There was a recall on the air bag system immediately after purchase 6/28/2025 and it was taken in July of 2025 to Mazda of Columbia, SC and it was “repaired.” The airbags of the truck that collided with the Mazda did deploy.

I am filing a safety complaint regarding my 2023 Kia Sportage Hybrid. The vehicle developed a “Check Hybrid System” warning and was diagnosed by a Kia dealership as having a failure involving the high-voltage hybrid system, specifically the MCU (Motor Control Unit). The dealership informed me that the vehicle may be unsafe to drive due to the possibility of fire risk associated with the high

The repair estimate was approximately $5,000, in addition to approximately $750 charged for diagnostics alone. The vehicle is reportedly out of warranty due to mileage, even though this appears to involve a major hybrid system component and a potential safety defect. This concerns me because: - the vehicle is a relatively new 2023 model, - the failure involves the high-voltage hybrid system, - Kia advised the vehicle may catch fire, - and independent mechanics generally cannot service or repair these proprietary hybrid components. I believe this may represent a broader safety issue or defect involving the hybrid system or MCU components in Kia hybrid vehicles. The touch screen display malfunctions and is often either unresponsive or it has ghost touches and moves across various screens while you are driving and during navigation. The system will make calls, activate voice assistance, move the map when navigation is active to where you can no longer see your route, switch between audio sources, open random apps, etc while driving leading to distraction.

This is dangerous while driving because it is hard to end calls, get back to your navigation, or focus on the road when voice assistants are activated randomly and difficult to turn off. There is not a reset button for this issue so it may continue the duration of your drive. The issue has been confirmed by a dealer with the suggestion to replace but online forums suggest this is a widespread issue and replacement does alleviate the problem. The vehicle has not been inspected by police or insurance. There were not any warning lamps, messages, or systems prior to the issue.Read more

