Cascading air conditioning system failures in my 2018 Subaru Forester. The failures occurred in the following sequence: 1. Expansion Valve Failure – The AC expansion valve failed, requiring repair. 2. Evaporator Core Failure – Shortly thereafter, the evaporator core failed, causing complete loss of air conditioning and requiring a second very costly repair. 3. AC Compressor Failure – Then th
... e compressor started failing as well, resulting in a third major costly repair. Based on my research, a significant number of 2018 Subaru Forester owners have reported the same pattern of cascading AC component failures, suggesting a systemic design or manufacturing defect in the AC system. Subaru of America, to my knowledge, has not issued a recall.Read more

My 2025 GMC Yukon's infotainment system has repeatedly failed. The huge center screen frequently goes black upon starting the car. Once the screen goes black you are unable to control literally dozens of the vehicle's features including but not limited to climate controls, safety features, heated/ventilated seats, gps, lane assist, the radio, no backup camera, etc. When the screen blacks out, the
... vehicle automatically turns the heat on to 76 degrees every single time and blows hot air from the front windshield defogger vents only. The list of features controlled by the touch screen is quite enormous. I have taken the vehicle to the GMC dealer four (4) times for software updates, none of which solved the problem and they have admitted they have no fix for this issue. The issue is widespread, as I have encountered literally hundreds of owners online like myself dealing with the very same problem; brand new cars that just do not work. Without any warning, on March 15, 2026, I turned out of a parking lot onto a major road and I noticed it wouldn't accelerate above 20 mph, so I pulled off the road. I tried again and my vehicle still failed to accelerate. And on my third attempt, my vehicle decelerated and was about to completely stall. I managed to turn into a parking lot and avoid getting rear ended. However, this could have
... been a fatal event. While I was trying to accelerate, the vehicle was quiet as if the car wasn't even running. After shutting the engine off for awhile, I was able to drive the vehicle with caution. I reached out to two Genesis service departments regarding my vehicle failing to accelerate while driving and neither had heard any previous complaints, and the earliest appointment would be mid April. There was no sense of urgency, as I'm left to hesitantly drive my vehicle at the risk of it happening again and getting rear ended. And when my vehicle does go in for service, I'm weary whether the problem will be resolved properly since the service department is unaware of the issue. In addition, I have had this vehicle for only 4 months and 1 month into it, I already took it to service because of fuel odor that had not gone away since the day 1. But the service department didn't find anything wrong with the vehicle, so nothing was done to it. I also swung by another Genesis service department, in case they happened to be familiar with the cause of the odor. My vehicle still has the fuel odor and now I'm wondering if that is related to my vehicle not accelerating. I am seeking your assistance to ensure that the problems with my vehicle are confidently resolved. And I wouldn't want this to happen to anyone else. I am thankful, I was driving and not my teen.Read more

