I am filing this complaint regarding a recurring and dangerous safety issue with my 2021 Subaru Forester. The auto stop/start feature intermittently fails to restart the engine after it shuts off when stopped. ... art again. This has happened multiple times at intersections, traffic lights, and freeways during rush hour (stopping and starting frequently), leaving me temporarily stranded in live traffic. The local Subaru dealership service department was unable to replicate the issue, however upon inspecting the vehicle's charging system, the battery was found unable to hold charge from the alternator and was replaced on June 9th, 2025. Now after less than 1 year of replacing the battery, the problem persists. I have been made aware of TSB 16-118-24R (TCM reprogramming for auto start/stop restart failure) and TSB 09-80-21 (Thermostatic Control Valve failure) which appear directly related to this issue. This is a serious safety hazard. A vehicle that cannot restart in live traffic puts the driver and other motorists at significant risk of a crash. Consequently, the trunk lid acts as a sail in windy conditions, easily overcoming the lift support and turning the heavy lid and metal latch assembly into a falling hazard for any consumer loading or unloading the vehicle. While Federal Motor Vehicle Safety Standards (FMVSS) focus heavily on internal trunk releases (Standard No. 401), there is a significant gap regarding the prevention of sudden, gravity-induced or wind-induced lid collapse. I am requesting that the NHTSA: Investigate the frequency of "unintended closing" injuries related to Nissan Altima trunk lift systems. Evaluate whether the lift-support specifications for this model are sufficient to withstand common wind gusts. I am filing a safety complaint regarding my 2017 Ford Explorer XLT with the 3.5L V6 engine due to a known internal water pump design defect. The water pump is located internally and driven by the timing chain. ... rd because the engine can suffer catastrophic damage, sudden failure, loss of power, or complete shutdown, sometimes with little or no warning. In my case, the vehicle experienced symptoms consistent with internal water pump failure, including my car overheating out of nowhere, forcing me to coast to the side of the highway in very unsafe conditions. This defect can destroy the engine and create dangerous driving conditions, especially if power loss occurs while driving with children or passengers, thankfully I didn't have any of my three children with me when this happened. This appears to affect many Ford Explorer vehicles from similar model years, and numerous owners have reported similar failures. The repair cost is extremely high because replacement often requires major engine disassembly or full engine replacement. I am currently financing this vehicle with a balance of $14,000 left to pay and have already paid close to $28,000. This is extremely unfair and the cost to repair is close to $5,000. I don't see a way for myself to afford that. I believe Ford should be required to address this issue because: The design can allow coolant contamination inside the engine without obvious external leakage. Failure can occur suddenly and create safety risks. Repair costs are excessive for what appears to be a widespread defect. Owners may not receive sufficient warning before catastrophic failure. After picking it up for the second time, the malfunctioning started again within the first 20 minutes of riding it. This is not just a user error, or a operating error like the dealership tried to tell me it was. This is a bigger issue. This almost cost me my life and I won't let this be brushed under the rug. This specific incident was the worst of the bunch. It was about 6 hours after I picked the bike up from the dealership the 1st time. I was out riding with a friend making frequent stops at resturaunts and gas stations. The E-clutch light had already been on at this point but per the dealership they told me it should still be safe to ride. Obviously not. After our last stop of the day, the bike decided (as I was getting up to speed) to junk uncontrollably for about 3 seconds, shut the engine off, then turn the engine back on and accelerate. At this point I hit the kill switch and glided to a safe spot to pull over and assess the issue. This could have easily costed my life if I had been at a higher speed. My vehicle was returned to me on April 25, 2026, after being at the dealership for approximately one week. My husband left a voicemail with the dealership (Evans Kia) reporting the problem, and I reported the issue to Kia America that following Monday. Since then, I have made multiple attempts to resolve this issue through Kia America. I opened a case on April 17, 2026, and have followed up several times. The case has been escalated multiple times (including April 22 and April 28). I was informed that Kia America also reached out to the dealership, but despite this, I have not received any direct response from the dealership or instructions to bring the vehicle in for repair. Despite clearly stating that the vehicle may be unsafe to drive, there has been no urgency in correcting the issue. I am requesting that this matter be reviewed as a potential safety concern involving improper vehicle servicing and release of a potentially unsafe vehicle. Poor drainage resulting from vehicle design defect. BMW needs to remove or modify the drain outlet to prevent repeat flooding. The warranty and extended warranty never checked for this-- drainholes functionality which results in electronic malfunctioning and safety concerns. 