Vehicle primarily failed while parked at owner residence [XXX] and remained inoperable for approximately three weeks prior to tow to Cadillac dealership. The ignition key became trapped and could not be removed. A manual override release beneath the driver-side steering column was required to free the key — confirming the lock was held electronically, not mechanically jammed. Fault confirmed by
... authorized Cadillac dealer: intermittent electrical fault within the shifter assembly lock mechanism. A similar intermittent concern was previously documented on this vehicle in 2011 (logged as "Customer Concern Not Duplicated"), establishing a recurring pattern. GM Technical Service Bulletin 07-07-30-032D (2006–2011 Cadillac CTS, automatic transmission) explicitly documents this exact condition: key sticking, Shift to Park DIC message, shifter binding, and battery drain. OEM replacement part is now discontinued. Manufacturer declined goodwill or recall coverage. Dealer refuses repair unless owner assumes full financial responsibility for a used part with no warranty and no guarantee against recurrence. Safety concern: ignition/shift interlock failure causing vehicle immobilization, key entrapment, and loss of transportation for an elderly owner dependent on the vehicle for medical care. Defect evades shop diagnosis due to intermittency. No safe OEM remedy exists. Prior recall: NHTSA 14V394000 / GM Campaign 14172 addressed same interlock system. Current failure represents recurrence beyond that recall's scope. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Description of Defect: When the vehicle is shifted into reverse, the rearview camera display freezes for several seconds before going black. This malfunction creates a direct and serious safety hazard — during the period the display is frozen, the image shown does not reflect actual real-time conditions behind the vehicle. Any pedestrian, child, or object that moves into the path of the vehicle
... after the display freezes will not be visible to the driver. This significantly increases the risk of a backover collision. Dealership Inspection Outcome: I brought the vehicle to an authorized Ford dealership for diagnosis. Upon inspection, the technician was unable to detect the diagnostic trouble code associated with Recall 25S89, and as a result, the vehicle was deemed to have passed inspection. However, the malfunction continues to occur. The failure to trigger the recall code does not resolve the underlying issue — the camera display still freezes and blacks out during reverse operation, leaving the driver without a functioning rearview safety system. Safety Concern: A rearview camera is a federally mandated safety feature. A display that intermittently freezes and then goes dark is functionally equivalent to having no camera at all during those critical moments. This is not a minor inconvenience — it is a safety system failure that puts pedestrians and other individuals behind the vehicle at risk every time the vehicle is put in reverse. Requested Action: I am requesting that NHTSA investigate whether Recall 25S89 adequately captures all failure modes associated with this defect, and whether vehicles that present with this symptom but do not trigger the associated diagnostic code are being improperly cleared at dealerships. A remedy should be identified and applied to all affected vehicles regardless of whether the recall code is detected during inspection.Read more

The transmission shift cable on the vehicle may fail to maintain its connection to the transmission. According to the manufacturer’s safety notice, this defect results in an unexpected and unrecoverable loss of PARK function. The manufacturer has acknowledged that this condition can lead to unintended vehicle movement in an unintended direction, even when the shifter indicates the vehicle is i
... n "Park." This creates an immediate risk of a vehicle crash without prior warning and poses a severe threat of injury to the driver, passengers, and bystanders outside the vehicle. The manufacturer issued the safety notice on March 8, 2026, but stated that a remedy is "not currently available." As of May 12, 2026 (over 60 days later), the manufacturer has failed to provide a timeline for the repair, failed to respond to formal written inquiries regarding a safety remedy, and has not provided interim safety measures or alternative transportation. The vehicle remains an active safety hazard that cannot be safely parked or operated.Read more

My 2025 Tesla Cybertruck AWD (VIN: [XXX] ) was returned to me after the first service visit (Invoice XXX Tesla Palo Alto, 8/22/2025) with the Pedestrian Warning System (PWS) speaker disconnected. The disconnection occurred during front-end service work at that visit, which included front fascia inspection and removal. The technician did not reconnect the PWS speaker before returning the vehi

The disconnection was not discovered until the second service visit (Invoice XXX, Tesla Palo Alto, 9/18/2025), at which point Tesla's own invoice documented: "The issue was caused by a disconnected front pedestrian warning speaker. Technician reconnected the front pedestrian warning speaker to resolve the issue." The [XXX] s therefore operated with a disconnected PWS for approximately 4 weeks — from August 22 to September 18, 2025 — without my knowledge. During this entire period, the vehicle was not emitting the required audible low-speed alert. The PWS emits an audible alert at low speeds to warn pedestrians, cyclists, and visually impaired individuals of the vehicle's approach. This system is required under FMVSS 141 (Minimum Sound Requirements for Hybrid and Electric Vehicles). This is a service-inflicted deactivation of a federally-required pedestrian safety system. The deactivation was caused by Tesla's Palo Alto service center (4180 El Camino Real, Palo Alto, CA 94306) during routine warranty repair work. Hyundai - DAW - Driver Attention Warning / Forward Attention Warning System This issue warrants immediate consideration for a recall. Despite wearing glasses, sunglasses, or no glasses, the problem persists. I occasional do remove my glasses while driving, and the warning continues to sound. This occurs regardless of the time of day, weather conditions, or whether I am sitting upright or reclining
... . The warning randomly activates and deactivates, sometimes without any intervention. Occasionally, I simply allow the warning to sound without taking any action, and it remains active for a few seconds or until it triggers a warning that the system will be turned off, at which point it deactivates. I have attempted various troubleshooting methods, such as keeping my eyes open, adjusting my seating position, wearing and removing my glasses, and tilting them to minimize glare. None of these methods have proven effective in preventing the warning from activating or in making it deactivate. This is a persistent problem. Hyundai dealerships initially informed me that the warning might be an annoyance and their response was to simply refrain from using it. However, if a basic feature like cruise control in a 2026 model is rendered unusable due to a design flaw (which this certainly is), it constitutes a clear violation of safety standards and should be addressed through a recall. The incessant alerts are highly disruptive and can be quite frustrating. For instance, driving with an arm in the 12 o’clock position triggers a warning, while wearing polarized sunglasses also results in an alert. The presence of a setting to disable the warning is a welcome feature but doesn't exist, and the system’s persistent alerts can be overwhelming. The alerts continue even after taking a break. It is distracting and a safety hazard. Drivers should be able to adjust the sensitivity of the feature turn off this feature. Currently, there is no way to disable or temporarily disable this feature.Read more
