I am writing to report a critical safety defect in my vehicle, a 2019 Ram 1500 Big Horn , involving a catastrophic failure of the heater core (heater coil). This failure creates an unreasonable risk to motor vehicle safety as defined under 49 U.S.C. § 301. Safety Risk 1: Acute Toxicity and Driver Impairment The failed component is releasing vaporized ethylene glycol directly into the passenger ca
... bin through the HVAC vents. Per CDC and ATSDR documentation, these fumes are known to cause respiratory distress and neurological impairment, including dizziness and confusion. While operating the vehicle, these fumes caused [describe your symptoms, e.g., dizziness or coughing], creating an immediate risk of a crash. Safety Risk 2: Severe Visual Obstruction The vapor creates a persistent, oily film on the interior of the windshield that cannot be cleared by the defrost system. This "flash-fogging" has severely restricted my field of vision during operation, which mirrors safety risks identified in NHTSA Recall 24V-011. Safety Risk 3: Electrical and Fire Hazard Liquid coolant is currently leaking into the dashboard area, which houses critical wiring harnesses and electronic control modules. This exposure to conductive fluid poses a high risk of electrical short-circuits and potential vehicle fire, as documented in previous NHTSA investigations (e.g., Recall 26V012). Supporting Information: * VIN: [XXX] * Current Mileage: 93000 * Incident Date: January 17 2026 I request that the NHTSA open a Preliminary Evaluation into this model’s heater core assembly to determine if a safety-related recall is warranted. 1. I'm not sure what component or system malfunctioned. The dealership has said something different every time it's had to be taken in. The first time they said it could have been the dip stick. The second time they said they didn't know but did an update to the car's system. Last week, was the third time we've had to take it in and they said they are doing an oil change and replacing the air inta
... ke hose assy, GDI fuel pump, High pressure fuel pump, valve cover assy, oil filter and oil but didn't say if this would fix the issue. 2. We have loss confidence in this car's safety and reliability and the dealership's ability to repair it. It's been in the shop multiple times and continues to fail. The issue with the vehicle is that when we got a warning the power to the car shuts off. It doesn't allow you to do anything with the gas pedal no matter what speed you are going. We have experienced this multiple times on city streets and the highway. The potential for an accident is very high when the car does this. 3. We have taken it to the dealership and they don't know what's wrong with it. I created a case with Kia and have not heard back from a case manager at all. 4. The vehicle has only been inspected by the dealership and no one else. They have contacted Kia corporate for solutions via messaging but this is the third time the car has just loss power while we are driving. 5. There were some warning signs like check hybrid batter system on the dashboard. The kia Access app allow shows diagnostic alerts like ADAS_DRV. I have attached some photos and some of the documentation we have. There have been multiple dates but I put the first date that the car broke down. Other dates include (1/28/26 and 2/6/26).Read more

Three separate original equipment Pirelli Scorpion MS tires experienced sidewall structural failures on my 2025 Hyundai Santa Fe Calligraphy within the first 8,000 miles. One tire suffered a sudden blowout at approximately 3,000 miles. Two additional tires developed visible sidewall bubbles at approximately 6,000–8,000 miles. These failures occurred without impact damage or misuse and represent
... a serious safety risk. The dealership acknowledged observing one sidewall bubble during service but failed to inform me. Due to repeated structural failures, I was forced to replace all four original equipment tires at my own expense to ensure vehicle safety. I am concerned this may represent a pattern defect involving the factory-installed Pirelli Scorpion MS tires on 2025 Hyundai Santa Fe Calligraphy models. Happy to provide my receipts and documentation, and my out of pocket expense was $1,396.17. The first failure was in August, 2025 (car purchased in July) on country road setting, the second and third were both in early January under primarily highway conditions.Read more

UNKNOWN December 25th, 2025, my vehicle started to shift poorly, breaking very harshly, giving a clonking sound when putting the vehicle into any gear, and when putting it into reverse, the car rocked roughly and threw a wrench light onto the dash. That wrench light gave a torque converter/transmission code. I took it to a dealership to get it diagnosed and they told me nothing was wrong with it a

