As previously noted, the mbrace system on my 2018 GLE-Class includes SOS/Emergency Call and Automatic Collision Notification functionality intended to contact emergency services in the event of a crash or airbag deployment, particularly where the driver may be incapacitated. These are safety systems, not discretionary convenience features. Your replies confirm that these safety functions are being permanently disabled, that no retrofit or replacement solution exists, and that the vehicle will continue to operate without the emergency communication capability it was originally sold with. I appreciate your acknowledgment, and I am documenting Mercedes-Benz USA’s position that these safety systems have been retired with no alternative provided. This is the second time the horn has stopped working. The first time was a year or two ago (records available on request), and both of the two horn units failed. The manufacturer Hyundai replaced them both and told me the issue they were having was the stamped housing of the horn has a seam that folds up and collects water, may freeze, and causes the unit to fail. Clearly they installed two replac

The way I found out was someone almost changed lanes into me, I tried to honk to alert them to not collide with me, and the horn didn't work. Thankfully I was able to stop fast enough to avoid the collision. I will be taking it to Hyundai for replacement again within the next few days. This needs to become a recall because clearly Hyundai hasn't accepted yet that they need to correct their design of this safety feature.Read more

Dear NHTSA Safety Defects Investigation Team, I am filing a safety complaint regarding my 2017 Lincoln MKC involving a child entrapment incident. On 05/17/2026, my vehicle locked itself while the engine was running, the key fob was inside the vehicle, and my [XXX] child was still inside the car. I was unable to gain entry to the vehicle and had to call 911/emergency responders for assistance. It
... took around 30 minutes from the time the vehicle got locked and for me door to be opened by the towing company sent by the 911 operator. Thankfully, the AC was turned on and my child was unharmed. The vehicle should not have allowed itself to lock under these conditions with the key present inside the cabin. This created a serious safety hazard involving a trapped child. Compounding the issue, the LincolnWay remote access system no longer functions because the vehicle’s original 3G modem became obsolete after U.S. 3G cellular shutdowns. As a result, I no longer had access to remote unlock functionality that had previously been available with this vehicle. Vehicle Information: • Year/Make/Model: 2017 Lincoln MKC • VIN: [XXX] • Approximate Mileage: [INSERT MILEAGE] • Incident Date/Time: 05/17/2026 at 4.15pm • Location: [XXX] Additional Details: • Vehicle was running at the time • Key fob was left inside the vehicle • Doors became locked unexpectedly • Child was trapped inside • 911/emergency assistance was required • LincolnWay remote services were unavailable due to obsolete 3G modem support ending I believe this issue represents a significant vehicle safety concern and request that NHTSA investigate whether similar incidents have occurred in other Ford/Lincoln vehicles equipped with these systems. The 911 call was placed around 4.15pm and I have the receipt from the Towing Service company Please let me know if any additional information or documentation is needed. I am reporting a serious safety issue involving my 2025 Nissan Murano SL, purchased on April 21, 2026. On May 9, 2026, while driving from [XXX] to [XXX] , the vehicle began experiencing repeated failures at stoplights and in traffic. While stopped at red lights with the vehicle in drive, the vehicle unexpectedly shifted into neutral and shut off when I removed my foot from the brake pedal and at
... tempted to accelerate. This first occurred on [XXX] at [XXX] . The issue then occurred again at [XXX] and [XXX] . Concerned for my safety, I drove directly to the dealership for service evaluation. The dealership performed a diagnostic scan but reported that no fault codes were found. I was advised to return for further service evaluation. While attempting to drive home, the same issue occurred again at XXX and XXX. At another intersection, XXX and XXX, I recorded video evidence of the vehicle shifting into neutral and shutting off immediately after releasing the brake pedal at a red light. I returned to the dealership, where dealership staff also reportedly experienced the failure during a test drive. The dealership retained the vehicle for approximately one week for additional testing and evaluation. On May 15, 2026, I was informed that a software update had been performed and the vehicle was returned to me. On May 17, 2026, the same issue reoccurred multiple times in XXX. While stopped in a pickup line at Home Depot, the vehicle shifted into neutral and shut off when attempting to move forward from a stopped position. The same issue then occurred again while waiting in line at 7 Brew Coffee. On one occasion, the vehicle shifted into neutral while my foot remained on the brake pedal and the transmission was still in drive. The issue occurred again at the intersection of XXX and XXX while waiting at a traffic light. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Hello, I am contacting Ford regarding a serious concern with my newly purchased 2026 Ford Explorer ST. Within days of purchase, we discovered water leaking from the moonroof area into the interior of the vehicle, including water coming through the seatbelt pillar area , wetting the roof as well, during rain and vehicle washes. In addition to the leak, there is noticeable wind and air noise coming
... from the moonroof area even when the roof is fully closed, (we notified them the day we got the vehicle about this) which raises concerns that the seal is not properly seating or sealing against the vehicle. After noticing these issues, we inspected several similar vehicles on the dealership lot and observed that multiple vehicles appeared to have uneven or improperly fitted moonroof seals as well. This has increased our concern that this may be a manufacturing or assembly defect rather than normal wear or an isolated issue. Because this vehicle is brand new and the issue was reported almost immediately after purchase, we are requesting that Ford review this matter seriously and assist in ensuring the vehicle is properly repaired or otherwise resolved. We are concerned about long-term damage to the interior, electronics, airbags, mold or mildew risks, and the overall value and reliability of the vehicle. We have documented the leak and seal concerns with photos/videos and are currently working with the dealership for inspection. Thank you for your time and assistance.Read more

