While driving a 2020 Kia Forte on the expressway at normal speed, the vehicle was operating normally with no prior warning lights or known issues. Suddenly, the brakes began to feel weak, as if they were failing. Immediately after, a popping sound was heard from the vehicle, followed by smoke coming from the side/underneath area. Within moments, the smoke intensified and the vehicle caught fire.
... The driver pulled over to the side of the road as quickly as possible. The fire spread rapidly, and within approximately 1–2 minutes the vehicle was fully engulfed in flames. There were no prior warning indicators, messages, or symptoms before the incident. The cause of the fire is currently unknown. The vehicle is available for inspection by insurance or manufacturer if needed. My drivers side front window shattered while I was driving on XXX Since it’s shattered, the window itself isn’t available for inspection. There were no prior cracks to the window and nothing hit my window when the it shattered. I was going 75 mph on the highway and had two passengers in my car. I ended up with glass all over me. Luckily, i was wearing sunglasses, so none it went in my eyes. I
... didn’t swerve or lose control of the vehicle, but myself and the two passengers could have been seriously injured as a result of this defect. I am unaware of whether this problem has been confirmed by a dealer or independent safety center in other vehicles besides my own. I did take my car to the Honda dealership in [XXX] where I bought it from, and they will look at it tomorrow during business hours. Incident date: Mar 5, 2026
The suspected failure involves the front axle/CV (constant velocity) joint assembly, part of the drivetrain system that transfers power from the motor to the wheels while allowing steering movement. The vehicle produces a repetitive clicking/clunking noise during turns, particularly under light acceleration from a stop. This symptom is consistent with a failing outer CV joint or axle shaft compone
... nt. The vehicle and all components are still installed and available for inspection upon request.A failing CV joint or axle can lead to loss of drive to a wheel, joint separation, or sudden drivetrain failure, especially during turning or acceleration. This creates a risk of loss of vehicle control, particularly when entering traffic, turning at intersections, or maneuvering at low speeds in populated areas. The clicking occurs during normal driving conditions, indicating a potential mechanical integrity issue that could worsen without warning. If the joint fails completely, it could result in sudden immobilization in traffic or inability to control vehicle movement safely. The vehicle has been presented to only the dealer, and they initially replaced but the problem persists. They do not have an answer other than when I presented bc my tires were showing serious wear, they refused to replace and said it was not connected to CV joint problem. No warning lamps or messages have appeared.Read more

While operating the vehicle, an intermittent audible warning sound occurs without any corresponding alert, message, or notification displayed on the dashboard or infotainment screen. The specific component or system causing the warning is unknown. The vehicle is available for inspection upon request. This condition puts my safety at risk because the unexplained warning sound occurs while driving
... and causes me to divert my attention from the road in an attempt to identify the source or meaning of the alert, creating a driver distraction. The issue has been reproduced multiple times under normal driving conditions. I demonstrated the condition to the dealership and provided video evidence; however, the issue has not been diagnosed or confirmed, and no root cause has been identified. The vehicle has been inspected by the dealership, but no explanation or resolution has been provided. The manufacturer is aware of the issue through an open case. There were no warning lamps, messages, or prior indicators associated with this condition. The audible warning occurs without any visible alert or explanation. The condition began shortly after purchase in November 2025 and was later documented and demonstrated to the dealership in January 2026.Read more

This is a safety-defect complaint regarding the Front Seat Active Head Restraint (AHR) on our 2015 Chrysler. In March 2025 the front passenger AHR SPONTANEOUSLY deployed with no collision or impact. The head restraint is now permanently dislodged and dangling from its original position. This creates an immediate safety hazard: it distracts the driver, protrudes into the front passenger seat area,
... and can cause pain or injury to front-seat passengers. This defeats the very purpose of the AHR and introduces new risks of harm. This is the exact defect NHTSA investigated in PE19-014 (inadvertent AHR deployment without crash). NHTSA closed that investigation in 2026 without a recall, citing FCA’s 10-year / Unlimited Miles warranty extension as sufficient remedy. However, FCA NEVER NOTIFIED US of the extension — even though our vehicle's VIN has been registered in our names at the same home address for ALMOST 5 years. We only discovered the warranty extension days ago through independent research. Our vehicle has extremely low mileage (59,666 miles total as of February 2026 — only 17,265 miles added in the 4.6 years of our ownership). We are the second owners and there is no history of accidents. Because we were never informed of the warranty extension, we could not request inspection or repair even though we were still within the 10-year extension when the headrest deployed. FCA customer service has now denied coverage, despite our initiating multiple phone calls and a detailed demand letter we sent in February 2026 with no response. As [XXX] and [XXX] on fixed Social Security income, we cannot afford an expensive replacement. The dangling head restraint remains an ongoing safety risk in our primary family vehicle. Please investigate this matter, particularly the failure to notify affected owners of the warranty extension that NHTSA relied upon to close PE19-014. The manufacturer’s “remedy” is ineffective if owners are never told it exists! INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA),Read more

In November 2022, I started getting a dashboard "shift to park" on my 2019 Acadia after I had parked and shut the vehicle off. At the time I took it into service to have it looked at/repaired, they told me there would probably be a recall on this issue,as it seemed to be a frequent problem, and I would get a refund for the repair. That recall never happened, and now my car is once again displaying
... the same issue. The dealership will not cover the repair, and I will be responsible, as it is out of their 2 year warranty window. I can't just ignore this issue and keep driving, as with the "shift to park" display, there is a constant alarm that doesn't stop unless the park switch recognizes the car is in park, even when it is shut off. While on/idol, the 3rd row seat (left/drivers side) folded down on my 9 year old niece. Myself, mother, 6 year old daughter, sister in law, brother and 12 year old niece all witnessed this happen as we were loading into the car after playing at the park (I had the car turned on). Her head got pinned to the headrest of the 2nd row captain seat. The buttons did not respond as they usually/supposed t
... o do to stop and reverse the folding action. The seat continued to press against her skull. After realizing the buttons in the cargo were not going to work, I had an idea and ran to the 2nd row door and pressed the captain seat button to slide it forward (my child was in that captain seat in a rear facing carseat). It wasn't able to move much but enough that the trapped girl was able to force her head out and her mom pulled her the rest of the way out to release her. It was very traumatizing. That 3rd row seat remained in that almost closed position as the buttons still would not function so we drove home. Eventually I tried the buttons in the again and it unfolded the seat. I have tested these buttons over and over since then to figure out what happened. Every time I test them, each button allows me to either release it and the action stops, or press the button again and the action stops. I do not know why it wouldn't respond to doing that same that day. Prior to that incident, sometime between getting the car on 7/31 and mid-November, I had an empty unbuckled backless booster car seat in the third row right/passenger side as a backup for daughters friends to use, and without remembering it was there, I pressed the button to fold down the 3rd row right side chair and it kept stopping and auto reversing, several times before I realized oh ya, the booster is back there. So when the innocent happened with my niece, it came as a big surprise because that booster weighs nothing compared to a 9 year old. It has not happened againRead more
