The vehicle is experiencing premature paint failure and delamination on factory-painted body panels with Quartz White Pearl (W8) paint. Paint is peeling, chipping, and separating from the underlying surface without collision damage, abnormal wear, or neglect. The issue appears consistent with widely reported Hyundai white paint adhesion defects affecting similar vehicles and paint codes. The cond
... ition developed gradually and continues to spread to additional areas of the vehicle. The affected paint failure exposes underlying surfaces and may reduce long-term corrosion protection of the body panels. No warning lights or messages appeared prior to the issue. The vehicle remains operational, but the paint defect appears to involve loss of adhesion of the factory-applied paint system rather than normal cosmetic wear or isolated rock chips. The issue is available for inspection upon request. The manufacturer has issued warranty extensions and programs related to similar white paint delamination conditions on Hyundai vehicles, but this vehicle was reportedly excluded despite having the affected paint code and similar failure pattern.Read more

I am filing a safety complaint regarding a defective diesel emissions system and persistent warning condition in my 2017 Land Rover Range Rover Sport Diesel vehicle that presents a serious safety hazard to myself and others. The vehicle was purchased from Ewing Buick GMC in Plano, Texas, [XXX] and was advertised as being in good condition and ready for sale. At the time of purchase, the vehicle ap
... peared to have undergone preparation and inspection-related services, which gave the impression that it was safe and roadworthy. Dealer informed me of no inspection being performed. Shortly after purchase, the vehicle began displaying persistent emissions-related warnings (DEF system / engine warning). I initially had the vehicle inspected, and serviced for repairs by Royse City Auto Care, where diagnostic information indicated emissions-related faults. Due to the seriousness of the issue, the vehicle was then taken to an authorized Land Rover service center for further diagnosis. Land Rover technicians indicated that potential causes may include a knock sensor or related module failure; however, they stated that these repairs are not guaranteed to resolve the issue. The warning condition remains active and unresolved. The vehicle now presents a critical safety concern. The emissions system warning includes a mileage countdown that will eventually prevent the vehicle from being driven once it reaches zero. This creates a dangerous situation where I may be unable to operate the vehicle at all, potentially leaving me stranded in unsafe conditions. Additionally, the vehicle may enter reduced power mode (limp mode) or experience loss of power while driving. These conditions significantly increase the risk of an accident, especially on highways or in traffic. As a result, my safety and the safety of others on the road are put at risk. This ongoing defect, combined with the inability to guarantee a repair, raises serious concerns about the reliability and safety of the INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIRead more

I purchased a 2023 Hyundai Elantra Hybrid 2023 on December 15, 2023, and since early ownership I have experienced persistent jerking/clunking while slowing to a stop, harsh downshifts, hesitation during braking, and rough transitions between electric and engine power. The issue was first documented around 5,744 miles and continued through approximately six to eight repair/work orders, where dealer
... ships repeatedly described the condition as normal hybrid/DCT behavior while performing software resets, adaptive relearns, and diagnostic checks. After continued complaints, Hyundai Techline became involved and the dealership concluded the vehicle “jerks pretty bad,” leading to a full transmission replacement under warranty. Despite the transmission replacement, the issue persisted and the dealership later confirmed slight clunking during downshifts and discovered a torn engine mount, which was also replaced. The dealership additionally noted that the issue did not occur while driving another Elantra Hybrid or the loaner vehicle, suggesting the condition was specific to my vehicle. I also opened a case with Hyundai Motor America Consumer Affairs regarding the recurring defect and repeated failed repair attempts, but the issue continues despite major drivetrain repairs. Hyundai Motors America Consumer Affairs has not been able to help me with this either. They also didn't provide me with the Federal Agency number when I requested them in order to reach out and seek help from the agencies. Hyundai didn't help me in any way. There were no warnings. They have been able to reproduce the issue but do not want to resolve the issue. I strongly believe no vehicle is designed in a way that it jerks and downshifts in a harsh manner causing distress to the driver/owner.Read more

