I am the original owner of a 2020 Kia Telluride. The white paint on the car is coming off in huge pieces all over the front. The primer is still attached to the metal. Huge chunks are coming off as we drive it. We have spoke to the dealership and Kia corporate as well and they do not care because it is out of the 3 year warranty. We had a paint shop inspect it and get an estimate for it to be fi

A recall repair was completed in 2023 related to an overheating warning. Recently, the same issue has returned. The engine cooling fan runs at a very high speed for several minutes and then shuts off, even when the vehicle is cold or operating under normal conditions. An overheating warning message appeared briefly while driving, but the temperature gauge remained within the normal range. This su
... ggests the engine may not actually be overheating and that the issue may be related to a faulty sensor, electrical system, or control module. The problem has not yet been fully diagnosed by a dealer due to diagnostic costs, but the symptoms are consistent with the original issue that led to the recall repair. This condition could pose a safety risk because the driver may receive inaccurate overheating warnings or may not be alerted if a real overheating condition occurs. The issue is intermittent and has occurred both while driving and when the vehicle is stationary. Attempts were made to contact the manufacturer to report this as a repeat issue after a recall repair, but no resolution was provided and a formal case was not successfully established.Read more

I am reporting an unreasonable delay in the remedy for NHTSA Recall 25V-668 (Toyota Recall 25TA12). My 2025 Toyota Sienna has been subject to Interim Notice 25TB12 for approximately 170 days. Despite this recall being acti
... ve since October 2025, Toyota has failed to provide the necessary replacement seat rails or a firm repair date. This defect involves structural weld failures that pose a high risk of seat detachment in a collision. By rendering the primary passenger area of this minivan unusable for nearly 6 months, the manufacturer has caused a substantial loss of utility and safety. I am requesting NHTSA investigate the lack of remedy availability and Toyota’s failure to support owners who cannot safely use the vehicle for its intended multi-passenger purpose.Read more

When driving on the highway going 65mph the car slipped in neutral and error showed on dashboard "transmission malfunction stop." I could steer and break, but not accelerate. Pulling over on side of highway, I was able to put in park and then drive. I pulled off the exit and the car did the same thing within 10 mins. Car was towed to Mercedes dealership in Warwick, RI. I was made aware after
... inspection that there was nothing wrong with the vehicle nor the transmission, but rather there was a needed software update that was causing the issue that had been identified previously. Regular service was due, but no indication whatsoever of the error I saw nor any other error codes. No error showed up for the technicians in inspecting the car. I was further informed, after research, that the software update was being drafted and that it would have to pass NHTSA governance. So I'd like to know if this is accurate? If NHTSA is aware? If the software update is sitting with you for approval? What is Mercedes obligation to share this with drivers noting they were aware of issue? If my [XXX] was driving she could have been killed. The vehicle experienced a failure directly related to a manufacturer recall involving fuel tank expansion. The incident occurred while the vehicle was in use during normal operation on a work trip approximately three hours away from home. This failure presents a serious safety concern due to the involvement of the fuel system, including the risk of fuel leakage, system failure, or potential fire
... hazard. The condition made the vehicle unsafe to operate and required immediate attention. The vehicle was taken to a dealership, where it was inspected and the issue was confirmed to be related to the recall. The dealership determined that the vehicle is a total loss and cannot be repaired. The failure has not been reproduced as the vehicle is no longer operable. The vehicle remains at the dealership and should be available for inspection upon request. UNKNOWN if warning lights or messages appeared prior to the failure. The issue occurred during normal driving conditions. The manufacturer has been notified and a case was opened; however, no resolution or response has been provided to date despite multiple attempts to follow up.Read more

I am reporting a safety-related electrical failure involving the heated steering wheel system in a 2023 Jeep Wrangler. While driving, the heated steering wheel malfunctioned and produced smoke from the steering wheel area. This created an immediate safety concern due to the presence of smoke and the potential for electrical overheating or fire in a critical driver control component. This was not a
... n isolated incident. The same system required a prior repair under warranty in late 2023. Despite that repair, the failure recurred in late 2025 with more severe symptoms, including overheating and damage to electrical components. The vehicle was taken out of service for repair, and the total time out of service related to this issue was approximately 115 days due to parts delays and repair time. The problem was confirmed and repaired by authorized dealerships, including replacement of the steering wheel and related electrical components. There were no meaningful warning indicators prior to the failure. The issue occurred during normal operations. The repeated failure of the same system, combined with smoke and electrical malfunction in the steering wheel, raises concern about a potential defect that could pose a safety risk to drivers and passengers.Read more

Hyundai Tucson Sport 2016 GDI engine. I am the original owner of this truck I have taken care of this truck so much it being my first brand new purchase. No oil leaks NOTHING! A few years passed started to notice frequent oil changes were supposed to be

Barely made it home. Got a tow truck to tow it to Hyundai dealership they told me they will not see it unless I pay the diagnostic fee of $200 and even then they think it’s the engine with everything I described to them. They said since there are no recalls on the Tucson that Hyundai will not do anything for me I would have to pay out of pocket for everything. I am deeply concerned about my safety and the safety of other drivers due to a serious and worsening infotainment system malfunction in my 2022 Honda Accord Hybrid. The system malfunctions spontaneously and repeatedly while I am driving, causing significant distraction and creating a serious risk of accident. Symptoms include the system switching between apps on its own, the radio turning on and
... off without any input, and the screen responding as if phantom touches are occurring. I cannot predict when this will happen and have been constantly distracted while driving. The problem first appeared approximately one year ago and has become significantly worse in recent weeks, occurring more frequently and more severely. The escalating nature of this defect is alarming and I believe it poses an imminent danger to myself, my passengers, and other drivers. This is a known defect. Honda has issued Technical Service Bulletins on this infotainment system and faced class action litigation covering 2020–2022 Honda models with this same system. I am requesting that NHTSA investigate this defect and consider a mandatory recall so that Honda is required to repair this at no cost to affected owners.Read more

