... ss. After completing the roof panel installation, my wife and I closed the doors and trunk and we walked toward the building. A witness (the owner of the other car that was involved in car collision ) confirmed that the Bronco was still parked where we left it. A short time later, we learned that the Bronco had rolled out of its parking spot and struck a parked car. The Bronco had not been touched, driven, or moved by anyone after we walked away that we were aware of. There were no warning signs that we were aware of. The police were at the scene and filed a report. This was a Enterprise Car Rental Vehicle with approx. 1,300 miles. I am unsure if this vehicle has been inspected by Enterprise. Damage / Injuries: •Collision with a parked car. •No injuries reported. •Police report filed (attached). •Photos available (attached). Why This Is a Safety Defect: A vehicle that rolls away while in Park violates the intent of Federal Motor Vehicle Safety Standard (FMVSS) 114, which aims to prevent accidental rollaway. This Bronco should not have moved under any circumstances once placed in Park. Safety Defect: Data Communication Module (DCM) Failure Affecting Hands-Free Microphone and Safety Connect / SOS Emergency System Vehicle: 2019 Toyota RAV4 Hybrid LE The DCM (which controls both the CarPlay/Bluetooth hands-free microphone and the Safety Connect SOS emergency button) suddenly failed without any warning or accident. Symptoms: - No one can hear me during CarPlay or Bluetooth calls

This is a known recurring issue documented in Toyota TSBs T-SB-0112-20 and T-SB-0089-21. Toyota of Hackensack diagnosed it and quoted $1,300 for DCM replacement/update while charging me $200 for diagnosis. They did not disclose during the multipoint inspection that this is a documented safety-related DCM fault. I performed the manufacturer-recommended 12V battery disconnect reset (standard TSB procedure), which fully restored the microphone and returned the SOS light to solid green. However, this is only a temporary fix for a known defect. This failure compromises hands-free calling while driving and the emergency SOS system — both critical safety features. I request NHTSA investigation and urge Toyota to issue a recall or service campaign for all affected 2019–2025 RAV4 Hybrids.Read more

... backward down the driveway and then across the sidewalk (on our side of the street), then across the street, and then across the sidewalk on the other side of the street and into the neighbor’s yard where it finally came to a rest. She said she heard something like grinding gears. I presume my son was trying to put it in park. Thankfully no property damage or personal injuries occurred during the incident. I [XXX] ) was not home when this happened. When I returned home, I inspected the van to see if my son was able activate the emergency shift lock release. I found the cover was intact and evidence of tampering was not observed. I then checked to see if I was able to pull the shifter out of park. Initially, I was unable to, but after a few tries and a little force, I was able to pull the shifter out of park (no keys in ignition, no brake compression, vehicle off) and the van began to roll. Our driveway is somewhat sloped. I replicated this procedure several times: a little force and a little wiggle and clunk, the shifter released and the van rolled. I discovered there was a recall for this very problem on 2010 Sienna’s (NHTSA Recall No.: 17V-657). I entered the Sienna’s VIN number on NHTSA’s website and found no active recall for the VIN number entered. I contacted Toyota of North America (1-800-331-4331) and chose to advocate and spoke to an agent and reported the problem. I was told the issue would need to be duplicated or that they could only inspect it if a code had been stored once the car is repaired. The car is currently being repaired with close to 7,000 in front end damage. I am reporting this because a sudden unintended acceleration presents a serious safety risk and could easily have caused a more severe crash or injury.
