I feel Tesla has introduced two safety issues with a recent version of software relating to the "Full Self Driving (Supervised)" feature. Issue #1. ... D. This dialog box does not dismiss by itself. The only way for the user to dismiss it is to tap one of the 4 options presented or leave a voice message. This is an unnecessary distraction that in no way is required for the user to operate the vehicle. Tesla is mandating feedback and introducing distractions when users take over control of the vehicle for their sole benefit. It is also akin to a penalty for disabling FSD, as it a generally negative experience for most people. Issue #2. Tesla has introduced a mandatory "FSD Streak" counter. This number appears on the smaller instrumentation screen for the S & X models. The counter counts the number of miles consecutively traveled by FSD. When a driver disables FSD for any reason, the counter resets to 0. The gamification of keeping FSD enabled is a major safety issue. Some people are highly susceptible to increasing this number as high as possible as if it were a video game. In situations where they should likely disable FSD and make manual control, there are inevitably users that will hesitate or maybe even not take control at all because they do not want to reset the counter. Tesla does not have any options to disable or remove this counter, so even people who know they are susceptible to this cannot remove it. I took my vehicle to a land rover dealership based upon 2 service bulletins issued by the manufacturer N768, sfcc_apr2022_17 and NHTSA item number 10211119. The Land Rover dealership refused to service the vehicle for those bulletins. I have a heart condition with 20% ejection factor. The dealer and manufacturer are not flashing or upgrading the telematics to modern standards which can be done easily by installing new software and/or changing out the telematics unit for 4G/5G compatible units and/or reprogrmming older units. As a result drivers with medical conditions are being put at risk if a medical emergency arises because the car cannot communicate to first responders as to its location to help an incapacitated driver.Read more

There were absolutely no warning signs and I immediately turned the car back on with no issues at all and drove normally after that (other than the fact I was scared to death about what just happened). The second 2 times have been when the car is on park. It is extremely random and my family and I fear for our life and safety. I ignored it after the first time that we took it in and they said nothing was wrong after running a diagnostic, but it has happened 2 times after that. I took it back in 2 days ago and they still found nothing wrong after doing the diagnostic. I am in “contact” with a regional affairs specialist but if you don’t answer them the first time there’s no way to get back in contact with them because they DO NOT answer your call at all. I called the general line and they said to wait until the 17th, which is almost in a WEEK. I know in my heart that I’m not lying and there’s something severely wrong with this car. I don’t even want to sell it because I don’t want to put other people’s safety at risk and feel responsible for that. I need an engineer an actual specialist someone who can help.Read more

The dealer has been unable to provide a confirmed diagnosis after multiple days, citing either the compressor or heating core as potential causes, but has not committed to a repair timeline or parts order. Of additional concern, the dealer has suggested 'outside influence' then said on the 4th say it maybe mechanical failure, but we have to check more of the compressor and heating coil as a possible cause despite my vehicle having no accident history, no modifications, and being only 4 years old. This appears to be an attempt to deny a valid warranty claim without evidence. I have an open case with Kia Consumer Affairs, however corporate has deferred entirely back to the dealer with no resolution or timeline. I am currently incurring out-of-pocket rental car expenses due to the dealer's backlog of loaner vehicles. I am requesting that the NHTSA investigate this matter. A complete AC system failure on a nearly new vehicle with no external cause raises serious concerns about manufacturing defects in the 2022 Kia Forte AC system. ... low-speed traffic transitions. It is worse on cold starts. I have experienced sudden unintended deceleration while attempting to enter traffic. This vehicle is covered under Safety Recall 25TA14 / NHTSA 26V320 ("Engine May Stall During Driving"). The TSB remedy was applied at my dealer on June 11, 2026. The remedy did not resolve the condition. I have OBD-II logs proving the defect persists post-remedy. Using Torque Pro at 10 Hz, I captured 55 instrumented hesitation events on June 11 alone -- 18 on the drive to the dealer before the TSB, and 37 after leaving. The worst single measurement shows a 1.7-second delay between pedal input and throttle blade response. My full dataset spans 131 events across multiple drives. Highway driving is completely normal throughout; the defect is exclusively a low-speed tip-in condition. The dealer has been unable to duplicate the condition across five repair order presentations since February 2025. The vehicle spent 105 days in dealer possession with no repair performed. This significantly reduces rearward visibility while backing up and increases the risk of striking pedestrians, children, pets, vehicles, or objects that may not be visible through mirrors alone. Drivers have come to rely on the backup camera system as an important safety feature, and its sudden failure creates a safety hazard during parking and low-speed maneuvers. The problem was diagnosed and confirmed by an authorized Nissan dealership. The dealership determined that the Around View Monitor (AVM) control unit had failed and advised that the module requires replacement. The vehicle has not been inspected by police or insurance representatives. Nissan corporate has not yet physically inspected the vehicle to my knowledge. The backup camera system functioned normally until it suddenly stopped working. There were no warning lights, messages, prior malfunctions, collisions, or damage that would have indicated a problem. The failure occurred without warning. I purchased this 2023 Nissan Pathfinder only one month ago, and the AVM system suddenly failed after just a few weeks of ownership. The dealership diagnosed a failed AVM control unit and quoted approximately $3,400 ($2800 parts and $600 labor) for repair. A critical rear-visibility safety system should not fail unexpectedly in a vehicle this new. The repair cost is prohibitive, leaving us owners without an important safety feature. The affected component is the turn signal stalk switch pack (multifunction switch), available for inspection. Diagnostic scanning confirmed DTC B124C stored in the SCCM. Based on my research this code indicates a Turn Indicator Stalk Switch Pack fault. The safety risk is threefold: first, a stalk that does not self-cancel sends false signals to other drivers. Second, manually forcing the stalk back to center diverts driver attention from vehicle control. Third, when the stalk releases under applied force it does so suddenly and without warning, and if the driver's hand is on the steering wheel this can cause an involuntary steering input, risking unintended lane departure at highway speeds. I am the second owner, having purchased from a dealership at approximately 60,000 miles. The defect was present at purchase and has worsened progressively to approximately 76,000 miles. The vehicle has not been inspected by a dealer as it is outside warranty. No warning lamp is present; however DTC B124C is confirmed via diagnostic scanning. The component that is failing is the Radio. The radio consistently turns off when using it and power cycles itself back on which creates a concern when I am using the phone or the Maps feature. I had the same issue with a 2019 Volkswagen Passat, that I previously owned and I now see that there are known issues with other Volkswagen vehicles for the same thing, but there is no recall on my vehicle.