We received the notice about this recall in October 2025, more than six months ago. I am submitting a safety complaint regarding my 2020 Mercedes-Benz GLE 350 due to what appears to be a significant air conditioning refrigerant leak. The vehicle rapidly loses refrigerant shortly after being recharged. When the system is operating, a loud seeping/hissing sound can be heard from the HVAC system area, followed by complete loss of cold air within a very short period of time. This is
... sue appears to indicate a major refrigerant system failure and raises concerns about potential defects in the evaporator, condenser, hoses, or related AC components. The leak rate is excessive and ongoing. I am concerned this may be a broader manufacturing defect impacting other Mercedes GLE vehicles. The vehicle has been properly maintained, and the failure occurred under normal operating conditions. Given the frequency of similar complaints online involving Mercedes HVAC refrigerant leaks and evaporator failures, I believe this issue warrants investigation for a potential recall or manufacturer accountability. The failure impacts driver and passenger comfort and visibility in humid or high-temperature conditions due to inability to properly defog windows. I respectfully request that NHTSA investigate this issue for potential safety and defect trends affecting the 2020 Mercedes-Benz GLE platform.Read more

The vehicle has experienced ongoing electrical and safety-system related malfunctions shortly after being purchased as an INFINITI Certified Pre-Owned vehicle in April 2025. The issues initially included flickering interior and exterior lights, intermittent door lock malfunctions, and other electrical irregularities. In June 2025, I received notification through the INFINITI app regarding a major
... engine recall involving engine bearing failure concerns affecting my vehicle. I brought the vehicle to Harte INFINITI for inspection regarding the recall and my electrical concerns. During that visit, dealership personnel acknowledged there were multiple vehicles experiencing similar engine bearing issues related to the recall. Despite my concerns, I was told the vehicle was “safe to drive” because no diagnostic codes were present at that time. The problems continued to worsen over time. On February 26, 2026, during a routine oil change shortly after expiration of the basic warranty, I was presented with approximately $2,600 in recommended repairs despite previously being told there were no issues with the vehicle. Most recently, the vehicle developed Forward Emergency Braking (FEB) malfunction warnings and Lane Assist malfunction warnings involving critical driver assistance and collision-avoidance systems. These warnings raised serious concerns regarding the safe operation of the vehicle because they involve systems designed to help prevent collisions and maintain vehicle control. I was concerned that these systems could fail unexpectedly while driving, potentially increasing the risk of an accident or injury to myself, passengers, or others on the roadway. The vehicle was returned to the dealership for inspection, where I was presented with an additional repair estimate exceeding $4,600 along with diagnostic charges. Despite the active safety-system malfunction warnings, I was again advised that the vehicle was safe to drive.Read more

