On 11.25.25, my MDX was involved in a multi-car hit and run on highway 101 SB at the Laurel Drive off ramp. The vehicle sustained two points of impact: (1) substantial front/driver side impact (2) rear-passenger side impact. Despite the severity, location of impact and speed of travel at the time of the collision, all airbags deployed except for the driver side curtain airbag. Importantly, at t
... he time of the crash, the Lane Keeping Assist System (LKAS) and associated AcuraWatch driver assist systems were activated. The vehicle’s safety assistance technology was engaged and functioning immediately prior to impact. Considering the above mentioned facts, driver side airbag failing to deploy, raises serious concerns regarding Possible software or signal-processing malfunction or electronic communication between radar/camera systems and the SRS module. The nature and severity of the impact clearly met deployment criteria for the driver side airbag. This failure raises concern that there may be a defect in the SRS system, including but not limited to: Airbag inflator component or propellant issue. Supplemental Restraint System (SRS) crash sensor calibration. SRS control module deployment logic. Crash signal sensors. Event-trigger logic or interconnnect failure. Wiring harness or inflator defect. Whether the system registered steering correction or impact prediction, the failure to deploy increased my risk and resulting injury, as well as compromised the fundamental safety system designed to protect drivers in front-impact crashes.Read more

IT FELT LIKE IT WAS GOING TO STALL SO I COULDN'T COME TO A COMPLETE STOP FOR VERY LONG IF AT ALL. IF I DID COME TO A COMPLETE STOP AND I WAS ON A HILL, IT WOULD START TO ROLL BACKWARDS. THE JEEP WOULD NOT ALLOW ME TO DRIVE IN E-SAVE, OR ELECTRIC, EVEN THOUGH THE BATTERY WAS 100% CHARGED AND HAD A HALF TANK OF GAS. THE JEEP WOULD ONLY DRIVE IN HYBRID MODE. WE HAD THE JEEP AT THE DEALERSHIP FOR ONE OF THREE RECALLS THIS JEEP WAS APART OF. IRONICALLY MY JEEP DID NOT START HAVING ISSUES UNTIL THAT SPECIFIC RECALL WAS "FIXED". The rear window on my 2017 GMC Sierra shattered unexpectedly without any impact. The vehicle had been remote started in very cold weather and the rear window glass suddenly shattered while the truck was parked. There was no rock strike or external damage that would explain the failure. I later learned about GM Customer Satisfaction Program N192265660 regarding rear window defroster thermal events
... affecting certain 2014–2019 GMC Sierra and Chevrolet Silverado trucks. Based on the sudden failure and the conditions at the time, this appears consistent with that issue. The Auto Rear Defog feature in the vehicle settings was turned off, however the truck had been started using the remote start feature when the glass shattered. Photos of the shattered window are available. I am submitting this report because the rear window defroster system may cause the glass to overheat and shatter unexpectedly.Read more

On Sunday, March 15,2026, I was driving from Houston, TX to Pharr, TX at the speed of 75mph on the left lane to pass an 18 wheeler, with another 18 wheeler behind me, and my truck started idling erratically. I lost speed and my service engine light came on. My truck slowed down and I almost got in a car accident with the 18 wheelers because the truck did this. The truck behind me slowed down on ti
... me and it went immediately to the right lane to avoid hitting my truck. I feared for my life. All the vehicles behind me and the 18 wheelers also went to the right lane to avoid me. By this time, the TRAC light had already come on also. I managed a space of no vehicles and steered towards the right safety lane. I pulled over at the local truck stop and stoped my truck to rest. After about 5 minutes I tried starting my truck and it wasn’t starting. I am a little mechanically inclined, so I started troubleshooting the truck. My code reader gave these codes: P1605-00, P0300-27, P0300-85, and P0302-00. Unscathed to start the truck after a while and got back on the road. The drive was brutal, as the truck kept burping erratically, but I made it home safely. I took my truck to the mechanic and he did some repairs and told me the issue is the fuel pump. I have checked the recalls for my truck but it shows zero recalls for my truck. Luckily I am alive to report this life threatening safety issue that Toyota has on this model. How do I get Toyota responsible to make the repairs on my truck? The part alone is over $1200 at O’Reily auto parts.Read more

As of today, March 17, 2026 , my 2025 Hyundai Tucson keeps having safety issues. On March 8th , 11th, 14th and 25th the car once again shut down mid-drive. This issue occurred last month as well but dealership claims no DTCs were found. I am noticing that the forward collision warning comes on without any cars or objects in front periodically since the purchase of my car. The light will remain o
... n for a few seconds and disappear. The automatic brake system seems to be causing issues as well which I believe causes the car to shut off completely mid-drive. Once the car shuts off the panel lights turn on to include the engine light, but then after a few seconds I able to change gears back to park in order to turn car back on and continue driving. This has happened now several times since last month. However, I have had issues with my brand new vehicle since I purchased it last year. My car remained in the dealership for 3 months last year due to an engine, ac and power train issues. Prior to the car shutting off, No actual warning lights illuminate other than the forward collision warning light and the notification of headlights are turned off even though it’s daylight and headlights do not need to be turned on . Periodically and intermittently an alert about consider taking a coffee break alert has also popped up in the past. It seems Hyundai has a severe issue with the FCA operating system and over 100 complaints have been made about this vehicle nationwide and no recalls have been issued. I am afraid for my life and the life of others on the road as this car should not be stopping and shutting off while driving for no apparent reason.Read more