The Hill Hold Assist system on my 2024 Volkswagen GTI (manual transmission) repeatedly holds the brakes too long when starting on an incline. The brakes remain engaged for 1–2 seconds even after I begin releasing the clutch and applying throttle, which causes the car to stall or hesitate before moving. This creates a safety risk when pulling into traffic or when other vehicles are behind me on a
... hill. This issue has occurred at least four times within the first 1,000 miles of driving. It is easily reproducible on normal hills and happens under normal driving conditions. I have many years of experience driving manual transmission vehicles, including a 2019 GTI, and I have never experienced this kind of delayed brake‑release behavior on any other vehicle. The system’s interference is abnormal and unexpected. There is no option in the vehicle settings to disable or adjust Hill Hold Assist, and many other 2023–2024 GTI manual owners report the same problem. This appears to be a calibration issue that results in unexpected stalls, hesitation, and delayed acceleration, creating a potential safety hazard.Read more

FORMAL DEFECT AND SAFETY HAZARD COMPLAINT TO: National Highway Traffic Safety Administration (NHTSA) Office of Defects Investigation (ODI) 1200 New Jersey Avenue SE, West Building Washington, DC 20590 DATE: May 17, 2026 SUBJECT: Urgent Request for Recall Investigation: Systemic Cloud-Dependency, Hypervisor Power Management Failures, QNX Boot-Looping/Blackouts, Unix Epoch Clock Drifting, High
... -Voltage Contactor Thermal/Fire Hazards, and Core Low-Voltage Module Malfunctions via Telematics Disconnection in the Fisker Ocean Fleet 1. Executive Summary This petition requests an immediate, systemic safety defect investigation into the software architecture, operating system stability, power management, and network subsystems of the Fisker Ocean vehicle fleet (Model Years 2023–2024). Following the liquidation bankruptcy of Fisker Inc. and its affiliates—under Case No.XXX (XXX) in the U.S. Bankruptcy Court for the District of Delaware, overseen by Judge [XXX] —the vehicle's intellectual property, cloud infrastructure, and over-the-air (OTA) software management assets were assumed by a commercial entity, American Lease, and its vehicle management arm (OV Loop/Ocean Loop). Due to the subsequent imposition of a predatory $499/year subscription paywall, thousands of retail owners have been cut off from cloud connectivity. While a missing cloud connection in a standard passenger car traditionally results in a benign loss of convenience features, in the Fisker Ocean's "software-defined" architecture, it triggers a severe, un-throttled software failure loop. This architectural defect forces safety-critical operating systems into permanent boot loops, pins CPU utilization, drops the system clock into a broken chronological state, and drives an aggressive parasitic drain on the 12V battery system. This directly leads to cascade control module failures, key fob inoperability, and accelerated mechanical degradation of high-voltage components, introducing an acute INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACRead more

I am writing to file a complaint regarding the air conditioning system failure in my 2020 Honda Civic Sport. The air conditioner in my vehicle began blowing hot air instead of cold air. I took the vehicle to Honda for inspection and paid $545.15 to have the system recharged and dye inserted in order to diagnose the issue. After this service was completed, I was informed that the evaporator has fa
... iled and needs to be replaced. I was then quoted $2,475.13 for the repair. While researching this issue, I discovered that many other Honda Civic owners have reported similar air conditioning problems involving the evaporator and AC system. I also became aware of a 2021 letter involving American Honda Motor Co. and Honda dealership personnel regarding evaporator leaks in vehicles with VIN numbers beginning with 2HG or 19X. The letter stated that Honda was researching these vehicles in an effort to better understand the issue. Given the number of similar complaints and Honda’s apparent awareness and investigation into evaporator leaks, this appears to be a potentially widespread defect affecting these vehicles, even when properly maintained. The failure of the air conditioning system has caused significant inconvenience and financial burden. I believe this issue should be investigated further, as it may indicate a defect in the vehicle’s air conditioning system. I respectfully request that the NHTSA review this matter and investigate whether the evaporator failure in the 2020 Honda Civic Sport is a recurring manufacturing defect that may warrant further action or assistance for affected vehicle owners. Thank you for your time and attention to this matter.Read more

