I am writing to formally report a critical safety failure involving Ford recall number 19S40 / NHTSA recall 19V767, which affects the driveshaft flexible coupling on my vehicle. While traveling with my wife and five children on a recent trip from Ludowici, GA to Fort Bragg, NC, our van suddenly experienced violent vibrations and loud moaning sounds, forcing us to stop in South Carolina where no ne
... arby Ford service center was available. We were stranded and faced not only travel disruption, but real danger with children in the vehicle. Upon inspection, the flexible coupling failed catastrophically, causing driveshaft damage and rendering the vehicle unsafe to drive. This exact failure is the subject of the ongoing recall, and it is unacceptable that such a well-documented and acknowledged issue is continuing to place families at risk. I am requesting the following: 1. Immediate authorization and support for full repair of the driveshaft and related components damaged by this failure. 2. Assurance that all components are replaced, not temporarily inspected or patched, to prevent recurrence. 3. Reimbursement for all expenses incurred related to towing, diagnosis, or temporary transportation. 4. A formal explanation of how this critical recall is being enforced and monitored. As a soldier and a father, I cannot afford to risk my family’s safety due to an unresolved and clearly documented manufacturer defect. I respectfully urge Ford to treat this matter with the urgency and seriousness it deserves. Thank you for your immediate attention.Read more

The Thor motorhome has several windows. The windows they installed are laminated windows, with two panes of glass and a seal or glue which connects the outer glass to the inner glass. The attachment method for the outer glass is ineffective. As a result, the outer glass pane comes loose from the inner and is prone to fall off or fly off at highway speeds. The risk is that the pane could strike
... another vehicle and possibly cause an accident. Already Thor has replaced two of the window units on this unit around 2021, not under warranty but as a customer courtesy. I now have a third window unit that is experiencing the same problem. It is apparent to me Thor has acknowledged there is a risk present when they replaced the other units. In addition, Thor has issued a recall for several year models after my year model. It seems clear they are aware there is a risk of window failure in model years outside of the recall. Unfortunately, they are unwilling to address the risk and hazard.Read more

> I purchased a 2023 Nissan Rogue brand new and it now has only 32,000 miles. It has a major water leak—flooding the interior through the lights, vents, and floor—yet Nissan won’t cover it under warranty and State Farm denied my comprehensive claim. The vehicle has sat exposed for 3 weeks at the body shop during constant rain, worsening the damage. The car is now unsafe to drive. Nissan fail
... ed to disclosure valid and pertinent information regarding the limitations on their warranty coverage. Furthermore, Nissan failed to disclose maintenance guidelines for maintaining a sunroof. Additionally, regardless of any debris build-up, the size of the water lines are inadequate to hold large volumes of water at a given time. The amount of rain that has occurred in Alabama over the past 3 months super exceeds the capacity of these drainage tubes, leading to interior flooding that is detrimental to the integrity of the safety features in this vehicle. My lights, electrical panel, air bags, and electrical ports have all been impacted by this intrusion and now I'm left with a useless, unsafe, and unpredictable vehicle that has a total of 32,000 miles. This is fraud and deceptive behavior that should be made public. I believe others need to know this could happen. I’d appreciate your help.Read more

The vehicle experienced multiple critical system failures within one week of purchase: Transmission failure (slipping, delayed shifting, and stalling during acceleration) Braking system malfunction (brakes jump or pulse when pressed, inconsistent stopping) Fluid leakage inside the cabin near the gas and brake pedals (believed to be either brake fluid or transmission fluid) The vehicle is curre
... ntly undrivable and can be made available for inspection upon request. The vehicle became a severe safety hazard while I was operating it with my infant child in the car. The brakes were unresponsive and unpredictable, creating near-miss situations when trying to stop. The transmission failure caused stalling at intersections and hesitation when accelerating, putting me at risk of being rear-ended. The fluid leak near the pedals presents a possible fire hazard or the risk of further brake failure. I feared for my life and my child’s safety. The problem has not been confirmed by a mechanic yet because the dealership, Platinum Auto Sales in Arlington, VA, refused to take the vehicle back or inspect it after I reported the issues. I was dismissed when I returned to report the failures. I am currently seeking an independent inspection, but I want this formally documented as an urgent safety matter. I am in the process of filing reports with the Federal Trade Commission, state consumer protection, and local police regarding fraud and sale of a dangerous vehicle. I can make the vehicle available for inspection by NHTSA or a certified safety inspector. There were no warning lights or error messages at the time of purchase. The problems appeared within 24–48 hours of driving the vehicle: Difficulty shifting and hesitation when accelerating began almost immediately Brake issues and pedal fluid leakage appeared by Day 3 By Day 5, the vehicle became unsafe to drive altogetherRead more

I am reporting a known paint defect involving the White Orchid Pearl paint on my 2018 Honda Fit. The paint has begun to peel and delaminate, particularly on the roof and other panels, exposing bare metal. As a result, the exposed areas are now beginning to rust. Honda issued a Service Bulletin covering this issue for seven years from the original in-service date. However, I am not the original ow
... ner, and by the time I discovered the issue, the paint had already begun to deteriorate, and I was told I do not qualify for coverage under the extended paint warranty. This defect appears to be a manufacturer issue related specifically to the White Orchid Pearl paint, which has been widely reported on multiple Honda and Acura models. Honda is aware of this problem but has not offered consistent support to all affected vehicles and owners, especially those who are second-hand buyers, despite the fact that the defect originated from factory-applied paint and is unrelated to ownership. I believe this issue warrants further investigation due to the frequency of complaints, the potential for structural corrosion, and the financial burden on owners. I urge the NHTSA to formally investigate and consider action that will compel Honda to support all affected vehicles, regardless of ownership history. Photos of damage and rust progression are available upon request.Read more

