At approximately 41,000 miles, approximately a month ago, water began leaking through the front edge of the sunroof despite the sunroof never being opened prior to the issue. These issues created a significant distraction and potential safety hazard, especially during night driving. The dealership diagnosed the issue and confirmed that the repair required modification of the sunroof assembly and track to correct a water intrusion problem. This was not a routine maintenance issue. The total cost of repair was $1,259.18, which I paid out-of-pocket. Additionally, the service advisor at the dealership stated that this is a frequent problem seen on Audi Q3 vehicles and expressed surprise that it has not already been subject to a recall. Given that this issue involves water intrusion affecting electrical components and actively interfering with safe vehicle operation, I believe this may represent a broader design or manufacturing defect impacting vehicle safety. I have requested that Tesla inspect and complete the following federally mandated safety recalls and service bulletins, none of which appear in my vehicle’s service history: • NHTSA Recall 21V-835 /

Tesla’s response does not address that these are federal safety recalls, not warranty repairs. Federal law requires recall repairs to be performed free of charge regardless of warranty status, age, or mileage. Tesla has also failed to provide written documentation confirming whether these recalls were completed prior to selling me the vehicle. Instead, I have been redirected to a self-service VIN lookup tool and a general data privacy request form — neither of which confirms when, where, or whether the recall work was actually performed. On Thursday, May28 I was driving my 2025 Ford Mach-2 on Schrock Rd. in Columbus, Ohio at approximately 3:30 PM when the hood (the "frunk") of the car unexpectedly unlatched and opened, flying up to a vertical position and obscuring my view of traffic. I was away from home when this occurred. When I got in the care to drive home, there was a warning that came on, for the first time, that read as follows: FRUNK FAULT DETECTED; SERVICE REQUIRED. There was no advice to avoid driving the car. I checked the hood/frunk to be sure that it was firmly closed, then started for home, avoiding my usual freeway drive at 65mph for a slower trip on local roads. It is a good thing I did, because the risk of a major accident with injury and serious property damage would have been far greater at higher speed. I arrived home without the frunk opening again, called for Ford Roadside Assistance, and arranged to have the car towed to Germain Ford of Columbus, where it still is, awaiting the delivery of a new frunk latch system. The dealer has determined that there is likely a defective sensor in the frunk latch. This is a known issue with the Ford Mach-e, affecting not only the frunk but also sometimes the hatch lid in the rear.This is a serious safety hazard, and a review of reports over several models years from Mach-e owners indicates that I am not alone in having experienced it. At highway speeds a suddenly unlatched frunk/hood would not only open to the vertical, but conceivable also be smashed against the windshield or ripped off the vehicle and become airborne. The number of reported incidents suggests this may be a widespread design or manufacturing issue rather than normal wear and tear. I believe this condition presents a safety risk because water can damage electrical systems, airbags, sensors, and other critical vehicle components. Owners should not be expected to repeatedly repair a drainage system that appears prone to clogging and causing interior water damage. I am submitting this complaint regarding a catastrophic CVT transmission failure in my 2019 Honda HR-V at approximately 97,500 miles. I incurred significant expenses for towing, diagnostic fees, transportation, lodging, and related costs due to the breakdown. After diagnosis, I was informed that the CVT transmission had failed and required replacement at an estimated cost of approximately $10,000. I later learned that Honda had issued Service Bulletins 21-046 and 21-047 concerning CVT belt deterioration and transmission issues in 2016-2020 Honda HR-V vehicles. These bulletins acknowledge that an incorrect PCM value could cause excessive pressure on the CVT belt, potentially leading to partial or complete belt breakage and sudden loss of acceleration. Honda also extended warranty coverage on affected vehicles due to premature CVT belt deterioration. My vehicle falls within the affected model years and experienced symptoms consistent with those described in Honda’s service information. Despite this, Honda denied my request for goodwill assistance, even though the failure occurred at relatively low mileage for a modern transmission and only shortly after the warranty extension period expired. I am concerned that this transmission defect creates a safety risk because it can result in sudden loss of acceleration or vehicle propulsion while driving, potentially placing drivers and passengers in dangerous situations. I respectfully request that NHTSA investigate the CVT transmission failures affecting Honda HR-V vehicles and determine whether additional action is warranted.Read more