[XXX] U.S. Army), Active-Duty Service Member (Stationed at Fort Knox, KY / Residing in Brandenburg, KY) I am reporting a severe structural manufacturing defect involving the room slide-out assembly on the driver-side flank of the vehicle body. Certified diagnostic testing and structural tolerance layouts executed by the component manufacturer, Lippert (LCI Case #[XXX]), and servicing dealership t
... echnicians (Camping World) have confirmed that the main slide-out housing and framing elements were installed, aligned, and integrated completely out of structural tolerance directly at the Thor Motor Coach assembly plant during production. This structural defect constitutes an uncorrectable manufacturing deviation. Because the slide-out mechanism is fundamentally misaligned within the main load-bearing sidewall of the motorhome, it repeatedly binds, undergoes severe component stress, and fails to close or seat flush against the secondary seals. Operating a 11,000lb+ commercial-chassis motorhome with a structurally out-of-tolerance slide-out housing introduces substantial safety hazards to public highways: Catastrophic Unintended Deployment: The mechanical stress and structural warping prevent the locking mechanisms and gear teeth from seating safely. Vibration at highway speeds risks structural latch failure, which could result in a partial or full unintended slide-out deployment while traveling at high speeds. Because the slide-out frame cannot close symmetrically, the structural integrity of the main driver-side wall is severely degraded. This creates asymmetrical aerodynamic drag and hazardous crosswind vulnerability, risking dynamic instability and potential vehicle roll-overs or lane-drift. In the event of an evasive driving maneuver, the shifting, misaligned mass of the slide-out can cause unpredictable chassis twisting and severe stability control failures, endangering my passengers and sharing motorists. The car was also returned to me by the dealer. When I asked the dealer on 05.12.2026 to indicate the root cause of the damage, the dealer replied that he was not aware of such a similar damage and indicated that the warranty on the car has expired and there are no open recalls. But The dealer's initial assessment from 05.13.2026 (estimates $9325.25) indicates the fire was caused by a failure of the Exhaust Gas Recirculation (EGR) cooler, which is a known issue (similar to recall VB1/19V-757) where the cooler cracks, leaks coolant, and causes a fire in the intake manifold. This is a component failure resulting in an explosion/fire, not typical wear. I need a diagnostic report that specifically mentions the EGR cooler failure as the source of the fire. My 2017 Honda CR-V's air conditioning system has repeatedly failed to cool properly over the past several years, occurring approximately 4 to 5 times. Honda extended the warranty on these components to 10 years with no mileage limit, however no formal safety recall has been issued and owners are not proactively notified. This is a recurring defect affecting thousands of CR-V owners across multiple model years (2017–2022). While driving down the road my passenger went to open the sunroof. That involves retracting the shade first. While the shade was retracting I heard a pop and then a crunching noise. The shade then drooped into the back seat area blocking any vision to the rear of the vehicle. It also was quite startling and distracting while driving. Contacted the dealer and set an appointment to get it looked at.
... Brought the vehicle in for its appointment about a week later. Was notified that a trim moulding ( part # LJ6Z5450036A) that was held up by two sided tape fell off and caused the shade to jam up and break. The moulding had fallen off with the shade closed so there was no way to know it had failed. Was initially quoted $2500 to repair it. The vehicle was less than 60 days out of Ford’s 3 year 36000 mile warranty. I even have a Ford 8 year 75000 mile extended warranty but was told that trim pieces were not covered by warranty. I asked the dealer to contact Ford to ask for assistance in paying for the repair. Ford came back and offered to repair it for $498.38. I authorized that repair and received the vehicle back. Shade is now functional but it appears that there has been no improvement to the moulding to keep it from having a repeat failure. If the moulding had fallen with the shade open it could have possibly struck an occupant and caused issues if driving.Read more

Thankfully, the AC was turned on and my child was unharmed. The vehicle should not have allowed itself to lock under these conditions with the key present inside the cabin. This created a serious safety hazard involving a trapped child. Compounding the issue, the LincolnWay remote access system no longer functions because the vehicle’s original 3G modem became obsolete after U.S. 3G cellular shutdowns. On May 15, 2026, I was informed that a software update had been performed and the vehicle was returned to me. On May 17, 2026, the same issue reoccurred multiple times in XXX. While stopped in a pickup line at Home Depot, the vehicle shifted into neutral and shut off when attempting to move forward from a stopped position. The same issue then occurred again while waiting in line at 7 Brew Coffee. On one occasion, the vehicle shifted into neutral while my foot remained on the brake pedal and the transmission was still in drive. The issue occurred again at the intersection of XXX and XXX while waiting at a traffic light. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Hello, I am contacting Ford regarding a serious concern with my newly purchased 2026 Ford Explorer ST. Within days of purchase, we discovered water leaking from the moonroof area into the interior of the vehicle, including water coming through the seatbelt pillar area , wetting the roof as well, during rain and vehicle washes. In addition to the leak, there is noticeable wind and air noise coming
... from the moonroof area even when the roof is fully closed, (we notified them the day we got the vehicle about this) which raises concerns that the seal is not properly seating or sealing against the vehicle. After noticing these issues, we inspected several similar vehicles on the dealership lot and observed that multiple vehicles appeared to have uneven or improperly fitted moonroof seals as well. This has increased our concern that this may be a manufacturing or assembly defect rather than normal wear or an isolated issue. Because this vehicle is brand new and the issue was reported almost immediately after purchase, we are requesting that Ford review this matter seriously and assist in ensuring the vehicle is properly repaired or otherwise resolved. We are concerned about long-term damage to the interior, electronics, airbags, mold or mildew risks, and the overall value and reliability of the vehicle. We have documented the leak and seal concerns with photos/videos and are currently working with the dealership for inspection. Thank you for your time and assistance.Read more

