The vehicle experienced failure of the air conditioning system caused by an A/C compressor shaft seal leak on my 2021 Honda Pilot. The issue was diagnosed and confirmed by both an independent mechanic and an authorized Honda dealership. The dealership confirmed that the recommended repair is replacement/repair of the compressor shaft seal consistent with Honda Service Bulletin (TSB) 23-050. The ve
... hicle and failed component are available for inspection upon request prior to repair. The failure created a safety concern because the inability to properly operate the air conditioning and defrost system can reduce driver visibility due to windshield fogging and create unsafe driving conditions during warm and humid weather. Excessive cabin temperatures can also negatively impact driver concentration and passenger safety during travel. Prior to visiting the dealership, my mechanic identified the likely issue as an A/C compressor shaft seal leak and advised me to bring the vehicle to Honda because it appeared to be a known manufacturer issue. The Honda dealership later confirmed the same defect after completing a paid diagnostic inspection. The vehicle has been inspected by an independent mechanic and an authorized Honda dealership service department. A case has also been opened with Honda Corporate regarding the issue and denial of coverage assistance though was told it could take several days to weeks to be resolved. Prior to the failure, there were no warning lamps or dashboard warning messages displayed. The first symptom was the air conditioning system no longer cooling properly when first used during warmer weather around May 2026. Because the A/C system had not been used during the winter months, the problem only became apparent once temperatures increased and the system was needed again. This appears to be a known issue already acknowledged by Honda through the publication of TSB 23-050 covering 2021 Honda Pilot models.Read more

My 2016 Hyundai Sonata is experiencing severe oil consumption due to a known Hyundai engine defect. The engine burns oil rapidly and loses more than a quart between oil changes. This creates a safety hazard because the engine can stall, seize, or fail while driving. The Hyundai dealership did NOT perform the official Hyundai oil-consumption test. They only checked the dipstick, noted the oil was
... low, and submitted a request for a paid combustion cleaning. Hyundai declined to pay for the cleaning, and the dealership is refusing to perform the required oil-consumption test unless I pay $750.00. Hyundai Corporate also refused to authorize the test, stating the vehicle is out of warranty, even though this is a known engine defect affecting many Hyundai vehicles. Without the official test, Hyundai will not review the engine defect or consider a goodwill engine replacement. This is a safety issue because the engine can fail without warning. The dealership and Hyundai are refusing to perform the required diagnostic test unless I pay $750, and refusing to review the defect without that test.Read more

The infotainment system in my Volkswagen ID.4 is experiencing intermittent failures while driving. The issue primarily affects the vehicle’s connectivity with my mobile phone (Apple CarPlay / Android Auto), music playback, and the use of navigation applications such as Google Maps. In several instances, the central display has also gone completely black, temporarily becoming unresponsive without
... warning. The affected component is the vehicle’s digital infotainment system, including the central touchscreen, Bluetooth connectivity, and multimedia and navigation functions. This system is available for inspection upon request. While driving, the system randomly disconnects from my phone, which interrupts Google Maps navigation, music playback, and other phone-based functions. This is particularly concerning because I rely on Google Maps for daily navigation, as the vehicle’s built-in navigation system is not sufficiently accurate or reliable. In my experience, the factory navigation does not provide strong real-time traffic detail and sometimes routes me through significantly longer or less efficient paths. The only advantage of the built-in navigation system is that it displays information on the driver’s head-up/front display, which Google Maps does not support. These failures create a potential safety risk, as the sudden loss of navigation while driving can lead to distraction when attempting to reconnect the phone or restore system functionality. Additionally, interruptions in audio guidance and the occurrence of a completely black screen may further affect driving focus. The issue occurs intermittently and has happened multiple times during normal vehicle operation. It is not consistently reproducible. To date, the vehicle has not been inspected by a dealership, the manufacturer, police, or any insurance representatives in relation to this issue. In most cases, there are no prior warning lights or messages; the system simply disconnects or the display goes blaRead more