I am reporting a severe saftey and crash-compatibility defect on the front passenger side of the 2026 Subaru Outback Premium trim. This vehicle is equipped with a fixed-height passenger seat that is positioned unsafely high relative to the headliner and sun visor assembly. Under standard deployment of the vehicle brakes, forward occupant kinematics cause the passenger’s forehead to travel collide into the rigid, unyielding lower edge of the sun visor, which act as a hard/sharp parts cut the forehead and twisting the neck seriously. During a recent braking event, this lack of structural clearance caused a direct head-strike incident. The occupant’s forehead collided with the sharp bottom edge of the visor, acting as a blunt knife edge that resulted in a laceration to the forehead and a hyper-extension strain injury to the neck. This is a critical design failure where occupant safety clearance envelope was poorly designed for this elevated fixed-seat configuration. In a high-speed collision, this layout poses an imminent risk of severe facial trauma, skull fractures, or catastrophic cervical spine injuries from the visor before the seatbelt pre-tensioner can fully lock. This requires immediate investigation and a mandatory safety recall to provide height-adjustable/lower seat frames to affected vehicles.Read more

Kia Consumer Affairs denied warranty coverage under the federal emissions warranty despite the vehicle being within the 8 year/80,000 mile coverage window at approximately 58,309 miles. No written explanation for the denial was provided. This failure appears to be a known pattern on this platform based on owner reports. The battery chiller and inverter coolant system are integral to battery thermal management and should be covered under the federal emissions warranty. My wife was driving at about 40 MPH when the instrument panel displayed a warning "System Failure" and the digital speedometer stopped working and showed the current speed as two dashes instead of a number (ie "--" instead of "40"). After about 5-10 seconds the warning went away and the digital speedometer appeared to start displaying the correct speed again.

On September 16, 2025, I was operating a powered industrial forklift inside Trailer No. [XXX]— a 2017 Great Dane refrigerated semi-trailer, VIN [XXX] — during routine dock-loading operations at [XXX] of the T.G. Lee Dairy / DFA Dairy Brands Fluid facility, [XXX] . The trailer was detached from its tractor, stationary, dock-locked, and wheel-chocked in the standard configuration for loading. W
... ithout prior warning, visible deformation, or any indication of structural distress, the right front K-bracket supporting the trailer’s JOST International Corporation landing gear failed structurally. The right landing gear leg collapsed, the trailer tipped to the right, and the front right nose of the trailer impacted the ground. I was thrown against the interior wall of the trailer and sustained cervical, thoracic, and lumbar injuries resulting in days away from work and ongoing medical treatment. The landing gear system installed on this trailer has been identified as a JOST International Corporation product (Grand Haven, Michigan). Identification is confirmed by: (1) the repair invoice of J-KO Mobile Semi Trailer Services, Inc., dated September 19, 2025, reflecting fabrication and installation of a replacement K-brace and identifying the trailer make as Great Dane; (2) the fleet operator’s internal communications identifying JOST landing gear in connection with Trailer No. [XXX] and the incident; and (3) written acknowledgment by JOST International Corporation’s legal counsel dated May 21, 2026, that the trailer collapse resulted from a failure of the JOST landing gear. Prior to the failure, the subject trailer was inspected twice on the date of the incident Driver Vehicle Inspection Reports completed at 3:30 AM and 10:55 AM on September 16, 202 both of which reflected “No Defect” for the landing gear. There is a promotional pop up which shows from Sirius XM, it appears several minutes into driving, while moving. This pop up advertises a deal on the subscription and is not only distracting, but it disables any other functions on the head unit including navigation assistance, drive modes and climate controls. While driving the LCD screens would go blank intermittently. Sometimes staying off for 30 secs or more. I could still hear sound and could control volume. This has happened on two different occasions while traveling to work. I have a 35 minute commute on interstate highways.