My 2022 Ford F150 B&O Unleashed Sound System 18SPK explodes from time to time with a deafening noise (pop, then static) that makes you jump. Turning off the radio does not stop the deafening noise. In the past, at times, I was able to stop the deafening noise by switching from satellite to AM radio. This last time, nothing I tried stopped the deafening noise and I could not pull off the road to tu
... rn off the truck for 10 minutes. My ears were still hurting the next day. During the 10 minutes I had to wind down the windows which did not help much. The speakers were getting hot to the touch, especially the little tweeter speaker, with a plastic burning smell. Each subsequent time I turned the truck back on (after waiting about a half minute) the deafening noise started again. Then I waited 10-15 minutes before starting the truck and the noise stopped. My truck is available for inspection upon request. My safety and the safety of others are put at risk because you jump when the problem occurs and are distracted when endeavoring to stop the problem. The Ford dealership cannot diagnose the problem unless the deafening noise appears at the time of service. The sound system has not been inspected by the manufacturer. There were no warning lamps, messages or other symptoms of the problem prior to the start of the deafening noise (pop, then static).Read more

The Hill Hold Assist system on my 2024 Volkswagen GTI (manual transmission) repeatedly holds the brakes too long when starting on an incline. The brakes remain engaged for 1–2 seconds even after I begin releasing the clutch and applying throttle, which causes the car to stall or hesitate before moving. This creates a safety risk when pulling into traffic or when other vehicles are behind me on a
... hill. This issue has occurred at least four times within the first 1,000 miles of driving. It is easily reproducible on normal hills and happens under normal driving conditions. I have many years of experience driving manual transmission vehicles, including a 2019 GTI, and I have never experienced this kind of delayed brake‑release behavior on any other vehicle. The system’s interference is abnormal and unexpected. There is no option in the vehicle settings to disable or adjust Hill Hold Assist, and many other 2023–2024 GTI manual owners report the same problem. This appears to be a calibration issue that results in unexpected stalls, hesitation, and delayed acceleration, creating a potential safety hazard.Read more

FORMAL DEFECT AND SAFETY HAZARD COMPLAINT TO: National Highway Traffic Safety Administration (NHTSA) Office of Defects Investigation (ODI) 1200 New Jersey Avenue SE, West Building Washington, DC 20590 DATE: May 17, 2026 SUBJECT: Urgent Request for Recall Investigation: Systemic Cloud-Dependency, Hypervisor Power Management Failures, QNX Boot-Looping/Blackouts, Unix Epoch Clock Drifting, High
... -Voltage Contactor Thermal/Fire Hazards, and Core Low-Voltage Module Malfunctions via Telematics Disconnection in the Fisker Ocean Fleet 1. Executive Summary This petition requests an immediate, systemic safety defect investigation into the software architecture, operating system stability, power management, and network subsystems of the Fisker Ocean vehicle fleet (Model Years 2023–2024). Following the liquidation bankruptcy of Fisker Inc. and its affiliates—under Case No.XXX (XXX) in the U.S. Bankruptcy Court for the District of Delaware, overseen by Judge [XXX] —the vehicle's intellectual property, cloud infrastructure, and over-the-air (OTA) software management assets were assumed by a commercial entity, American Lease, and its vehicle management arm (OV Loop/Ocean Loop). Due to the subsequent imposition of a predatory $499/year subscription paywall, thousands of retail owners have been cut off from cloud connectivity. While a missing cloud connection in a standard passenger car traditionally results in a benign loss of convenience features, in the Fisker Ocean's "software-defined" architecture, it triggers a severe, un-throttled software failure loop. This architectural defect forces safety-critical operating systems into permanent boot loops, pins CPU utilization, drops the system clock into a broken chronological state, and drives an aggressive parasitic drain on the 12V battery system. This directly leads to cascade control module failures, key fob inoperability, and accelerated mechanical degradation of high-voltage components, introducing an acute INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACRead more
