My vehicle is subject to Safety Recall 20UF (Suction Jet Pump failure / Fire Risk). I have attempted to contact Pacific Volkswagen in Hawthorne, CA to schedule this safety-critical repair. On two separate occasions, upon mentioning that the call was regarding the 20UF recall, the dealership staff disconnected the call. This repeated 'recall avoidance' at Pacific Volkswagen is preventing me from re

My 2023 Mazda CX-30, which is currently under full manufacturer warranty, was brought to the dealer on October 10, 2025, specifically to address a safety defect involving white chemical particulates blowing through the HVAC vents. The dealership confirmed the failure of the "evaporative coating", which resulted in unidentified chemical byproduct being discharged directly into the passenger cabin.T
... he vehicle remained out of service for 103 days due to this manufacturing defect. Upon receiving the Repair order (RO# 77004584) today, January 20, 2026, I discovered a significant safety concern that violates standard warranty repair protocols: the technician admitted to receiving two separate heater unit boxes that were damaged during shipping and subsequently 'transferred all salvageable items "from one damaged unit to the other to make one complete unit". As a consumer with a vehicle under warranty, I am entitled to a repair using new, undamaged factory parts. I am extremely concerned about the structural and functional integrity of a "reconstructed" HVAC system assembled from damaged components. Furthermore, despite the confirmed chemical emission, no professional decontamination of the interior was performed. The manufactures is now using financial threats of storage and rental car fees to force me to accept this substandard, "cannibalized" repair.Read more

To Whom It May Concern, I am submitting this formal safety complaint regarding my 2021 BMW X3, which is currently subject to an active safety recall involving a potential engine starter defect that may result in a vehicle fire.BMW of North America has notified me that water may enter the engine starter, causing corrosion and overheating, which can lead to a fire even when the vehicle is parked and
... turned off. BMW has advised owners, including myself, to park the vehicle outside and away from structures due to this risk.Since receiving this recall notice, my vehicle has displayed a check engine light, and I have personally experienced a burning or fire-like smell coming from the engine compartment. These symptoms have caused serious concern for my safety and the safety of my child.I contacted BMW regarding this issue and was informed that there is currently no available remedy to repair the defect. Despite the known fire risk and the warning signs my vehicle is exhibiting, BMW has not provided a temporary repair, replacement vehicle, or other permanent solution. This vehicle is my only means of transportation. I am a single mother and rely on this vehicle daily to transport myself and my child to work, school, and essential activities. Being advised to continue driving a vehicle with a known fire hazard, while also being instructed not to park it in my garage, places an unreasonable and unsafe burden on my family.I am not refusing a reasonable solution. I am open to trading my vehicle into a comparable BMW model while maintaining the same existing car note or loan terms. My primary concern is having a safe and reliable vehicle for myself and my child, given the unresolved safety defect and the lack of an available repair.I am submitting this complaint so that the National Highway Traffic Safety Administration is aware that consumers are currently operating vehicles with a known fire risk and no available remedy. Incident Date: December 11, 2025 Vehicle: 2026 Tesla Model Y Location: DFW Airport, Irving, TX System: Full Self-Driving (Autopilot / FSD) Description: On December 11, 2025, my 2026 Tesla Model Y was operating under Full Self-Driving while exiting an airport when the vehicle struck a gate arm, causing property damage and windshield/body damage. No injuries occurred, but the collision happened whil
... e the FSD system was actively controlling the vehicle. Following the collision, I requested the vehicle operational and FSD engagement data from Tesla for the incident timeframe. Tesla provided a CSV dataset; however, the FSD engagement and autonomy decision-layer data were almost entirely missing, despite the vehicle being in motion and presumably under FSD control. On follow-up requests, Tesla stated they are unable to provide additional autonomy data and that “Tesla does not collect all your vehicle data,” despite marketing the system as Full Self-Driving and collecting extensive telemetry. This raises a safety concern because: 1.A collision occurred during FSD operation. 2.Tesla is not providing complete autonomy data for safety assessment. 3.There appears to be no transparency into FSD decision-making, object detection, or control authority at the time of impact. 4.Owners, insurers, and potentially regulators cannot review how FSD behaved during a safety-related event. I am submitting this complaint so NHTSA is aware that: (1) A collision occurred under FSD control, and (2) Tesla refused complete operational autonomy logs for evaluation.Read more

I have a 2024 MAZD CX-90 Preferred Plus AWD. I was driving my [XXX] grandson to preschool and it sounded as though the passenger's door opened while I was driving. He was buckled in his car seat and I believe the s

I pulled over, got out of the car and looked. the passenger's back door handle was positioned up but the door was closed. being so shook up, I didn't think to take any pictures. I then had messages on my car saying Dynamic Stability control malfunction - have the vehicle inspected and Engine Malfunction - have the vehicle inspected. I called my dealership to find out if there were any recalls on this type of problem (they said that there weren't any). I have come across a forum where a person states: "So can confirm 1st row passenger door does not provide any warning as well. Looking at the manual, I see that there is supposed to be a warning sound when any door/hood/liftgate is open while driving, as quoted below. If someone else confirms this is normal behaviour, then I think we should file a safety defect complaint with the NHTSA and Transport Canada. “Door-ajar Warning Light/Liftgate-open Warning Light/Hood-open Warning Light The light turns on if any door/liftgate/hood is not closed securely. In addition, the warning sound is activated if any door/trunk lid/liftgate is not closed and the vehicle is driven. Close the door/liftgate/hood securely.” This was a very scary situation. My vehicle was at a manufacturer‑authorized dealership for a federally mandated recall repair. While the vehicle was in the dealership’s exclusive care, custody, and control, it was struck by a third‑party cleaning vendor operating on dealership property and was declared a total loss. I did not hire or authorize this vendor. After the incident, the manufacturer informed me in writing that th
... e dealership would repair the damage at their own expense, separate from the recall work. The manufacturer also stated they were covering the recall‑related repairs and were coordinating with the dealership. They provided weekly updates, monitored the recall repair, and escalated a backordered recall part. Later, after the dealership confirmed the vehicle was a total loss, the manufacturer reversed its position and stated it was “not involved.” The dealership has refused to identify the cleaning company responsible for the loss, their insurer, or any claim information, preventing any direct follow‑up. The manufacturer continues to direct me to “work with the parties involved,” even though the responsible vendor has not been disclosed. This raises concerns about the safety and oversight of the recall repair process, the handling of vehicles during recall service, and the manufacturer’s response when a vehicle is destroyed during a recall procedure.Read more

