Component/System Failure: The turbocharger system failed, specifically the wastegate arm, resulting in loss of boost and reduced engine performance. The failed component should be available for inspection upon request through the servicing dealership. Safety Risk: The failure caused a sudden loss of engine power while driving, placing me and others at risk, especially in traffic situations where
... acceleration is required (such as merging, crossing intersections, or maintaining speed). This created a hazardous driving condition. Confirmation of Issue: The problem was diagnosed and confirmed by an authorized dealership. The diagnosis stated: “turbocharger wastegate arm damaged causing loss of boost and DTC,” which matches known issues described in GM’s Special Coverage. Inspection History: The vehicle has been inspected by the dealership service department. It has not yet been inspected directly by the manufacturer, police, or insurance representatives. Warning Signs Prior to Failure: Yes. The vehicle displayed a check engine light and reduced engine power message prior to the failure. These symptoms began shortly before the vehicle experienced noticeable loss of acceleration and performance. Additional Information: This failure is consistent with a known issue acknowledged by General Motors under Special Coverage for similar vehicles experiencing turbocharger-related defects. Despite having the same failure and symptoms, my vehicle was excluded based on lot number, not condition or diagnosis.Read more

The high-pressure fuel pump (HPFP) failed internally while I was driving at night, causing the vehicle to suddenly stall and shut off completely while my children were in the vehicle. This resulted in an immediate loss of power and created a dangerous situation in traffic. The failure caused metal debris to spread throughout the entire fuel system, and the dealership confirmed that the system req

This vehicle previously had a manufacturer recall related to the fuel pump that was completed in 2022 and is now listed as closed. Despite the recall repair, the same failure occurred again. This demonstrates that the recall remedy did not correct the defect. The sudden engine shutdown without warning presents a serious safety hazard, especially at night and with passengers in the vehicle. Its been at the dealer ship for a month.Read more

My car started shutting down while driving. After the engine started seizing the key wouldn't turn in my ignition. Took car to dealership immediately on 10, 2026. Took again in 2024 for recall and other repairs. Found out after requesting previous service incidents they failed to do recall repairs and never notified me of it, or to come back. I've requested current diagnostic report several times
... in writing and verbally with codes for this current service, with non compliance. My car is still at the dealership needing work and I need current diagnostic report before I pay for repairs. I am attaching a dealership diagnostic e-Report for my 2013 Kia Optima from 2024. While the report lists “No Current DTC” for several systems, it also documents multiple “Communication Fail” errors across critical modules, including Smart Key, Head Lamp, Air Conditioner, Brake, and other systems. These communication failures indicate a broader electrical or network issue within the vehicle, not simply a failed battery. The report also shows that certain diagnostic scans were not properly performed. Additionally, there are historical fault codes related to TPMS and weight classification systems, further supporting that the vehicle has experienced ongoing electrical issues. Despite these findings, the dealership attributed the vehicle failure solely to a bad battery and did not provide a current battery diagnostic test. They also failed to address an open recall related to electrical/fuse issues prior to this failure. Based on this past diagnostic report, I believe the root cause of my vehicle’s failure is an unresolved electrical defect, potentially related to the outstanding recall, rather than normal battery wear. This has resulted in my vehicle becoming inoperable and poses a serious safety concern.Read more

