I was informed that only a “temporary fix” could be performed, and I was required to pay approximately $500 for this repair in order to regain use of the vehicle. The vehicle has not received a permanent repair, and the dealership has taken several months without completing a permanent fix. I have also been waiting several months for reimbursement of the almost $500 paid for the temporary repair, with no resolution. I am concerned that I was charged for a temporary repair related to a serious safety failure and that continued delays in both reimbursement and permanent repair place me at ongoing risk. I am filing this complaint due to the dangerous nature of the brake failure, the lack of a permanent repair, the extended delay in resolving a critical safety issue, and the financial burden placed on me for a temporary solution.Read more

Vehicle: 2016 Ford F-150, 5.0L V8, 6R80 automatic transmission Mileage at time of failure: Approximately 118,000 miles Approximate date of occurrence: Late 2025 (Late October to Early November) While driving at speeds above approximately 40 mph, the transmission would randomly and unexpectedly downshift, causing a severe and sudden jerking motion accompanied by extremely high engine RPMs. After s
... everal seconds, the transmission would violently upshift back into the original gear. Most occurrences involved the transmission downshifting one or two gears; however, on multiple occasions it downshifted from 6th gear directly to 2nd gear while traveling at highway speeds. This caused the rear wheels to momentarily lock up, creating a dangerous loss of traction and loss of vehicle control. These events occurred frequently, including during short trips, and were not related to driver input, braking, or road conditions. During these incidents, the check engine light and wrench warning light would illuminate. Diagnostic trouble code P0720 (Output Shaft Speed Sensor Circuit) was consistently stored. This pointed to a failure within the transmission molded lead frame, which houses the output shaft speed sensor and related circuitry. Due to the severity of the symptoms and the known issues with the Ford 6R80 transmission molded lead frame, I replaced the molded lead frame myself. After replacement, the erratic downshifting, wheel lockup, warning lights, and fault codes were completely resolved. The vehicle has operated normally since the repair. Ford has a documented history of issues related to molded lead frame failures in 6R80 transmissions, including erratic shifting and loss of speed sensor data. This failure presented a significant safety risk, particularly at highway speeds, due to sudden and uncontrollable deceleration and momentary wheel lockup.Read more

Prior to this incident, the vehicle exhibited no warning signs, including no pulling, steering issues, warning lights, or diagnostic codes. We immediately pulled over and had the vehicle towed to a mechanic who inspected the truck but could not replicate the issue. Although the vehicle was later driven approximately 120 miles home and short distances locally without recurrence, we believe it remains unsafe due to the unpredictable nature of this failure. A vehicle should never abruptly and forcefully change lanes without driver input. We have also experienced ongoing issues with the key becoming stuck in the ignition and intermittent starting problems, raising concerns about potential electrical or mechanical defects. This incident involved two uncommanded lane deviations under normal driving conditions with no identified cause or warnings. Due to the risk of serious injury or fatal collision, this remains an unresolved safety concern requiring investigation. We are requesting that NHTSA review this complaint, evaluate whether similar incidents indicate a pattern or defect involving the steering, transmission, or electronic control systems in the 2016 Ford F-150, and take appropriate action to protect public safety. ... ng with a smell of burning rubber from the tires as they spun on the road, as my car lurched forward, and hit the rear of the stopped car ~ 5-7 feet ahead of me. I felt that I had no control of my car and turned the engine off after the collision. I called 911 twice at 3.55 PM and 4.12 PM and waited as advised, for the Police Officer who arrived about 30 mins later. I filed the report and the officer was able to retrieve my auto license plate which had fallen off from the road where I could clearly see the tire marks on the asphalt from the spinning tires at the time of the accident. The only message I recall receiving on the display was one that informed me that I had experienced a crash after the collision. I had never experienced this problem before. Later ,after the Police Officer had arrived and documented the accident, I was able to drive the car home. The burning smell from the tires persisted for some time. The next day I called the BMW dealer in the AM, and with great trepidation, drove the car to the BMW dealership. I explained to the Service Advisor on duty on the Saturday, what exactly had transpired during the accident. I also explained this later to the BMW Body Shop manager when I spoke with him. I requested a full inspection and investigation by BMW. I personally do not feel safe driving the car again after this most unnerving and frightening episode.Read more

