While decelerating at a low speed behind an 18-wheeler semi-truck, the vehicle failed to continue slowing down. Despite keeping the brake pedal fully depressed, the vehicle did not decelerate, resulting in a front-end low-speed collision with the metal rear bumper of the semi-truck. Because the collision occurred at a very low speed, the operator of the commercial truck was unaware of the impact and safely continued driving forward without stopping. The vehicle's brakes entirely refused to re-engage until I frantically pumped the physical brake pedal and cycled/pressed vehicle control power switches. This action seemingly forced the electronic braking modules to reboot or re-establish hydraulic pressure, allowing the vehicle to finally come to a stop. The vehicle was subsequently removed from the active roadway. This is a severe, life-threatening safety defect involving a total loss of electronic brake-by-wire and friction braking application without any mechanical override intervention, presenting an extreme hazard. The vehicle has been towed directly to an authorized Toyota dealership for a comprehensive electronic diagnostic pre-scan of the Skid Control ECU, Electronic Parking Brake system, and 12-volt auxiliary power systems to retrieve the stored DTC fault logs and freeze-frame data.Read more

Weather conditions were clear and normal at the time of the incident. There was no obvious impact point or bullseye pattern visible that would clearly indicate an object strike. The failed component was the panoramic sunroof/front movable glass panel. The damaged component is still available for inspection. This created a serious safety concern because shattered glass could have entered the vehicle while driving at highway speeds and could have caused distraction, injury, or loss of visibility/control. The vehicle has been inspected by a Hyundai dealership. I was informed the panoramic roof assembly may require replacement. I am filing this complaint because the failure appeared spontaneous and may indicate a defect or structural stress issue with the panoramic roof glass. Hyundai repaired the hood under this warranty in 2025, during which the vehicle was out of service for 77 consecutive days. Since that repair, the defect has migrated to the adjacent fender and has also appeared on a door, demonstrating that it is spreading across the vehicle. Hyundai has offered to repaint only the single currently-visible panel rather than addressing the systemic defect. My 2019 BMW X3 equipped with a B46 engine developed a significant coolant leak at approximately 80,000 miles. The vehicle displayed a low coolant warning, and coolant was visibly leaking underneath the vehicle. Around the same time, I began noticing excessive white exhaust/steam during cold starts and, for the first time since owning the vehicle, had to add engine oil between scheduled oil changes
... . The vehicle is currently undergoing inspection by an independent repair facility. Preliminary findings indicate concerns involving premature failure of critical plastic cooling system and/or oil filter housing components. Based on my research, these failures appear consistent with widespread complaints involving BMW B46/B48 engine cooling system and oil filter housing defects. I am concerned this defect could lead to sudden coolant loss, overheating, engine failure, or unsafe driving conditions. Prior to the coolant warning appearing, there were no significant alerts indicating a major or imminent cooling system failure. The apparent failure of critical plastic engine cooling components under normal driving conditions raises serious concerns regarding the durability and safety of these parts. I have photographs documenting the coolant leak, as well as detailed notes and diagnostic information from the repair facility, available upon request. The evaporator for the air conditioning failed. BMW used an aluminum evaporator with yf coolant. Those combined cause the coolant to eat away the evaporator. Once the yf comes in contact to the air it turns toxic. This shouldn't fail at 82,000 miles.

My 2026 Hyundai Tucson is experiencing a stuck update loop for the 2nd time since I purchased it new from a Hyundai dealer on February 2, 2026. The first stuck update occurred about Feb 18, 2026. I am filing this complaint because both times this occurred Verizon’s internet WiFi service was terminated because the provider couldn’t authenticate their service token and that this becomes a safety issue due to the fact that the Automatic Crash Notification and the SOS Emergency Call button are both disabled until the dealer flashes the system software and reestablishes a clean network handshake. The first attempt to correct the issue took about 3 weeks. Currently, I are waiting for an appointment to be made with the deal’s serve department so that they can flash the software again and clear the second stuck update loop. In the meantime, the above mentioned safety features are disabled. I fear that this stuck update issue will be never ending and I will be constantly returning to the deal to have the software flashed and the safety features will be disabled until they do so. All that said, you are aware of the reason why this occurs but continue to allow the manufacturer to flash the software and apply a bandaid. I am referencing NHTSA TSB #25-01-024H (Service Campaign 9C1). I would like to encourage NHTSA to reconsider their position on this ongoing network failure as more than a technical service communication issue but, rather, as an emergency safety recall as many Hyundai customers are experiencing this same stuck software update loop and loss of vital safety features.Read more

AWD vehicle where fronts should not be significantly outpacing rear wear. Left-right asymmetry on front axle suggests alignment issue. Dealer and BWM North America refused in investigation and call it normal. Requesting NHTSA agent review and case escalation.