I purchased a 2024 Tesla Model 3 a little over a year ago. Last month this malfunction has caused an accident, and my car has been totaled, my insurance will not cover it, and I have been advised by two separate attorneys to sue. This has been litigated in court with lawsuits against Tesla (See Benavides v. Tesla; see also IN RE: TESLA ADVANCED DRIVER ASSISTANCE SYSTEMS LITIGATION). I had notified Telsa twice of this issue, the car left the lane and did not notice a concrete barrier on the left side going north on I-95 in or around Broward County. To my knowledge no one has inspected the car. I was not warned that this car would cause an accident, the problem first appeared when I reported the issue using the voice reporting feature around November 2025. January 18, 2025 Subject: Request for Review – Windshield Crack on 2025 Honda CR-V Hybrid Dear Honda Customer Relations Team, I am writing to respectfully request a review of a windshield issue on my wife’s Subject 2025 Honda CR-V Hybrid, which we purchased new in the summer of 2025. ... ring roughly four inches in length. The crack originates at the driver-side edge near the frame and began with a very small divot. There is no visible impact crater or evidence of damage from a foreign object. Based on its location and appearance, I believe this may be consistent with an stress-related edge crack rather than road debris. Our local dealership has declined coverage, which I understand may be standard procedure at the dealership level. However, given the vehicle’s age, mileage, and the nature of the crack, I am respectfully requesting that Honda Corporate review this matter for possible warranty consideration or goodwill assistance. My wife and I have been Honda owners for over 20 years. We have consistently chosen Honda vehicles for their quality and reliability, and we have also purchased Honda vehicles for our children, who continue to drive them today. Our loyalty to the Honda brand has always been rooted in trust—trust in engineering, and trust in standing behind the product. I am not seeking special treatment, only a fair review of the circumstances. We value Honda and hope to continue that relationship for many years to come. I have attached four photographs of the windshield, including both full-view and close-up images of the crack for your review. Thank you for your time and consideration. I appreciate your review and look forward to your response. Sincerely, [XXX] [XXX]

I am writing to demand the immediate buyback of my 2024 Jeep Wrangler 4xe due to catastrophic safety defects, multiple recalls, and Stellantis's own actions that have rendered this vehicle unsafe, unmarketable, and virtually worthless. The confluence of critical safety recalls, stop-sale orders, and the announced phase-out of this model line has created an untenable situation that warrants full re
... purchase of this defective vehicle. CRITICAL SAFETY DEFECTS AND RECALLS My vehicle is subject to the high-voltage battery fire risk recall, which poses an imminent danger to my safety, my family's safety, and public safety. The risk of spontaneous battery fire—even when the vehicle is parked and turned off—is unconscionable and represents a fundamental failure in engineering and quality control. I purchased this vehicle with the reasonable expectation that it would not pose a fire hazard that could destroy my property or endanger lives. Additionally, this vehicle is affected by the engine casting contamination defect, which compromises the structural integrity and reliability of the engine itself. This is not a minor defect; it strikes at the core functionality of the vehicle and raises serious questions about long-term durability and safety. STOP-SALE ORDER AND PHASE-OUT ANNOUNCEMENT Stellantis has issued a stop-sale order on these vehicles, effectively acknowledging that they are unfit for sale to consumers. If this vehicle is too dangerous and defective to sell to new customers, it is equally unacceptable for me to continue owning and operating it. The stop-sale order is a public admission of the severity of these defects. Furthermore, Stellantis has announced the phase-out of the Wrangler 4xe line, which has immediate and devastating consequences for vehicle value, parts availability, service support, and resale potential. This decision effectively orphans current owners and guarantees dramatic depreciation far beyond normal market conditions. CATASTROPHIC FINANCIAL IMPRead more

Incident date: Jan 17, 2026
While using the vehicle’s BlueCruise driver assistance system, the driver monitoring system generates frequent false driver-attention alerts despite normal posture and active attention to the roadway. The system often fails to recognize attentive driving unless the driver deliberately exaggerates head position or facial expression. Routine and unavoidable behaviors—including briefly touching t
... he face, wiping the nose, or checking side-view mirrors for traffic—frequently trigger alerts, even though these actions are part of safe driving. These false positives create alert fatigue and increased cognitive load, forcing the driver to divert attention from driving to manage the monitoring system itself. The intrusiveness of the alerts becomes distracting and undermines the intended safety benefit of the system. Due to the frequency of false alerts, the driver is often compelled to disengage BlueCruise entirely. I am concerned the current driver-monitoring calibration may inadvertently increase distraction for attentive drivers and warrants review.