Around 11 am on Sat., May 2, 2026, I was sitting in a very long line in Starbuck's drive thru when my 2025 Lexus RX hybrid suddenly and uncontrollably accelerated--like an uncontrollable bucking horse. Brakes didn't work. I couldn't stop my out of control car, and the multiple computer sensors also didn't stop the car. It was like the throttle was engaged because of a computer glitch. In an effort
... to avoid hitting the car in front of me, I immediately tried to swerve, but still hit the car, then jumped the exceptionally high curb and ended up in the grass below and almost into the busy road ahead of me. It was terrifying. The car in front of me hit the car in front of her. The exact same issue happened to me on June 18, 2025 when my car unintentionally accelerated--also in a very tight space and going at 2 mph. There was no accident at this time because my car's computer stopped me from plowing into the building immediately in front of me after unintentionally accelerating. The next day at the recommendation of the person who sold me the car, I took my new Lexus to the service department. They gave me a loaner and told me to contact Toyota's/Lexus' Brand Engagement department in order to start the review, which I did. The review was conducted and ended around July 9, 2025 . On Mon., May 4, 2026 I took my car back to the Lexus dealer where I purchased, but was not given a loaner, which I requested, and was told I would need to wait several hours to have this inspected. I then took my car to Boyce Body Werks. A State Farm agent examined my car and initially approved almost $24K worth of damages. Boyce Body Werks is saying when they disassembled my car that there was more damage underneath my car. I also notified Toyota/Lexus about the same issue that happened one year prior. They also refused to give me a loaner, and I am now forced to pay my rental fees. Boyce Body Werks--a Lexus certified dealer has multiple pictures of the damage. My car is still under warranty.Read more

While driving, the vehicle suddenly and without warning experienced a severe, dangerous loss of acceleration. The dashboard immediately displayed a "Reduced Engine Power" warning message along with a "Service StabiliTrak" light, forcing the vehicle into an immediate electronic fail-safe "limp mode." This sudden, drastic reduction in engine power severely restricts the vehicle's ability to maintain
... safe roadway speeds, accelerate from a stop, or merge safely into traffic, posing an immediate rear-end collision hazard to myself and other motorists. Furthermore, the defect simultaneously disables critical active safety systems by shutting down the StabiliTrak and Traction Control networks. A diagnostic scan revealed code P2138 (Accelerator Pedal Position Sensors 1-2 Correlation Not Plausible). Investigation reveals that this is a widespread, systemic safety defect caused by design flaws in the vehicle's body. Rainwater and moisture from clogged factory sunroof drains or cowl seams leak directly down into the interior cabin wall, pooling into the primary instrument panel-to-body wiring harness block (X218 connector) in the driver's side footwell. This water intrusion causes green corrosion on the delicate pins, short-circuiting the 5-volt reference sensor circuit for the throttle controls while the vehicle is in motion. General Motors is fully aware of this exact defect and has documented it internally via Technical Service Bulletin (TSB) #10-00-89-017H (and related GM TSBs). Because this defect causes unexpected, immediate engine failure and disables traction safety networks while driving, it constitutes a severe safety hazard that should be escalated to a mandatory safety recall.Read more

There is no interlock preventing the door from auto opening while the fuel tank is open - which is a critical design flaw, that can result in injury to the individual fueling and hazardous material spills/ hazardous conditions where the pump attendant is directly drenched in fuel. A simple sensor on the door with a software interlock would resolve this issue and prevent future instances. In this case the injury was minor/bruise and discomfort, but it could have been more severe had the driver not reacted quickly, or had the driver had underlying medical conditions such as brittle bones or was on blood thinners.Read more

The exterior paint on my 2019 Volvo XC90 has experienced significant premature peeling and delamination on the front bumper and surrounding body panels. The paint failure appears to be a factory defect related to improper adhesion, primer application, or a substandard clear co
... at process during manufacture. The affected components are available for inspection upon request. Safety concern: Peeling and delaminating paint can obscure visibility of underlying corrosion, accelerate rust formation on structural body components, and create road debris hazards if large sections separate at highway speeds — posing risk to following vehicles. The concern was submitted to Volvo Cars USA Customer Care and assigned a formal case number. After requesting photographs and inquiring about paint protection documentation, Volvo denied any financial assistance without conducting a physical inspection, technical review, or providing a written basis for denial. No dealer inspection was performed, and no independent technical evaluation was offered or facilitated by Volvo. There were no warning lamps or electronic symptoms prior to the paint failure. The defect became visually apparent gradually and has progressively worsened. No prior accidents, repaints, or aftermarket modifications have been made to this vehicle. It is my belief that this paint defect may be a systemic manufacturing issue affecting multiple 2019 XC90 vehicles, and that Volvo Cars USA is aware of or should be aware of this condition. I am requesting that NHTSA investigate whether a defect trend exists across this model year and whether a safety recall or technical service bulletin is warranted.Read more

