Afterwards when attempting to close the sunroof after opening, it wouldn't close. We had to force it shut due to rainy weather. ... le frequently displays a message stating “Rear View Camera Unavailable, Contact Ford Dealership” and then goes to a black screen. This results in a complete loss of rear visibility while backing up, increasing the risk of a collision, especially involving pedestrians or other vehicles. I received the safety recall notice in September 2025. Since then, I have made numerous attempts to have the vehicle inspected and repaired. Despite repeated contact with my local dealership, has informed me that they cannot perform the inspection or recall repair because they do not have an SSCC repair procedure available and therefore cannot proceed. I have also contacted Ford Motor Company customer service by phone multiple times, but these calls have not resulted in a resolution or a clear path to getting the vehicle repaired. As a result, I am left operating a vehicle with a known safety defect and no access to the recall remedy. I am deeply concerned for my safety and the safety of my family. Operating this vehicle without a reliable rearview camera significantly increases the chance of a crash, and I believe the continued delay in providing a remedy defeats the purpose of a safety recall. I respectfully request that NHTSA document this complaint and take any appropriate action to ensure that Ford Motor Company provides a timely inspection and repair for this safety defect.Read more

In June 2019, while driving my 2018 Nissan Maxima, the vehicle suffered a sudden and violent mechanical failure. Without warning, the steering wheel jerked forcefully to the side, and the vehicle became uncomtrollabel leading to an approx 100-foot spin across a median. I have since verified this vehicle was subject to NHTSA Recall 19V-807 for an ABS Actuator leak that shorts the circuit board.

On the morning of January 28, 2026 I arrived to work and came to a complete stop in my works parking garage. I used my key card to raise the gate. Once the gate was fully raised I pressed the accelerator to drive up the ramp. Shortly after accelerating, my engine revved really loud and high rate, then my car automatically increased speed. I immediately pressed the brake pedal but it didn’t stop.
... It continued accelerating at a speed not controlled by me with my brakes not engaging even though I was pressing the brake as hard as could. I estimate I travel about 100’ with no control of the gas or brake. It lasted about 15-20 seconds. If a car were in front of me at that time, I would have crashed into them because I could not stop the car. I immediately called Davis Moore Mazda. I had the vehicle towed there because I didn’t feel safe driving it. Information from my cars computer program was sent to Mazda. Mazda didn’t see any issues and their diagnosis was a foot was likely on the accelerator causing the acceleration. The general manager called me to discuss. He said the only thing that registered was acceleration. I asked does the computer show my attempt to brake (stop) the car. His response was the computer wouldn’t read that because that was a normal driving process that wouldn’t be recorded. I asked why the acceleration would trigger an alert in the computer, since acceleration is also a normal driving process. He then said the computer picked it up because it accelerated quickly. There were no warnings present prior to this issue.Read more

I am reporting a safety-related defect involving unintended vehicle movement on my Hyundai Ioniq 5 (Model Year 2023, SEL AWD). Observed behavior: With the vehicle in Drive and the Auto Hold function active (indicated by the white “AUTO HOLD” indicator in the instrument cluster), I brought the vehicle to a complete stop by pressing the brake pedal. ... le remained stationary, and I removed my foot from the brake. After approximately three minutes of remaining stopped, a driver-assistance system error message appeared on the instrument cluster. At that point, Auto Hold disengaged (the “AUTO HOLD” indicator disappeared), and the vehicle began to creep forward on its own. I did not command acceleration, did not press the accelerator pedal, and did not intentionally release the brake. The movement was unexpected and created a safety risk. The first occurrence happened in a school drop-off line with the vehicle stopped near other vehicles and pedestrians. I have since reproduced the behavior and captured it on video. Related observations: Similar driver-assistance system errors have occurred while the vehicle was in Park and in Drive, with or without Auto Hold active. In those cases, the vehicle did not move because it was either mechanically locked or I had my foot on the brake. In all cases, Auto Hold deactivated. Safety concern: This behavior raises concerns about the reliability of Auto Hold and its interaction with braking and driver-assistance systems, particularly after the vehicle has been stationary for an extended period. Unintended forward movement presents a significant safety hazard. I am reporting this issue for investigation of potential defects in software, sensors, or control logic. Broken CV Axles (Officially diagnosed at approx. 6,000-7,000 miles) •On May 31, 2025, the vehicle was returned to the dealership for backup camera installation and to revisit the knocking noise, this time properly documented. •That same day, both the left and right CV axles were diagnosed as broken, and I was told the vehicle was not safe to drive. •I had unknowingly been driving in this unsafe condition for almost three months. •The vehicle remained in the dealership’s possession until June 5, 2025 (6 days), Recurrence of Knocking Noise and Third Service Visit (January 10, 2026 – Approx. 15,000 miles) At approximately 15,000 to 16,000 miles, the knocking noise had returned (in both directions of steering) and I brought the vehicle in to be diagnosed •The dealership acknowledged hearing the noise. •I was told the sound is a “well-documented issue” with this model and considered “normal.” •I was informed that the vehicle was safe to drive. •I was told Honda is working on a redesigned part and that a recall is expected in approximately six months to one year. •I was specifically told Honda will not authorize replacement of the same axle parts again, as they continue to fail, and that I must wait for a recall remedy.Read more

