I am writing to formally document an urgent and ongoing safety issue with my new Audi Q8 (VIN [XXX] rrently has fewer than 1,100miles. On my recent visit to my local Audi authorized dealer's service department (Tuesday, 4/21/2026), I brought the vehicle in for a “central lock malfunction” warning and was advised that the issue had been resolved. However, the next morning the same error reap
... peared. This warning has been present intermittently, beginning the day I purchased the vehicle at 0 miles. More critically, this malfunction is affecting the driver-side door’s ability to open from the inside. On multiple occasions, I have been unable to exit the vehicle without repeated attempts. This presents a serious safety concern, as it temporarily prevents me from exiting the vehicle in a normal and timely manner. Given the nature of this issue, I want to clearly document that: - This is a safety-related defect impacting my ability to exit the vehicle - The issue was not resolved after the initial repair attempt - The problem has reoccurred immediately INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6) INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Coolant leak observed at the upper radiator hose quick-connect fitting. The leak appears to originate from the internal seal at the radiator inlet connection. This issue developed without warning and is consistent with a known defect in the radiator hose seal design used in 2023 GMC Canyon vehicles. Vehicle experienced coolant loss and visible residue at the connection point. No visible external
... damage to the radiator or hose assembly was observed. GM has issued a Special Coverage program for similar radiator inlet hose seal failures in related vehicles using the same component design. Concern is that this defect may lead to coolant loss and potential engine overheating if not addressed.Read more

The frame of my 2018 Chevrolet Silverado 1500 has rusted through to the point of failing Maine state safety inspection at only 41,000 miles. A licensed Maine inspection station identified structural frame corrosion and determined the vehicle is unsafe to operate. A Chevrolet dealer confirmed the frame rust and stated the truck must have rusted sections cut out and welded to be made roadworthy. The
... vehicle is available for inspection upon request. Safety risk: A rusted-through frame on a pickup truck is a serious structural defect. Frame failure while driving could cause loss of vehicle control, separation of suspension or drivetrain components, or catastrophic structural collapse, particularly at highway speeds or under load. The vehicle is currently unregisterable in Maine due to this safety failure. The failure has been confirmed by both an independent licensed inspection station and by a Chevrolet dealership service department. This failure is consistent with a known manufacturing defect documented in GM Technical Service Bulletin 19-NA-255, which acknowledges that the wax coating applied to the frames of 2011–2024 Silverado and Sierra trucks was improperly applied by a third-party frame supplier, causing the coating to peel and exposing bare steel to premature corrosion. GM has issued this TSB but has not issued a recall, and Chevrolet dealers are not consistently performing the repair. I contacted GM Customer Assistance to request goodwill repair and was refused; the representative also declined to open a case. There were no warning lamps or dashboard messages prior to discovery, as frame corrosion is not monitored by vehicle systems. The defect was identified visually during state safety inspection. I am requesting that NHTSA investigate this defect pattern and consider a recall, as was ultimately ordered for the similar frame corrosion defect on Toyota Tacoma trucks.Read more

Was driving my vehicle and the service light came on that said (SVC 4X4) when I took the vehicle in the Fury Dealership to get a diagnostic check, I was told that my PTU system need to be replaced and it would cost $4500.00 to repair, upon doing some research it seems that Jeep Cherokee has had MANY problems with this particular parts failure. Upon going back and forth with the Service manager ab

*The end of March, my car suddenly started making an awful grinding noise, jerking, slowing down even with foot on excellerator. Towed to nearest Lia in Leesburg. Couldn't find anything wrong. Happened two more times didn't take in. Next time it happened in April towed in. *My engine was replaced in April of 2025. *Was fine until 12/10/25. Same grinding, etc. Was worse than before. Found out my 5

That has expired. I thought I had purchased 5 year extended warranty when I bought out lease but I don't. So I don't know what I paid for. I do not feel confident that it is fixed nor do I trust dealer. *I have asked both dealerships to give me a written explanation on what they have tested for and dates and serial # on parts. I was told that info was only for dealer and won't let me know. I want to know if any damage to parts that were source of grinding. I have all or most of paperwork for service. I am taking car to another reputable auto repair to check it over for so I can decide to get rid of it. *I want to make sure it is on record that I have let you know that fixes haven't worked. While traveling at normal highway speeds (55 MPH), the vehicle's battery capacity rapidly dropped from 80-90% down to 10% in less than 30 seconds, resulting in activation of the vehicle's "turtle mode" and power throttling, which made it impossible to maintain posted highway speed of 55 MPH. A warning appeared on the dashboard, which stated, "Warning Power Reduced". The vehicle traveled at approxi
... mately 40 MPH creating a serious hazard to others on the roadway who were traveling at the posted speed limit of 55 MPH. Upon pulling over to the side of the freeway, the battery charge rebounded back up to 75%, but maintained power throttle, making it unsafe to reenter the roadway, as the vehicle was incapable of accelerating to the posted speed limit of 55 MPH. It was extremely slow to accelerate at all, making it from 0 - 40 in about a minute, which again, was extremely hazardous to others on the roadway. Nissan has since acknowledged that this was due to a factory defect, and covered under their factory warranty. I have attached photos of the warning message displayed, and have videos of the rapid battery loss, but those files are too large to attach.Read more