2025 Chevrolet Blazer EV AWD purchased 5/23/25 (under 3,100 miles) The vehicle's Telematics Control Module (TCM) has failed, disabling the built-in navigation system, OnStar services, remote start, and My Chevrolet app connectivity. The dealership confirmed the TCM needs replacement and the part is on backorder with no ETA. This is a known issue affecting this model. I am reporting this as a saf
... ety defect for the following reasons: The 2025 Blazer EV does not support Apple CarPlay or Android Auto. With the built-in navigation disabled, the only way to navigate is by using a phone, which requires visual attention to a separate small screen while driving. Under Pennsylvania's hands-free law (75 Pa.C.S.A. § 3316.1), use of handheld devices while driving is a primary offense, making phone-based navigation both unsafe and legally problematic. On at least one occasion, the navigation system appeared to be functioning normally at the start of a trip but was providing materially inaccurate directions mid-route (e.g., instructing me to turn onto a road I had already been traveling on for miles). A system that displays as operational while delivering incorrect routing data is dangerous because the driver has no way to assess its reliability before departing. GM's EV Concierge characterized the loss of navigation as a non-safety feature issue and suggested using a phone holder. This response is inadequate given the vehicle's lack of CarPlay integration, which makes the built-in navigation the only safe, integrated means of route guidance. This vehicle has also required three prior unrelated warranty repairs in 10 months: a recall for a faulty rear parking brake harness (9/25), a 12V battery failure requiring flatbed towing (11/25), and a faulty tire pressure monitor (12/25). The pattern of recurring failures across unrelated systems at minimal mileage indicates a systemic quality issue with this vehicle.Read more

On [XXX], while driving my 2021 Ram ProMaster 2500 at highway speed in Florida, the van suddenly lost all power and came to a near stop, then re-engaged. Vehicles were in front and behind me, creating an immediate risk of serious collision, injury, or death. I pulled over ~30 minutes, restarted, but it could not exceed 20 mph while RPMs were high. AutoZone confirmed limp mode and advised a dealer.
... I drove it home to Port Charlotte, then had it towed to Gettel Dodge Chrysler Jeep Ram, Punta Gorda, FL, where after two weeks it was diagnosed with a failed transmission requiring replacement. The transmission can be inspected at the dealership. No warning lamps appeared before failure. Problem confirmed by AutoZone and the dealer. I contacted Stellantis (Case #[XXXX], contact Regina Knox; dealership contact Noel), but repair assistance was denied. The van also has two active recalls, including risk of rolling out of park, unremedied. Incident Date,March 07, 2026] Current Mileage, 31625 miles I am reporting a catastrophic structural failure of a safety-critical suspension component on my Tesla Model Y. Upon inspection, I discovered that one of the bolts securing the driver-side lower control arm has completely detached and fallen off. A second bolt is approximately 50% loose. This has caused a significant safety hazard
... , as the control arm is no longer properly secured to the subframe, leading to a high risk of total steering loss and wheel detachment while driving. Furthermore, when I contacted Tesla Service, they demonstrated extreme negligence by instructing me to drive the vehicle in this lethal condition to their service center. They also initially refused to cover the towing and repair costs, claiming it was not a factory defect despite the clear evidence of fastener failure (improper torque or thread locker application during manufacturing). This is a clear manufacturing defect that poses an immediate threat to the driver, passengers, and other road users. I am requesting NHTSA to investigate this batch of Model Y vehicles for suspension fastener integrity issues to prevent potential fatal accidents.Read more

I am writing to report a serious safety concern involving my 2024 Chevrolet Silverado that resulted in a child being locked inside the vehicle with the key fob present inside the car. On March 25 2026, after parking and turning off the vehicle, I exited the driver’s seat with my key fob inside my purse, which remained in the front of the vehicle. I then walked around to remove my daughter from
... the back seat. During that time, she climbed into the front seat and pressed the interior door lock button, locking all doors. My daughter has autism and is not able to follow instructions or unlock the vehicle on command. As a result, she was locked inside the vehicle with the key fob present inside, and I was unable to gain access from the outside. The vehicle did not prevent the lock from engaging, nor did it allow me to open the door despite the key being inside the vehicle. This situation required emergency assistance (911) to safely remove her from the vehicle. I understand that many vehicles have “lockout prevention” features; however, in this case, the system did not account for a vulnerable occupant inside the vehicle. Allowing the vehicle to be locked from the interior while the key fob is inside—and without any external override—creates a significant child safety risk. I am requesting that Chevrolet review this design behavior as a potential safety issue, specifically: - Why the vehicle allows interior locking when a key fob is detected inside - Why there is no external override to unlock the vehicle when the key is present - Whether a software update or safety feature enhancement can address this risk This is not just a convenience issue—it is a safety hazard that can lead to dangerous outcomes, especially for children or individuals with disabilities. Please confirm receipt of this concern and advise whether this issue has been previously identified or is under review.Read more

