The driver-side power sliding door latch, motor and cable system on my 2019 Honda Odyssey Elite failed. Honda performed Recall A88 (sliding door handle cable repair) in 2021. After this repair the door began intermittently failing to close fully and would close about 80% before the sensor triggered it to reopen. This continued until this week when the door suddenly stopped latching completely. The
... automatic door buttons no longer work and the door can only be moved manually. I believe my vehicle should also be included in recalls Z33 and O36 addressing sliding door latch failures affecting similar 2019 Odyssey vehicles. The failed components are available for inspection upon request. This defect creates a serious passenger safety hazard. Because the door will not latch, it opens while driving. The automatic function does not work and the door can be moved manually but will not latch. This creates risk of passenger ejection, especially children, and the possibility of the door opening during a crash. The vehicle is unsafe to operate. Yes. The failure was reproduced and confirmed by a Honda dealership that inspected and diagnosed the sliding door failure. The vehicle was inspected by a Honda dealership and I was charged a diagnostic fee. Honda refused recall or goodwill coverage because my VIN is not included. I request the repair be performed free under recalls Z33 and O36 and the diagnostic fee reimbursed. I am waiting for NHTSA inspection and request Honda be required to include my vehicle and repair it at no cost. Before total failure the door would close about 80% and automatically reopen. When it stopped working this week, the dashboard displayed a “Left Sliding Door Problem” warning and the vehicle produced continuous beeping while driving.Read more

On January 17, 2026, I was driving my Hyundai Ioniq 5, when it began to decelerate and lose power while I was pressing the accelerator pedal. The deceleration progressed. A small flashing battery icon appeared on the upper display. This icon had never been on the display before. Fortunately, I was traveling downhill and able to safely pull the car over to a stop. I turned the car off. I restarted
... the car with the same issue recurring. I turned the car off and called the dealer and scheduled an appointment. After about 5 minutes I restarted the car and was able to safely drive home without further incident. The dealer checked the car out and determined that nothing is wrong with it. The dealer was unable to replicate the problem. I have no confidence in continuing to drive the car safely as a loss of power could cause a serious accident. I contacted Hyundai USA customer service and a case was opened. They closed the case explaining that since the dealer found nothing wrong there is nothing they could do. Thank you.Read more

Incident date: Jan 13, 2026
The transmission blew in 2023. I purchased the vehicle from Fox Ford (first mistake) and they sent me to the local Nissan dealer to fix it. It was covered under dealer warranty at the time. Fast forward to 1/30/26. My car is fully paid off and I’m driving home from work with my daughter in the backseat and, my worst nightmare occurred; the transmission blew again! The only reason I purchased thi
... s vehicle in the first place by the way is because a police officer ran a red light in 2020 and smucked us and didn’t have a single repercussion but, I digress. Anyway, I was able to pull into the median on the highway (luckily I was able to get there and not have the car die in the middle of the highway lane) and call 911 for support. My car was towed to Riverside where it still sits. I don’t have any updated paperwork because Nissan is indicating it’s not covered under warranty even though this is a known issue with Rogues and clearly already an immediate issue to me because it had been fixed previously in this particular vehicle. I would like this safety issue to be known as well as the fact that instead of the dealership and Nissan as a company caring or trying to help me in this process, they’ve had me go on a wild goose chase and the saga continues. I simply want the issue to be fixed and justice to be served so that hundreds of thousands of other families don’t have to suffer. Luckily my husband was able to rearrange his schedule and assist with transportation until I begrudgingly was able to purchase a new vehicle but, many individuals do not have that privilege. What a shame that money is cared about more than human lives.Read more

