I purchased my truck from Karl Chevrolet in Ankeny, IA around September of 2019. Approximately 1.5 years after purchasing the vehicle, I began having issues with my infotainment system intermittently resetting itself while I was driving. On or about the end of 2021 (after this had occurred a few time), I brought the truck into the Karl Chevrolet in Ankeny, IA, as it seemed to become more frequent

The technician told me that this thing happens all the time with the infotainment systems and it was nothing to be concerned about and because it was intermittent, they may not be able to identify the issue unless it was actively occurring and I was told to come back in if it continued to happen. Shortly after, I had moved to [XXX] in April of 2022, I began to have more frequent issues with the infotainment system randomly resetting and eventually it began impacting phone calls, where people would cut in and out or disconnect, beginning to cause even more of a safety and distraction issue. After conducting some of my own research, I learned that I was not the only one that had issues with the infotainment system in the 2019 Colorado and that GM had published several Technical Service Bulletins (TSBs) regarding the infotainment system, and there had been a class action lawsuit, [XXX] , et al. v. General Motors LLC, that covers the same issues I am having. On January 29, 2025, after the issue progressively getting worse, I took the truck to Gregg Young Chevrolet in Omaha, NE. After conducting a diagnosis they verbally confirmed there were TSBs regarding radio issues that after contacting the GM service center, GM's response was to replace the radio. This was to be at my own cost however, considering GM confirmed through TSBs there are issues with the infotainment system, coupled with safety concerns around distracted driving, I felt compelled to make this complaint. A passenger in the vehicle — a minor child with a documented mold allergy — has experienced allergic reactions as a result of exposure to the vehicle's air conditioning output. This constitutes a health and safety concern. The mold contamination in the HVAC system poses a risk to vehicle occupants, particularly those with respiratory conditions or mold sensitivities. The failure created a significant safety concern because the ESC and traction control systems are critical safety features designed to assist with vehicle stability, traction, and driver control, particularly during emergency maneuvers or adverse road conditions. The warning indicators raised concerns that the vehicle could unexpectedly lose stability assistance, traction management, or engine performance while driving. This placed the driver and others at risk because the vehicle’s ability to respond safely during normal or emergency driving conditions could have been compromised. In my case, both sides of the dual-zone system simultaneously rise to maximum temperature, regardless of the settings selected. This behavior appears to be linked to a failure of the main blend door actuator. While this may seem like a comfort issue at first glance, it can quickly become a safety concern. Sudden, uncontrolled heat output can fog windows, distract the driver, and create uncomfortable cabin conditions that may impair focus. I am reporting a serious and repeated safety defect involving Hehr side windows installed on my 2018 Thor Palazzo 36.1 motorhome (VIN: [XXX] ). Multiple operable side windows on this RV have experienced adhesive bond failure between the scissor arm/operator mechanism and the vent glass pane. This has caused windows to partially detach and violently flap while driving at highway speeds. One rear b

Additional adhesive failures have continued occurring from 2023 through 2025 involving: - Kitchen window - Living room window - Dinette window - Rear bathroom window (including replacement assembly) In several incidents, I was forced to pull onto the highway shoulder to perform emergency temporary repairs using tape and adhesive to prevent windows from fully detaching from the vehicle. In December 2025, while driving near [XXX] , the rear bathroom vent window again separated from the operator arm while traveling at highway speed despite prior repair work. The window was violently flapping in the wind and appeared at risk of completely detaching from the RV. This defect creates a serious road hazard because detached glass/window components could strike nearby vehicles or motorists. I am aware of prior Hehr/Lippert recalls involving adhesive-related window failures, but I have not found a recall covering this specific failure point involving separation between the operator arm and vent glass. The defect appears widespread across multiple windows on the RV and has persisted despite repeated repair attempts. I believe this issue warrants investigation and possible expansion of existing Hehr/Lippert window recalls. They replaced the emergency breaking module and it still does it however, now my vehicle no matter what I do every time I put the vehicle even into park, my emergency brake kicks in. I only recently learned about the existence of this program after visiting a Toyota dealership regarding another issue. At that time, I was told the program had already expired in April 2024 and that Toyota would no longer provide assistance. I believe this is unfair because: - Toyota acknowledged this as a manufacturing-related defect. - I was never properly notified. - Toyota dealerships had multiple opportunities to inform me during regular service visits. - The defect became progressively worse over time. I am requesting that Toyota reconsider my case and provide goodwill assistance or repair coverage despite the expiration date, due to the lack of notification and the severity of the defect.