Without any warning, on March 15, 2026, I turned out of a parking lot onto a major road and I noticed it wouldn't accelerate above 20 mph, so I pulled off the road. I tried again and my vehicle still failed to accelerate. And on my third attempt, my vehicle decelerated and was about to completely stall. I managed to turn into a parking lot and avoid getting rear ended. However, this could have
... been a fatal event. While I was trying to accelerate, the vehicle was quiet as if the car wasn't even running. After shutting the engine off for awhile, I was able to drive the vehicle with caution. I reached out to two Genesis service departments regarding my vehicle failing to accelerate while driving and neither had heard any previous complaints, and the earliest appointment would be mid April. There was no sense of urgency, as I'm left to hesitantly drive my vehicle at the risk of it happening again and getting rear ended. And when my vehicle does go in for service, I'm weary whether the problem will be resolved properly since the service department is unaware of the issue. In addition, I have had this vehicle for only 4 months and 1 month into it, I already took it to service because of fuel odor that had not gone away since the day 1. But the service department didn't find anything wrong with the vehicle, so nothing was done to it. I also swung by another Genesis service department, in case they happened to be familiar with the cause of the odor. My vehicle still has the fuel odor and now I'm wondering if that is related to my vehicle not accelerating. I am seeking your assistance to ensure that the problems with my vehicle are confidently resolved. And I wouldn't want this to happen to anyone else. On March 18, 2026, while driving a leased Honda Prologue in rush-hour traffic on a major highway in [XXX] , the vehicle suddenly shut down and would not restart for approximately two minutes. The incident occurred without any warning messages, alerts, or notifications and created an extremely dangerous situation. At the time of the incident, traffic had been stop-and-go for approximately three mi

On approximately the seventh or eighth attempt, the vehicle finally restarted, allowing me to safely pull off the highway. Fortunately, the slow-moving traffic prevented a rear-end collision with vehicles behind me. I began leasing the vehicle this past summer, and at the time of the incident it had fewer than 2,600 miles on the odometer. I took the vehicle to the dealership that same evening. During their inspection, the dealership identified several “Lost Communication” diagnostic trouble codes in the vehicle logs. The most significant code identified was B1B42 – Start/Stop Switch Not Plausible. The service team also noted that the manufacturer’s troubleshooting checklist references codes B1B15 or B1B15 2A; however, those codes were not present. Based on the findings, the dealership replaced the S38 Start/Stop Switch. I resumed driving the vehicle two days ago and have not yet experienced any further issues. However, I remain extremely concerned about the seriousness of this failure. If this same issue were to occur again at higher speeds or in faster-moving traffic, it could result in a severe accident. Shortly after I put it in park and turned it off it "released" and started rolling spontaneously. I could have run over my neighbors; people, babies...pets, me... I piled logs and big rocks behind all 4 tires (put it park with the parking brake on of course,) & called the Mercedes dealer & made an appointment. I did not drive it again until it was "repaired," except for the fact that nothing showed up on the scanned as wrong. I am writing to file a formal safety complaint against Tesla, Inc. regarding a software update failure that has disabled legally required vehicle functions on my Tesla for over two years. For more than 26 months, my vehicle has been unable to complete a software update through Tesla's over-the-air (OTA) update system. As a direct result of this failure, my vehicle's display screen does not functi
... on for the first several minutes of every drive. This prevents me from accessing my speedometer, turn signals, and other legally required safety features, rendering the vehicle non-compliant with federal and state motor vehicle safety standards during this period. I have made multiple documented attempts to resolve this issue through Tesla's customer support channels over the course of these two years. Despite these efforts, the issue remains unresolved. Tesla has now informed me that resolving the defect will require a paid service visit — despite the fact that the failure originates within Tesla's own software update infrastructure, not from any action or negligence on my part. I am requesting that NHTSA investigate this matter on the following grounds: 1. The software defect disables legally required vehicle safety functions, including the speedometer, turn signals, hazard lights, and more. 2. The defect has persisted for over two years despite repeated good-faith efforts by the vehicle owner to resolve it through the manufacturer. 3. Tesla's OTA update system is manufacturer-controlled infrastructure. A failure within that system constitutes a manufacturer defect, not a maintenance issue. 4. Tesla is requiring payment from the consumer to correct a defect in their own systems. I believe this issue may not be isolated to my vehicle and could affect other Tesla owners experiencing similar OTA update failures. I respectfully request that NHTSA investigate whether a broader safety defect exists and whether a recall or corrective action is warranted.Read more

