Incident date: Jan 24, 2026
I am submitting a safety complaint regarding my 2024 Jeep Grand Cherokee 4xe (plug-in hybrid), VIN: 1C4RJYB69RC714841, due to repeated loss of propulsion events while driving, including during highway merging. On multiple occasions, the vehicle has: Shifted into neutral without any driver input and failed to accelerate while merging onto a highway, behaving as though it was stuck in a gear or in
... a protective failsafe state Most recently, while accelerating onto a highway, the vehicle would not respond to the accelerator pedal, forcing me to pull off the roadway. I had to place the vehicle into Park, wait briefly, and then resume driving in order to restore propulsion. Twice i had to perform this on the on ramp. Very dangerous! These are the most recent incidents that have occurred, but the issue has happened at least half a dozen times before. Only now, more often. While driving in Hybrid mode (on January 17th, 18th with my daughter present; also occurred on Jan 22 (after leaving Jeep dealer and having performed TCM & PCM updates), Jan. 23 & 24) With the hybrid battery depleted (normal operating condition) With no warning lights, alerts, or messages displayed before or during the events The vehicle was taken to an authorized Jeep dealership (Safford Brown in Sterling, VA), where PCM and TCM software updates were completed, and the dealer reported “no issues found.” Despite this, the loss of propulsion issue has continued to occur. This condition presents a serious safety risk, particularly during highway merging and acceleration, where the inability to accelerate could result in a collision. The issue appears to be intermittent and does not consistently trigger fault codes or warning indicators. I am requesting that NHTSA review this issue for potential investigation, as it involves unexpected loss of drive power while the vehicle is in motion. Thank you for your attention to this matter. Sincerely, Danette SprustonRead more

The Gas Door did not open in order to pump gasoline. There is no mechanical device for the driver, passenger, owner of the vehicle to manually open the gas door from the inside. There was no electronic warning within the Mercedes Benz to alert driver about malfunction of gas door not working. Driver took vehicle to local Firestone mechanic who said the "gas door actuator" needed to be replaced. Ac
... cordingly, vehicle with the remaining gasoline was taken to local Mercedes Dealer who after the vehicle being looked at, "confirmed gas door to not opening due to internal mechanical fault" and replaced with new gas door actuator at a cost of $820.28. Vehicle owner mentioned to service rep, that he lives from parents about 150 miles away and had the vehicle owner stopped at gas station in between, the vehicle owner would have been stranded without any access to a dealer to fix this critical safety issue. Vehicle owner then advised service rep this issue should have been elevated as critical safety recall issue and that a complaint would be filed with NHTSA. Any vehicle owner, their family if being transported in a deserted place hoping to go to the next gas station for pumping gas would have faced a severe danger to their self and their families. Vehicle owner has sent a detailed letter to Mercedes Benz corporate office in USA detailing the incident and a copy of the letter will be attached as an attachment if it is permitted in this complaint. Requesting a recall of this issue with an order for reimbursement of service charges paid by vehicle owners to fix this "malfunction of gas door". There are multiple complaints to this effect on various message boards regarding the occurrences of "gas door not opening". This also needs to be looked at.Read more

I am submitting this complaint to report a potential vehicle safety defect involving the factory-installed running boards on a 2023 Chevrolet Tahoe. The running boards become extremely slippery during light snow or minimal winter precipitation, creating a significant risk of slipping while entering or exiting the vehicle. This occurs during normal use and does not require heavy snow or ice. I hav
... e experienced multiple near-falls despite stepping carefully, and the condition is repeatable. Given that the Chevrolet Tahoe is commonly used as a family vehicle and may be accessed by children, elderly individuals, or passengers with limited mobility, I am concerned that the running boards present an unreasonable slip hazard under common winter driving conditions. At this time, no injury has occurred; however, the frequency and severity of the slipping raises concern for potential injury if the issue is not addressed. The running boards are factory-installed and have not been modified. I am requesting that NHTSA review this issue to determine whether the design or materials of the factory running boards meet reasonable safety standards for normal winter conditions. Thank you for your attention to this matter. I am reporting a failure of the Around View Monitor / camera system on my 2023 Nissan Pathfinder. The system stopped functioning completely without warning. Pressing the camera button does nothing, the camera cannot be activated through vehicle settings, and no camera views (rear, front, or surround) are available. Parking sensors and the infotainment screen continue to work, indicating a system-l
... evel electronic failure rather than user error or a single camera issue. This vehicle relies on the camera system for rear visibility and low-speed maneuvering. The loss of this system reduces driver visibility and increases the risk of collision, especially when reversing or parking. The failure occurred despite normal use and maintenance, with no prior warnings or messages to the driver. The vehicle is only a few years old, yet the repair involves replacement of a major electronic control module at significant cost. This appears to be a premature failure of a safety-related electronic system. I am concerned this may affect other vehicles of the same model year and configuration.Read more

