My vehicle's factory applied paint which is Blizzard Pear 070 is failing and peeling on entire vehicle in multiple areas including roof, hood, interior doors, side panels, and rear door. This started as chipping back in 2019 and then peeling around late 2019 and has progressively gotten worse with even being garaged. This is a know issue acknowledge by Toyota customer support program that has s
... ince expired. The paint is delaminating down to the primer with bare metal exposed. Yes it is available for inspection and actually had my local Toyota dealer who I purchased from take pictures last month and tell me the peeling paint is from the know manufacture defect. This is a could pose a safety risk as peeling paint can detach while driving and and also the exposed metal is susceptible to corrosion which could lend to body panels deterioration over time. Toyato manufacturer was contacted and I was told the customer support warranty is expired. Although Toyato said they sent out recall noticed I never received them. Back in 2019 I approached the dealership service dept with issue but they didn't document it and at the time told me it was general wear and tear. Now Toyato manufacturer is not standing behind it. They dealership also told me there are many others having the same issue when I took my car in to get assessed. If there are this many people Toyato should stand behind the paint defect and extend the opportunity for it to be fixed.Read more

I purchased a new 2026 Lincoln Nautilus only 3 days ago. While driving on a public road, both side mirrors suddenly auto-folded on their own without any driver input. This created a serious safety hazard because I immediately lost side visibility while operating the vehicle. I had to pull over in the middle of traffic and manually restore the mirrors before continuing to drive. This malfunction
... is extremely dangerous and unacceptable in a brand-new vehicle. Side mirrors are critical safety equipment, and unexpected folding while driving could easily lead to an accident. I request that this issue be investigated as a potential safety defect affecting the Lincoln Nautilus.Read more

Issue: Tailgate unlatches unexpectedly Erratic behavior consistent with electronic release switch malfunction I am reporting a safety concern involving the tailgate on my 2022 Chevrolet Silverado 3500 HD. The tailgate exhibits erratic behavior and has unlatched unexpectedly without user input. I can support this claim with documented video evidence I took while the vehicle was parked. This condi
... tion is consistent with the defect described in General Motors safety recall N232426400, which involves water intrusion into the tailgate exterior electronic release switch, potentially causing a short circuit and unintended tailgate opening. Although my vehicle is not included in the recall population based on VIN, it is the same model year, platform, and equipped with the same power-unlatching tailgate system. The failure mode I am experiencing matches the recall description exactly. I brought the vehicle to a dealership for diagnosis. The dealership acknowledged familiarity with this issue and indicated they have observed similar failures on other trucks outside the recall VIN population. However, because my VIN is not included in the recall, I was told the repair would not be covered. General Motors offered only minimal goodwill assistance, and I was required to pay for the repair. This condition presents a safety risk, as an unlatched tailgate can allow cargo to exit the vehicle and create a roadway hazard, increasing the risk of a crash. I am submitting this complaint because the defect appears to extend beyond the currently identified VIN population, and I believe additional vehicles may be affected.Read more

I am filing this claim because a mandatory remote software update pushed by Volkswagen, combined with recent invasive repairs to the interconnected safety restraint network (PODS), caused a critical emergency safety feature (SOS) to fail, and the manufacturer is refusing to restore the vehicle to compliance. 04-01-2026: Received mandatory manufacturer software update v3.8.1. Updated failed and h
... ad to be reset. 04-16-2026: Vehicle was brought to the dealer to repair the Passenger Occupant Detection System (PODS). The technician unplugged electrical harnesses and recalibrated the safety network to install the new sensor mats. 04-29-2026: Received mandatory manufacturer software update v3.8.1. 04-30-2026: Immediately following these two events, the emergency SOS system threw a critical hard failure light and became non-functional. 05-01-2026: The dealership and VW Corporate both denied Goodwill coverage, claiming it is an out-of-warranty hardware failure, despite it being a globally documented software side-effect. The v3.8.11 update is known to overwrite the measured state of health of the emergency backup battery, artificially timing it out after 5 years regardless of its actual physical health. The PODS system that the dealer just serviced actively talks to the vehicle's Supplemental Restraint System. I am requesting that Volkswagen covers 100% of the cost to restore the vehicle's emergency SOS system to working order—whether that requires a physical replacement of the telematics module/battery or a manual software reset of the battery history timer.Read more

