My vehicle has had recurring interior water intrusion and related safety concerns. The most serious symptom was interior window fogging on the inside of the vehicle for multiple days during or following rainy, snowy, and cold conditions, which significantly reduced visibility while driving and made it difficult to see out of the windshield and side windows. To clear the fogging quickly, I have had
... to turn off the heated seats and defrosters and open the windows; otherwise, I have needed to pull over and wait until I could safely see through the windshield again. A strong mildew odor would also develop and could last for days after rain. Sometimes the seats felt damp and the odor became worse when the heated seats were turned on. The first repair visit for this issue was January 13 to 19, 2026 and during that visit, the dealer was unable to recreate the issue. The problem continued, and after my navigation/infotainment screen began having issues, I returned the vehicle to the dealer again on March 7, 2026. I was then informed there was water intrusion and water-damaged carpeting, and I was told there was a known issue involving a faulty or broken seal on this model and there was a recall, but I have not been able to confirm that recall by VIN online. It remains unclear whether the leak is the full cause of the repeated interior fogging condition. In addition to the water intrusion issue, the vehicle has also had other electrical and safety-related concerns, including the infotainment screen turning white for weeks, abnormal turn-signal behavior after completing turns, lane-assist steering irregularities that pulled the vehicle toward the lane line, and braking irregularities, including the brakes locking up while backing into a parking space or during slow stop-and-go traffic. It is currently unknown whether moisture intrusion contributed to any of these other issues. My vehicle is still with the dealer for repairs until around April 15.Read more

I am reporting a recurring safety issue with the power windows on my 2023 Toyota Tundra. Specifically: 1.The driver-side window and front passenger-side window have stopped moving partway during operation. The window halts halfway, the window becomes stuck and cannot be closed even when attempting to push the window up manually with my hands. 2.The problem has occurred with two separate windows o
... n the same truck. 3.This behavior has occurred under normal operating conditions and appears similar to reports from other 2023 Tundra owners, suggesting a possible design or manufacturing defect. 4.The issue could pose a safety risk, as the window may trap fingers, limbs, or objects unexpectedly, or fail to close during adverse weather or security situations. Actions Taken: •I have taken the vehicle to a Toyota dealer for inspection and attempted repairs. •The dealer has documented the issue in service records, but the problem has occurred in a different window. Additional Notes: •I am aware of multiple owners experiencing the same issue with 2023 Tundras. •I am submitting this complaint to request that NHTSA investigate the potential defect and assess whether a formal recall or safety advisory is warranted.Read more

A recall was issued for a roof leak defect that allowed water intrusion into my vehicle. The recall repair was performed at Autonation Toyota of Weston dealership, where the roof was replaced due to water staining. However, water intrusion which led to black mold growth into the interior carpeting and floor board remained after the repair. The dealership stated that the remaining water intrusion a

Documentation showing that the recall repair procedure was completed according to the manufacturer’s requirements was requested but has not been provided. In under a year I have had to replace my windshield three times due to cracks from debris. The total cost would be greater than $4,000. I, nor my wife and family have ever had to replace a windshield on any previous car we had owned this many times in under one year or even at all. I suspect there is a defect in the build of the windshield or the design of the car that may be causing this.

2020 Honda HR-V — OBD Monitors Fail to Reset After Battery Replacement. Dealer Refused Help. Cannot Pass MA State Inspection. Following a routine battery replacement, my 2020 Honda HR-V OBD monitors will not reset, making it impossible to pass Massachusetts state inspection. ... ystem still shows “not ready.” I have made multiple trips to inspection stations, waited in long lines each time, and been turned away every single time. I have lost significant time, wasted fuel, and face real risk of registration penalties — for a car that runs perfectly. When I contacted Honda of Newton Corner — my exclusive service dealership — they told me they could not help me. No escalation. No fix. No support. I am not a mechanic. I am a customer who followed every instruction, trusted the brand, trusted the dealer, and has been left completely alone to deal with this! This is a safety concern. If Honda’s OBD system cannot confirm its own monitors are functioning after 562 miles and 142 cycles, there is no assurance the vehicle’s emissions and safety systems are working properly. This issue is not isolated — multiple HR-V owners across 2019–2022 model years report identical problems. Honda has issued no software fix and no dealer protocol to resolve it. I am requesting NHTSA investigate Honda’s OBD monitor reset failure across HR-V models and require Honda to issue a remedy — including dealer-authorized in-shop resets and a software update. Honda created this problem and has abandoned its customers entirely.I should not have to choose between driving an uninspected vehicle and being stranded. This is a defect in Honda’s drive cycle reset logic. Honda needs to issue a software update and instruct dealers to perform monitor resets!A dealership that exclusively services your vehicle should have all the necessary tools and the obligation to help. The reverse indicator and camera continued flashing. After slowing I turned my hazard lights on and slowly drove the vehicle directly to the Toyota dealership. I told them what happened and provided a video of the flashing light and screen. I told them that the truck was no longer safe to drive and that I was fortunate that I didn’t cause an accident. They ran diagnostics on the vehicle which had many error codes for various modules. They have cleared the codes and are requesting that I take the vehicle back. They reported that they could not replicate and no repairs were made. The accelerator system malfunctioned in Jan. I had to pay for a replacement fuse despite the warranty. The vehicle is modified with Braunability equipment for wheelchair access. These modifications are approved by Toyota and were completed by MobilityWorks. Aside from the vehicle itself being unsafe to drive, the passenger sliding door has had numerous problems. My 2022 Kia Stinger first went into the dealership because it would not accelerate properly even at full throttle, creating an immediate safety risk in traffic. Over multiple visits, the dealership replaced the ECM, ABS module, brake booster, vacuum pump, and a fuel injector, and confirmed repeated electrical shorts across major systems including throttle control, braking, and fuel delivery. Despi
... te months of repairs, the loss of power, brake assist issues, ABS faults, and electrical failures continued, and the dealership never identified the root cause. The problems were reproduced and documented by the dealer, the vehicle was inspected multiple times on behalf of the manufacturer, and warning lights and drivability symptoms appeared before each failure. The vehicle is currently not in my possession, and all replaced components are available for inspection. The combination of engine hesitation, brake system failures, and repeated electrical shorts makes the vehicle unsafe and unreliable to operate.Read more