While driving on the highway, I heard a loud pop that made me flinch and swerve. After parking I noticed the glass in the sunroof had exploded upward like a volcano, leaving a large hole. Fortunately the shade was closed, or glass would have come into the car while I was driving.

When moving forward at very slow speed, the steering wheel bounces as if it had been hit by something and we hear a loud "thunk". The whole front end of the car vibrates. This has also happened once while completely stopped at a traffic signal. This has been happening for several months, and we know that others have reported the same problem, but the dealer is unresponsive. We are worried

My car failed to connect/stay connected to a charger. When a successful charge started, within 5 minutes , my dash lit up with warning messages to stop moving the car. Place car in park. Battery needs to be replaced. My car was in park at the time. Then the charging session ended and wouldn't reconnect. My car wouldn't start either. Leaving me stranded at the charging station . I called for tow

Due to the design of the grill located at the front end of the 2025 toyota grand highlander the vehicle poses vulnerability for multiple debris items to fly up into exposed air conditioning condensor. The damage caused by the debris is not covered by the warranty because it is considered an outside occurrence. However the design creates the vulnerability for the ac to become damaged. It is also s
... itting below the radiator posing an additonal risk for damage and should have additonal protection to help prevent damage. I am currently having to dispute the refusal to cover the damage to my own ac condensor done by a rock flying into the grill space causing damage and losing freon, because they state it is an outside occurrence regardless of the fact that the design is faulty. There are multiple people who have experienced this happening to their vehicles some not even at the 2000 mile mark and are responsible for the financial cost. Upon doing research the 2026 toyota grand highlander model was created with an even lower setting bumper that provides better coverage and protection for the air conditioning condensor and all other important mechanical components. Suggesting that Toyota is aware of the issue if they created an additonal part to help protect that area from damage. I am literally at risk each time I drive my car just for the fact that rocks are on every road way as well as other debris. I have called the customer relations department with no resolution given only to be told I am responsible for the cost of the repair. I believe this is something that needs to be addressed as well as fixed for the sake of the buyer and the already large cost of owning and maintaining a vehicle. If it happens more than a few times its not bad luck, it's a flaw in the creation of the product and consumers should not have to be held financially responsible for someone else mistake, whether individual or corporate entity. I have previously owned 4 other toyotas with no issues!Read more

I was driving on the highway going approximately 75mph, everything was 100% normal. All of a sudden, and at once, my steering wheel started to shake, the car lost power/acceleration and speed quickly dropped. The engine was making loud noises. We pulled over, called my mechanic I've had for 35yrs. We were able to get the car to the garage driving very slowly, it wouldn't go above maybe 30mph. My m
... echanic initially did a diagnostic test which indicated issues with coils??After further investigation he, and 2 of his fellow mechanics determined it was a much more serious issue that has resulted in a total malfunction of the motor. I bought my car in 2021 with 46,740 miles on it, it currently has 84,756 miles. I still owe $4349.63, 1yr left on my loan. My mechanic said he is aware of a number of recalls on jeeps . I looked up my VIN bought found nothing. Similar issues, but nothing with that VIN.Read more

I'm reporting a rearview camera issue in my Hyundai Santa Fe 2020. The camera image was previously static/distorted and has now completely stopped working. When the vehicle is placed in reverse, no image is displayed. This creates a safety risk, as the rearview camera is essential for visibility and preventing accidents while backing up. I am aware there has been a recall for similar rearview came