On March 18, 2026, while driving a leased Honda Prologue in rush-hour traffic on a major highway in [XXX] , the vehicle suddenly shut down and would not restart for approximately two minutes. The incident occurred without any warning messages, alerts, or notifications and created an extremely dangerous situation. At the time of the incident, traffic had been stop-and-go for approximately three mi
... les. Without warning, the radio turned off, all dashboard lights went dark, and the vehicle lost power and stopped moving. I attempted to restart the car multiple times, but it would not turn on. On approximately the seventh or eighth attempt, the vehicle finally restarted, allowing me to safely pull off the highway. Fortunately, the slow-moving traffic prevented a rear-end collision with vehicles behind me. I began leasing the vehicle this past summer, and at the time of the incident it had fewer than 2,600 miles on the odometer. I took the vehicle to the dealership that same evening. During their inspection, the dealership identified several “Lost Communication” diagnostic trouble codes in the vehicle logs. The most significant code identified was B1B42 – Start/Stop Switch Not Plausible. The service team also noted that the manufacturer’s troubleshooting checklist references codes B1B15 or B1B15 2A; however, those codes were not present. Based on the findings, the dealership replaced the S38 Start/Stop Switch. I resumed driving the vehicle two days ago and have not yet experienced any further issues. However, I remain extremely concerned about the seriousness of this failure. If this same issue were to occur again at higher speeds or in faster-moving traffic, it could result in a severe accident. The complete loss of power without warning represents a significant safety risk and warrants further review. In August 2025, I had taken the van for a drive to charge things up, got home, put it in park and parked it in my driveway, which has a mild slope. ~ 30 seconds - 1 minute after me leaving the vehicle, it rolled backward out of the driveway, across a busy street and 1/4 into my neighbors yard. I was mortified, there is usually a lot of people walking their dogs, kids etc...and cars going by. The v
... ehicle was stopped by the curb vs the 2nd back tire (Thie first one cleared the curb and was in my neighbors lawn. It ALMOST hit a BIG tree. I pulled it back into the driveway & checked it again, worried I may have somehow not put it in park, but it did it over and over again. Shortly after I put it in park and turned it off it "released" and started rolling spontaneously. I could have run over my neighbors; people, babies...pets, me... I piled logs and big rocks behind all 4 tires (put it park with the parking brake on of course,) & called the Mercedes dealer & made an appointment. I did not drive it again until it was "repaired," except for the fact that nothing showed up on the scanned as wrong. They kept if for 3 weeks but "it never did it again," & they "never found anything." They charged up the battery and said maybe it was because the battery got too low and that the chassis battery charges the transmission battery and maybe the transmission battery was low. They mentioned that they had had one other person come with the same problem. The local Winnebago dealership mentioned that they had had someone come in with the same complaint. With the vehicle unchanged after this potentially catastrophic event, how can I trust it? What happens if I am in it and it is parked on a slope and it does it again? The service 4WD light is on in this vehicle. Upon taking it to my personal mechanic to fix the issue, it was determined that the code was associated with a recall. Upon calling again, I was advised that I would have to incur the entire cost to have the issue fixed and if it was determined that my vehicle would be included in the recall at a later date i could submit receipts and be reimbursed. The code is PTU failure related and could cause possible loss of power to the wheels. This is a major safety concern as well as a huge expense to fix. I am writing to file a formal safety complaint against Tesla, Inc. regarding a software update failure that has disabled legally required vehicle functions on my Tesla for over two years. For more than 26 months, my vehicle has been unable to complete a software update through Tesla's over-the-air (OTA) update system. As a direct result of this failure, my vehicle's display screen does not functi
... on for the first several minutes of every drive. This prevents me from accessing my speedometer, turn signals, and other legally required safety features, rendering the vehicle non-compliant with federal and state motor vehicle safety standards during this period. I have made multiple documented attempts to resolve this issue through Tesla's customer support channels over the course of these two years. Despite these efforts, the issue remains unresolved. Tesla has now informed me that resolving the defect will require a paid service visit — despite the fact that the failure originates within Tesla's own software update infrastructure, not from any action or negligence on my part. I am requesting that NHTSA investigate this matter on the following grounds: 1. The software defect disables legally required vehicle safety functions, including the speedometer, turn signals, hazard lights, and more. 2. The defect has persisted for over two years despite repeated good-faith efforts by the vehicle owner to resolve it through the manufacturer. 3. Tesla's OTA update system is manufacturer-controlled infrastructure. A failure within that system constitutes a manufacturer defect, not a maintenance issue. 4. Tesla is requiring payment from the consumer to correct a defect in their own systems. I believe this issue may not be isolated to my vehicle and could affect other Tesla owners experiencing similar OTA update failures. I respectfully request that NHTSA investigate whether a broader safety defect exists and whether a recall or corrective action is warranted.Read more