My 2010 Volkswagen CC became completely inoperable because of the active fuel pump control module recall (NHTSA 17V509). The vehicle stalled and nearly caused an accident. The check engine light came on just before I noticed a burning smell coming from the backseat of my car immediately after smelling that the car suddenly stalled out in the middle of a busy intersection and the engine would not s
... tart back up. I had to push it to a gas station parking lot across 3 lanes of traffic. I lifted up the backseat and found that the fuel control module was overheated and melting on top of the fuel pump so I carefully and properly disconnected it from the vehicle. The next day I had the vehicle towed to Lokey Volkswagen in Clearwater, FL. Today 3/14/2026 They did the recall repair and told me my vehicle is safe to drive. Today 3/14/2026 I drove the car 1 mile from the dealership when the exact same thing happened again the car stalled out, and I had to carefully maneuver to the shoulder of hwy us 19 s narrowly missing vehicles in the other lanes. The vehicle was then towed back to lokey Volkswagen. It is pretty clear that the issue was not resolved, and that this fuel control module failing may have affected other parts of the fuel system/wiring and or the fuel control module failed again. I have pictures of the fuel control module, and I gave the initial melted/failed fuel control module to the service department and they took it. The dealership service department is stating that this may be to do something other than the recall repair and that I may be liable for the diagnostic and towing charges. This is a very serious safety issue that almost cost me to get in accidents on the highway twice. I feel that they (Volkswagen) needs to be held responsible for rectification and to find out if there is a connection between this recall and the continuing failure of the vehicle to operate safely.Read more

The high-voltage battery on my 2022 Nissan Leaf SL+ is subject to Recall NHTSA ID: 25V-655. The recall notice states that the battery cells can develop excessive lithium deposits, which increases electrical resistance and can cause rapid battery heating during Level 3 fast charging. Nissan instructs owners not to use Level 3 charging at all until a “remedy” is available. The issue is that the
... remedy described by Nissan is not an actual repair of the battery defect and has been significantly delayed, with no remedy provided for the loss of use of my vehicle, due to the dangers of the battery, which is still covered by warranty. Nissan states it will install software that monitors for “state-of-charge fluctuation” and, if detected, will prevent the vehicle from restarting or recharging in order to avoid a thermal incident. This means the defect inside the battery cells is still present, and the car may disable itself if the defect begins to appear. The recall materials also state there is no warning before overheating occurs. This creates multiple safety concerns: • The underlying battery defect remains uncorrected. • The vehicle can become immobilized (unable to restart or recharge) if the software detects the condition. • Loss of Level 3 charging capability affects the ability to travel safely or plan charging when needed. • A battery fire risk exists during Level 3 charging if the defect is not detected in time. Nissan’s documents state that the software is meant only to prevent the “progression” of a thermal incident, not to repair the defective battery. I am concerned that my vehicle contains a known hazardous defect that has not been physically repaired and that the software-only response is inadequate to ensure safety. I am filing this complaint so NHTSA is aware that the remedy being offered does not address the internal battery defect itself and may leave owners exposed to continued safety and reliability risks.Read more

