defect in 2025 Ram ProMaster vehicles is strongly requested. On April 20, 2026 I was pulling into a parking space. I began to break but something went terribly wrong. My car accelerated on it's own, went over the curb and crashed into a building. I have asked my insurance company to inspect the car for any malfunctions but they won't. They deem it totaled and say I am at fault because it's my responsibility to control my car. The police arrived but they did not inspect the car either. I have contacted the dealership (Parkway Hyundai), but they are of no help either. There were not warning lamps, messages or other symptoms of the problem prior to the failure. However, I did notice recently that when I put my foot on the break to start the car that sometimes the break was very stiff and other times it was not. I thought it strange but thought it was just the way my car was. My insurance is Progressive. I purchased a new Mercedes Benz GLC300 2023. Last month, my friend and I were driving on I75 in Florida when we heard what sounded like a loud boom or gunshot. We quickly discovered that my sunroof had exploded. The sunroof was closed, and fortunately so was the sunshade, as large pieces of glass flew off the roof and smaller shards of glass collected on top of the sunshade. My friend and I were
... fortunate to have not been insured and were able to safely navigate the vehicle to the side of the road to assess the damage. In contacting the MB dealership, I learned that the sunroof is considered "glass" and thus not covered under warranty even though the vehicle is only 3 years old with less than 50,000 miles. What is more concerning to me is that the roof was not under a recall, and there have apparently been other MB owners that have experienced the same issue with class action lawsuits pending. My insurance company covered the repairs following payment for the deductible. This huge safety concern is that we could have been seriously injured by flying and/or falling glass. I write to express my deepest concern over the safety issue the exploding roof presents-the cost of repair relative to what could have happened is minor. Thank you for the attention to this matter.Read more

The display on the dashboard that includes the speedometer and other information goes completely blank. So does the heads-up display on the windshield. I've had it to the dealership and they have noted the problem but have not
... been able to fix it. I know that Genesis, which is part of Hyundai has had recalls about this problem, but so far Hyundai has not. On 3/15/2026 I was on my way home from Massachusetts to Pennsylvania, and about 35 minutes into my drive I got an alert stating "High Engine Temperature Stop Safely", so I pulled over immediately and turned the car off. After allowing the engine to cool down, I opened the hood and started the car to see if I could identify any issues, the "Engine Fault Service Now" alert came up. This was the firs
... t time this ever happened, this put me, my two dogs who were in the car with me, and possibly others on the road at risk, had the conditions escalated. (i.e. under hood fire). The vehicle was towed to an Authorized Ford Dealership, Mathieu Ford Sales, Inc. who diagnosed the issue on 3/17/2026. The dealership removed the spark plugs to inspect cylinders and found coolant intrusion into cylinders significantly on cylinder 2. Dealership referenced TSB 22-2322, is recommending short block assembly and related parts to remedy concern. Dealership advised it was safe and okay to drive the vehicle slowly to a nearby residence (to store the vehicle/retain possession), noting to pull over if an alert pops up and to have it towed if I received another message like before. The dealership advised it was not safe to drive the vehicle back to Pennsylvania. There weren't any issues driving the vehicle back on 3/17/2026 following the dealership's guidance; the car hasn't been driven since. The car needed to be jumped to start the vehicle on 4/17/2026 for a tow truck to load the vehicle onto its bed. The vehicle is currently back in possession of the dealership who diagnosed the issue. Upon doing research, this appears to be a widespread manufacturing design defect that Ford has failed to properly address/recall. My vehicle met the criteria under their Customer Satisfaction Program in 2022 - the vehicle is an Escape, 2017-2019, assembled in Louisville, that was Built before April 8, 2019, and which exhibited a covered condition, having low coolant. Ford Motor Co is failing to remedy-cure.Read more

11686537 prior complaint #. I want to add to my prior complaint. I have has this car going on almost a year. During the winter months there was no issues with the informatic system. Once we experienced our first 80 degree day it immediately started again. This is becoming unsafe as it causes anyone driving the car to become distracted due to the constant switching of screens. If you try and use t

I had my car set to autolock and at some point it reset that and then my car was broken into and stuff was stolen from inside. The informatic system is completely unusable when this happens and is pretty much every time you use the car when the weather is over a certain temperature. (NHTSA Tracking Number 11726105) I am updating my existing complaint regarding my 2024 Hyundai Tucson (NHTSA Tracking Number 11726105) for an ongoing safety concern involving an intermittent warning chime while driving. This issue began shortly after I purchased the vehicle in Nov 2025 and has been reported to the dealership multiple times over several months. The warning chime occurs randomly wh
... ile driving without any visible warning message or dashboard alert. This creates uncertainty because I do not know what the vehicle is trying to warn me about, which is distracting and creates a safety concern while driving. The dealership previously replaced both front seatbelt buckle assemblies after identifying intermittent high resistance as a possible cause. However, after that repair was completed, the warning chime occurred again while I was driving to the dealership for a follow-up visit. This confirmed that the issue was not fully resolved. I immediately returned the vehicle to the dealership, and they are now performing further diagnosis. I was advised that if the issue reoccurred, the next possible areas for diagnosis could involve the instrument cluster or the control module, both of which are safety-related systems connected to warning alerts and seatbelt functions. In addition, during this same visit, the dealership confirmed there is an actual oil leak requiring repair. This is significant because when I first raised this concern after an outside inspection in December 2025, I was told it was likely leftover residue from a prior oil change. The dealership has now confirmed it is a real leak and kept the vehicle for repair while providing a loaner vehicle. Because this vehicle was sold as Certified Pre-Owned and these safety and mechanical concerns continue shortly after purchase, I remain concerned about the reliability and safety of the vehicle.Read more