There are no warnings such as lamps or anything else prior to the failure happening besides if the radio has sound coming through it, you won’t hear the sound anymore and within 60 seconds it will power cycle down for about a minute and then come back on and have to completely reload. When it fails, the entire screen goes dark and I lose the backup camera and surround-view cameras, all HVAC/climate controls, audio, navigation, and most secondary controls. During a ride-along at Edmond Hyundai on [XXX], a technician directly witnessed and confirmed the occurrence. Despite this confirmation, it was not documented on the repair order. Two prior repair attempts returned cannot-reproduce results: March 2, 2026 at Bob Howard Hyundai and March 10, 2026 at Edmond Hyundai. Additional Repair/Inspection Attempts: On April 7, 2026, Norman Hyundai opened and closed a repair order without the consumer's knowledge and refused to provide a copy despite an explicit request. The noise itself could have caused a crash had I been driving. There was no warning about this, and when I called the dealer, they said they hadn't heard of it. I have two case numbers filed with Kia America about this issue. I have taken it in for servicing at a Kia dealer as well. They can't reproduce the problem and are not clear on what it is, understandably. They changed some settings on the car the last time I took it in (June 2025) and said to bring it back if it continues. I asked and they said there were no recalls about such an issue.Read more

Safety Risk: Immediate safety hazard from extreme environmental exposure. In regional desert temperatures exceeding 115°F-118°F, the complete loss of cabin cooling creates a severe risk of heat exhaustion, dehydration, and heat-induced physical impairment for the driver while operating the vehicle on public roads. Additionally, during high humidity, the system emits a chemical-smelling mist/vapor from the vents causing an acute burning sensation in the driver’s eyes. 3. Reproduced/Confirmed: Yes. Right Honda (Scottsdale, AZ) inspected the vehicle and confirmed the evaporator leak on 07/11/2025. I have also observed the chemical mist/vapor emission, which was corroborated by a witness while driving in California. 4. Inspected by Others: Reviewed by Right Honda service department. Escalated to American Honda Motor Co., Inc. (Case #16391364). Despite 5 years of ownership and previous failures in the same AC loop, the manufacturer refuses to cover the repair of this systemic defect. 5. Warning Lamps/Symptoms: No dashboard warning lamps. Initial symptoms began at ~59,857 miles with a gradual decline in AC performance, degrading to total loss of cooling at idle. The rear air conditioning refrigerant lines on a 2019 Toyota Highlander SE failed due to excessive rust/corrosion, causing the A/C system to leak refrigerant and stop holding a charge. The dealer recommended replacing the rear A/C lines/cooling lines as a set, evacuating and recharging the A/C system, adding dye, and checking system operation. The repair was completed by the Toyota dealer but not covered or reimbursed. The vehicle/component was inspected by an independent service center and by a Toyota dealer. Toyota corporate customer support was also informed of the dealer diagnosis. The failed component may no longer be available for inspection because the repair has been completed, but repair records, dealer diagnosis, invoice, and related documentation are available. Safety concern: The A/C system failure can affect occupant safety and visibility because air conditioning is used for windshield defogging/defrosting and cabin humidity control. Loss of A/C can reduce the ability to clear the windshield in humid or rainy conditions. It can also create a risk to rear passengers, including children, during hot weather because the rear A/C system is no longer functioning as designed. The failure also involved leaking refrigerant from corroded lines. Toyota will not accept that this is a premature and dangerous parts failure. There were no warning lamps or dashboard messages before the failure. The symptom was that the A/C stopped cooling and would not hold refrigerant. The cause was later confirmed, by Toyota, as leaking rear A/C lines due to excessive rust/corrosion. Prior to replacement, the vehicle exhibited increasing humming, roaring, and grinding noises from the rear, which worsened with vehicle speed. No warning lights were illuminated. I am concerned about repeated premature rear wheel bearing failures and the potential safety risk associated with wheel bearing deterioration, including loss of wheel stability and vehicle control if the condition progresses while driving.Read more
