Twice since I purchased my car, once in 2025 and two weeks ago in 2026, I turned my car off in the garage and the engine kept running. I had to start the car up and turn it off again in order for it to shut down. I mentioned that to Arapaho Kia at my last visit in November 2025 and they just said that they didn’t find anything on the computer.

Component Affected: Service Brakes / ABS Module / Hydraulic Braking System Description of Safety Defect: There is an active, intermittent, catastrophic failure in the front braking system/ABS module causing the front brakes to completely lock up mid-transit while operating at high speeds (60+ MPH). First Incident (April 24, 2026): During operation on a busy interstate at over 60 MPH, the front br
... aking system experienced a sudden, catastrophic thermal lock-up. This caused violent vehicle shuddering, extreme friction heat, and heavy, thick, acrid smoke. The vehicle was brought to a dealer shop for repair. Second Incident (April 29, 2026): Four days after delivery, the identical catastrophic brake lock-up recurred mid-transit, creating an immediate fire hazard due to friction heat and smoke. An independent repair facility diagnosed the failure and replaced the ABS module. Third Incident (June 4, 2026): The identical component failure recurred while traveling at over 60 MPH on Interstate 694 with a minor child in the vehicle. The front brakes fully locked up mid-transit, creating an immediate risk of a high-speed collision, severe thermal friction, and heavy burning odors, rendering the vehicle entirely un-roadworthy. Current Status: > The vehicle has sat grounded at a repair shop for 30 cumulative days out of 54 days of ownership due to the persistent re-locking of the front hydraulic braking system. The vehicle is unsafe to operate on public roads. No crash or fire occurred, but the failure poses an imminent thermal and collision hazard.Read more

2024 Chevrolet Equinox EV. Beginning approximately two infotainment software updates ago, the vehicle's Bluetooth system repeatedly drops the connection to my paired iPhone, and the contact-sync feature stopped functioning entirely. The problem did not exist when the vehicle was new and started only after a vehicle software update, indicating a software-introduced defect rather than a phone fault.
... I have reset the infotainment system and performed a full delete-and-re-pair on both the phone and the vehicle; the dropouts and contact-sync failure persist. Because the system no longer reliably supports hands-free calling, I am unable to consistently place or receive calls without handling the phone directly while driving, which I consider a driver-distraction safety concern. No warning was issued and I am not aware of a recall or software fix addressing this.Read more

I am reporting a recurring safety defect regarding the exterior door handle assembly on my 2024 Tesla Model 3. On 6/15/26, the exterior door handle of caused a finger entrapment incident, resulting in an avulsion of a gel nail and significant trauma to the nail bed. The artificial nail remains trapped within the door assembly. Despite seeking service at the local Tesla Service Center, technicians
... failed to retrieve the object or address the entrapment hazard. This design creates a severe pinch-point hazard that poses an ongoing risk of injury to all users Despite bringing the vehicle in for service on 6/18/26 to have the hazardous object removed, the service center performed only a cursory visual inspection and refused to disassemble the handle, leaving the entrapment hazard in place. The design of the door handle creates a clear pinch-point/entrapment zone that poses a safety risk to all occupants. I am reporting this to document both the design defect and the failure of the manufacturer to address the resulting safety hazard upon request.Read more

This was an auto stop-start system failure. The Honda dealership in Frisco inspected and attributed this to battery life but did not guarantee this was the cause of the issue. There were no other signs of failure prior to the incident. A description of the incident is below. When accelerator was pushed to proceed, the car started flashing lights with a warning message on the dashboard, the car was then automatically put into park by the system, and would not proceed to move without turning the vehicle off and back on. If this had occurred in another location, it would have been extremely dangerous as it took some time to turn the vehicle back on.Read more

Summary of issue The vehicle developed an engine/emission warning light and failed mechanically without any crash or impact. The car drove fine until about one week ago. I have driven the vehicle normally and performed regular maintenance. Events and observations A warning light appeared indicating engine and emission problems. I took the vehicle to two separate mechanics: Mechanic A diagnosed e
... ngine damage and catalytic converter issues. Mechanic B diagnosed engine damage and a possible fuel pump problem. I am not a mechanic and cannot independently interpret technical findings, but both inspections indicate serious engine-related failures. The vehicle has approximately 80,000 miles and was purchased for more than $20,000 less than two years ago. No collisions, impacts, or abuse occurred that could explain the failure. I believe this is a potential manufacturing defect. I contacted a Honda dealer about the problem; they stated it is not a manufacturing defect and declined to accept responsibility for repair. I believe the dealer is avoiding responsibility. Requested action I request NHTSA open an investigation (or add my report to any existing investigation) into this vehicle’s engine and related components to determine whether a safety defect or noncompliance exists. I request guidance on next steps and that this complaint be considered in any recall evaluation.Read more

