am reporting an issue involving unexplained cracking of my Tesla windshield with no known impact or debris strike. Last night at approximately 6:20 PM, while in Chapel Hill, NC, I entered my vehicle and noticed a crack on the driver’s side of the windshield. The outside temperature at that time was about 28°F. There was no audible rock strike and I have no recollection of anything hitting the g
... lass. The morning low temperature was approximately 14°F. To my awareness, the passenger-side crack was not present earlier that morning. Approximately 15 minutes ([XXX]EDT) before writing this statement, I was seated in the passenger seat and noticed a second crack on the passenger side. The temperature had risen to about 34°F. Again, there was no strike event that I recall while driving or parked. Both cracks appeared within a short time span during an unusually cold period with temperature changes between 14°F and 34°F. The passenger crack visually resembles an impact point, but I cannot identify any incident that would have caused it. My concern is that these cracks may indicate thermal stress, structural flex, a material defect, or bonding/installation issue. Windshield failure without impact presents a safety hazard due to reduced visibility and potential compromise of structural integrity during a collision. I am requesting that NHTSA document and evaluate this issue for potential safety implications. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am writing to report an unresolved safety recall for my vehicle. I received Safety Recall Notice 25V695 in October 2025, and as of today, the recall has not been resolved because the required repair parts remain unavailable. This recall involves a safety-related defect, and the extended delay in providing a remedy raises serious safety concerns. I have contacted authorized dealerships, and I ha
... ve been informed that parts are still on backorder with no estimated availability date. Given the amount of time that has passed since the recall notice was issued, I do not believe this recall has been addressed within a reasonable timeframe. The ongoing lack of an available repair increases the risk associated with operating the vehicle under normal conditions. I am requesting that this issue be reviewed to ensure the manufacturer complies with its obligation to provide a timely and effective remedy for this safety recall. Thank you for your attention to this matter.Read more

Car was assessed for recall related to service bulletin 23-032. Recall was performed, but the mechanic who performed the recall says the car is not repaired because the rear frame corrosion, as described in bulletin 23-032, is so severe that a large piece is missing from the rear frame. The mechanic says the repair as described in the bulletin is not enough to fix the car. The service bulletin rem
... edy was only performed because the rear trailing arm bolts were able to be removed, but the car is still unsafe due to rear frame corrosion that Honda has admitted to as per the recall. The car is considered unsafe due to the frame damage and likely cannot pass state inspection. After calling Honda, Honda says the repair is complete even though the mechanic who performed the recall says the repair is not complete and the car is unsafe due to issues with rear frame corrosion that led to the recall in the first place. Honda is refusing to discuss the issue any further. A screenshot of the damaged part of the rear frame has been attached. This is part of a video taken by the mechanic who performed the recall. In this video the mechanic also states that “this is not a repair.” He also states that part of the damage is obscured by the brace that was attached to the frame, so the image provided does not show the full extent of the damage.Read more

Doors not opening renders the vehicle unusable or unsafe for any passengers besides the front seat passenger. I bought my Dodge Grand Caravan to be a family car, and I can only have one child ride safely with me. In the event of a wreck, passengers would be trapped in the vehicle. I have submitted this concern before, and I have written to the warranty company associated with Dodge. The upper radiator hose on some huge percentage of these trucks fail (mine has failed at just over 37K miles). You can check the Colorado Chevy forum, youtube, etc., et al, and find plenty of instances where this has failed. As this is the cooling system, it can cause the engine to overheat and potentially ruin the engine. The flaw seems to be an engineering issue with the rubber seal they use (in

Instead, my local Chevy dealer, since its not a recall item, said they could schedule a time next week for me to LEAVE the truck and then they would get to it in 7-10 days. serious safety-related defects in my 2023 Kia K5 involving the fuel/emissions system &other vehicle components. From early ownership & throughout the lease period, the vehicle experienced repeated& unresolved issues including fuel system abnormalities, emissions failures, fuel odors, check engine light activation, abnormal noises, &performance concerns consistent with fuel pressure or fuel tank-re
... lated problems. I reported these issues multiple times to the dealership &to Kia Consumer Affairs beginning well within the first 24 months of the lease, continuing for over a year. Despite repeated complaints and service visits, the vehicle was repeatedly returned to me- deemed safe to drive, my concerns were ignored. In June 2024, the dealership identified fuel tank expansion, confirming the same safety concerns I had been reporting. In November 2025, Kia issued Safety Recall SC356 (NHTSA Recall No. 25V794), stating that a damaged check valve can allow air to enter the fuel tank, causing expansion, contact with hot exhaust components, melting, and increased fire risk. My vehicle is included in this recall, which remains incomplete, &I was never notified while the defects were actively occurring. Prior to the recall and before the fuel tank expansion was diagnosed, I paid significant out-of-pocket costs in March and April 2024 for emissions-related repairs based on representations that the vehicle was safe to drive. This vehicle was intended as a graduation gift for a minor, & minors in my household regularly operated it. Kia’s continued assurances that the vehicle was safe, despite known and reported defects, placed my family, especially minors, at serious risk of injury or death. In addition to fuel/emissions issues, the vehicle had persistent seat defects and other unresolved problems that were never fully repaired. I first experienced the ABS, Traction Control, and Brake warning lights on December 20, 2025, while driving. I knew something was wrong because whenever I turned the steering wheel, braked hard, or drove over potholes, all three lights would come on and remain lit for about a minute. Each time I turned the car off and restarted it, the lights would disappear. However, if I drove the vehicle for an

I ordered new brakes and rotors and had them installed by my certified mechanic. Unfortunately, about an hour after the repair, the ABS, Traction Control, and Brake lights came on again. I immediately contacted my mechanic and brought the vehicle back in for a diagnostic test. The diagnostic results showed that the ABS module is faulty. My mechanic explained that this malfunction is what is causing all three warning lights to appear on my dashboard. I was advised that the ABS module must be replaced by the manufacturer or a dealership. After researching this issue, I discovered that this same problem has occurred on multiple occasions with Honda Crosstours over the years, indicating that this may be a known or recurring defect. I am filing this complaint regarding the ongoing safety risk and lack of remedy for NHTSA Recall 24V-700, which affects my 2019 Nissan Leaf. This recall was officially initiated in September/October 2024 due to a risk of high-voltage battery fires during Level 3 (DC Fast) charging caused by lithium deposits in the battery cells. It has been approximately 16 months since Nissan issued the interim
... notification advising owners to stop using Level 3 quick charging. To date, no functional remedy has been provided. This delay has resulted in: 1. Unaddressed Fire Risk: The vehicle contains a known defect that can lead to a thermal incident. While Nissan’s interim "solution" is to avoid fast charging, the underlying hardware defect remains in the vehicle. 2. Loss of Essential Functionality: I am unable to use the advertised DC fast-charging capability of my vehicle. This severely limits the vehicle's utility for trips beyond its immediate range and forces me to rely exclusively on slower Level 1 or Level 2 charging, which is not always feasible. 3. Unreasonable Delay: Under 49 U.S.C. Chapter 301, manufacturers are required to remedy safety defects within a reasonable time. A delay of over a year for a software-based fix (which Nissan has stated is the intended remedy) is unreasonable and leaves the consumer with a diminished and potentially hazardous product. I am requesting that the NHTSA investigate the cause of this extensive delay and compel Nissan to either provide an immediate repair, offer a vehicle buyback, or replace the defective battery modules, as the proposed software "patch" appears insufficient to address the physical cell degradation.Read more