I am writing regarding the defective condition of the 2022 Nissan Pathfinder (approx. 16,431 miles) purchased on Nov 10, 2025. With the onset of spring, the A/C system has failed, blowing only ambient air. This aligns with known manufacturer defects: Jump Tube Defect (PC899/PC939): Faulty seals causing refrigerant leaks. A/C Amplifier (NTB23-033A): Irregular airflow and spontaneous surges. Notably
... , I reported HVAC and mechanical concerns within the 90-day warranty period. On Dec 20, 2025, I notified the dealership of whistling sounds/airflow bursts and clicking noises in the steering system. Although technician dismissed these as "normal" at the time, reporting them within the warranty period preserves my right to repair. While previous issues (loose bolts/chassis vibration) were resolved, the HVAC irregularities persist. On April 18, 2026, I returned to report the A/C failure and was denied a complimentary diagnostic, despite the vehicle being sold in a defective condition. Requested Actions: Full Diagnostic: A comprehensive, no-cost inspection of the entire HVAC system. Complete Repair: Replacement of the condenser, jump tube, seals, A/C amplifier, and related components under dealer responsibility/manufacturer goodwill. The sunroof wind deflector mechanism on my 2021 Kia Telluride failed due to what appears to be a known defect. The passenger side wind deflector arm broke off and upon trying to close the sunroof it made grinding noises. This caused the sunroof to malfunction and created a situation where the sunroof could not operate properly and is now stuck open. Kia has acknowledged issues with this compone
... nt in a Technical Service Bulletin, indicating prior knowledge of defects in the wind deflector arms. However, no recall or owner notification was issued. Because of this lack of notification, I was not aware of the need to inspect or repair this part when the issue was minor. Instead, the failure progressed and now requires a significantly more expensive repair involving major sunroof components. The sunroof being stuck open is a distraction when driving and during rain has allowed water to leak into the car. Fortunately, I was parked when the wind deflector arm failed. Had this occurred while driving, the abrupt malfunction could have created a dangerous distraction and compromised safe vehicle operation.Read more

Hi! I brought my car in to the Ford dealership in Lexington, SC to get a recalled A Frame Pillar repaired. They took the part off and didn’t repair it because they didn’t have the parts in. When I called corporate to check on the status of that part, I found out they had marked the recall as “complete” which was not the case. Corporate opened the case again for me, so it should’ve read i
... ncomplete in their system. (Therefore, maybe they would call me to let me know the part was in stock. That didn’t happen.) When I called back 2 months later, they had apparently marked it as complete a second time. Which again, was false. 12 months later, they’re finally agreeing to replace the part. I also found out from corporate in that same phone call that I had a recall to a power steering component. When I called the shop, they said that had been marked as complete too so I was not eligible for a second replacement. I have a hard time trusting that the original was actually replaced now. And the steering feels like a major safety concern! Point is, I’m worried that location may be doctoring their documentation, marking recalls as repaired when repairs have clearly not been made. I’d hate for it to cause a major accident or something. Bringing this up in case it warrants an audit. I’m sure I’m not the only person they’ve done this to. I’ve opened a case with Ford’s Automatice Safety Office as well and they are investigating but I thought it prudent to mention to an outside source. Like I said, I’d hate for someone to get in accident because they thought a repair had been made when it actually never was.Read more

My 2022 Ram 1500 TRX has recurring safety-related failures of the infotainment system affecting Uconnect 5 (12” screen). These issues are widespread in 2021–2024 Ram TRX and Ram 1500 models and began or worsened after forced over-the-air updates. The primary defect is black-screen failures while driving. When the screen goes black or freezes, the truck loses all functions controlled through t
... he display, including HVAC/defrost, backup camera, vehicle settings, seat controls, and performance gauges. This creates an immediate safety hazard: I cannot adjust climate/defrost, cannot see the required rear-view camera when shifting to reverse, and cannot access key vehicle information. Owners across multiple years consistently report: • Black screens, freezes, and reboots • Loss of HVAC control and dual-zone functions • Backup camera not displaying • Backup camera freezing with picture • Vehicle/Apps tabs failing to open • Uconnect error messages • Smaller, harder-to-read gauges after updates • Wireless CarPlay/Android Auto failures • No option to decline or roll back OTA updates These failures distract the driver, remove access to federally required equipment (rear camera), and eliminate visibility and climate controls that directly affect safe operation. The system becomes unpredictable and unstable despite the vehicle being in motion. My vehicle experiences the same issues, including what was listed above Because this affects essential safety-related controls and visibility, I believe this is a safety defect in Ram/Stellantis Uconnect 5 systems. I request NHTSA open a defect investigation and require Ram to correct these failures.Read more