I took it to a different shop for a second opinion and as soon as my vehicle was in the air, the Mechanic found that ALL of my mounts were bad, I had a transmission leak and might possibly have a bad torque converter. I am submitting this complaint regarding a recurring safety defect in my 2024 Honda Prologue (VIN: [XXX] ), serviced at AutoNation Honda Chandler in Chandler, Arizona. Most recent service reference: Invoice #XXX. The vehicle has experienced a repeated knocking noise coming from the right front wheel area, particularly when turning right or left at low speeds with the steering wheel fully turned.
... The dealership diagnosed the issue as a failure of the front wheel drive half shaft (outboard passenger axle) and replaced the component. This same part has now failed three times. Despite multiple repairs and part replacements, the problem continues to return. The front axle/half shaft is a critical drivetrain component, and failure of this part can affect steering control and vehicle stability, especially during turning. Between all service visits, my vehicle has been at the dealership for nearly one month in total. Most recently, my third appointment was canceled, and I was informed that there is currently no solution available for this issue. I am extremely concerned about the safety of operating this vehicle due to the repeated failure of the same drivetrain component and the lack of a permanent repair. This ongoing situation has caused significant stress and raises serious safety concerns. I am requesting that this matter be investigated, as it appears to be a recurring defect affecting a critical vehicle component. I am reporting a safety-related defect in my 2017 Ford Focus equipped with the PowerShift dual-clutch (DPS6) transmission. The vehicle has experienced ongoing transmission and clutch-related issues, including shuddering, jerking, hesitation from a stop, delayed engagement, and unpredictable acceleration behavior. These issues are most noticeable in low-speed driving, stop-and-go traffic, and when
... accelerating from a complete stop. The clutch has previously required replacement/repair, yet the same drivability problems persist. The vehicle continues to jerk and hesitate in a way that makes it unsafe to enter intersections, merge into traffic, or drive in congested conditions. I have been informed that further repairs are not cost-effective and that the transmission/clutch issues are likely to continue despite repair attempts. Being advised that the vehicle is “not worth repairing” due to this known transmission design is concerning and suggests a systemic defect rather than normal wear. I am aware that Ford has faced widespread complaints, investigations, and class-action litigation related to similar PowerShift dual-clutch transmission failures in earlier model years. Although my 2017 model year was excluded from the settlement, the symptoms and behavior of my vehicle are consistent with the same known defect pattern. The condition appears to be worsening over time and presents a clear safety risk due to hesitation, sudden engagement, and loss of driver control during normal driving situations.Read more

⭐ 1. The Airbag Control Unit (ACU) and associated restraint‑system components appear to have malfunctioned. The Event Data Recorder shows inconsistent airbag deployment staging, a pretensioner firing while the belt was recorded as unbuckled, a seat‑track sensor fault, unsupported occupant‑classification data, incomplete curtain‑airbag deployment during rollover, and missing crash‑even
... t data. Yes, the vehicle and the full 29‑page EDR report are available for inspection upon request. ⭐ 2. The restraint system did not protect the occupants as expected. I suffered chest injuries from hitting the steering wheel, and my passenger sustained injuries to their hand and arm. These injuries occurred because the driver airbag did not deploy correctly, the pretensioner logic was inconsistent, and the curtain airbags did not deploy on the driver side despite multiple full vehicle rotations. The system’s irregular behavior increased the risk of serious injury or death. ⭐ 3. No. The issue occurred during a crash event and cannot be safely reproduced. However, the malfunction is confirmed by the vehicle’s own Event Data Recorder, which documents the deployment irregularities, sensor faults, and data‑integrity issues. ⭐ 4. The vehicle was inspected by insurance representatives after the crash. The manufacturer has not performed an engineering‑level inspection of the Airbag Control Unit or restraint‑system logic. The EDR report was obtained through the insurance process and serves as the primary source documenting the malfunction. ⭐ 5. No warning lamps or messages appeared prior to the crash. There were no symptoms indicating that the airbag or restraint system would malfunction. The failures were only discovered after reviewing the Event Data Recorder.Read more

I own a 2021 GMC Yukon Denali XL with approximately 81,000 miles. On 4 separate occasions, after coming to a complete stop in traffic and then attempting to accelerate, the vehicle violently jerks and shifts itself into park. I am not pressing the brake when this occurs. I must manually shift the vehicle back into drive in order to continue moving. This happens so fast, that I am only able to see
... a red outlined box that intermittantly flashes on the dashboard but I cannot see what it says before it disappears. The incident feels as though the vehicle has been struck from behind due to the abrupt force. This has occurred more than once and creates a dangerous situation in traffic, particularly when vehicle are behind me. The vehicle has been taken to a GMC dealership, which has serviced the vehicle since 2021. The dealership reports they were unable to duplicate the condition after driving approximately 60 miles and therefore cannot perform a repair. This issue remains unresolved. Given that the vehicle is shifting into park while in motion or during acceleration, I believe presents a significant safety hazard.Read more