I am filing a formal safety complaint regarding my 2017 Ram 1500. The Anti-Lock Braking System (ABS) module has been non-functional for approximately one year. I have been informed the replacement part has been on national backorder for nearly two years, with no clear timeline for repair. The ABS system is a critical safety component designed to prevent wheel lock-up, maintain steering control du
... ring emergency braking, and reduce stopping distances in wet or icy conditions. Without a functioning ABS module, the vehicle is at increased risk of skidding, loss of steering control, and extended stopping distances. In a full-size pickup truck, this creates a substantial risk of serious injury or death to occupants and others on the road. Additionally, related systems such as traction control and electronic stability control may be compromised. Online owner forums indicate this is an ongoing issue affecting many Stellantis-built vehicles, including Ram 1500 models. While recalls have been issued for other model years for braking-related defects, no recall has been issued for the 2017 model year despite similar failures. It is unacceptable for a manufacturer to leave owners operating vehicles with a known safety defect while critical braking components remain unavailable for extended periods. If a safety system cannot be repaired within a reasonable timeframe, regulatory intervention is warranted. I respectfully request that NHTSA investigate this matter and determine whether a recall is necessary. Should an accident, injury, or fatality occur due to this unresolved ABS defect, the manufacturer should be held accountable.Read more

While driving at highway speeds, my 2025 Nissan Pathfinder suddenly loses power and enters limp mode with no warning. The dashboard displays: “Engine Malfunction – Power Reduced – Service Now.” When this happens, the vehicle cannot accelerate and struggles to maintain speed, creating a serious safety hazard in traffic. This has occurred multiple times. First time was on January 6th - enter
... ed highway and car stalled, was able to pull over and have the car towed to the dealer and they stated, "cannot replicate the issue". Second time February 15th - car stalled on highway and Husband was able to pull over and restart the car. Third time was February 18th - car entered limp mode on the highway, car was towed to the dealer and they have yet again stated, "cannot replicate the issue". During each event, the Pathfinder rapidly decelerates, and I am forced to pull over immediately to avoid being hit from behind. The loss of propulsion happens suddenly and without any driver input. This places me and other drivers in danger, especially at high speeds and on icy Alaska roads. Two of the times was during snowstorms with low visibility. Attempts to Repair: I have taken the vehicle to the Nissan dealership. The dealer states they “cannot replicate the issue” and returns the vehicle without repair. However, the warning message and limp-mode event clearly indicate an engine or powertrain failure. The issue continues to reoccur despite these visits. Safety Concern: Because the vehicle loses power in fast-moving traffic and enters limp mode, it poses a substantial risk of collision or injury. This defect could cause a serious accident, especially at highway speeds or in winter driving conditions.Read more

I am reporting a potential safety defect involving the panoramic sunroof of my 2026 Kia Sportage, purchased new in August 2025. On February 19, 2026, at approximately 4:45 PM, while driving around 65 mph, the panoramic sunroof suddenly shattered without warning. The failure was preceded by a loud, explosive sound, after which the tempered glass fractured and collapsed inward into the passenger co
... mpartment above the driver’s seat. There was no observable external impact. I did not see any debris or object strike the vehicle, and the glass had no prior damage. Weather conditions were mild (approximately 75°F), and the vehicle was operating under normal highway conditions. Glass fragments fell into the cabin, including onto the driver’s seat and interior surfaces. The sudden noise and glass intrusion created a safety hazard, including risk of injury and driver distraction while traveling at highway speed. The vehicle was taken to a Kia dealership the same day. I was later informed by the dealership in San Angelo, Texas, that the damage would not be covered under warranty and that I would be responsible for the repair. Given the lack of observable impact and the inward failure of the glass, I am submitting this report for evaluation as a potential safety-related defect. The incident raises concerns due to the sudden failure at highway speed, inward breakage into the occupant compartment, and occurrence in a vehicle less than one year old. Here is your updated review with that included: ⸻ I am very disappointed with Hyundai Motor America and the service I received at Hyundai of Renton. They performed the software update and were supposed

The ignition was damaged during the theft, and I had to replace it myself just so I could drive the car. When I returned to the dealer, they refused to install the ignition cylinder protector, saying the ignition was broken. It was broken because of the theft — which happened due to Hyundai’s known defect. After the recall software update, my key fob also stopped working properly. I went back to the dealership, and they refused to fix or properly reprogram my key fob. On top of that, the service manager, John, was very rude and unprofessional. This has been the worst experience I’ve had with a vehicle and a dealership. Because of this situation. Very disappointed.Read more