Tesla inspected the vehicle and stated the failure was related to the battery coolant heater. The failure resulted in a complete vehicle immobilization and access failure. I had previously experienced restart warnings on the dashboard, but I did not expect a total lockout and loss of access. The inability to access the vehicle was unexpected and alarming. It may be a known safety issue. It should be recalled. This condition presents a serious safety risk because occupants could be trapped inside the vehicle and the vehicle cannot be accessed or moved without external power. In extreme heat or cold, or in a medical or emergency situation involving a child or pet, this failure mode could create a life-threatening situation. Importantly, even after multiple requests, I have not received a copy of the diagnostic report, or anything other than informal emails or calls. Tesla has not provided information regarding this failure other than broad statements, and visuals of the location of the battery pack, in an attempt to deny warranty coverage. The Tesla reports a High Voltage System alert currently. The vehicle is subject to Tesla’s Battery and Drive Unit Limited Warranty. Tesla has stated that the failed component was the battery coolant heater, but has refused to cover the repair under warranty. ... evere risk of crash or injury. As of today February 5th, 2026, my vehicle has been out of service for over 37 consecutive days with no available remedy or estimated repair date. This constitutes an unreasonable delay in repair and a failure by the manufacturer to remedy a safety defect in a timely manner as required by the National Traffic and Motor Vehicle Safety Act (49 U.S.C. § 30120), to my best understanding. This grounding has created a severe family hardship primarily because this is our only vehicle. We are a family of three, including a [XXX] toddler with daily transportation needs. Toyota’s attempts to mitigate our hardship have been substandard: (1) Initially, I was provided an economy SUV (Corolla Cross) which I was forced to return on January 16th because the dealership allowed the temporary registration to expire; (2) I was then downgraded to an economy sedan (Corolla) which was unfit for local winter conditions (snow and ice) and failed to meet the safety and transportation needs of my toddler; and (3) Only on February 5th was I offered a gas-powered RAV4. Despite owning a premium plug-in hybrid, I have now been forced to pay unfairly for gasoline for 37+ days due to Toyota's inaction. Because my vehicle is unsafe to drive and the manufacturer cannot provide a timely fix, I am formally requesting a prompt manufacturer action in the form of a prompt repair or a fair repurchase (buyback) to resolve this ongoing safety and financial burden. The center console screen has a default screen that in part includes interior fan speed temperature. To adjust the fan speed and other A/C settings, the driver has to click press a small button at the bottom of the screen to open a new window. ... XXX when she bought it on 10/31/1999 for $23, 470 with 8 miles on the odometer; salespeople never mentioned the door handles were plastic. The failure was complete fracture all at once, not progressive; I suspect the plastic strain hardened from repetitive stress, making this a design defect rather than material quality. The driver's door latch is a safety issue; now I must struggle to enter the car, and you never know where you might be when you need quick entry, like in a rain. Paid cash w/ trade in for car. Door handle not yet repaired, but claim filed with Toyota Customer Experience Center (ref. # XXX). Request Toyota replace or repair door handle at their expense, preferably with aluminum handle on driver door lest problem recur. Also the car needs an OE solar panel to keep the battery charged for low mileage use. I replaced previous battery with new 7 yr warranty one on 1/22/25 and despite idling to recharge for 1 hr at a time, the battery now remains recurrently low. The vehicle was not sold for a given yearly mileage use, and my mom, now deceased, bought it for reliable safety during retirement & old age as my father died; the vehicle now belongs to our family trust, of which I am trustee. I request Toyota pay for a solar panel to be installed above rear window deck, on rubber tubed bolts spaced above the speakers, to keep battery recharged. Battery charge is obviously a safety issue, and idling is a pollutive, unforeseen expense. Both these issues should likely be recall issues because of the vehicle purchase price involved, as design negligence.Read more

UNKNOWN - I was driving the car on a highway, there were no alerts and my engine light was not on, and my car started smoking and within 5 minutes my car was completely on fire. After the fire was put out, the whole front of the car was gone and then engine looked melted.

My fiancée and I just recently bought this car with the 1.6T-GDi engine I immediately replaced the oil even though the dealership said that he just replaced it.
