On [XXX], I was involved in an accident in my Toyota Camry while the vehicle was stopped on the freeway [XXX]. The car suffered an impact at approximately 20 MPH or less. My friend [XXX] was driving with my permission while I was a passenger, and our two dogs were also in the vehicle. We were rear-ended by another driver, which pushed us into the vehicle in front of us, causing both rear and fro
... nt impacts. The vehicle was totaled. During the collision, the airbags did not deploy. Immediately after the accident, the vehicle displayed an “SRS Airbag Malfunction – Visit Your Dealer” warning message. I suffered whiplash and nerve-related injuries. I am concerned the airbag/SRS system did not operate properly and may have failed during the accident, potentially increasing the severity of injuries. I am requesting an investigation into the failure of the SRS/airbag system. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

My 2024 Used Toyota Grand Highlander Hybrid Platinum Max was at a slight decline stopped at a stop sign in Echo mode. The whole car began to violently shake and lurching forward. No one was in front of me and my foot was on the brake or I probably would have run into a car in front of me. The dashboard then flashed a warning about the hybrid system overheating. The only way to stop it was to t
... urn off the engine, then turn it back on. The car then began driving normally. I have also had an issue with hesitation felt while accelerating 0-40 mph with study throttle. Upon researching both issues, they should be resolved with TSB calibrations to my car of T-SB-0028-25 and T-SB-0017-24. This is not an error that would come up when doing a diagnostic check on my car's computer per research I have done. I took my car to get the updates at Toyota of Redlands, CA, and they refused to do them unless they could recreate them themselves. I called Toyota to report this, and they said that each Toyota dealership/service is privately owned at their own discretion. I was basically told to return if I experience it again. I do not want to experience this again and they wouldn't take my word on it anyways. I feel like next time I could injure myself or someone else if this glitch happened again. I therefore would like to implore to make this a mandatory preventative TSB calibration for cars that have had this reported/models/year which mine has. This goes for any TSB calibrations should be done preventatively, but especially ones that can cause a potential accident like T-SB-0028-25 and T-SB-0017-24. Here is information on why this would not show up on a diagnostic test: Your Toyota didn't generate an error code because the shudder is a software calibration issue rather than a component failure. The onboard computer (ECU) only triggers a check engine light or throws a Diagnostic Trouble Code (DTC) when sensors detect a hard mechanical or electrical failure.Read more

All o

We have to wait for it to decide to stop and fix itself. It is a safety issue because You are driving and randomly volume goes to full blast an startles you, cannot hear what is going on around you, and hurts your ears. This is distracting and I have swerved my car because of it. Bluetooth capability is suppose to be handsfree, so if it keeps muting my call or switching to my phone, that is now a safety hazard as well. vehicle was inspected in 2021 by Mazda dealership. At that time they said the only thing they can do it remove the SIM card, and we would lose navigation. However the issue did not stop. It would cost us about $1000+ between labor and parts and inspection to get this fixed. The vehicle experienced failure of the HVAC heater flap/blend door mechanism on the driver’s side. Specifically, the plastic gear assembly broke at its mounting point, causing the gear to spin freely and preventing the airflow door from opening and functioning properly. As a result, the driver’s side heating and airflow system no longer operated correctly. The failed component is available fo
... r inspection upon request. Because the manufacturer does not sell the small gear component separately, I was forced to obtain a custom 3D-printed replacement part in order to restore operation. This defect created a safety concern because the inability to properly control heat and airflow can affect windshield defrosting and cabin visibility during cold or wet weather conditions. Reduced defrost performance can impair driver visibility and increase the risk of an accident. The problem was confirmed during inspection and repair. Accessing the failed gear required extensive disassembly of the vehicle’s front dashboard area and HVAC system components. The repair process was extremely labor-intensive for what is essentially a small plastic gear failure. Prior to complete failure, the HVAC system began making abnormal clicking/grinding noises and airflow operation became inconsistent. Eventually, the gear mounting point completely failed, causing the actuator gear to spin freely without moving the heater flap. This appears to be a common problem affecting Nissan Rogue vehicles, particularly involving the weak plastic HVAC gear and mounting design. The manufacturer does not offer the individual failed gear as a replacement part, which unnecessarily increases repair complexity and cost.Read more