First off, I should say that I generally love my Polestar 2. I've enjoyed owning it. However, I the infotainment system performance is dangerous. The incredibly slow time to open apps and the lag in response to inputs forces a driver to split attention between the screen and the road for far too long. For example, after clicking on Google Maps it may take 3-4 seconds to load, an additional few sec
... onds to be ready to accept inputs (such as an address), a few seconds to return search results, a few seconds to respond to a tap on a location, and a few more seconds to load directions. All told it can be 15-30 seconds to load directions, even for saved locations. The issues are most noticeable with Maps given how often I use them. But there are also lags in vehicle controls, such as climate, radio controls, and other apps like Amazon Music. As best as possible I use the infotainment system before I start driving or while I'm stopped at a light. Sometimes I will pull over to avoid distraction. But there are times when that isn't possible (such as freeway driving). But it shouldn't be this hard. Or this dangerous. You should not be forced to split attention for this long between the screen and the road. Unfortunately, voice controls are similarly slow and unreliable. I try to use OK Google when possible but even here I have to split my attention. After saying "OK Google" it can take several seconds to load and I have to watch the screen to see when it's ready to listen. And then it responds to commands at best half the time with lags or failures for command that should work "Navigate to location X" or "Play Musician Y on Amazon Music". You then have to go back to the slow manual inputs on the screen. This goes beyond an inconvenience to a danger. This is a known issue and Polestar has not taken steps to resolve it. I believe they should for all Polestar 2 owners.Read more

I am reporting a manufacturing defect involving the dashboard on my 2022 Audi A6. The dashboard has begun bubbling and warping, particularly near the area close to the windshield. This defect appears to be common among Prestige‑trim Audi A6 vehicles, as other owners have reported similar dashboard warping. The warping causes the dashboard material to rise upward, which reflects additional glare
... and partially obstructs visibility through the lower portion of the windshield, especially in certain lighting conditions. This reduces my ability to clearly see hazards low on the road ahead, creating a potential safety risk. Additional Details: - The vehicle has not been modified. -The issue has progressively worsened over time. - The defect appears to affect normal forward visibility and could distract the driver. Audi has not issued a recall for this known problem, despite multiple similar complaints from other owners (Effected Models per online research: ETron, R8, A6, A7, A8, Q7, Q8). I am asking NHTSA to investigate this dashboard defect as a potential manufacturing defect and safety hazard due to obstructed visibility and distracted driving caused by glare/reflection from the warped dashboard surface.Read more

Dear NHTSA Investigators, I am writing to formally report what I believe is a significant safety defect in certain 2007–2011 Toyota Camry vehicles, specifically related to excessive engine oil consumption in models equipped with the 2AZ-FE 2.4L four-cylinder engine (primarily 2007–2009, with some reports extending to later years). This issue causes the engine to burn oil at an abnormally high
... rate—often 1 quart or more every few hundred to 1,200 miles—without any visible external leaks. Owners frequently report no adequate warning from the dashboard oil light or other indicators before the oil level drops critically low. If the engine runs out of oil while driving, it can lead to rapid overheating, internal component damage (such as scored bearings or cylinder walls), and sudden engine seizure or catastrophic failure. Such a failure while the vehicle is in motion poses a serious safety risk, including: potential for crashes, including rear-end collisions if the vehicle stops abruptly without warning, or veering into other lanes/obstacles. Sudden loss of engine power. Increased likelihood of loss of control etc. This problem has been widely documented through thousands of owner complaints (e.g., hundreds on sites like CarComplaints.com for the 2007 model alone), NHTSA ODI reports, and Toyota's own Technical Service Bulletins (TSBs) and Warranty Enhancement Programs (such as ZE7 for certain models). While Toyota addressed it via a warranty extension (not a full recall) involving piston/ring replacements for qualifying vehicles after an oil consumption test, many owners—particularly those outside the original coverage period or with higher mileage—have faced expensive engine repairs or replacements out of pocket. The lack of a mandatory recall means the issue persists in many vehicles still on the road. I am concerned that this defect creates an unreasonable risk to drivers, passengers, and other road users. I had an issue with this and I needed to fill up 4.5QRead more