This concerns an unresolved airbag safety issue that went undetected due to a negligent recall inspection at a Kia dealership. On March 16, 2026, I brought my vehicle to Ed Voyles Kia of Chamblee, GA for a recall inspection to determine whether damaged wiring under the passenger seat was causing my airbag warning light to remain on. I dealt with the service advisor throughout the process. After th
... e appointment, I was told the inspection was complete and no wiring damage was found. Upon leaving the dealership, I immediately noticed that my exterior and interior lights, radio, seatbelt indicators, and numerous other systems had stopped working, none of which were issues before my appointment. I contacted the dealership the same day and returned on March 17, 2026. The service advisor relayed the technician's position that these failures were pre-existing and unrelated to the service. After I pushed back, the manager became involved and pointed to a separate BCM recall to explain the replacement, rather than acknowledging that the BCM had failed immediately after their service. My BCM had been functioning normally before the appointment. The service advisor and manager continued to deny any wiring damage. On April 20, 2026, I personally inspected the area under the passenger seat while cleaning my vehicle I immediately found visibly damaged wires, the exact area the technician had examined during the recall. I found this damage within minutes. The damaged wiring under the passenger seat is the very issue this recall was designed to identify. The technician failed to find it, leaving a known safety hazard unresolved. My airbag warning light has been on since before the appointment and remains on today. I have been driving my family in this vehicle with a compromised airbag system. I have photo evidence of the damaged wires and request that NHTSA log this as a formal safety complaint and investigate if this reflects a broader pattern of incomplete recall inspectionsRead more

I am reporting an issue related to a safety recall involving the electronic throttle system on my 2025 Nissan. I scheduled two separate service appointments with Mossy Nissan Kearny Mesa (San Diego, CA) to have this recall addressed, and in both cases, the dealership failed to inspect or service the vehicle on the scheduled day. First appointment – March 6 at 8:45 AM: I dropped off my vehicle
... at the scheduled time and left it at the dealership for the entire day. I arranged transportation home at my own expense. At approximately 4:00 PM—over 7 hours later—I was contacted and informed that the dealership’s computers were down and that my vehicle had not been inspected at all. No prior notice was given, and the vehicle remained untouched for the entire duration of the appointment. Second appointment – April 8 at 2:30 PM: I scheduled a second appointment and again arranged my schedule to bring the vehicle in. Upon arrival at the scheduled appointment time, I was informed that the vehicle would not be inspected that day and would need to remain overnight before any evaluation could begin. In both cases, despite having scheduled appointments and leaving the vehicle in their possession for extended periods, the dealership did not perform even a basic inspection related to a safety recall. The recall notice indicated that the repair would take approximately one hour. Instead, I lost two workdays and incurred out-of-pocket transportation expenses without any progress on a safety-related issue. COMPONENT/SYSTEM: High-voltage battery system (traction battery). The vehicle is subject to a manufacturer recall for risk of internal short circuit and potential thermal event. The vehicle and battery are available for inspection upon request. SAFETY RISK: According to the manufacturer and dealer, the defect presents a risk of battery fire, including while the vehicle is parked and not in operat
... ion. As a result, the manufacturer has instructed safety precautions, including limiting charging to no more than 80% and parking the vehicle outdoors and away from structures. These precautions materially limit driving range and require the vehicle to be parked away from the residence, which increases risk exposure to occupants, visitors, and surrounding property. REPRODUCTION/CONFIRMATION: The defect has been confirmed by the manufacturer through a formal recall applicable to this vehicle. The dealer has acknowledged the recall and the associated safety instructions. INSPECTIONS: There has been no individualized inspection specific to this vehicle because the issue is a known, manufacturer-identified defect affecting recalled vehicles of this model. The condition and risk have been acknowledged by both the manufacturer and dealer. WARNINGS/SYMPTOMS: There were no prior warning lights, messages, or observable symptoms before receiving notice of the recall. The safety issue was not detectable to the driver absent the manufacturer’s notification. REPAIR STATUS/TIMING: The manufacturer has advised that replacement batteries are not expected to be available for several months (estimated beginning December 2026 or later). In the interim, the vehicle must continue to be operated under the above safety restrictions, while customer must still bear the known, significant risk of fire until then.Read more