My 2007 Toyota Tacoma has an open recall on the rear leaf springs due to the risk of fuel tank puncture and fire. However, Toyota and their authorized dealership are unable to perform this safety recall because the vehicle’s frame is too severely rusted. The dealership informed me that attempting the repair could further damage the vehicle, and they requested I sign a waiver releasing Toyota of
... liability if harm occurs during the recall process. They also informed me that replacement frames are no longer available, so the recall cannot be completed as I was interested to pay for this to get the recall safely completed This leaves me with a vehicle that presents a known, life-threatening safety hazard, which Toyota cannot resolve. The frame corrosion is due to a previously acknowledged defect for which Toyota offered limited-time frame replacements that expired before I owned the vehicle. I believe Toyota is violating its obligation to provide a remedy for this open recall. I am requesting that NHTSA investigate this issue, as it appears Toyota is leaving customers with dangerous, unrepaired vehicles and no available remedy.Read more

I was driving down the road when the check engine light began to flash. The vehicle started to decelerate and began to sputter. This was a serious safety issue as there were vehicles behind me which put myself and other drivers at risk. I was able to pull over on the side of the road. I cut the vehicle off and waited a few minutes. I cut the vehicle back on and the check engine light was still bli
... nking. I was able to drive the vehicle back to my home but was not able to go over 40 mph and had to repeatedly pull over to ensure my safety. Diagnostic of the vehicle indicated a code P1326. The vehicle was taken to a local Hyundai Dealership to get a second diagnostic. This was truly terrifying for me. I’m glad I didn’t have my son in the car with me at this time. Also done research and it states that the P1326 code in Hyundai Santa Fe sport vehicles, particularly those with 2.0L turbo and 2.4L engines, is linked to a recall concerning connecting rod bearing wear and potential engine failure. This issue is addressed through a recall or a product improvement campaign, which may involve engine replacement or software updates. The knock sensor detection system (KSDS), associated with code P1326, is designed to detect excessive bearing wear and activate engine protection mode. My Hyundai only has 82000 miles on it. So I’m writing this to let you know that this is a big safety issue and needs to be addressed and fixed on my Hyundai.Read more

The MMI (multi-media interface) frequently reboots during driving, resulting in a black screen and the inoperability of key systems such as the backup camera, rear obstacle detection, navigation, and cell phone tethering (for hands-free operation). These reboots require user input, including entering a PIN to log into the user account (if set up) and repeatedly tapping on the MMI, which can be hig
... hly distracting while driving. By its very nature, the reboot catches the driver by surprise and requires diverting attention from driving.Typically, this user input is provided while stationary, immediately after starting the vehicle. This issue constitutes a safety concern, as reboots can occur during reverse parking maneuvers, rendering the backup camera and obstacle detection sensors inoperable until the system reloads. Additionally, reboots completely disable the navigation screens and necessitate multiple user inputs to restore functionality, which can be hazardous while driving. Many states have enacted hands-free laws and requirements that mandate the use of dash-mounted systems to comply with these regulations. The reboots render such systems inoperative. Numerous reports on online user forums suggest that this issue is widespread and affects the same model year vehicles. For instance, see [XXX] . Several Technical Service Bulletins (TSBs) filed with the National Highway Traffic Safety Administration (NHTSA) indicate that available software updates should be applied to address infotainment-related issues. Relevant TSB numbers include: TSB 2067021, TSB 2076404, and TSB 2069112. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

On Thursday, [XXX], I took my vehicle to the authorized dealership Alberic Chrysler Jeep Dodge, located on Kennedy Avenue in San Juan, Puerto Rico, for routine maintenance. While handling the vehicle, the service personnel lifted it using the shop's lift equipment, raising it all the way to the ceiling, which caused severe structural damage to the vehicle's roof. The dealership contacted the Puert
... o Rico State Police, who filed report number [XXX]. Additionally, I was asked to complete a written damage claim at the dealership. The vehicle was under full manufacturer's warranty at the time of the incident. However, the structural damage has now compromised the vehicle's integrity, specifically: - The rear doors became misaligned. - The roof suffered a visible crack, which exposed the interior of the vehicle to weather conditions. - The damage potentially affects warranty coverage, especially related to corrosion protection. We requested that the dealership retain the vehicle on-site to allow for inspection by the manufacturer's technical team. The dealership refused to do so unless I agreed to pay a $25 daily storage fee. We also requested substitute transportation, which they denied. Additionally, according to the NHTSA database, this vehicle currently has an open safety recall. I was never informed of this recall by the dealership at the time of the maintenance appointment, which is a serious failure in the dealership's responsibility to notify the owner and address potential safety risks. The vehicle is used for both personal and professional transportation, and at the time of the incident, it had 8,402 miles. This incident raises significant safety concerns regarding the vehicle's structural integrity, the open recall, and the dealership's handling of the matter. My eyes are drawn to the screen instead of the road, which puts my safety and the safety of my passengers at risk. When I first reported this issue to the dealer, they explained that I was outside the "limited warranty" for the display unit and that it would cost me around $1000 to replace. I declined. About a month ago, I received a class action lawsuit notice in the mail. This notice described my exact issues. I immediately made an appointment with the dealer to get my vehicle inspected and the issue resolved under the settlement. Upon taking my vehicle to the dealer, they explained that the settlement only covers the issue if the CMU needs to be replaced- not if the problem lies with the display screen itself. They inspected my vehicle and said that my CMU does not have issues and that my screen is causing the glitches. They refused to cover the replacement of the screen, even though the Mazda Connect Infotainment Settlement describes my EXACT issues. Why isn't the settlement covering replacement of the screen if that's where 99% of consumer issues lie (according to the dealer)?