The moonroof spontaneously shattered when I was driving on the freeway which sounded like a gunshot. There were no external factors such as debris from other vehicles and the temperature outside was relevantly constant at about 78F. No other cars were close to me or in front of me. For almost the entire 2 years I had my car, nothing has ever damaged the moonroof glass. I do want to note an observa
... tion that may or may not be linked to the potential safety defect: I do recall hearing strange pebble-like noises occasionally on the freeway & off the freeway at speeds of ~35-65 mph. It sounds like crumbs falling down the sides/top of the car in a horizontal pattern. I observed this as early as 1 month into ownership. Upon closer inspection after stopping to inspect my vehicle, the glass shows to have shattered/blew up & outward in cube shaped pieces and the pattern of the glass appears spiderweb-like, not originating from an impact point. The fracture pattern of the remaining glass appears to match a smooth edge breakage pattern which indicates stress failure due to a spontaneous glass breakage. The dealer has denied coverage due to the technician 'finding no evidence of structural damage.' Yet, my moonroof shattered upwards and there is not evidence of an impact crater. My car is still under warranty and under 2 years old and currently is only at about 20,500 miles. The dealership quoted me ~ $2500+$185 diagnostic. I will be elevating to Toyota corporate to request a closer inspection. Based on research, I discovered numerous reports of similar spontaneous sunroof failures in Toyota vehicles. The failure presents a high risk for public safety because the glass could fall into the eyes and face of the driver at highway speed. Additionally, drivers could be startled & swerve on the road at high & low speeds, on the freeway/streets. There were no warning signs/alerts. One moment I was driving to work on the quiet road & the next I heard what sounded like a gunshot.Read more

Within months of purchase, I reported to James Wood Hyundai in Decatur that my children were repeatedly becoming unbuckled in the rear bucket seats. I was informed repairs would be delayed several weeks because Enterprise fleet vehicles were being prioritized and was told I am the “only person” experiencing this issue a statement that conflicts with a formal safety recall. I am reporting a safety concern involving a 2018 Dodge Journey SE (VIN: [XXX] ) related to a failure in the vehicle’s emissions control system, specifically the catalytic converter and oxygen sensors. On January 13, 2026, the vehicle began producing a strong sulfur or “rotten egg” odor while driving, and shortly afterward the check engine light illuminated. Diagnostic testing revealed troubl
... e code P0420 (Catalyst System Efficiency Below Threshold – Bank 1). This code indicates that the catalytic converter is not properly converting exhaust gases, which can result in increased emissions and potential exposure to exhaust fumes. The vehicle was inspected by an authorized Chrysler, Dodge, Jeep, and Ram dealership in Maurice, Louisiana, which confirmed that the catalytic converter and oxygen sensors are failing and must be replaced. The vehicle remains available for inspection upon request. This vehicle was purchased for our growing family and is regularly used to transport our children, including a [XXX] infant and a [XXX] child. Since the emissions system issue began, occupants have noticed persistent exhaust-like fumes and strong sulfur odors while the vehicle is operating. Our infant has experienced recurring respiratory issues, and our [XXX] began experiencing respiratory symptoms after we purchased this vehicle. While we cannot definitively determine the cause, we are concerned that exposure to exhaust fumes from the malfunctioning emissions system may be contributing to these health issues. In addition, adult occupants have reported feeling unusually tired or fatigued after longer car rides in the vehicle, which raises additional concerns about potential exposure to exhaust gases inside the cabin. Because catalytic converter failures can allow harmful exhaust gases such as carbon monoxide and other pollutants to pass through the exhaust system improperly, we believe this issue presents a potential safety risk to vehicle occupants, p INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 URead more

Dear National Transportation Safety Board, I would like to bring a situation that happened to me while driving my 2019 Subaru Forester last Saturday March 7th, 2026 at 10:40am. I was coming back to my house from an errand. While waiting for my garage door to go all the way up, my car suddenly accelerated forcefully and lunged through the garage and into my home through the wall. There was a c
... hest freezer that was against the wall in the garage which was against the back wall. This freezer was also taken through the wall and into my laundry room. The freezer was crushed. The front of the car and tires were up through the wall and in the home. There was a water spigot also in the garage that came into my home that broke and dispersed water. The furnace was crushed, water heater damaged, washer and dryer damaged, a water cooler damaged along with crushed metal bifold doors that were ripped off HVAC closet. And other things in garage totally crushed. Thankfully, no-one was killed. I was barely able to get out of car as when it suddenly accelerated, and clipped the bottom of garage door that wasn’t finished being up, then it went towards the left and corner of garage instead of of how I would normally pull in and park which would have been straight and middle of garage. The car went so fast and would not stop until it had hit the items in house that would make it not be able to go any further. This vehicle definitely had something faulty with it that caused this car to suddenly take off and crash. And the air bags did not go off. Surprised with such a forceful impact. My car had 62,039 miles on it and was well maintained. I bought brand new at Dellenbach Subaru in Fort Collins, CO in December 2018.