While my vehicle was still under factory warranty, a mechanical issue was diagnosed by the dealership and a replacement part was ordered. The part was placed on backorder, and I was advised I would be contacted once it arrived. I was never notified when the part became available. By the time I was contacted weeks later, the warranty period had expired. I immediately scheduled service once I learn
... ed the part was available. During the repair visit, I was informed that the prolonged delay resulted in additional damage, including components related to braking and sealing. These components are available for inspection and were identified by the servicing dealership. Brake system involvement raises safety concerns, as reduced braking effectiveness can increase stopping distance and risk to occupants and others. The condition was confirmed by dealership technicians during service. The issue was not caused by misuse and occurred due to a delayed repair that originated under warranty but could not be completed due to parts unavailability and lack of notification. The vehicle remains under dealer inspection and repair. Warning symptoms were present prior to service, and the condition was confirmed by the dealership upon inspection.Read more

On October 29, 2025, my Infiniti QX50 was repaired at Naples Infiniti after the engine under shield detached while driving, ripping off the passenger-side wheel well liner. The technician stated the failure was likely caused by missing bolts from a prior oil change approximately two weeks earlier. Based on that representation, I authorized the repair. That explanation later proved to be incorrect.
... On January 18, 2026, the entire under shield detached again while driving, once more tearing off the wheel well trim and additional underbody plastic components. This second incident also caused a visible gouge in a new tire. This was the second identical failure, creating a significant and ongoing safety hazard. Subsequent research shows a well-documented pattern affecting 2019–2021 Infiniti QX50 vehicles, where underbody components detach while driving. This strongly suggests a design or materials defect, not isolated maintenance error. On January 19, 2026, while driving to Naples Infiniti to drop the vehicle off following the second failure, additional plastic components detached from the vehicle, nearly causing an accident. Photographs are available documenting the damage and debris. I immediately reported this to Infiniti, and they deny any wrongdoing or vehicle design defects.Read more

Summary of Risk: 2025 Jeep Wrangler 4xe Thermal Runaway/Fire Hazard: Per NHTSA Recalls 25V741000 and 25V766000, the high-voltage battery contains damaged separators, causing internal short circuits that lead to fires even when the vehicle is parked and turned off. Non-Stop "Turnover" Failure: The vehicle’s current state (constantly attempting to start/turn over without stopping) is a documented
... electrical/hybrid system malfunction. This indicates the high-voltage system is in a state of failure, significantly increasing the risk of a "thermal event" (fire). Proximity Danger: Per manufacturer guidelines, this vehicle is currently a Life-Safety Risk as it is immobilized within 50 feet of a residential structure housing a toddler and pets, violating the "Park Outside and Away from Structures" safety directive. Absence of Remedy: The dealership has admitted there is no current fix or available parts to resolve this hazard, rendering the vehicle "permanently impaired" under Maine Law. Summary of Dealership Response Failure to Secure Safety: Despite the vehicle being a non-drivable fire hazard, the dealership has not provided an emergency tow or a no-cost loaner vehicle to remove the threat from the residence. Predatory Pricing: The dealership is attempting to leverage a safety crisis to increase the lease payment by over 60% (from $500 to $800+). Misuse of Incentives: The dealership is failing to apply the $8,000 Stellantis "Recall Relief" rebate to lower the monthly payment. Instead, they are using it to absorb "negative equity" on a defective vehicle that should legally be subject to a full manufacturer buyback. Unreasonable Terms: The dealership is effectively demanding a $10,000+ total cost increase over the life of a new lease to replace a vehicle they admit is unfixable and dangerous.Read more

My truck continues to have the radio distortion problems that it has had after numerous software updates and repair actions. Approximately the first week of November in 2022 is when the first radio problem occurred. I only had the vehicle for about a month. The radio played an extremely loud distortion sound, close to an alarm tone, immediately after starting the truck. 3 different Ford dealers at
... tempted software updates and replacing the APIM module, but the issue came back several times. After the last Over the Air software update on SEP 17, 2025, the radio distortion has happened at least 6 times since then. The SEP 17 update specifically addressed the audio issues while using android auto. My warranty expired on July of 2025. I did not take delivery of the truck until late September because of shipping issues. Even with the delayed delivery of the truck, Ford told me that my warranty term was from JUL of 2022 to JUL of 2025. I always keep a 10mm socket in my truck in case I have to disconnect the battery in order to stop the radio distortion. In some cases, I'm able to change the radio source and the distortion will stop. Since the software update on SEP 17, the radio distortion has happened every 3 - 4 weeks.
