The high-voltage battery on my 2022 Nissan Leaf is subject to Recall 25V655. The recall notice states that the battery cells can develop excessive lithium deposits, which increases electrical resistance and can cause rapid battery heating during Level 3 fast charging. Nissan instructs owners not to use Level 3 charging at all until a “remedy” is available. The issue is that the remedy describe
... d by Nissan is not an actual repair of the battery defect. Nissan states it will install software that monitors for “state-of-charge fluctuation” and, if detected, will prevent the vehicle from restarting or recharging in order to avoid a thermal incident. This means the defect inside the battery cells is still present, and the car may disable itself if the defect begins to appear. The recall materials also state there is no warning before overheating occurs. This creates multiple safety concerns: • The underlying battery defect remains uncorrected. • The vehicle can become immobilized (unable to restart or recharge) if the software detects the condition. • Loss of Level 3 charging capability affects the ability to travel safely or plan charging when needed. • A battery fire risk exists during Level 3 charging if the defect is not detected in time. Nissan’s documents state that the software is meant only to prevent the “progression” of a thermal incident, not to repair the defective battery. I am concerned that my vehicle contains a known hazardous defect that has not been physically repaired and that the software-only response is inadequate to ensure safety. I am filing this complaint so NHTSA is aware that the remedy being offered does not address the internal battery defect itself and may leave owners exposed to continued safety and reliability risks.Read more

VEHICLE INFORMATION Year: 2018 Make: Honda Model: HR-V VIN: [XXX] Mileage at Time of Incident: 96,993 Incident Date: 03/10/2026 I am filing this complaint regarding the premature and complete failure of the CVT (Continuously Variable Transmission) on my 2018 Honda HR-V with approximately 96,993 miles. DESCRIPTION OF FAILURE: The transmission on my 2018 Honda HR-V failed without prior warning.
... Symptoms included sudden loss of acceleration, vehicle unable to move under its own power, etc. The vehicle was taken to Page Honda of Bloomfield where it was diagnosed with a complete CVT transmission failure. The repair cost totaled $8,873. SAFETY CONCERN: This failure poses a serious safety risk. A sudden loss of transmission function while driving — particularly at highway speeds resulted in loss of vehicle control and created dangerous conditions. This is not a minor mechanical inconvenience; it is a critical safety defect. HONDA'S RESPONSE: I opened a case with Honda Customer Service; Case # XXX requesting goodwill assistance reimbursement for the repair costs. Honda denied my claim. After submitting a formal written demand, Honda agreed to reconsider, but has yet to provide full reimbursement. PATTERN OF FAILURE: I believe this is not an isolated incident. I am aware of other 2018 Honda HR-V owners who have reported the same CVT transmission failure pattern. I urge NHTSA to investigate this and help me with resolving this case. I am happy to provide additional documentation, repair invoices, and correspondence with Honda upon request. My son, [XXX] , has opened a complaint/Case #XXX with Kia. His 2023 KIA Forte had been idle for at least 24 hours when the engine compartment caught fire. He became aware of this when the Fire Department knocked on his apartment door around 4:00 PM 12/26/25 and said they were evacuating the building because a car was on fire. He went outside and was shocked, to say the least, that it was his car.
... The Fire Department had taken off the hood to extinguish the fire. He removed his belongings once the fire was out. He contacted his insurance company but since it was a holiday weekend, he did not get a call back until 12/29/25. He then filed a claim with State Farm Insurance which was required to get his car towed to a Kia repair location. On 1/9/25, [XXX] [XXX] t Castrucci Auto Shop in [XXX] [XXX] he car was totaled. I talked with [XXX] who was [XXX] between the auto shop and State Farm, told me that once the repair exceeded the value of the car, he was instructed by State Farm it was totaled and there was no need to continue. On or around 3/4/26, Kia sent engineers to assess the damaged vehicle. Kia determined it was an engine fire and the cause was undetermined due to the condition of the engine department. Incident date: Mar 22, 2026
I was told the best thing to do was report it here. Our son was driving his 2019 Honda Accord and suddenly everything on his dashboard lit up. Was told it is a blown head gasket. We were also told that the shop has had over 81 Accords in with the same issue. They said Honda is trying to sweep it under the rug rather making a recall. We were told this is what they see: The head gasket mate
... rial between cylinders is very tiny, any minor disturbance will cause coolant to leak into the cylinder. Usually failures are happening between cylinders 2&3 where the material is the thinnest. 2) Honda is really trying to get a lot of power out of this small engine with the inclusion of the turbocharger. Higher octane fuels are needed to prevent premature detonation / knock, especially with forced induction (turbo charger). You can still run the car using 87 octane, but the chances of problems like a blown head gasket happening increase. 3) The head bolts that keep the engine head to the engine block are not strong enough. These bolts are torque to yield at around 50 ft lbs (if that). There have been instances where these bolts don't have enough clamping force to withstand the pre-detonation. This force causes the head to jump and the bolts to stretch. The heat from the combustion in the cylinder is no longer contained, which warps and damages the head gasket over time, leading to its failure. It was suggested to use ARP studs which we were told are a higher quality than the OEM Honda head bolts and have a stronger clamping force. Our son ended up stranded 3 states away with this issue that we then learn is VERY common yet there are no recalls.Read more