Started to smell a musty odor in my 2020 Tiguan. Had no idea where it was coming from. Repeatedly wiped down and vacuumed the car; but the smell would not leave. Mold has now started to develop and grow on the car
... pet on both sides of the car in the back. This is very bad as I don't want me and my family breathing in mold. Also, I purchased the car in late March of 2024 and have only had it for just a little over two years. Since I've had the car we have gotten all routine maintenances completed including the 80K mile service. After reading online I see that this is a COMMON problem with VW Tiguan's where either the Sunroof leaks into the car or the A/C drain causes issue; in both cases it causes water leaks and mold. Tried calling VW and Audi of America to explain and see if they would buy the car back, but was advised due to the year and mileage they could not. This is sad to me because this is dealing with MOLD. VW is aware of these issues and seems to not care if people are left finishing out financing on a loan with a car that leaks and causes mold. I'm even seeing that on the NHTSA website complaints section for Tiguans, that multiple people have registered complaints about the same issue. Attaching picture of mold that is growing in back area.Read more

The problem is in the power-locking manual tailgate’s exterior release switch assembly. Water intrusion into the switch can cause a short circuit, leading the tailgate to unlatch while the truck is in Park. Once moving, momentum and air pressure can cause the tailgate to open further, potentially dumping cargo onto the road and creating a hazard.

Hyundai - DAW - Driver Attention Warning / Forward Attention Warning System This issue warrants immediate consideration for a recall. Despite wearing glasses, sunglasses, or no glasses, the problem persists. I occasional do remove my glasses while driving, and the warning continues to sound. This occurs regardless of the time of day, weather conditions, or whether I am sitting upright or reclining
... . The warning randomly activates and deactivates, sometimes without any intervention. Occasionally, I simply allow the warning to sound without taking any action, and it remains active for a few seconds or until it triggers a warning that the system will be turned off, at which point it deactivates. I have attempted various troubleshooting methods, such as keeping my eyes open, adjusting my seating position, wearing and removing my glasses, and tilting them to minimize glare. None of these methods have proven effective in preventing the warning from activating or in making it deactivate. This is a persistent problem. Hyundai dealerships initially informed me that the warning might be an annoyance and their response was to simply refrain from using it. However, if a basic feature like cruise control in a 2024 & 2025 model is rendered unusable due to a design flaw (which this certainly is), it constitutes a clear violation of safety standards and should be addressed through a recall. The incessant alerts are highly disruptive and can be quite frustrating. For instance, driving with an arm in the 12 o’clock position triggers a warning, while wearing polarized sunglasses also results in an alert. The presence of a setting to disable the warning is a welcome feature, but the system’s persistent alerts can be overwhelming. The alerts continue even after taking a break. It is distracting and a safety hazard. Drivers should be able to adjust the sensitivity of the feature to turn off this feature. Currently, there is no way to disable or temporarily disable this feature.Read more

The vehicle SOS communication/warning system began malfunctioning in February 2026. I am receiving an alert stating there is an SOS Malfunction and to service the vehicle immediately. On February 18, 2026, the VW dealership conducted extensive testing on the vehicle and determined that the J949 SOS module required replacement as it could not be reset or remedied. Diagnosis - internal electrical f
... ailure. The dealership advised that this is a known issue to VW and that there are several other customers with the same issue and waiting for the modules, which are on backorder with VW. There is no communication between the vehicle and VW as a result, and the infotainment system is unable to update. The service tech advised that there are a number or potential issues that will arise because this is a vehicle communication failure. I was forced to prepay for the module and labor, which was ordered on 2/18 (in excess of $500). On 5/26/26 the VW dealership service manager confirmed that the module still has not arrived from VW and that there may be a recall issued because this is such a widespread problem, but that they have no estimated delivery time for the part. The vehicle has had no problems prior to this. If the car is in an accident, there will be no SOS function. The infotainment system, which controls CarNet and the ability to communicate using CarPlay will also be compromised, which may result in the inability to call for assistance in event of an incident. VW Customer Service states they have no ability to track service orders or provide information on the ongoing issue, and stated that the dealership has the only ability to track the part. The dealership cannot check the status beyond acknowledging that the part is on backorder. This is a widely reported problem online in multiple forums and with multiple VW models. We are paying for a part and waiting multiple months to obtain it, and the vehicle is compromised due to the failure of VW.Read more