Vehicle primarily failed while parked at owner residence [XXX] and remained inoperable for approximately three weeks prior to tow to Cadillac dealership. The ignition key became trapped and could not be removed. A manual override release beneath the driver-side steering column was required to free the key — confirming the lock was held electronically, not mechanically jammed. Fault confirmed by
... authorized Cadillac dealer: intermittent electrical fault within the shifter assembly lock mechanism. A similar intermittent concern was previously documented on this vehicle in 2011 (logged as "Customer Concern Not Duplicated"), establishing a recurring pattern. GM Technical Service Bulletin 07-07-30-032D (2006–2011 Cadillac CTS, automatic transmission) explicitly documents this exact condition: key sticking, Shift to Park DIC message, shifter binding, and battery drain. OEM replacement part is now discontinued. Manufacturer declined goodwill or recall coverage. Dealer refuses repair unless owner assumes full financial responsibility for a used part with no warranty and no guarantee against recurrence. Safety concern: ignition/shift interlock failure causing vehicle immobilization, key entrapment, and loss of transportation for an elderly owner dependent on the vehicle for medical care. Defect evades shop diagnosis due to intermittency. No safe OEM remedy exists. Prior recall: NHTSA 14V394000 / GM Campaign 14172 addressed same interlock system. Description of Defect: When the vehicle is shifted into reverse, the rearview camera display freezes for several seconds before going black. This malfunction creates a direct and serious safety hazard — during the period the display is frozen, the image shown does not reflect actual real-time conditions behind the vehicle. Any pedestrian, child, or object that moves into the path of the vehicle
... after the display freezes will not be visible to the driver. This significantly increases the risk of a backover collision. Dealership Inspection Outcome: I brought the vehicle to an authorized Ford dealership for diagnosis. Upon inspection, the technician was unable to detect the diagnostic trouble code associated with Recall 25S89, and as a result, the vehicle was deemed to have passed inspection. However, the malfunction continues to occur. The failure to trigger the recall code does not resolve the underlying issue — the camera display still freezes and blacks out during reverse operation, leaving the driver without a functioning rearview safety system. Safety Concern: A rearview camera is a federally mandated safety feature. A display that intermittently freezes and then goes dark is functionally equivalent to having no camera at all during those critical moments. This is not a minor inconvenience — it is a safety system failure that puts pedestrians and other individuals behind the vehicle at risk every time the vehicle is put in reverse. Requested Action: I am requesting that NHTSA investigate whether Recall 25S89 adequately captures all failure modes associated with this defect, and whether vehicles that present with this symptom but do not trigger the associated diagnostic code are being improperly cleared at dealerships. A remedy should be identified and applied to all affected vehicles regardless of whether the recall code is detected during inspection.Read more

The transmission shift cable on the vehicle may fail to maintain its connection to the transmission. The manufacturer issued the safety notice on March 8, 2026, but stated that a remedy is "not currently available." As of May 12, 2026 (over 60 days later), the manufacturer has failed to provide a timeline for the repair, failed to respond to formal written inquiries regarding a safety remedy, and has not provided interim safety measures or alternative transportation. Hyundai - DAW - Driver Attention Warning / Forward Attention Warning System This issue warrants immediate consideration for a recall. Despite wearing glasses, sunglasses, or no glasses, the problem persists. I occasional do remove my glasses while driving, and the warning continues to sound. This occurs regardless of the time of day, weather conditions, or whether I am sitting upright or reclining
... . The warning randomly activates and deactivates, sometimes without any intervention. Occasionally, I simply allow the warning to sound without taking any action, and it remains active for a few seconds or until it triggers a warning that the system will be turned off, at which point it deactivates. I have attempted various troubleshooting methods, such as keeping my eyes open, adjusting my seating position, wearing and removing my glasses, and tilting them to minimize glare. None of these methods have proven effective in preventing the warning from activating or in making it deactivate. This is a persistent problem. Hyundai dealerships initially informed me that the warning might be an annoyance and their response was to simply refrain from using it. However, if a basic feature like cruise control in a 2026 model is rendered unusable due to a design flaw (which this certainly is), it constitutes a clear violation of safety standards and should be addressed through a recall. The incessant alerts are highly disruptive and can be quite frustrating. For instance, driving with an arm in the 12 o’clock position triggers a warning, while wearing polarized sunglasses also results in an alert. The presence of a setting to disable the warning is a welcome feature but doesn't exist, and the system’s persistent alerts can be overwhelming. The alerts continue even after taking a break. It is distracting and a safety hazard. Drivers should be able to adjust the sensitivity of the feature turn off this feature.