I am submitting this complaint regarding a known exhaust manifold/flex pipe failure affecting my 2020 Subaru Ascent Touring (VIN: [XXX] ). My vehicle was diagnosed by a Subaru dealership with a “broken flex pipe on the header” causing excessive exhaust noise and exhaust leak concerns. Repair estimate was slightly over $2,000. ... her Subaru Ascent owners describing identical failures involving cracked exhaust manifold welds and failed flex pipe sections. Subaru has also issued TSB 09-106-23 involving exhaust manifold weld cracking and redesigned replacement components for this issue. Symptoms began in the low 70,000-mile range and progressively worsened before formal diagnosis at approximately 83,000 miles. This appears to be a widespread defect involving emissions/exhaust components. Many owners online report loud cold-start exhaust noise, cracked manifold welds, failed flex pipe sections, exhaust fumes/smells, and expensive manifold replacement repairs. My concern is both the widespread nature of the defect and Subaru’s handling of complaints regarding the issue. Initially, Subaru denied assistance because the vehicle has a branded title. After documentation was provided showing federally mandated emissions-related coverage/extensions were active for this VIN regardless of title status, the denial basis changed to mileage. After explaining symptoms began prior to the mileage threshold, Subaru then cited lack of dealership service history after my ownership despite the vehicle being regularly maintained by a local mechanic. I am requesting NHTSA review whether this exhaust manifold/flex pipe defect represents a broader reliability and emissions-related defect trend affecting Subaru Ascent vehicles. The attendant said they apparently didn't know about the recall until I returned the car. I find that hard to believe. I checked NHTSA's website a day after I returned the car [XXX] ), and it said that the Kia Telluride I rented had been recalled on February 23, 2026. The reason for the recall was because "THE DRIVER AND PASSENGER SEAT BACK FRAME ASSEMBLIES MAY FAIL TO PROPERLY RESTRAIN OCCUPANTS IN A CRASH". That's a terrifying thought. According to Avis's recall policy [XXX] they're not supposed to rent the car and they are supposed to notify me about the recall. Avis did neither of these things. Luckily the trip was accident-free and my daughter and I are physically fine. But I think Avis should be held accountable for allowing me to drive a rental car that may have had a higher likelihood of killing us in a crash. At the very least they didn't notify me in any way. They weren't even going to mention anything about the recall, and I had to ask about it after I saw what they wrote on the windows. I also submitted a review afterward (Avis sent me a standard email asking about my experience), and I explained my frustration about them renting me a recalled car. Avis replied, but it was just a standard apology and they didn't say anything about the recall. All around a terrible experience. ... Pleasant, Pennsylvania, on or about May 20, 2026, for a turbocharger failure covered under GM Special Coverage Bulletin N232395330. The vehicle had recently passed Pennsylvania safety and emissions inspection on April 30, 2026. The turbo-related diagnostic code appeared on or about [XXX]. Crivelli completed the minimum turbocharger warranty repair but then informed the customer that additional intake/manifold-related repairs were needed and would not be covered. The customer disputed this because the additional failure appeared during the same repair sequence and appears directly related to the covered turbocharger failure. A written request was faxed asking Crivelli to review the invoice, compare the repair to GM’s covered parts list, and submit the related intake/manifold issue to GM for collateral coverage, goodwill review, or warranty consideration. Crivelli did not respond to the fax notice and did not return the customer’s calls. When the customer attempted to follow up with the service department, Scott from the service department refused to review the concern, refused to provide further service, and refused to contact GM regarding the related warranty issue. Based on the conduct, tone, refusal to review the documentation, refusal to escalate to GM, and refusal to provide equal service, I believe the customer was denied fair consumer service because of race.The customer is not asking for special treatment. Because the Pacifica Hybrid disables operation while a charging connector is detected, the vehicle became completely undrivable and required towing. Multiple AAA tow operators were unable to move the vehicle normally. We ultimately had to personally research and guide the tow operators through the manual neutral override procedure in order to get the vehicle into neutral and onto the tow truck. The dealership has suggested that the connector may have fused or arced together during charging and is attempting to characterize the situation as “not the vehicle’s fault.” Regardless of the precise electrical mechanism, this event occurred during ordinary intended use of the vehicle’s charging system. The end result was that the vehicle trapped the connector, immobilized itself, and stranded the owner without any meaningful owner-accessible recovery method. This is not ordinary wear and tear or a cosmetic inconvenience. This was a charging-system immobilization event involving a vehicle platform that already has a documented history of charging-system recalls and software-related charging concerns. The vehicle was being used exactly as intended. No misuse, modification, or improper operation occurred. A vehicle shouldn't become completely inoperable during routine charging. I am requesting review of this matter as a significant charging-system failure and requesting goodwill assistance regarding the substantial diagnostic and repair costs now being imposed on the customer for an event that occurred during normal operation.Read more

Despite keeping the brake pedal fully depressed, the vehicle did not decelerate, resulting in a front-end low-speed collision with the metal rear bumper of the semi-truck. Because the collision occurred at a very low speed, the operator of the commercial truck was unaware of the impact and safely continued driving forward without stopping. The vehicle's brakes entirely refused to re-engage until I frantically pumped the physical brake pedal and cycled/pressed vehicle control power switches. This action seemingly forced the electronic braking modules to reboot or re-establish hydraulic pressure, allowing the vehicle to finally come to a stop. The vehicle was subsequently removed from the active roadway. This is a severe, life-threatening safety defect involving a total loss of electronic brake-by-wire and friction braking application without any mechanical override intervention, presenting an extreme hazard. The vehicle has been towed directly to an authorized Toyota dealership for a comprehensive electronic diagnostic pre-scan of the Skid Control ECU, Electronic Parking Brake system, and 12-volt auxiliary power systems to retrieve the stored DTC fault logs and freeze-frame data.Read more