I am submitting a safety complaint regarding my Ford vehicle’s infotainment screen failure, which has been identified under a recall but currently has no available remedy or repair timeline. When the screen goes blank or becomes unresponsive, I lose access to critical safety features, including the rearview camera, which is federally required. This significantly increases the risk of accidents
... while reversing, especially in areas with pedestrians. The system also affects access to navigation, hands-free communication, and important vehicle alerts, forcing me to either drive without these features or become distracted trying to compensate. Although Ford has acknowledged this issue, there is no clear timeline for a fix, and no accommodations have been provided. I am expected to continue driving a vehicle that does not meet expected safety standards, which puts both myself and others at risk. I rely on my vehicle daily, including transporting passengers, and the lack of a functioning safety system is a serious concern. The absence of communication and support from Ford regarding a resolution is also concerning. I respectfully request that NHTSA investigate this issue and require Ford to provide a timely remedy or appropriate accommodations, such as a loaner vehicle or interim solution, to ensure driver and public safety while awaiting a permanent fix.Read more

The touchscreen dedicates 2/5th of the screen to what they call "traffic visualization" which is the car depicting where the surrounding vehicles, traffic lights, etc. are. While this display is useful for the self driving software engineers to compare what the car is seeing vs. the actual traffic near it, for the actual driver this display is nothing more than a VERY DISTRACTING VIDEO GAME! w
... hich diverts driver's attention away from the road to the touch screen. And this display cannot be turned off by the driver. In fact it can be expanded to cover the entire touchscreen....even MORE distracting! The visualization changes to a vertical view when parking to display other vehicles and parking lines which IS useful, but during driving the traffic visualization is the same as watching a movie on the touchscreen.....and I believe NOT allowed both by Tesla and NHTSA. Please have Tesla remove the dangerous traffic visualization display while moving more than say 10mph so I can be used for parking, but not for anything else. I am not sure why Tesla wastes so much of the limited touchscreen space on a useless safety distraction when their are other items that can be displayed in that screen space most drivers would appreciate. Most Tesla models only have the one screen compared to other brands that now feature 2 or more that seem to be appreciated by buyers so when Tesla complains about removing it, remind them of this point!!Read more

Safety Complaint Description Vehicle: 2025 Kia Sportage Hybrid LX 2025 Mileage: ~6,000 miles Issue status: Ongoing, multiple dealer visits (3 attempts), unresolved Related Kia case number: [XXX] Problem Description The vehicle demonstrates significant highway instability and unsafe handling behavior under normal driving conditions. At highway speeds (approximately 50–70 mph), the vehicle exhib
... its excessive lateral movement and instability, especially after hitting potholes or uneven road surfaces. The car shifts horizontally after impacts, requiring immediate steering correction to maintain lane position. On grooved concrete highways (such as interstate routes toward [XXX] [XXX] ), the vehicle feels like it is floating and swaying continuously, even without strong wind conditions. The vehicle is also highly sensitive to moderate crosswinds (around 20–25 mph). At highway speeds, this results in noticeable rear-end drift and instability, requiring frequent steering corrections and creating concern about potential loss of control. Overall, the handling behavior is beyond normal expectations for this vehicle class and creates a safety concern at highway speeds. Safety Concern These conditions create a risk of loss of vehicle control, particularly: -After hitting potholes at highway speeds -During moderate crosswinds -On grooved or uneven concrete highways The instability increases driver workload and raises concern about potential accident risk if overcorrection occurs. Repair Attempts The issue has been reported to the dealer and service center three times, but no permanent resolution has been achieved. The problem remains reproducible under normal driving conditions. Additional Notes Concern exists regarding whether proper pre-delivery inspection (PDI) or alignment verification was fully completed prior to delivery, as the issue has been present since early ownership and persists at ~6,000 miles. I am filing this complaint regarding a recurring and dangerous safety issue with my 2021 Subaru Forester. The auto stop/start feature intermittently fails to restart the engine after it shuts off when stopped. When this occurs, multiple dashboard warning lights come on, an alarm sounds, and the vehicle requires putting the transmission into "Park" and shutting it down completely before it will st