Due to the design of the grill located at the front end of the 2025 toyota grand highlander the vehicle poses vulnerability for multiple debris items to fly up into exposed air conditioning condensor. The damage caused by the debris is not covered by the warranty because it is considered an outside occurrence. However the design creates the vulnerability for the ac to become damaged. It is also s
... itting below the radiator posing an additonal risk for damage and should have additonal protection to help prevent damage. I am currently having to dispute the refusal to cover the damage to my own ac condensor done by a rock flying into the grill space causing damage and losing freon, because they state it is an outside occurrence regardless of the fact that the design is faulty. There are multiple people who have experienced this happening to their vehicles some not even at the 2000 mile mark and are responsible for the financial cost. Upon doing research the 2026 toyota grand highlander model was created with an even lower setting bumper that provides better coverage and protection for the air conditioning condensor and all other important mechanical components. Suggesting that Toyota is aware of the issue if they created an additonal part to help protect that area from damage. I am literally at risk each time I drive my car just for the fact that rocks are on every road way as well as other debris. I have called the customer relations department with no resolution given only to be told I am responsible for the cost of the repair. I believe this is something that needs to be addressed as well as fixed for the sake of the buyer and the already large cost of owning and maintaining a vehicle. If it happens more than a few times its not bad luck, it's a flaw in the creation of the product and consumers should not have to be held financially responsible for someone else mistake, whether individual or corporate entity. The high-voltage battery on my 2022 Nissan Leaf is subject to Recall 25V655. The recall notice states that the battery cells can develop excessive lithium deposits, which increases electrical resistance and can cause rapid battery heating during Level 3 fast charging. Nissan instructs owners not to use Level 3 charging at all until a “remedy” is available. The issue is that the remedy describe
... d by Nissan is not an actual repair of the battery defect. Nissan states it will install software that monitors for “state-of-charge fluctuation” and, if detected, will prevent the vehicle from restarting or recharging in order to avoid a thermal incident. This means the defect inside the battery cells is still present, and the car may disable itself if the defect begins to appear. The recall materials also state there is no warning before overheating occurs. This creates multiple safety concerns: • The underlying battery defect remains uncorrected. • The vehicle can become immobilized (unable to restart or recharge) if the software detects the condition. • Loss of Level 3 charging capability affects the ability to travel safely or plan charging when needed. • A battery fire risk exists during Level 3 charging if the defect is not detected in time. Nissan’s documents state that the software is meant only to prevent the “progression” of a thermal incident, not to repair the defective battery. I am concerned that my vehicle contains a known hazardous defect that has not been physically repaired and that the software-only response is inadequate to ensure safety. I am filing this complaint so NHTSA is aware that the remedy being offered does not address the internal battery defect itself and may leave owners exposed to continued safety and reliability risks.Read more

VEHICLE INFORMATION Year: 2018 Make: Honda Model: HR-V VIN: [XXX] Mileage at Time of Incident: 96,993 Incident Date: 03/10/2026 I am filing this complaint regarding the premature and complete failure of the CVT (Continuously Variable Transmission) on my 2018 Honda HR-V with approximately 96,993 miles. DESCRIPTION OF FAILURE: The transmission on my 2018 Honda HR-V failed without prior warning.

I was told the best thing to do was report it here. Our son was driving his 2019 Honda Accord and suddenly everything on his dashboard lit up. Was told it is a blown head gasket. We were also told that the shop has had over 81 Accords in with the same issue. They said Honda is trying to sweep it under the rug rather making a recall. We were told this is what they see: The head gasket mate
... rial between cylinders is very tiny, any minor disturbance will cause coolant to leak into the cylinder. Usually failures are happening between cylinders 2&3 where the material is the thinnest. 2) Honda is really trying to get a lot of power out of this small engine with the inclusion of the turbocharger. Higher octane fuels are needed to prevent premature detonation / knock, especially with forced induction (turbo charger). You can still run the car using 87 octane, but the chances of problems like a blown head gasket happening increase. 3) The head bolts that keep the engine head to the engine block are not strong enough. These bolts are torque to yield at around 50 ft lbs (if that). There have been instances where these bolts don't have enough clamping force to withstand the pre-detonation. This force causes the head to jump and the bolts to stretch. The heat from the combustion in the cylinder is no longer contained, which warps and damages the head gasket over time, leading to its failure. It was suggested to use ARP studs which we were told are a higher quality than the OEM Honda head bolts and have a stronger clamping force. Our son ended up stranded 3 states away with this issue that we then learn is VERY common yet there are no recalls.Read more