2019 SIB should have enforced extended warranty to investigate and cover this design defect in this vehicle. We are submitting complaint to the office of defects and investigation and requesting a good will repair and replacement of faulty systems and mold abatement which will require replacement of the damaged and mold in the pax and rear trunk areas. ... in home estimate told me that a 2024 F 350 has an Automatic Return to Park shift feature that should have immediately moved the column mounted shift to Park and applied the emergency brake. This feature was apparently installed in all Super Duty trucks in 2023 when the electric column shifter was added. It did neither of these things and collided with the wall damaging the wall and drivers door. I believe this is a defect in a safety critical system and filed a complaint with the Ford Customer Service Concerns department. They told me they would escalate to their general counsel but if I filed an insurance claim, that the complaint would no longer be active. No lawyer I contacted would take my case because a significant personal injury did not occur. I was advised to file insurance claim for both home and property policies.Read more

The issue has not been reproduced or independently duplicated by a dealer or service center; however, it was inspected and documented by the insurance company and repair facility during the claims and repair process, which confirmed that full glass replacement was necessary. The vehicle or component has been inspected by insurance representatives and a repair shop, but it has not been inspected by the manufacturer or law enforcement. This condition presents a safety risk due to intermittent stalling/shutdown while driving, electrical system instability, and failure of critical systems including exterior lighting and HVAC. These issues create unpredictable vehicle operation and reduced safety for occupants and others on the road. The problem has been reproduced and confirmed by the dealership over at least four separate service visits, for which documentation is available. Despite repeated repair attempts, the condition has not been resolved and the same faults continue to return. The vehicle has been inspected multiple times by the dealership. The manufacturer, Stellantis, has been contacted but has declined further escalation or engineering review, citing warranty expiration. Warning indicators, including the check engine light, have repeatedly appeared prior to and immediately after service visits. ... o honor promised repair warranty despite their own negligence that created the damage. I found out that Hyundai models including mine have coupler issues involving recalls, years ago. I was not aware of this, and so not sure if my vehicle was part of the recall. However, Hyundai Motor America failed to notify me to take my vehicle in for a safety check for the vehicle coupler. And while I found that that my vehicle qualified for an extended warranty due to the recall, HMA so lacking in responsibility, sound business and moral ethics, let alone serious concern for my safety, claims that my vehicle is past the extended warranty by a completely insignificant mileage. As far as am concerned my vehicle is not safe to drive, and it remains at the dealership while I exhaust all avenues to have my car fixed. I have documents including clear repair notes and service request documents to support my complaint. ... dash and it would say charging malfunction once I got home we took a look at the engine and noticed that none of the belts were working at all and then I started to google what could possibly be the problem and a couple of different recalls came up recall number 12V550000 and that is engine and engine cooling engine diesel what the name of it is and the recall date is 1127 2012 and then the next one is 2012 BMW X5 diesel SUVs from the model year 2012 923 BMW X fives have been recalled because they have defective engine belt, tensioners and cracks that develop along the welds if the belt tensioner fails there could be a sudden loss of power steering assist that is sounds like exactly what happened to my car and then there’s another one recall number B121918 which is from September 2019 and it is called idler pulley bolt and come to find out that actually was a recall on my vehicle, but the previous owner is the one that had it repaired so I’m not sure if BMW is willing to repair it again but there’s not a lot of miles on this cards a 2012 I bought it with 95,000 miles on it and it currently only has 131,000 miles on it. I’m just hoping for some kind of resolution here this is the fifth Bmw that I bought in my lifetime and I for the most part I’ve had pretty good ultimate driving experience and good customer service and some good financing but I can’t see why just recall wouldn’t cover my vehicle.