The local Subaru dealership service department was unable to replicate the issue, however upon inspecting the vehicle's charging system, the battery was found unable to hold charge from the alternator and was replaced on June 9th, 2025. Now after less than 1 year of replacing the battery, the problem persists. I have been made aware of TSB 16-118-24R (TCM reprogramming for auto start/stop restart failure) and TSB 09-80-21 (Thermostatic Control Valve failure) which appear directly related to this issue. This is a serious safety hazard. A vehicle that cannot restart in live traffic puts the driver and other motorists at significant risk of a crash. Dear NHTSA, I am writing to formally report a dangerous design oversight and safety hazard regarding the trunk lid support mechanism on the Nissan Altima. On [Date of Incident], while the vehicle was parked, a gust of wind caused the trunk lid to slam shut unexpectedly. The metal door latch struck me directly in the frontal region of the skull, resulting in an injury that required medical monitor
... ing for potential brain trauma. The current design relies exclusively on torsion bars or gas struts to maintain an open position. These components are prone to wear and environmental fatigue, and they lack a mechanical "positive lock" or secondary safety catch. Consequently, the trunk lid acts as a sail in windy conditions, easily overcoming the lift support and turning the heavy lid and metal latch assembly into a falling hazard for any consumer loading or unloading the vehicle. While Federal Motor Vehicle Safety Standards (FMVSS) focus heavily on internal trunk releases (Standard No. 401), there is a significant gap regarding the prevention of sudden, gravity-induced or wind-induced lid collapse. I am requesting that the NHTSA: Investigate the frequency of "unintended closing" injuries related to Nissan Altima trunk lift systems. Evaluate whether the lift-support specifications for this model are sufficient to withstand common wind gusts. Mandate a mechanical locking mechanism or more robust secondary safety catches to prevent these lids from becoming dangerous projectiles. Safety Issue: Defective interior cabin camera causes Tesla Full Self-Driving (FSD) to disengage abruptly and without adequate warning at freeway speeds. I am a [XXX] driver. I purchased FSD capability with this vehicle in December 2018. In October 2025, I had the FSD hardware upgrade (HW2.5 to HW3) installed at Tesla's Encinitas, CA service center (B.A.R. License [XXX] ). Within days, FSD would
... became completely unusable due to a recurring "Cabin camera unavailable — schedule service" alert that instantly disengages the system. The disengagement occurs on the freeway at highway speed, without meaningful advance warning. As a [XXX] driver, the sudden loud alarm and loss of assisted driving is genuinely alarming — it is not immediately clear in the moment whether the vehicle has lost power or experienced a critical failure. This is exactly the kind of dangerous situation driver assistance systems should prevent, not cause. Tesla's own diagnostic confirmed the defect (Diagnostic Invoice No. XXX, November 27, 2025, Encinitas service center): "Cabin camera unavailable" alert verified Intermittent stream exit faults detected in the selfie/cabin camera Low-voltage circuit and wiring checks were normal — ruling out wiring Conclusion: fault is within the camera module itself; technician recommended replacement The defect is FSD-specific. The cabin camera works correctly for Sentry Mode, live security viewing, and all normal driving. It fails exclusively when FSD is engaged and demands sustained driver monitoring. The camera housing also becomes extremely hot to the touch during FSD use, consistent with reports from other 2018 Model 3 owners describing identical symptoms after their HW3 retrofits. The vehicle experienced failure of the Anti-lock Braking System (ABS), including illumination of ABS and traction control warning lights. The failure was confirmed by a dealership, which identified an internal ABS module failure and wheel speed sensor issues. These components are available for inspection upon request. This issue is associated with Safety Recall 94B, which addresses defects within
... the ABS system. At the time of failure, there was no remedy available under this recall. The failure impacted critical safety systems, including braking stability and traction control, creating a potential safety risk, particularly during braking or adverse driving conditions where ABS functionality is essential. The issue was reproduced and confirmed by an authorized dealership, which replaced the ABS module and related components after diagnostic testing. Prior to failure, warning indicators including ABS and traction control lights were present on the dashboard. A recall remedy for Safety Recall 94B was later released. However, despite the failure involving the same system identified in the recall, the manufacturer has denied that the repair is related, without providing a technical explanation. This raises concern regarding whether similar safety-related failures are being consistently identified and addressed.Read more

