My 2023 Jeep Wagoneer experienced a catastrophic panoramic sunroof failure while parked and unattended. The vehicle had been driven normally earlier that day with no known impact or debris strike, and I have footage from before arriving showing the roof intact. The vehicle was parked in a school parking lot for approximately 2.5 hours, and when I returned, the panoramic roof glass had spontaneousl
... y shattered. No impact event was witnessed, and there was no known object strike to the vehicle. The dealership determined the damage was caused by outside influence; however, no specific impact point or evidence of an object strike has been identified to us. Because the glass shattered unexpectedly while the vehicle was stationary, we are concerned this may represent a spontaneous tempered-glass failure and a potential safety defect.Read more

I own a 2024 Ford F-150 Lightning Platinum built in August 2024. The vehicle has been at the dealership for over 45 days for severe, unresolved electrical safety defects. I have attached photographic proof of the main defects. The first three photos show the driver's instrument display screen behind the steering wheel completely blacking out on startup, blinding critical safety gauges like the sp
... eedometer. The center screen stays on, but the main cluster is totally dead. Fourth and fifth photos show the driver's seat memory failure. I manually set my lumbar support to a comfortable 3-4-3 setting and save it, but every time the truck sits, the seat completely deflates to a flat zero default on startup. This causes an immediate driving distraction and ergonomic hazard. The factory navigation also randomly shouts driving directions out of nowhere without any driver input. The dealership is currently trying to install a temporary "bypass wire" hack on my factory harness just to see if it fixes the seat memory. However, Ford's internal hotline (SSSC) refuses to authorize repairs for the black screen or phantom navigation until a tech physically catches it glitching on command. Ford’s policy is to return the vehicle unrepaired if it does not fail during active observation. I am requesting NHTSA to investigate Ford's active evasion of safety recall 25V540 (Ford 25S88). The official recall paperwork states the defective supplier software entered production on May 14, 2024. My truck was manufactured in August 2024—squarely within the defective window—yet Ford is using a model-year loophole to exclude 2024 vehicles from the recall population. They are denying owners a permanent factory software remedy and forcing dealers to use unapproved, temporary physical workarounds. The vehicle is at the dealership and available for federal inspection if need be.Read more

Greensboro Transit Agency (GTA), operated by RATP Dev USA, is formally notifying you of an engine compartment fire involving one of our transit vehicles. Vehicle Information: •Fleet Number: 764 •Manufacturer: New Flyer •Vehicle Type: Heavy-Duty Transit Bus •Incident Date: [XXX] •Location: Greensboro, North Carolina Summary of Incident: While in revenue service, the operator reported hear
... ing an abnormal noise and observed white smoke coming from the rear engine compartment area. The operator safely evacuated passengers and emergency responders arrived quickly to suppress the fire. No injuries or fatalities occurred. A third-party fire investigation was conducted by Halsey King & Associates. Based on the preliminary findings, the fire is believed to have originated from a catastrophic failure of the Thermo King A/C compressor clutch bearing assembly. The bearing reportedly overheated, seized, and caused the compressor belt to ignite while still being driven by the engine. The investigation identified evidence consistent with: •A seized A/C compressor clutch bearing •Extreme heat generation within the clutch assembly •Belt ignition and progression of fire through engine compartment components At this time, our maintenance records do not indicate any previously documented defects associated with this specific Thermo King assembly prior to the incident. Our fleet currently has inspection and preventive maintenance procedures in place related to belt alignment, pulley alignment, belt tension, clutch air gap measurement, and related A/C system inspections. We are providing this notification for awareness, documentation, and any further review deemed appropriate. Please let us know if additional information, photographs, maintenance records, or investigative documentation are required. I attempted to reach you by phone, but your voicemail inbox appears to be full, so I am sending this email instead. Regarding your concern about Safety Recall C4B: Hydraulic Control Unit, please note the following: At this time, the official Stellantis recall remedy is not yet available, even though the recall is currently in Phase 4 of 5 toward final remedy release. We also do not have a timeframe for when the final remedy will be issued. However, there is a suitable alternative repair option using parts associated with the recall remedy for the 2017–2018 Ram 3500 Cab Chassis (DD) and Ram 4500 <10K lb. Cab Chassis (DF). Below are the part numbers that may be presented to your dealership. They will be able to determine the appropriate next steps based on the vehicle's condition. Parts Information (Alternative Repair Option) Part No.: 68143491AB Qty.: 1 Part Name: Anti?Lock Brake System (Control) Applicable Models: – 2017–2018 (DF) Ram 3500 <10K lb. Cab Chassis – 2017–2018 (DD) Ram 3500 Cab Chassis Part No.: 04318080AD Qty.: 3 Part Name: Brake Fluid Please understand that this is not a mandatory repair option, but rather an alternative if you prefer not to wait for the official recall remedy. This repair would initially be an out of pocket expense, but you may later file for recall reimbursement. The cylinder head was rebuilt last year, associated with a similar instance. I am now fully aware that the 2019 Tiguans are known to experience all of these problems, but when I brought my car in on previous occasions, that never got mentioned. I am told XYZ is an issue, then billed for repairs. The car was originally leased, and when I purchased it at the end of the lease, I also purchased a full premium warranty. When the issue happened in May of 2025, the policy eventually covered this, but it took over a month of justifications and records before the third-party insurance company agreed to cover the repairs. I apologize if this does not meet the NHTSA bar of a safety hazard. It does reduce the ability for hands free navigation and acts as a distraction for drivers. There are dozens, maybe 100+, reports online of people having sim