Owners across multiple years consistently report: • Black screens, freezes, and reboots • Loss of HVAC control and dual-zone functions • Backup camera not displaying • Vehicle/Apps tabs failing to open • Uconnect error messages • Smaller, harder-to-read gauges after updates • Wireless CarPlay/Android Auto failures • No option to decline or roll back OTA updates These failures distract the driver, remove access to federally required equipment (rear camera), and eliminate visibility and climate controls that directly affect safe operation. A few minutes later, I tried to turn on the car and some displays were off, there was no drive power, the steering wheel did not turn, the door locks were flakey, and I wasn't able to tell if I had shifted into Park or if the parking brake was engaged. When the tow truck arrived, the car was completely unresponsive and there was no way to release the parking brake or shift the car in neutral, so the car was dragged across the pavement and onto the tow truck. The failure was quick and comprehensive and could have easily caused injury. There are many highways near me with fast, busy traffic and either no emergency shoulder or a very narrow one. If my car tells me that I need to stop the vehicle, that can create a very dangerous choice between stopping somewhere less safe quickly or risking the car being completely disabled while in the travel lane while I look for a safer spot. Loss of cabin heat can easily become dangerous very quickly in the winter. Many drivers may not even understand the urgency of the warning until the car starts failing to drive. The car is at a Kia dealership for repair who confirmed an ICCU failure. See related NHTSA recall no 24V-867 and others. ICCU failures are endemic to Kia and Hyundai EVs. I have a 2024 Tahoe that was purchased new from the dealer and has 13,700mi. The first service was completed as required in December with no issues identified. Less than 2 months later the front wheels started making a squeaking sound intermittently when accelerating, coasting or breaking. There were no warning messages. ... measuring 2mm (completely worn out) but no other issues. Before drop off the car computer said above 75% or more for front and rear breaks (green). I have been driving the car in normal condition in southern ca. It seems that the break pads were defective and degraded at an alarming/unsafe rate or that a different component on the car caused them the continuously rub and wear down. The dealership blamed it on owner operation even though they supposedly checked them 2 months before and didn't see abnormal signs of wear. I am very concerned with the QUALITY, RELIABILITY & SAFETY of the front breaking system due to the fact that the breaks wore out so fast. I am worried that the problem has not been properly identified and that that breaks will continue to have this problem if it wasn't caused by faulty pads. I have never had a break wear problem like this before on any of the other 5 new cars that I have owned. While performing a factory recall on the RV, the mobile RV tech discovered underlying issues. He suggested that we get an in-depth evaluation. I contacted and dropped the RV at the shop for diagnosis. The shop discovered extensive frame damage (weld break) at the main beam connecting the pin box and also total frame break and separation on the upper deck floor support. There is substantial floor s
... agging above the outriggers extending from the living area through the back of the RV. The whole rear of the RV is sagging, causing wall cracking and slideout damage. The shop also measured a considerable deflection (pin box to front bottom frame) while performing the manufacturer's instructions for the measurements. The manufacturer has denied our claim and we are tasked with getting home to permanently park the RV. The manufacturer communicated with the shop regarding their finds and the denial, but has not contacted me in any way. All recalls/TSBs have been performed. The rear TSB has not fixed the rear frame sagging and the front TSB was not 100% completed by a dealer, in that several lag bolts were not replaced and none of them had adhesive applied. We have been fortunate that we have had no doors open while traveling. I composed a list of the items we found and submitted it with photos to the manufacturer. They said that the RV qualified for the "Customer Satisfaction Campaign" and that it should take ~1hr for a visual inspection. They only paid 0.10 hrs.Read more

As of today, no OEM or modifier has accepted responsibility for diagnosing or repairing this safety-critical braking system. This leaves the vehicle unable to be safely or legally operated, with no authorized diagnostic or repair support despite OEM branding and VIN identification.