Transmission was supposedly rebuilt back July 26, 3021. Now transmission has had a new dual clutch replaced but other concerns are still there jerkiness on accelerations. In 2021 the transmission was not fully rebuild as per recall. Only clutch assembly was done and not the back end. Which now could be the problem. Second, colonial Ford sated transmission rebuilt has lifetime warranty which a lie.
... Colonial Ford doesn’t offer lifetime. The service advisors lie about this all the time. Now the car jerks on acceleration after clutch replacement. Dealer and Ford still take no responsibility over the product! The car has 145,000 miles still under the 150,000 mile warranty isssue by you!Read more

Water began accumulating in the front footwell area of the vehicle, resulting in a soaked carpet and persistent musty odor. The condition developed during normal operation of the air conditioning system, without any external water intrusion (no flooding or open windows). The issue appears to be caused by a failure of the A/C condensate drain system, which is not properly draining water outside th
... e vehicle. Instead, condensation is leaking into the cabin. This creates a risk of mold growth and potential damage to electrical components located beneath the floor, including wiring harnesses and control modules. The problem has been reproducible, especially when the air conditioning is in use. There were no warning lights or alerts prior to noticing the issue. Research indicates that BMW has identified this exact defect in Service Information Bulletin SIB 64 04 16, which states that the condensate drain hose may be improperly installed, kinked, blocked, or damaged during installation, leading to water ingress into the footwell area. This suggests the issue is due to a known design or manufacturing defect rather than normal wear and tear. This condition poses a safety concern due to possible electrical malfunction and reduced vehicle reliability. The affected components are available for inspection upon request.Read more

I am filing a complaint regarding my 2022 Acura MDX Type S with approximately 70,700 miles. The vehicle developed a suspension system warning, and upon inspection by an Acura dealership, I was informed that the suspension stroke sensor and floor wiring harness are corroded and require replacement. I was quoted approximately $12,000 for the repair. This failure is extremely concerning because the v
... ehicle is only a few years old and this type of corrosion affecting a major wiring harness should not occur under normal operating conditions. The affected components are part of the suspension system, which directly impacts vehicle handling and safety. Due to the failure, the suspension system may not function properly, potentially affecting ride height, stability, and overall control of the vehicle. This raises safety concerns, especially at highway speeds or during emergency maneuvers. There was no prior warning of water intrusion or external damage that would reasonably explain this level of corrosion. This appears to be a premature failure of a critical electrical component. Acura has only offered minimal goodwill assistance, leaving the majority of this extremely costly repair as the responsibility of the owner. I am concerned that this may be a broader issue affecting other vehicles and that it could pose a safety risk if not addressed. I am requesting that this issue be investigated. Thank you.Read more

Component that malfunctioned - "Keep you eye on the road" On a 300 mile trip the alarm went off numerous times even though my eyes were on the road and at times it would disengage the Cruise Control. The frame of the back window is cracking allowing water to come in through the back glass. This is causing the headliner and other components such as air bags and other electrical components to get wet and damaged. This could cause risk of fire or failure of safety components within the vehicle such as air bags, cameras and other sensors. With water intrusion comes the high risk for mold to devel

A GMC dealer has inspected the glass and said it was a faulty seal but since it's not in the 3 year warranty window, it's not covered. Independent auto glass companies state they have seen this issue many times and the glass seal is faulty. GMC should do the right thing and fix this issue. This devalues the vehicle and the GMC brand and poses serious risks to driver. I have created a case with GMC and submitted photos, videos and dealership estimates for proof.Read more