I called Hyundai and was told there was no information, no remedy, no compensation for the loss of use, no loaner car, or any other options. The representative then said "unfortunately we also cannot escalate your case because there have been no fatalities yet." I purchased the vehicle new, the first owner and not even a three months later the nightmare began. I have taken it to the Honda dealership serval times when it is happening and the course of action is a screen reset and the issue still persist. There were no warnings or lights that came on prior to it to happening, it just began and has never stopped. The reset is not a sufficient solution and the only other option there is to replace it which cost well over a thousand dollars and most customers can't afford it. ... he instrument panels. As well the one pedal function would switch into regular drive mode, as in a gas powered vehicle, randomly. After multiple tries and incidents the dealer service department has been unable to duplicate. Then on March 6 of this year I bought in the vehicle yet again for this one pedal failure, and was notified there was an existing recall. This software update was performed. On March 17, just 11 days after the update, while backing into my garage with the garage door going up the windshield wiper was clipped by the door, destroying both the wiper and shattering the back window. I have backed into my garage thousands of times and never had an issue. Why this happened on this date I do not know. What I do know is that the vehicle gave no warnings and did not break for the object. At the time my dogs were clipped into their seat belt harnesses in the center well. By a miracle no glass shattered beyond the back storage area and I was able to safely remove them. I checked after and all of the collision warning systems were listed as active, including automatic braking for rear collisions. I had the dealership run scans twice and all scans showed no DTC codes and all systems active. I can only assume that all prior updates have failed, and there is a substantial defect to the vehicles ability to communicate. I have a copy of the scan performed. The second scan was run on April 9 because the dealership failed to provide a printed copy of the first. I have that second scan. My vehicle fails to communicate or activate essential information, a safety hazard.Read more

... ng. In 2023, a Honda dealership performed the MOST Bus/FAKRA connector safety recall on this vehicle. That manufacturer recall remedy has FAILED, and the exact same safety defect has returned. I did schedule the vehicle for a formal diagnostic inspection at a Honda dealership this Friday to pull the network communication error codes. I am aware that many other Honda HR-V owners have reported similar air conditioning problems, and Honda has offered extended warranty coverage for certain Honda vehicles with A/C system defects. However, my vehicle has not been included in that extended coverage, despite experiencing the same recurring issues. In addition, the vehicle previously required a fuel pump replacement in 2021. I am concerned that this may indicate broader reliability and safety-related defects affecting this model. The vehicle is also experiencing mechanical and electrical problems involving the odometer/instrumentation system. The odometer has shown irregular behavior and appears to malfunction intermittently, raising concerns about the accuracy of vehicle information and overall reliability. I have been experiencing these issues for years without any resolution. I reached out to Honda Customer Service about these issues, but because my "VIN" is not included in the recall or extended warranty, I haven't been able to get much help. I believe these issues warrant investigation, particularly given the number of similar complaints from other Honda HR-V owners. I am submitting a safety complaint regarding the rear visibility system on my Tesla Cybertruck. The physical rear view mirror does not provide any usable visibility of the roadway behind the vehicle. ... the rear camera feed displayed on the center screen. This is the only way to see directly behind the vehicle during normal driving. However, the rear camera feed disappears every time the turn signal indicators are activated. When the indicators turn on, the screen automatically replaces the rear view with the side camera view. This removes all rear visibility at the exact moment when the driver needs full awareness for lane changes, merging, and turning. This behavior cannot be disabled or overridden. It is part of the default software design. As a result, the driver is left without any functional rear visibility method during critical maneuvers. This creates a clear safety risk and raises concerns regarding compliance with FMVSS 111, which requires reliable rear visibility. The loss of rear visibility during lane changes and turns increases the risk of collisions with vehicles approaching from behind. Since the physical mirror is unusable and the digital feed is removed by the software, the driver has no safe or compliant way to monitor rear traffic during these maneuvers. I request that NHTSA investigate this issue and require Tesla to provide a remedy. A potential remedy would include keeping the rear camera feed visible at all times unless the driver manually chooses to remove it, or adding a dedicated rear view camera feed that remains active during all driving conditions.Read more