I am reporting a serious nighttime visibility and driver-assistance safety issue with my 2025 Honda Passport. During evening driving, approximately 60% or more of the upper portion of the windshield appears blacked out or severely dimmed, significantly impairing forward visibility. This condition worsens on downhill grades, where an even greater portion of the windshield darkens. Visibility improv
... es when high beams are manually activated; however, driving continuously with high beams is unsafe and inappropriate. This creates an unreasonable risk of collision, particularly on unlit roads and downhill grades, as pedestrians, road curvature, and hazards are difficult or impossible to see without improper use of high beams. In addition, multiple ADAS systems operate erratically. Adaptive cruise control delays braking on downhill grades until the vehicle accelerates approximately 9–10 mph above the set speed, delays acceleration after braking and resuming, and disengages with minor steering inputs. Automatic high beams operate inconsistently. These issues persist despite recalibration. The condition has existed since early ownership and continues at approximately 20,000 miles despite multiple service visits. A Honda field technician confirmed all systems were calibrated but did not perform a nighttime drive. A dealership shop foreman conducted a nighttime drive but did not share findings, and I was told he was unavailable and had already spent “enough time” on the vehicle. My initial service record included the notation “do not diagnose.” I was initially told the dealership had never encountered this issue; later I was told they had seen it on other vehicles but did not know how to fix it. A dealership-recommended nighttime comparison drive with a 2026 Honda Passport did not exhibit these issues. I also regularly drive a 2024 Honda CR-V that shows none of these nighttime visibility or ADAS problems.Read more

Problem Description The core issue stems from a production fault in the battery cells that can cause increased self-discharge. In a high-voltage battery pack, which operates like a chain of interconnected modules, a single faulty module with elevated self-discharge becomes the “weakest link.” This leads to: • Degradation of the overall usable capacity. • Reduced driving range and power l
... imitations. • Potential cascading failures affecting additional modules over time. • Warning indicators, such as a yellow electrical system fault in the instrument cluster (fault code P0BBD00: Hybrid Battery Pack Voltage Variation Exceeded Limit). • Infotainment messages like “Electronic system does not work correctly. Please go to workshop.” • In severe cases, unexpected shutdowns or failure to pass technical inspections, potentially rendering the vehicle non-roadworthy under local regulations. The fault is linked to specific battery module part numbers from LG, including: • 0Z1.915.592.G/H/J • 0Z1.915.599.G/H/J/P These were produced through the end of 2023 (up to December 4, 2023) and include configurations like 12S2P (small capacity) and 8S3P (large capacity), meaning all MEB battery setups could be impacted. Volkswagen does not publicly disclose full VIN ranges, exact affected counts, or the precise technical cause of the manufacturing fault, but internal escalation procedures exist to identify additional defective batches.Read more

I purchased this vehicle, November 2023 and by August 2024 the vehicle went into Griffin Hubs to get the engine replaced. They had the vehicle for over a month and a half when I received it back after the engine I had it for a day and a half and had to take it back because it was making a loud grinding noise coming from the engine which then they had to replace a second time because the engine was
... shredded and was told they were going to put a new transmission in as well, but instead of just replaced a piece, and then two months later, the negative terminal for the battery was replaced in about four months later I take it into the dealership again because as I was driving the vehicle pulled back three times and then shot forward and that happened seven separate times dealership said they could not find an issue and then in November 2025, the vehicle goals back into the dealership for overheating needing a new water pump because I was losing coolant with no leaks. Get the vehicle back two days later my coolant is empty tied again call the dealership they said if it happened again, bring it back in two weeks later it did the exact same thing. My coolant was emptied, but there is no leaks coming from under the vehicle and now last week as I was driving, the vehicle suddenly stopped accelerating and my gearshift wouldn’t catch any gears and as the vehicle is still moving my tires, then locked. The vehicle has been at the dealership for five days and they still cannot figure out what the issue is.Read more

The CHIME SPEAKER in the failed with less than vehicle 11,000 miles on the odometer. This extremely low mileage indicates that the failure is due to a manufacturing defect rather than standard wear and tear. I am particularly concerned because this is not merely a convenience issue; it is a significant safety concern. The chime speaker is the primary method of delivering audible alerts for: - Turn
... Signal Indicators: The lack of an audible "click" prevents confirmation that signals are active, which can lead to driver error or unintentional signaling. - Seatbelt Warnings: Critical reminders for passenger safety are now silenced. - System Fault Alerts: Any immediate mechanical or electrical warnings from the vehicle’s diagnostic systems are currently uncommunicated to the driver. - Lane departure warnings. - Collision alerts - Blind spot alerts - Other advanced driving assist and safety systems. The fault has been reproduced and confirmed by the dealer (Porsche Tysons Corner). The fault is also confirmed by the vehicles onboard diagnostic and warning system. The dealers proposed fix is to replace the entire instrument cluster at a cost of more than $5000 or turn the repair over to an audio shop in hopes that they can replace remove the instrument cluster and replace the chime speaker with a non-OEM part as the OEM lowest replaceable part is the entire instrument cluster. This is safety issue because very few vehicle owners will be will to spend more than $5000 to repair/replace the chime speaker.Read more

Specific Recalls/Campaigns Not Addressed: I brought my vehicle to Fiesta KIA (Dealer Code 53232) to address the following open safety campaigns which they failed to remediate: NHTSA Recall #24V-757 (KIA SC326): "Instrument Cluster Screen May Fail." This is exactly the failure I am experiencing. NHTSA Recall #24V-443 (KIA SC302): ICCU Software Logic failure, which poses an immediate risk of loss of motive power while driving. Service Action SA563: Specific logic improvement for HUD and Cluster stability. Dealer Conduct (Safety Related): Despite the vehicle presenting with a "No Signal" instrument panel—a catastrophic safety failure—the Service Manager (Anthony) refused to provide the previously promised loaner vehicle. He suggested that because the vehicle was not purchased at his specific dealership, his obligation to facilitate this critical safety repair was diminished. This resulted in the vehicle remaining on the road with unresolved, safety-critical defects.Read more