While driving, the vehicle suddenly began handling abnormally. In response, we safely pulled over as quickly as possible. Before we could fully stop, the wheel catastrophically failed. The rim cracked around the entire circumference, the tire completely separated from the vehicle, and the vehicle lost stability and struck the guardrail. As a result of this incident, the vehicle sustained substanti
... al damage, including but not limited to: •Complete destruction of the wheel/rim; •Tire separation; •Axle and suspension damage; •Body damage to the driver-side front and rear panels; and •Additional mechanical damage that is still being assessed. Photographs taken immediately after the incident clearly show that the wheel fractured around the rim itself. The damage pattern is consistent with a catastrophic wheel failure, not ordinary cosmetic damage or curb rash. When I initially contacted the Kia dealership, I was told that they had "never seen anything like it." However, after the vehicle was towed to the dealership, the explanation changed, and I was informed that Kia now believes the vehicle must have struck something and that the impact caused the wheel to fail. That explanation is inconsistent with what occurred. At no point prior to the failure did we strike debris, a pothole, a curb, or any other object that would reasonably explain a complete structural fracture of the wheel and the tire separating from the vehicle. The wheel failed first. The collision with the guardrail occurred only after the catastrophic wheel failure and loss of control while attempting to safely pull over.Read more

**Vehicle Safety Complaint** I am experiencing a serious safety issue with my 2016 Chevrolet vehicle involving the power steering system. On a somewhat regular basis, I receive dashboard warnings including "StabiliTrak Not Working," and "Service Power Steering." Shortly after these warnings appear, the power steering fails while I am driving. When this occurs, I must use significant force to tur
... n the steering wheel in order to maintain control of the vehicle and make turns. When I pull over, turn off engine and wait a few minutes, the restart the engine, it appears to be resolved and works fine. This creates an extremely dangerous driving condition. I am concerned that I may not be able to turn quickly or forcefully enough to avoid an accident, potentially resulting in a collision, injury, or death. I am aware that certain 2015 model year vehicles were recalled for similar power steering issues. However, despite experiencing what appears to be the same defect in my 2016 vehicle, no recall has been issued. The issue remains uncorrected and continues to pose a significant safety risk. GM Customer Care (1-800-462-8782) states I am not eligible for any cost assistance because it is a 2016 model and not 2015. Given the serious safety implications of sudden power steering loss while driving, I am requesting that NHTSA investigate this issue and determine whether a recall is warranted for affected 2016 vehicles. The sudden loss of power steering while the vehicle is in motion presents a substantial safety hazard and should be addressed before it leads to serious accidents or injuries.Read more

In October 2024, I brought my recently purchased used Model S to the Cincinnati Service Center for an antenna inspection, MCU upgrade, and general inspection to ensure the vehicle was safe and operating properly. During the inspection, I was informed that additional issues had been discovered. Wanting the vehicle to be reliable, I authorized the recommended repairs and ultimately paid more than $1
... 0,000 for the work performed. When I picked up the vehicle, a technician had to access it because the keys had been locked inside, and the battery had not been adequately charged. Shortly after leaving, I heard unusual noises and, within approximately five miles, discovered the protective underbody panel had not been properly secured and was hanging from the vehicle. Because I live in West Virginia, I was unable to complete the trip home and required a tow to a charging station. During the drive, the vehicle displayed coolant-related warnings. When I contacted the service center, I was advised to continue driving the vehicle if it remained operable and return it when convenient. Only seven days later, the vehicle completely shut down and became inoperable. When I returned it to Cincinnati, I was informed that the main battery required replacement at my expense despite previously being told multiple times that the battery was functioning properly. I have never received a clear explanation as to how the battery failed, why I am being held responsible, or whether the prior service contributed to the issue. Tesla instructed me to remove the vehicle from its property and return it to the seller. Although Tesla later offered $4,500 as a resolution, I declined because I do not believe I should be responsible for a battery failure that occurred immediately after extensive service work.