This created a serious safety concern because shattered glass could have entered the vehicle while driving at highway speeds and could have caused distraction, injury, or loss of visibility/control. The vehicle has been inspected by a Hyundai dealership. I was informed the panoramic roof assembly may require replacement. I am filing this complaint because the failure appeared spontaneous and may indicate a defect or structural stress issue with the panoramic roof glass. Complaint: This vehicle exhibits a systemic, progressive factory paint defect — excessive peeling and bubbling of the white finish that spreads from one body panel to the next. ... Extension Z05 (TSB 25-BD-004H), which extends paint coverage to 10 years/unlimited miles for affected white-paint vehicles including the 2019 Santa Fe. Hyundai repaired the hood under this warranty in 2025, during which the vehicle was out of service for 77 consecutive days. Since that repair, the defect has migrated to the adjacent fender and has also appeared on a door, demonstrating that it is spreading across the vehicle. Hyundai has offered to repaint only the single currently-visible panel rather than addressing the systemic defect. I am filing this complaint to document the defect and its progression, and to support the public record regarding factory paint failure on these vehicles. No crash, injury, or fire has resulted. Corporate was immediately notified of this severe safety defect (Case No. 16372717. However, Honda Corporate has explicitly refused to send an engineer or inspector to evaluate the vehicle's electronic or mechanical logs, and they also refused to provide their denial statement in writing. My 2019 BMW X3 equipped with a B46 engine developed a significant coolant leak at approximately 80,000 miles. The vehicle displayed a low coolant warning, and coolant was visibly leaking underneath the vehicle. Around the same time, I began noticing excessive white exhaust/steam during cold starts and, for the first time since owning the vehicle, had to add engine oil between scheduled oil changes
... . The vehicle is currently undergoing inspection by an independent repair facility. Preliminary findings indicate concerns involving premature failure of critical plastic cooling system and/or oil filter housing components. Based on my research, these failures appear consistent with widespread complaints involving BMW B46/B48 engine cooling system and oil filter housing defects. I am concerned this defect could lead to sudden coolant loss, overheating, engine failure, or unsafe driving conditions. Prior to the coolant warning appearing, there were no significant alerts indicating a major or imminent cooling system failure. The apparent failure of critical plastic engine cooling components under normal driving conditions raises serious concerns regarding the durability and safety of these parts. I have photographs documenting the coolant leak, as well as detailed notes and diagnostic information from the repair facility, available upon request. We tried to get this fixed at our local dealership, they told us that it was under the 10y/100k miles warranty, we have 66k on the van. The evaporator for the air conditioning failed. BMW used an aluminum evaporator with yf coolant. Those combined cause the coolant to eat away the evaporator. Once the yf comes in contact to the air it turns toxic. This shouldn't fail at 82,000 miles.

My 2026 Hyundai Tucson is experiencing a stuck update loop for the 2nd time since I purchased it new from a Hyundai dealer on February 2, 2026. The first stuck update occurred about Feb 18, 2026. A case was opened with Bluelink and it was taken to the deal for diagnosis on March 26, 2026. The update loop was cleared by the dealer’s service department in mid April. I am filing this complaint because both times this occurred Verizon’s internet WiFi service was terminated because the provider couldn’t authenticate their service token and that this becomes a safety issue due to the fact that the Automatic Crash Notification and the SOS Emergency Call button are both disabled until the dealer flashes the system software and reestablishes a clean network handshake. The first attempt to correct the issue took about 3 weeks. Currently, I are waiting for an appointment to be made with the deal’s serve department so that they can flash the software again and clear the second stuck update loop. In the meantime, the above mentioned safety features are disabled. I fear that this stuck update issue will be never ending and I will be constantly returning to the deal to have the software flashed and the safety features will be disabled until they do so. All that said, you are aware of the reason why this occurs but continue to allow the manufacturer to flash the software and apply a bandaid. I am referencing NHTSA TSB #25-01-024H (Service Campaign 9C1). I would like to encourage NHTSA to reconsider their position on this ongoing network failure as more than a technical service communication issue but, rather, as an emergency safety recall as many Hyundai customers are experiencing this same stuck software update loop and loss of vital safety features.Read more

2024 BMW X1 xDrive, 8000 miles, original tires. Front right tire wear to 2.7mm, front left tire wear to 4mm, and rear tires wear to 5mm. AWD vehicle where fronts should not be significantly outpacing rear wear. Left-right asymmetry on front axle suggests alignment issue. Dealer and BWM North America refused in investigation and call it normal. Requesting NHTSA agent review and case escalation.