I was forced to continue driving until I could safely pull over. After parking, shutting the vehicle off, and waiting several minutes, the IHU restored itself. Premature frame rot
Incident date: Mar 12, 2026
I am reporting a structural failure of the rear subframe due to corrosion on my 2015 Mercedes-Benz C300 4MATIC. The failure was diagnosed by an authorized Mercedes-Benz dealer, who confirmed that the rear subframe has corroded and broken, compromising the structural integrity of the vehicle. This is not cosmetic rust. The subframe is a load-bearing structural component, and its failure creates a s
... erious safety risk, including potential loss of vehicle control. I have been advised that the vehicle may be unsafe to operate in its current condition. The vehicle has approximately 143,000 miles and has been registered and operated in Connecticut, a salt-use state. However, this level of corrosion leading to structural failure of a major suspension component is premature and not consistent with normal wear and tear, even in such environments. I contacted Mercedes-Benz USA and escalated the matter to an Executive Referral Manager. Mercedes-Benz acknowledged the seriousness of the issue but declined to provide any repair assistance, stating that my vehicle falls outside of warranty extension and goodwill parameters. Mercedes-Benz did not dispute the nature of the failure or its safety implications, but refused to address the issue based solely on internal policy criteria such as age and mileage. A rear subframe supports critical suspension components. Corrosion leading to structural breakage can result in instability, misalignment, or failure of suspension components while driving, increasing the risk of a crash. This issue appears to be consistent with known reports of rear subframe corrosion in similar Mercedes-Benz vehicles, raising concern about a broader safety defect. The failure was identified during inspection, and repair costs are estimated at approximately $5,000–$6,000. I am submitting this complaint due to the safety risk posed by structural failure and the manufacturer’s refusal to address the issue.Read more

I am filing a safety complaint regarding Tesla's removal of driver-controlled maximum speed in FSD (Supervised), a change that has alarmed a significant number of Tesla owners and safety-conscious drivers. Previous software allowed drivers to set a precise maximum speed via the steering wheel scroll wheel while FSD (Supervised) was active. Tesla replaced this with fixed "Speed Profiles" (Sloth, C
... hill, Standard, Hurry, Mad Max), none of which allow a driver to set a specific speed. Each profile operates at a programmed range relative to the posted speed limit. Even the slowest profile (Sloth) has been observed exceeding the speed limit. The system also frequently misreads posted limits entirely. This is a serious safety regression. In winter conditions, a cautious driver may need to travel well below the speed limit due to snow or black ice. Previously, they could reduce FSD's max speed precisely. That is no longer possible, the system determines speed, and the driver cannot override it downward while FSD remains engaged. This concern is not isolated to just me. Online forums and Tesla owner communities have seen widespread discussion and frustration over this change, with many drivers expressing that it has made them feel unsafe and less in control of their own vehicle. Removing precise speed control from a supervised autonomous system directly contradicts Tesla's stated position that the driver remains responsible for safe operation. This is an ongoing issue and not limited to a single date for the "Tell us the approximate date this incident occurred." I request NHTSA: 1. Investigate Tesla's removal of driver-adjustable maximum speed in FSD (Supervised); 2. Evaluate compliance with FMVSS and driver control guidelines for automated driving features; 3. Require Tesla to restore precise, driver-controlled maximum speed settings.Read more

On 03/13/2026, while driving my 2020 Tesla Model X at approximately 65 mph on a freeway, the front hood (frunk lid) suddenly flew open without warning and slammed into the windshield. The hood rotated upward violently and struck the windshield completely blocking the driver’s forward visibility. This created an immediate safety hazard while traveling at highway speed. Prior to the incident there
... were no warning messages or alerts displayed on the vehicle screen indicating that the hood was open or improperly latched. Tesla vehicles normally provide a dashboard and a sound alert if the hood is not properly latched before driving. No such warning occurred. The vehicle’s front camera recorded the incident. The video does not show any hood movement or flutter prior to the failure, suggesting that the hood was fully closed before the event. After the incident, Tesla service personnel inspected the vehicle. The technician stated that the hood may not have been properly latched prior to driving. However: The vehicle gave no dashboard warning No hood movement or vibration was visible prior to the event The hood opened suddenly and violently rather than gradually If the secondary latch alone had been holding the hood, some hood movement or vibration would normally be expected prior to full release. Safety Concern A hood opening at highway speed that: occurs without warning results in complete loss of forward visibility presents a severe safety risk to the driver and surrounding traffic.