Around 100,000 miles, my vehicle began experiencing intermittent hesitation, sudden loss of power, and frequent stalling. Prior to the complete failure, the vehicle would occasionally hesitate during acceleration and feel like it was losing engine power while driving. Over time, the problem became more frequent. One day, while driving approximately 50 mph on a side road, the vehicle suddenly stal
... led without warning and completely lost power. I was luckily able to avoid an accident, and no one was injured, but the sudden shutdown created a serious safety hazard and significantly increased the risk of being rear-ended by other vehicles. The vehicle continues to stall frequently and hesitate during operation, making it unsafe to drive. The failed component was confirmed by Nissan. The dealership diagnosed the issue and recommended replacement of the defective part for over $900. However, a Nissan mechanic explained that the repair only involved removing and replacing the common problem, which is the Mass air flow sensor, secured by two screws. I replaced the component myself for approximately $90, and the vehicle temporarily improved. Unfortunately, the stalling, hesitation, and loss of power have returned and now occur constantly. The failed component should still be available for inspection upon request. The issue was confirmed by Nissan personnel, although I completed the repair independently rather than paying the dealership repair cost. Prior to the failure, the vehicle showed symptoms including hesitation, intermittent power loss, and stalling beginning around 100,000 miles. These symptoms worsened over time until the vehicle completely shut down while driving. This appears to be a common issue affecting Nissan vehicles and creates a serious safety concern due to the risk of sudden engine shutdown while driving.Read more

On Friday, December 5, 2025, I was coming out of the driveway at the Jack In The Box on the corner of El Camino & Nobili Avenue in Santa Clara. My foot was on the break as I was going to stop at the end of the driveway to check to see if there was any traffic coming down Nobili before I exited the Jack In The Box driveway. All of a sudden before I even hit the end of the driveway while I had my fo
... ot on the brake, the car started to speed up (unintentional acceleration)and I continued to press my foot down harder on the brake, but nothing could stop it. A lot of black smoke came up out of the steering wheel area where it attached to the dashboard & all of the air bags around the car deployed. Then my car completely turned itself around and the front of my car was now facing the driveway that I had just come out of. This torque of acceleration and braking is why my car spun around 180 degrees. The vehicle had sustained damage to the front end, the hood, front bumper cover, right front head lamp was cracked, and the Toyota emblem was missing. (See attached photos taken at the time of the accident) Since all of the air bags in the whole car had completely deployed, I found that the only way I could get out of the car was to slide myself under the air bag that surrounded the driver side door. As I got out of the car, the rib that is directly under my right breast began to hurt very badly and it was hard for me to take a breath with the pain from that rib. I also had a bad headache, too. Plus, the top of my left hand was all red and felt like something had scratched the whole back of my hand. It hurt to touch it. I was also light-headed.The car, I was told, looked like I had hit a wall. The whole left front side of the car was severely mangled - the hood, the fender, and the tire was flat. The Auto Idle Stop (AIS) system malfunctions during normal driving. When coming to a stop, the AIS shuts off the engine as designed, but the vehicle then fails to restart when the brake is released. The vehicle shifts itself into Park automatically without driver input, either before or at the same time as a dashboard message appears instructing the driver to depress the brake pedal and press the
... start button. When the start button is pressed, the dash resets but the engine does not start. The only way to recover is to fully power the vehicle off and restart it, which is difficult and unsafe at a busy intersection or red light. This puts the driver and others at serious risk. The vehicle becomes immobilized in traffic with no ability to move it out of the path of oncoming or following vehicles, creating a risk of being rear-ended. The unexpected automatic shift into Park is also a vehicle control concern, as the driver does not initiate it. The problem is intermittent. The vehicle has been taken to an authorized dealer twice. On the first visit the dealer kept the vehicle several days with a loaner and attempted to replicate the issue but could not. On the second visit the dealer kept the vehicle one day and again could not reproduce it. A third diagnostic visit is scheduled, during which the service manager intends to drive the vehicle to attempt replication. As of now the failure has not been confirmed or reproduced by the dealer. The vehicle is available for inspection upon request. No warning lamps appear prior to the failure. The only symptom is the automatic shift into Park and the dashboard message to depress the brake and restart, which appear at the moment of the malfunction. The first occurrence was at approximately 25,000 miles. The vehicle has not been inspected by the manufacturer, police, or insurance representatives.Read more

There should be a recall put out from Stellantis about this known common issue. I've called Stellantis and they said on the phone that they’ve created a case and that was it. I truly believe Stellantis is running a scam and just trying to rake in more money and having customers order $700 parts when they may just need a $30 spring replaced. This rattling noise with the active exhaust for SRT Dodge Chargers is a very common issue and can easily be searched online, but no recalls or warnings have been put out from Stellantis about it. After the newest software update, the Tesla Model Y FSD now shows a pop-up menu every time you disengage full self-driving. The pop up menu is black, so it contrasts with the white screen and causes a visual distraction. Additionally, the pop up menu requires you to make a selection in order to make it disappear - there’s no way to swipe it away (which would still be problematic while driving).
... Because it’s mandatory, and it is located on top of the volume controls for music/podcasts/etc, you are unable to pause auditory stimulation such as music/podcasts. This happens at the exact moment you need to focus most: when a traffic situation is so dicey, disabling FSD is required to ensure safe driving. This is an absolute safety concern. I tried to disable the feature through Tesla’s support services, but that option is not available. This is a serious safety concern, and it’s frankly egregious that Tesla is allowed to do this. The pop-up menu does not benefit drivers in any way - it’s clearly there for Tesla to collect data to improve their FSD product. They need to find a way to collect that data without impacting the safety of the driver and other vehicles on the road. Furthermore, it’s not even good quality data collection - the variables they provide in the pop-up menu are vague and do not come with definitions. Their quality system needs tighter review.Read more