The latest Tesla autopilot software forces users to engage in an autopilot product survey after disengaging autopilot while driving in order to gain access to audio/radio controls. The survey is not dismissible without reading a complicated prompt and choosing a response, and it completely obstructs audio controls until you participate in the survey. The driver therefore cannot turn off music to l

I had just traded cars with my daughter so I could babysit my triplet 14 month old grandchildren. I drove my 2014 Scion Xd without any issues for 3 hours (from Bath, Maine to the border of Vermont). I traded cars with my daughter, mother of triplets, and she drove off with her husband to attend a reunion. Approximately 10 minutes after my daughter and her husband left, she called and said the car
... made a loud pop sound and she had to pull over. Then, a man ran across four lanes of rt 89 to tell her the car was on fire. My husband fielded the call from my daughter who screamed, 'The car is on fire!' - then the call went dead. I had to drive my husband and triplet grandchildren in the direction of the car, visible by the black smoke plumes, while not knowing if my daughter and her husband were alive. The fire department, a state trooper, and local police were on the scene and I am thankful everyone was safe. Needless to say, this situation was very traumatic for my family. My daughter reported the oil light and the engine coming on, as well as a dash symbol she didn't recognize, right before the car caught on fire. The right hand lane of route 89 had a 1/4 mile streak of oil leading up to my burned out Scion Xd.Read more

am filing a formal complaint regarding an official software update provided by Hyundai that completely corrupted the infotainment/head unit system in my 2017 Hyundai Santa Fe Sport. I purchased this vehicle from CarMax less than three weeks ago. The vehicle came equipped with Hyundai Blue Link, so I contacted Hyundai support to activate the service by pressing the Blue Link button inside the vehic
... le. Hyundai informed me that my vehicle required a software update because the current system did not contain the necessary software for Blue Link to function properly. A Hyundai representative directed me to Hyundai’s official update website and provided instructions for completing the update. Following Hyundai’s instructions exactly, I purchased and formatted a USB drive, downloaded the update using my VIN from Hyundai’s official website, properly extracted the files, and installed the update into the vehicle. During the installation process, the update failed and corrupted the head unit system. The display now continuously shows “Update Unsuccessful” with Error Code 106. Since the failed Hyundai update, multiple vehicle systems no longer function properly, including the backup camera, Bluetooth, hands-free calling, Blind Spot functionality, radio, map/display system, automatic headlights, and automatic radio shutoff when opening the driver door. I now have to manually operate systems that previously worked automatically. I contacted Hyundai customer support multiple times and was repeatedly transferred between departments. I was incorrectly referred to navigation support even though my vehicle has a non-navigation system. Navigation support confirmed this was a Hyundai issue and stated they could not assist. When I contacted Hyundai again, I explained every step I followed, and the representative acknowledged that I appeared to have completed the update process correctly. Despite Hyundai’s official software causing this failureRead more

Radio has recently starting making deafening static after driving for about 15 miles. Cannot reduce the horrible noise with volume control or by changing from radio to BT or USB or any other Source option. It is EXTREMELY loud to the point where I think it will affect my hearing...not to mention extremely distracting to be trying to drive with that deafening noise. This is very dangerous!!! I

There is still no remedy to this recall and my truck is affected - I have been driving with NO ABS AND NO TRACTION CONTROL for almost 4 YEARS. This has also deactivated my cruise control. This is unacceptable and compromises me and my families safety. This is not a question of if it is going to happen, but when. My vehicle isn't under the category of "may be susceptible to failure" it has failed a
... nd I have permanently lost those BASIC and CRITICAL safety features. It is simply a matter of WHEN the lack of those features will cause an accident. There must be something done. . I have spoken with Mopar and several dealerships and nobody has given me any solution other than to say they will call me when a remedy is available. That is NOT ENOUGH. Recall language for C4B "The brake hydraulic control unit (HCU) on your vehicle [1] may be susceptible to failure or malfunction which may result in a reduction or change of Anti-Lock Brake System (ABS) /Electronic Stability Control (ESC) /Traction Control performance. A failing HCU will initially result in the intermittent loss of ABS/ESC/Traction control progressing to permanent loss. The loss of ABS/ESC/Traction control may cause a vehicle crash without prior warning. "Read more
