While driving at highway speed, I accidentally brushed one of the paddle shifters on the steering wheel. On February 28, 2025, at approximately 8:43 a.m., while driving on Highway 3 in Alvin, TX, the panoramic sunroof glass on my vehicle suddenly and violently exploded/shattered without any warning. At the time of the incident, no other vehicles were around me, and there was no impact from debris, rocks, or any external object. Thank god my kids had just been dropped off at school or this would be a
... whole different situation. The explosion was extremely loud and occurred spontaneously while the vehicle was in motion. My husband was a passenger in the vehicle at the time. The sudden failure created an immediate safety hazard due to the risk of flying glass, distraction, and potential loss of vehicle control. My husband immediately looked around to note if someone threw something or if construction vehicles were around. It was a very slow quiet morning! Nothing was inside the car where the hole in sunroof appeared! There was no prior damage, no weather-related event, and no indication of stress or cracking before the glass failure occurred. After the incident, I researched this issue and discovered multiple reports from other Infiniti QX60 owners describing nearly identical panoramic sunroof glass failures, often referred to as the glass “exploding” or shattering outward while driving, and frequently without any impact or external cause. These reports include vehicles from 2022 and nearby model years, suggesting a recurring and known issue. Although there is currently no recall issued for this defect, the number and consistency of complaints indicate a potential manufacturing, material, or installation defect in the panoramic sunroof glass. I am submitting this complaint to formally document the incident and request that NHTSA investigate this serious safety concern, as spontaneous sunroof glass failure poses a clear risk to vehicle occupants. I am still a week later working with insurance to get this fixed. Costing me money and a vehicle I can’t drive!Read more

The A/C evaporator core in my 2020 Dodge Charger Hellcat has failed twice within a short period of ownership. The first failure occurred at approximately 20K miles in 2022, when the air conditioning suddenly stopped blowing cold air. The dealership diagnosed a leaking evaporator core and replaced it under warranty. On [XXX] at approximately 50k, the exact same failure occurred again — loss of r
... efrigerant and no cooling. The vehicle again required replacement of the evaporator core, a repair that involves removing the entire dashboard and is extremely labor-intensive and expensive. This appears to be a recurring defect rather than normal wear. I have learned that many other Dodge Charger and Challenger owners report identical evaporator failures, often multiple times, suggesting a possible design or manufacturing flaw with the HVAC evaporator used in these vehicles. Loss of A/C may seem like a comfort issue, but in extreme heat it can create a driver-safety risk due to overheating, window defogging problems, and driver fatigue. I live in Arizona where daytime temperatures regularly climb above 80 degrees, even in the winter. A major component like an evaporator should not repeatedly fail in a late-model vehicle. I am requesting that NHTSA investigate whether 2019–2022 Dodge Charger/Challenger vehicles are experiencing an abnormal rate of evaporator core failures that may warrant a recall, extended warranty, or technical service bulletin. Repairs Performed: • Evaporator core replaced in 2022 at Bill Luke Chrysler / Dodge in Phoenix, AZ • Evaporator core replaced again on February 4th, 2026 at AAA Auto Repair in Phoenix, AZ • Refrigerant leak confirmed both times • Dash removal required for repair Supporting Documents Available: Repair orders, dealership invoices, diagnostic reports INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Last month, while transporting my brother to his arm surgery in Chapel Hill, NC, I observed unusual and audible shifting behavior when engaging the vehicle's sport mode. This mode is typically utilized for extended journeys on highways and interstates. Although the anomaly was brief, I did not initially consider it significant. Several weeks later, upon departing from my community college campus,
... I encountered a red light near Santa Fe Drive. The vehicle exhibited a hesitant and labored acceleration, eventually moving forward. This incident was particularly concerning given the high volume of traffic during that time of day. I subsequently stopped at a nearby 7-Eleven for fuel, suspecting a low fuel level might be the cause. No engine warning lights or mechanical indicators were illuminated, and the oil life was at 70% with appropriate oil levels. Fast forward to the last week of January. On Wednesday the 28th, I began to notice recurring shifting issues with the vehicle during a class break. Additionally, engaging reverse consistently resulted in a harsh shift. As I was leaving my neighborhood to return to class, the vehicle completely failed to engage gears or accelerate. At this point, both the wrench and check engine warning lights illuminated. Upon returning home, I utilized the code reader provided by my mother last Christmas, which displayed the code "P0772." This confirmed my prior research regarding potential issues with the EcoBoost engine. I am relieved that this mechanical failure did not occur amidst the heavy traffic typically found on 210 Murchison South , I-295, Bragg Blvd Fayetteville/Spring Lake and other routes heavily trafficked due to the military base. The high speeds on these roads could have led to a dangerous situation, potentially causing a rear-end accident or other involving other drivers who may be speeding or inattentive. I am grateful that I did not experience a stall in the middle of traffic, which could have compromised my safety.Read more