Water began accumulating in the front footwell area of the vehicle, resulting in a soaked carpet and persistent musty odor. The condition developed during normal operation of the air conditioning system, without any external water intrusion (no flooding or open windows). The issue appears to be caused by a failure of the A/C condensate drain system, which is not properly draining water outside th
... e vehicle. Instead, condensation is leaking into the cabin. This creates a risk of mold growth and potential damage to electrical components located beneath the floor, including wiring harnesses and control modules. The problem has been reproducible, especially when the air conditioning is in use. There were no warning lights or alerts prior to noticing the issue. Research indicates that BMW has identified this exact defect in Service Information Bulletin SIB 64 04 16, which states that the condensate drain hose may be improperly installed, kinked, blocked, or damaged during installation, leading to water ingress into the footwell area. This suggests the issue is due to a known design or manufacturing defect rather than normal wear and tear. This condition poses a safety concern due to possible electrical malfunction and reduced vehicle reliability. The affected components are available for inspection upon request.Read more

I am filing a complaint regarding my 2022 Acura MDX Type S with approximately 70,700 miles. The vehicle developed a suspension system warning, and upon inspection by an Acura dealership, I was informed that the suspension stroke sensor and floor wiring harness are corroded and require replacement. I was quoted approximately $12,000 for the repair. This failure is extremely concerning because the v
... ehicle is only a few years old and this type of corrosion affecting a major wiring harness should not occur under normal operating conditions. The affected components are part of the suspension system, which directly impacts vehicle handling and safety. Due to the failure, the suspension system may not function properly, potentially affecting ride height, stability, and overall control of the vehicle. This raises safety concerns, especially at highway speeds or during emergency maneuvers. There was no prior warning of water intrusion or external damage that would reasonably explain this level of corrosion. This appears to be a premature failure of a critical electrical component. Acura has only offered minimal goodwill assistance, leaving the majority of this extremely costly repair as the responsibility of the owner. I am concerned that this may be a broader issue affecting other vehicles and that it could pose a safety risk if not addressed. I am requesting that this issue be investigated. Thank you.Read more