There are no warning signs and the GPS will just start giving poor directions. The "danger" is in the fact that the only workaround now is to engage with a phone or other device while driving. In theory a human can rise above this and be smarter than the car but isn't that true of many safety components like non-functioning wipers(drive slow) or blind spot protectiong(check your mirror). The vehicle began suffering rough idling, difficulty starting, and dangerous stalling events during operation. Cycling the ignition on and off provided only temporary relief. The check engine light has now become permanently illuminated. The safety hazards have severely escalated into a protective engine "limp mode," causing violent drivetrain shuddering and a catastrophic loss of driveability. BMW of Pittsburgh refuses to honor the recall workmanship, demanding a predatory $6,000 out-of-pocket teardown fee to bypass their liability. When I refused to take delivery of the un-remedied vehicle, a Ford corporate representative mandated that I drive it anyway, stating that if I suffered a respiratory crisis while operating the vehicle, I should simply "call the ambulance". This ultimatum caused an immediate severe hypertensive crisis (160/112 BP), requiring emergency medical transport to CHI Health Immanuel and cardiac evaluation. This represents a critical component failure causing toxic cabin air contamination, compounded by a complete failure of the manufacturer’s remedy. Ford Engineering and Woodhouse Ford Service admitted in writing that they have NO scientific test to verify cabin air safety and NO official technical procedure to decontaminate the internal HVAC housing following this chemical saturation. Ford’s only attempt at a remedy was an external spot clean and a simple filter replacement, leaving toxic residual chemicals inside the blower housing. Had not noticed anything unusual that give me any hint that the gas pedal would stick all the to the floor. When I researched unexplained acceleration there was reference to the throttle and 4 or 5 reasons why it would accelerate on its own.Read more

All warning lamps have been coming on at random times. The dealerships have always said there is no problem. Now, outside of warranty, I am reading that it's possible that the problem is that all of the fuel injectors need to be replaced. Online notes that this is a widespread issue with this particular vehicle. I was informed that I can disconnect the battery and it resets the system and warning

The reverse camera started pointing straight down one day so I took it in for a recall dealing with the screen and camera. Ther replaced the camera, and did some updating and when I got it back the camera is still pointing straight down. I called them immediately and told them my camera was still pointing straight down they asked me to bring it back. I was there 5 minutes and the service manager c
... ame to me and said the camera was pointing down when I brought it in and that the reason was probably because it was an aftermarket part, which it’s not it’s all factory, and that wasn’t in the recall. "Under FMVSS 111, S5.5, my vehicle is required to display a rearview image that covers a 10-foot by 20-foot zone directly behind the rear bumper. Because the camera is pointing at the ground, it fails to provide the 'clear and reasonably unobstructed view' mandated by federal law. This is a safety non-compliance issue that Ford is obligated to rectify to bring the vehicle back into a street-legal state." • "Federal safety standards for rear visibility do not have an expiration date for manufacturing defects. A mounting system that allows a safety-critical camera to fail and point at the ground is a defect in material and workmanship that creates a safety hazard."Read more

While driving my 2013 Chevrolet Malibu (2.5L), I experienced a sudden and severe failure of the braking system. [Insert exactly what happened here—for example: 'The brake pedal suddenly went completely to the floor,' 'The brakes unexpectedly locked up,' 'The vehicle experienced a sudden loss of braking power,' or 'The brake warning lights illuminated right as the brakes failed']. This occurred w
... hile traveling at approximately 25 mph on a one-lane road. This total/partial loss of braking power created an immediate safety hazard and a high risk of a crash. The vehicle had to be shifted into manual, then safely coasted to a stop. No prior warning signs or brake issues occurred immediately leading up to this event.Read more