I am filing a complaint regarding my inability to receive a safety recall remedy without first paying for a separate component replacement. Vehicle: Tesla Model 3 VIN: [XXX] My vehicle has an active safety recall related to hood latch functionality, which Tesla stated requires a firmware (OTA) update. Tesla informed me this update could not be installed unless the vehicle’s car computer (
... MCU/CPU) was replaced. Tesla classified this component as out of warranty and required me to pay $1,687.66 out of pocket (after a partial discount). Separately, my vehicle developed a charging failure requiring HV battery repair. Tesla confirmed in writing that they could not proceed with the HV battery repair or install required firmware unless the same computer was replaced. Tesla also confirmed that if I did not approve and pay for the computer replacement, they would not proceed with either the battery repair or the recall-related update. At the time, my vehicle was not drivable, and Tesla indicated the loaner vehicle would need to be returned if I did not approve the repair. This would have left me without a functional vehicle, leaving no practical alternative but to proceed. As a result, I approved and paid for the repair in order to restore vehicle functionality and receive the recall remedy. This payment was made under protest. Concern: This situation conditions access to a safety recall remedy on a customer-paid repair, creating a barrier to receiving a mandated safety fix. I request review of whether a manufacturer can require payment for a separate component in order to enable a safety recall remedy and essential repairs. I have written documentation supporting these statements. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Summary of the Safety Problem: A 2026 Ford Bronco rolled out of its parked position with no occupants, despite being left in Park, and collided with another vehicle. Full Description of the Incident: The Ford Bronco was placed in Park, was not running, and had no occupants. I exited the vehicle several times to reinstall the removable roof panels. The vehicle remained stationary during this proce
... ss. After completing the roof panel installation, my wife and I closed the doors and trunk and we walked toward the building. A witness (the owner of the other car that was involved in car collision ) confirmed that the Bronco was still parked where we left it. A short time later, we learned that the Bronco had rolled out of its parking spot and struck a parked car. The Bronco had not been touched, driven, or moved by anyone after we walked away that we were aware of. There were no warning signs that we were aware of. The police were at the scene and filed a report. This was a Enterprise Car Rental Vehicle with approx. 1,300 miles. I am unsure if this vehicle has been inspected by Enterprise. Damage / Injuries: •Collision with a parked car. •No injuries reported. •Police report filed (attached). •Photos available (attached). Why This Is a Safety Defect: A vehicle that rolls away while in Park violates the intent of Federal Motor Vehicle Safety Standard (FMVSS) 114, which aims to prevent accidental rollaway. This Bronco should not have moved under any circumstances once placed in Park. What You Request NHTSA to Investigate: •Whether the 2026 Ford Bronco has a defect in its Park mechanism, parking brake system, or electronic controls that can cause unintended rollaway. •Whether similar complaints exist for this model year. •Whether a recall or inspection campaign is warranted to prevent future incidents. Safety Defect: Data Communication Module (DCM) Failure Affecting Hands-Free Microphone and Safety Connect / SOS Emergency System Vehicle: 2019 Toyota RAV4 Hybrid LE The DCM (which controls both the CarPlay/Bluetooth hands-free microphone and the Safety Connect SOS emergency button) suddenly failed without any warning or accident. Symptoms: - No one can hear me during CarPlay or Bluetooth calls
... (microphone inoperative). - Safety Connect / SOS light became intermittent or inoperative (directly impacts emergency calling, crash notification, and roadside assistance). This is a known recurring issue documented in Toyota TSBs T-SB-0112-20 and T-SB-0089-21. Toyota of Hackensack diagnosed it and quoted $1,300 for DCM replacement/update while charging me $200 for diagnosis. They did not disclose during the multipoint inspection that this is a documented safety-related DCM fault. I performed the manufacturer-recommended 12V battery disconnect reset (standard TSB procedure), which fully restored the microphone and returned the SOS light to solid green. However, this is only a temporary fix for a known defect. This failure compromises hands-free calling while driving and the emergency SOS system — both critical safety features. Mercedes issued a recall for Mercedes C class for the model year up to 2015 for rear subframe that is rusting with holes in it. The dealer tells me my car is not covered by Mercedes even though it is a 2015 model. They gave me an estimate for the repair of over $5000. They say the model changed mid 2015 and mine is a model that is not covered. This could cause a serious crash at high speed. I am
... not sure why that repair is not covered for my 2015 model & I would like Mercedes to honor the repair.Read more