The frame of the back window is cracking allowing water to come in through the back glass. This is causing the headliner and other components such as air bags and other electrical components to get wet and damaged. Subject: Safety Complaint – Premature Frame Failure on 2018 Ford F-150 Raptor (VIN: [XXX] ) To Whom It May Concern, I am submitting a formal safety complaint regarding my 2018 Ford F-150 Raptor, VIN: [XXX] , with approximately 99,700 miles. My vehicle has been diagnosed by a Ford-authorized service department with severe frame corrosion and structural deterioration, including extensive rust a
... nd compromised integrity. In addition, I have obtained an independent evaluation from a private body shop, which also confirmed that the corrosion is premature and structurally significant. I have been advised that the vehicle is unsafe to operate, and the estimated repair cost is approximately $30,000, effectively rendering the vehicle unusable. This condition appears to be premature and not consistent with normal wear and tear, particularly given that the vehicle has been properly maintained and used under normal driving conditions. The failure of a vehicle’s frame represents a serious safety hazard, as it directly affects crashworthiness, structural stability, and safe operation. A failure at this level could lead to loss of control or catastrophic injury. I contacted Ford Motor Company to request assistance, but they declined to provide any support or remedy. Given the severity of this issue, I respectfully request that the NHTSA: •Investigate potential frame corrosion or structural defects in this vehicle model •Determine whether similar complaints have been reported •Evaluate whether this issue warrants a recall or manufacturer action I am prepared to provide supporting documentation, including inspection reports from both the Ford dealership and independent body shop, repair estimates, and photographs of the damage. The air conditioning system in my 2019 Honda Civic has experienced a total failure of the evaporator core due to a refrigerant leak. This follows previous documented failures of the AC condenser and compressor shaft seal, both of which are currently under Honda-issued 10-year warranty extensions. Various other users have also reported these issues with their 10th generation civics. The failure of
... the AC system prevents the defroster from effectively dehumidifying the cabin. During rain or high-humidity conditions, the interior of the windshield fogs up rapidly and cannot be cleared, severely obstructing my field of vision while driving. In high-temperature environments, the cabin temperatures reach dangerous levels within minutes. This poses a significant health risk for passengers, particularly children and the elderly, and causes driver fatigue/distraction due to heat exhaustion. Despite multiple repairs to other components in this specific R1234yf system, the system continues to fail. Honda has diagnosed a leaking evaporator core, a part known within the owner community to have the same manufacturing defects as the already-warrantied condenser and compressor. 2020 Volkswagen Passat SE 2.0L VIN: [XXX] Complaint Summary: I am reporting a persistent and unresolved fault involving the secondary air injection system (DTC P0491 – insufficient airflow) that has resulted in repeated, conflicting diagnoses across multiple repair facilities. This raises concerns about a potential systemic defect affecting this vehicle model. Detailed Description: The issue
... began prior to March 2025 while the vehicle was still under warranty. The vehicle displayed a Check Engine Light associated with diagnostic trouble code P0491 (secondary air injection system insufficient airflow). On April 3, 2025, a repair facility diagnosed the issue as a failed secondary air injection pump and replaced the component. However, the Check Engine Light returned within 24 hours with the exact same fault code, indicating that the repair did not resolve the issue. A second independent diagnostic evaluation was performed later in April 2025. This inspection confirmed that: The secondary air injection pump was functioning correctly Airflow through the system was present Electrical controls and PCM commands were operating properly The fault was attributed instead to a mechanically stuck secondary air injection combination valve Despite this revised diagnosis, the issue has remained unresolved. As of 2026, a dealership evaluation has now attributed the same fault condition to a potential failure within the cylinder head assembly, indicating a significantly more serious underlying condition. Safety Concern: This issue involves the emissions and engine air management system and has resulted in: Persistent Check Engine Light Repeated failure of emissions-related components Conflicting and escalating repair diagnoses across multiple qualified service providers I am requesting that the National Highway Traffic Safety Administration review this issue for potential investigation into a systemic defect. The power to the car wouldn't shut off. We were able to jump the car to get it started again.Read more

I am filing a complaint regarding the frame manufactured by Lippert Components on my 2021 Grand Design Solitude 380FL (VIN [XXX] ). Lippert is the OEM supplier of the frame and has been directly involved in inspection and repair of ongoing structural failures. The unit has experienced repeated structural failures, including frame flex, broken welds, cracking of the main I-beam rails above both ax
... les, torn steel upper sill plates and suspension failures. These issues began within the manufacturer’s warranty period and required extensive factory repairs from December 2024 to February 2025, including reinforcement of the upper deck, weld repairs, sill plate replacement, and added gussets. Despite these repairs, new failures developed shortly after. In September 2025, a Lippert technician (dispatched at Grand Design’s request) identified new frame cracks above both axles, a bent outrigger support, and repeated suspension failure. These were not pre-existing conditions but occurred after factory structural repairs. In December 2025, Lippert performed additional field repairs, including welding the cracked frame, adding fishplates, reinforcing outriggers, and installing cross-members. These repairs were performed under engineering direction, and Lippert confirmed no standard TSB applied to this condition. Despite multiple repair attempts, the unit continues to exhibit structural-related issues, including cargo doors opening during transit and recurring suspension failures. These conditions indicate ongoing and unsafe frame deflection and instability. Grand Design has now stated their repairs are done per a customer satisfaction campaign, and attributed responsibility for any ongoing frame issues to Lippert, and has abandoned any further assistance with cargo doors opening during transit. This represents a systemic structural defect affecting safety, including risk of debris or components entering roadways and loss of structural integrity during travel. On Friday March 20, 2026, we traded in a Chevy Equinox to get a newer model which is the VIN number we listed in this complaint. Driving home from the dealership the vehicle went from drive to park on its own (driving at 65 miles/hr) and my husband had to immediately put the car physically back into drive. Once we finally got the vehicle to our house, I went out to check it, and the vehicle woul
... d not even start. Every single light came on yet it would not start up. We called On-Star and they couldn't do anything and told us we'd have to take it to the dealership. We finally got it to start up again, but the check engine light never went off. The next day my husband was able to get it started and immediately took it back to the dealership it was bought from, which it was actually transferred to from another dealership within the state, but on the way to the dealership we bought it from the car went into park on its own 5 more times, very dangerous and unacceptable. Once the car made it to the dealership, they could not even get the vehicle to shut off once they pulled in into the service area. My husband luckily is a professional driver, but is this was someone not as familiar or calm, this could have been a horrible catastrophe.Read more