This model lacks a well-tested turn signal lever, replacing it with arrows on the steering wheel. This method of activating turns signals is fundamentally unreliable and consequently dangerous. As an owner of the subject Tesla for just over one year, I find that I regularly fail to achieve correct activation, even with repeated attempts to do so. The steering wheel control may: Not activate at
... all, even with multiple attempts to activate the signal. I find no consistent patter as to why pressing the arrow or vicinity of the arrow fails to achieve activation. Worse, may activate in the opposite direction to the pending movement. This is the most dangerous situation, misleading other drivers with regard to the intended movement. It’s also not apparent to me that the opposite direction has been activated. The problem is made worse by: Lack of tactile feedback from the controls. The switches do not move perceptibly, nor is there any other feedback, such as a click. The ridge between left and right arrows doesn’t provide sufficient indication of correct or incorrect placement of the thumb on the controls. Pressing on the controls within the vicinity of the arrows may or may not result in activation at all or activate the opposite one. The position of the control varies depending on the steering wheel angle. The control may be very difficult to access, particularly where a sequence of turns in opposite directions must be executed. Specifically, it is essentially impossible to activate for the second movement while making the preceding turn. I understand that Tesla recognizes the problem and owner dissatisfaction in that it offers a retrofit with a conventional and well-tested turn signal lever for $600. Tesla must be compelled to recall these vehicles and retrofit them with a conventional turn signal lever or other effective control at no charge to owners.Read more

It also freezes up and glitches which then makes the reverse camera not in real-time causing a minor accident. The screen sometimes turns black and sometimes blue. Secondly the trailer towing lights don't work properly and say that the trailer disconnected or not working properly. We tow or m
... ove many vessels at work and it is not the trailers. It is the truck. I am sure I saw a recall ref the trailer plug but now it shows that there are no open recalls or issues. Again that is another safety issue. I have a ford bronco that had a reverse camera and display issue and it has a active recall on it. This f250 experiences the same issues. I don't know why it is not recalledRead more

2022 GMC Canyon AT4 Crew Cab. After the vehicle is shifted into Park and the engine is shut off, the ignition key cannot be removed from the cylinder. The key is physically locked, the steering wheel will not lock, and the vehicle cannot be reliably secured. The only workaround is to restart the engine and cycle the shifter repeatedly until the interlock releases the key. Recurring. Safety implic
... ations: operator cannot reliably power the vehicle off in an emergency, cannot lock the steering column, and may be forced to leave an unattended vehicle in accessory mode — fire, electrical, and theft risk. The defect is documented in GM's own literature. GM Bulletin PI1429 and GM TSB 19-NA-206 address this exact symptom on the Canyon/Colorado platform. NHTSA-archived GM TSB MC-10113734 (March 2015), "Ignition Key Cannot Be Removed and/or Does Not Rotate to the 'Off' Position," names the 2015 GMC Canyon and Chevrolet Colorado. The 2022 Canyon uses the same shifter and ignition-interlock architecture. Documented cause: failure of the transmission control lever microswitch that signals "Park" to the interlock solenoid. Documented fix: replace the shifter assembly. Reported to an authorized GMC dealer while still under the New Vehicle Limited Warranty. Dealer recorded the concern verbatim — "key stays locked in ignition when shifting to Park" — and closed it "unable to duplicate," no repair, despite the open bulletins. A second GMC dealer later refused to inspect, with the service advisor stating on the service drive that there is no fix, that they see this on Canyons and Colorados all the time, and that he had seen another such vehicle the day before. No diagnostic, no repair order. GM's Technical Assistance Center has since taken the position that the bulletins above do not apply to this vehicle, which is inconsistent with the bulletins themselves. No crash, no injury, no fire. Reporting a manufacturer-documented ignition-interlock defect GM is refusing to repair.Read more

When you driving at any speed but mainly when the vehicle is traveling at least 45 mph or more. When you have to abruptly stop, occasionally you will hear a bike horn sound. I have no clue what's causing it and/or where the noise is coming from.