We purchased a 2025 Toyota Sienna Woodland Edition in July 2025. On October 7, 2025 we were notified that vehicle is subject to a safety recall [NHSTA Recall No. 25V-668, Toyota Recall No. 25TA12 (Interim Notice 25TB12)] due to improperly welded seat rails. The recall states: "The subject vehicles contain second-row seats that are mounted on seat rails that may have been improperly welded. If

All known owners of the subject vehicles will be notified to return their vehicles to a Toyota dealer. For all involved vehicles, the dealer will replace the second-row seat rails with rails that have proper welding, free of charge. For all involved vehicles, Toyota will notify customers by early December 2025." Following the recall notice, we returned our Toyota Sienna to the dealer due to the safety risk of putting our children (or any passenger) in the second row seats and we are currently driving a rental car. Problem with emissions system on my 2019 Honda CRV. Car was diagnosed and recommended to replace fuel injectors. This is a pervasive problem with Honda vehicles with a previous recall for Odyssey, Pilot and Ridgeline. Honda needs to recall any of their vehicles with this problem and fix it. In addition pervasive issues with their infotainment system which includes navigation. I have had the syste
... m go out when using the navigation option while in heavy traffic. Again both of these problems are well documented online with other owners but nothing being done by Honda to remedy either situation.Read more

My svc 4wd light came on in November of 2024. Took it to the dealer same month, they said my ptu failed and they couldn't do anything for it bc the parts were jot available. They weren't available on back order even. I paid for $130 dollars just to be told that they couldn't do anything. While at the dealer after they told me this news I went and talked to a salesmen, they told me they couldn't ta
... ke the jeep in on trade bc they don't know when they could get the part so they would loose money with it sitting. Recall came out on this in January 2025 and it now is February of 2026..I'm still waiting. The car is unstable and is basically worthless and a death trap in rain and is worse in snow. I got offered a rental out of my own pocket. I'm still making payments on this and I can't even fix it. The cars going to kill me and then what. Nothing. I was told in November of 2025 that they were hoping to have this fixed by the end of the year. Well it's February and nothing, I think they are just lieing. People are going to get hurt or killed bc of the stability.Read more

The component that appears to have malfunctioned is the front axle integral carrier (front subframe) and the related front suspension geometry, as the vehicle now exhibits a persistent pull to the left and degraded steering stability after the dealer replaced the front lower control arms, engine mounts, and performed alignments; this area of the vehicle is fully available for inspection upon reque
... st. My safety and the safety of others were put at risk because the vehicle requires constant steering correction to avoid drifting left, creating a danger of unintended lane departure and loss of control, which aligns with the steering‑related risks described in NHTSA Recall 21V990 / MBUSA 2022070009, where failure of the integral carrier can impair steering and increase crash risk. The problem has been reproduced by me on every drive, though the dealer stated they “did not find anything,” despite alignment printouts showing front camber imbalance and lack of caster correction. The vehicle has been inspected twice by the authorized Mercedes‑Benz dealer, but they did not inspect or address the front axle integral carrier even though the recall documentation shows that corrosion or structural issues in this part can affect steering performance and connection to other components. There were no warning lamps or dashboard alerts; the symptoms began immediately after the dealer’s suspension and mount repairs, and the steering pull and unstable handling have continued ever since.Read more

The vehicle continues to unlock itself. The dealer has been able to reproduce the problem. First repair. They said it was because of a recall to do with the door handles, even though the recall didn’t state any of that cause they were able to reproduce the vehicle unlocking itself after the recall so they did a software update once I took the vehicle home was able to reproduce, and the issue of
... it unlocking itself continued. I returned the vehicle to the same dealer and which they had the vehicle for an additional 26 days with another software update and then they had to call in to the tech line where they had to replace the control module that still didn’t fix the issue then had to replace a key then they released the vehicle back to me as they exhausted all their options and the issue is still present unable to drive the vehicle due to security concernsRead more