I am filing a safety complaint regarding my 2017 Ford Explorer XLT with the 3.5L V6 engine due to a known internal water pump design defect. The water pump is located internally and driven by the timing chain. When it fails, coolant can leak directly into the engine crankcase and mix with motor oil instead of leaking externally where it would be easier to detect. This creates a serious safety haza
... rd because the engine can suffer catastrophic damage, sudden failure, loss of power, or complete shutdown, sometimes with little or no warning. In my case, the vehicle experienced symptoms consistent with internal water pump failure, including my car overheating out of nowhere, forcing me to coast to the side of the highway in very unsafe conditions. This defect can destroy the engine and create dangerous driving conditions, especially if power loss occurs while driving with children or passengers, thankfully I didn't have any of my three children with me when this happened. This appears to affect many Ford Explorer vehicles from similar model years, and numerous owners have reported similar failures. The repair cost is extremely high because replacement often requires major engine disassembly or full engine replacement. I am currently financing this vehicle with a balance of $14,000 left to pay and have already paid close to $28,000. This is extremely unfair and the cost to repair is close to $5,000. I don't see a way for myself to afford that. I believe Ford should be required to address this issue because: The design can allow coolant contamination inside the engine without obvious external leakage. Failure can occur suddenly and create safety risks. Repair costs are excessive for what appears to be a widespread defect. Owners may not receive sufficient warning before catastrophic failure. I am requesting that NHTSA investigate this defect for safety implications and determine whether a recall or broader manufacturer accountability is warranted. Thank you for your time.Read more

The charging system randomly drops voltage causing the motorcycle to go into a limp mode while driving limiting speed to 55 and reduced throttle input which causes safety concerns while driving down the road. Harley Davidson is
... aware of the issues but will not repair the issues. This is a common problem with most Harley pan americas. Harley has updated the charging system on newer models along with the right hand control module but will not issue a recall to fix the previous model bikes affected by these problemsRead more

Vehicle E-Clutch system malfunctioning. When riding the e-clutch warning lights will flicker and cause the bike to jult uncontrollably for about 3 seconds. It happens at random, no one situation is the same, it could be in the middle of a turn, it could be at a stop light, it could be getting up to speed, etc. This has happened over 10 times while riding and after taking it to the dealership they
... were unable to reproduce the warning lights and the issue didn't show up when running the codes. After picking it up for the second time, the malfunctioning started again within the first 20 minutes of riding it. This is not just a user error, or a operating error like the dealership tried to tell me it was. This is a bigger issue. This almost cost me my life and I won't let this be brushed under the rug. This specific incident was the worst of the bunch. It was about 6 hours after I picked the bike up from the dealership the 1st time. I was out riding with a friend making frequent stops at resturaunts and gas stations. The E-clutch light had already been on at this point but per the dealership they told me it should still be safe to ride. Obviously not. After our last stop of the day, the bike decided (as I was getting up to speed) to junk uncontrollably for about 3 seconds, shut the engine off, then turn the engine back on and accelerate. At this point I hit the kill switch and glided to a safe spot to pull over and assess the issue. This could have easily costed my life if I had been at a higher speed. The repair order attached is from the 2nd repair. The first repair was able to get covered by warranty I guess so I didn't get any paperwork.Read more

I was driving my vehicle on multiple occasions in which the car started to alarm and say, "Check charging system" at which point the car stopped all together. Also, when the cruise control is engaged, it will turn off automatically. It doesn't matter the speed the cruise control is set at, it will turn off.
