After 3 weeks of inconsistent shifting with the automatic transmission and whining and clunking, the car failed to go into gear at all yesterday 1/5/26 while driving near Dulles airport. I had it towed, was told the transmission failed and then I found that this is a systemic design issue with the 2017-2022 cohort of gas V6, 8 speed, AWD Highlanders. Repair will be between $5K and $10K.

I am reporting a recurring safety defect involving the front bumper assembly on my brand-new Toyota Camry Hybrid. The front bumper repeatedly detaches, separates, and pulls away from the vehicle during normal driving conditions. This has occurred multiple times despite repeated repair attempts by the authorized Toyota dealership. Toyota and the dealership have acknowledged that this is a known is
... sue affecting other vehicles and have admitted that there is currently no permanent repair remedy available. Despite this, the defect has been misclassified as “cosmetic,” which is inaccurate and unsafe. The bumper is a critical safety component designed to absorb and distribute crash forces. A bumper that is already detached or partially unsecured compromises crash force pathways and reduces occupant protection in the event of a collision. If an accident occurs, the bumper cannot perform its intended safety function, increasing the risk of injury. Additionally, I can hear air intrusion entering the vehicle through the front area where the bumper is separating, confirming that the vehicle’s exterior seal and structural integrity are compromised while driving. This vehicle has never been in an accident and is operated under normal conditions. A front bumper should not detach on a new vehicle. This is not a cosmetic concern it is a safety defect that places occupants at risk. I am requesting that NHTSA review this issue as a potential pattern defect and require Toyota to address it as a safety matter rather than mislabeling it to avoid corrective action.Read more

The crimping on the A/C hose from the firewall to the compressor failed and leaked all the freon from the system. This is a design flaw, the mounting location causes excess vibration in the hose leading to the crimping failure. There are countless forum posts and other complaints filed by owners of the same vehicle for the exact same A/C hose. This should be a recall from Volvo. It is a major safe
... ty issue for customers like me who live in a humid environment, without A/C the windshield can fog over and hinder visibility. I contacted Volvo Customer Care for reimbursement and was denied as my car was not under warranty. My car only has 40,000 miles on it and this part shouldnt fail in this manner, especially at my low mileage. I have since purchased their extended warranty as this part will eventually fail again (from the design flaw in A/C system mounting location) and I don't want to pay the large repair costs again. The NHTSA should investigate this with all owners of this year/make/model and they will find Volvo needs to declare a recall and reimburse owners for the repair costs.Read more

Component or system that failed or malfunctioned, and availability for inspection The torque converter and transmission system in my 2020 Ford Edge have malfunctioned. The vehicle experiences transmission shuddering/stuttering during normal driving, particularly during low-speed acceleration and gear changes. The vehicle remains in my possession and is available for inspection upon request. Safe
... ty risk The transmission stuttering causes hesitation, jerking, and unpredictable power delivery while driving. This creates a safety risk, especially when accelerating from a stop, merging into traffic, or driving at low speeds in congested areas. The lack of smooth and reliable acceleration increases the risk of rear-end collisions or loss of control, putting both myself and other drivers at risk. Confirmation or reproduction of the problem The issue has been consistently reproducible during normal operation of the vehicle. The symptoms have been observed repeatedly and were confirmed by a dealer/independent service center, which identified the torque converter and transmission as the source of the problem and advised that significant repairs or replacement would be required. Inspections performed The vehicle has been inspected by a dealership and/or independent repair facility. The manufacturer has not yet directly inspected the vehicle beyond dealer diagnostics, but the vehicle is available for further inspection if requested. Warning lamps, messages, or symptoms prior to failure There were no immediate warning lamps at the onset of the issue. The primary symptoms were transmission shuddering, stuttering, and rough shifting, which gradually worsened over time. These symptoms first appeared during regular driving and became more frequent and severe, indicating progressive failure of the torque converter and transmission system.Read more

The front trunk (frunk) on my 2026 Tesla Model Y lacks internal lighting and an internal emergency release mechanism. This creates a foreseeable entrapment hazard, particularly for children or small adults. Unlike rear trunks regulated under FMVSS No. 401 (Internal Trunk Release), the frunk provides no illuminated internal release, no glow-in-the-dark handle, and no means of escape or signaling i
... f the hood is closed. In low-light conditions (nighttime or enclosed garages), the absence of lighting would immediately impair orientation and increase panic, raising the risk of injury or suffocation. I identified this hazard during routine cleaning when my [XXX] child was able to sit comfortably inside the frunk while it was open. Although no incident occurred, this demonstrates that a child can easily enter the frunk and could become trapped if it were accidentally closed during loading or unloading. Once closed, there is no internal method of release. While FMVSS 401 currently applies to rear trunks, it establishes a federal safety precedent that enclosed compartments capable of child entry must include internal, operable, and visible escape mechanisms to prevent entrapment deaths. The frunk presents an analogous hazard and fails to meet these basic entrapment-prevention principles. This is a design defect, not misuse. The risk is foreseeable, preventable, and severe. I request that NHTSA’s Office of Defects Investigation evaluate this condition, review similar complaints for this model year, and consider corrective action (recall or retrofit) requiring an illuminated internal emergency release. The vehicle was equipped with a roof rack and a mounted cargo box positioned directly above the sunroof plane, providing physical shielding of the glass from above. There was no collision, no road debris strike, and no external object interaction. The glass failure was sudden and explosive. After safely pulling over and retracting the interior sunshade to assess the damage, glass fragments fell into the passenger compartment and struck both front occupants. Two children seated in the second row were not injured. Visual inspection shows no identifiable impact point, crater, or localized strike origin. The vehicle is experiencing a malfunction involving the Power Transfer Unit (PTU), which is currently subject to an open safety recall. Our 2015 Jeep Wrangler JK Unlimited just caught on fire. I happened to be in my daughter’s bedroom, 20 feet away from where the Jeep was parked. I thought I was seeing flames reflecting from the TV, but I realized that they were real. My next thought was that my husband was building a firepit fire. I thought that was weird because he was doing it in the front yard, but I didn’t think
... any more about it because I was so focused on my work. I went back to work for a second, but something told me to look again. I did, and then realized that it was the Jeep that was on fire. I screamed for my husband, but couldn’t find him; I was scared that he was in the Jeep. Actually, he had been in the back yard and heard me screaming, so he ran up front to see what was happening. I called 911 while my husband tried to put the fire out with the water hose, which didn’t help- the flames kept getting higher. It took firefighters a while to extinguish the flames and to get it cooled off. The engine appears to be melted. It is in our yard and available for inspection. The fire fighters made a report that my husband has requested. We’re waiting on the insurance company to contact us. The fire smelled electrical. We smelled no gasoline. I was absolutely horrified thinking about what might have happened if it had ignited at night when no one was looking. My daughter, my pets, my husband, and I could have all been casualties of this spontaneous fire. The fact that it was day time, the unusual coincidence that I was where I was so that I saw the fire, and the fact that my husband parked on a different side of the yard than usual, all probably are the reasons we are alive today. Had any one of those events not been in play, it could have been a very different outcome. My husband had no indication that there was a problem with the Jeep. He’d just driven it. He reported no odors and no problems at all. He takes excellent care of it, so it was in great condition.Read more