Discontinued vehicle model still under active bumper to bumper warranty (thru 10/26); has faulty rear defroster/heated side mirrors that the "remedy" handed down to me was to "drive more" in an effort to charge battery to a threshold that allows said necessary safety feature to operate. If charge falls beneath 65% the vehicle terminates functionality of deemed "non-essential" operations to maintai
... n major functions. Problem is, this feature is not optional. It needs to be available on demand AT ALL TIMES. Especially in the midst of a brutal upstate NY winter where the windows are quintessentially frosted over EVERY morning. There is but one mention of rear defroster operation in the 2023 Dodge Hornet owner manual on page 47. There is ZERO mention of driving X amount of miles or X amount of time to maintain functionality of the rear defroster/heated side mirrors. Devoid of this information in the manual, my expectation is for the company to find a remedy as I have owned many vehicles in my life and never have I ever had this sort of selective operation issue for a SAFETY feature. These vehicles rely on an Intelligent Battery Sensor to dictate to the car what the state of charge is for the battery. My battery was replaced by the dealer I bought this from in May '25 (have paperwork to prove) and the readings per the local dealer I had looking into this on two occasions this month were in the 60-65% range for battery capacity. No way a battery this young has been drained by a third of it's potential already. And if so, something larger is amiss that needs to be diagnosed and remediated PERMANENTLY. Me "driving more" is not a solution. Especially since it makes me incur unnecessary miles and expend more on gas which I will absolutely not do. Bringing this to light before warranty expiration later this year. The company is seemingly trying to do anything and everything to not put money into vehicles that are discontinued. Factory warranty ends 10/26.Read more

Just after Thanksgiving, my vehicle displayed a “Service Emergency Calling” message. I brought it to the dealership and they documented the concern, referencing GM bulletin PIP6077B, but no repairs were performed. On December 30th, while parking, I shifted into reverse and the vehicle would not reverse and instead moved forward. A “Service Transmission” warning appeared and the vehicle was
... towed. The dealer cleared the codes and released the vehicle without performing any diagnostics. About two weeks later, when starting the vehicle after work, several warnings appeared, including “Service Safety Restraint System,” “Forward Collision System Unavailable,” and a rear-seat alert despite no passengers. While driving, lane keep assist, lane departure warning, and blind zone steering assist were not functioning. The next morning, the vehicle suffered a complete electrical failure. It would not start, the screens stayed off, and the doors would not lock or unlock. The vehicle was towed again. The dealer replaced the battery, claimed it had simply died, and attempted to return the vehicle to me. I refused because none of the prior warnings were diagnosed, and I was told the codes “erase when the battery is changed.” After I demanded full electrical and transmission diagnostics, including a parasitic draw test, the dealership informed me the vehicle was unsafe to drive. They stated the OnStar/telematics module was draining the battery and the replacement part was backordered with no estimated arrival date. I was told multiple customers are waiting for the same part, which means this is NOT an isolated event and needs way more attention than it's being given. This defect results in loss of essential safety systems and complete vehicle shutdown, creating a significant safety risk.Read more

A recurring malfunction occurs with the infotainment/radio and rear backup camera display. The system also cycles between screens (including switching to the camera view), and the infotainment freezes and becomes unresponsive (loss of screen/control). Audio playback and hand