The infotainment system in my 2022 Kia K5 began failing on or around early December, 2025, at approximately 44,000 miles and has continued to worsen. The vehicle is currently at approximately 47,000 miles. The screen displays the Kia startup screen repeatedly and does not fully load, flashing, becoming unresponsive, and ultimately causing distraction while driving. Audio may still function, but k
... ey features such as Bluetooth, Apple CarPlay, and auxiliary input do not work. The backup camera will either not function at all or will flash intermittently while reversing. This failure affects safe operation of the vehicle because it limits access to vehicle controls and creates distraction while driving due to the malfunctioning display. It also impacts visibility and usability of integrated systems that are expected to function reliably. The problem has been reproduced consistently and was evaluated by a Kia dealership. The dealership was unable to determine a clear cause for the failure but recommended full replacement of the infotainment system at a cost of approximately $6,700. There were no clear warning messages prior to failure. The issue has persisted despite attempts to reset the system and power cycle the vehicle. This appears to be a premature failure of a critical electronic system that should not occur at this mileage.Read more

I am experiencing repeated and unpredictable software failures in my vehicle that create serious safety concerns. The loss of safety systems while driving, combined with persistent audible warnings, creates a dangerous driving environment by increasing distraction and reducing available safety protections. This is not an isolated incident—it has happened multiple times. Safety Risk: The unexpected disabling of driver assistance and safety systems while the vehicle is in motion presents a significant safety hazard. The accompanying high-pitched warning noise further distracts the driver during critical moments. Together, these issues increase the risk of an accident. Component/System Failure: The turbocharger system failed, specifically the wastegate arm, resulting in loss of boost and reduced engine performance. The failed component should be available for inspection upon request through the servicing dealership. Safety Risk: The failure caused a sudden loss of engine power while driving, placing me and others at risk, especially in traffic situations where
... acceleration is required (such as merging, crossing intersections, or maintaining speed). This created a hazardous driving condition. Confirmation of Issue: The problem was diagnosed and confirmed by an authorized dealership. The diagnosis stated: “turbocharger wastegate arm damaged causing loss of boost and DTC,” which matches known issues described in GM’s Special Coverage. Inspection History: The vehicle has been inspected by the dealership service department. It has not yet been inspected directly by the manufacturer, police, or insurance representatives. Warning Signs Prior to Failure: Yes. The vehicle displayed a check engine light and reduced engine power message prior to the failure. These symptoms began shortly before the vehicle experienced noticeable loss of acceleration and performance. Additional Information: This failure is consistent with a known issue acknowledged by General Motors under Special Coverage for similar vehicles experiencing turbocharger-related defects. Despite having the same failure and symptoms, my vehicle was excluded based on lot number, not condition or diagnosis.Read more

My car started shutting down while driving. After the engine started seizing the key wouldn't turn in my ignition. Took car to dealership immediately on 10, 2026. I've requested current diagnostic report several times
... in writing and verbally with codes for this current service, with non compliance. My car is still at the dealership needing work and I need current diagnostic report before I pay for repairs. I am attaching a dealership diagnostic e-Report for my 2013 Kia Optima from 2024. While the report lists “No Current DTC” for several systems, it also documents multiple “Communication Fail” errors across critical modules, including Smart Key, Head Lamp, Air Conditioner, Brake, and other systems. These communication failures indicate a broader electrical or network issue within the vehicle, not simply a failed battery. The report also shows that certain diagnostic scans were not properly performed. Additionally, there are historical fault codes related to TPMS and weight classification systems, further supporting that the vehicle has experienced ongoing electrical issues. Despite these findings, the dealership attributed the vehicle failure solely to a bad battery and did not provide a current battery diagnostic test. They also failed to address an open recall related to electrical/fuse issues prior to this failure. Based on this past diagnostic report, I believe the root cause of my vehicle’s failure is an unresolved electrical defect, potentially related to the outstanding recall, rather than normal battery wear. This has resulted in my vehicle becoming inoperable and poses a serious safety concern.Read more

2025 Chevrolet Blazer EV AWD purchased 5/23/25 (under 3,100 miles) The vehicle's Telematics Control Module (TCM) has failed, disabling the built-in navigation system, OnStar services, remote start, and My Chevrolet app connectivity. The dealership confirmed the TCM needs replacement and the part is on backorder with no ETA. This is a known issue affecting this model. I am reporting this as a saf
... ety defect for the following reasons: The 2025 Blazer EV does not support Apple CarPlay or Android Auto. With the built-in navigation disabled, the only way to navigate is by using a phone, which requires visual attention to a separate small screen while driving. Under Pennsylvania's hands-free law (75 Pa.C.S.A. § 3316.1), use of handheld devices while driving is a primary offense, making phone-based navigation both unsafe and legally problematic. On at least one occasion, the navigation system appeared to be functioning normally at the start of a trip but was providing materially inaccurate directions mid-route (e.g., instructing me to turn onto a road I had already been traveling on for miles). A system that displays as operational while delivering incorrect routing data is dangerous because the driver has no way to assess its reliability before departing. GM's EV Concierge characterized the loss of navigation as a non-safety feature issue and suggested using a phone holder. This response is inadequate given the vehicle's lack of CarPlay integration, which makes the built-in navigation the only safe, integrated means of route guidance. This vehicle has also required three prior unrelated warranty repairs in 10 months: a recall for a faulty rear parking brake harness (9/25), a 12V battery failure requiring flatbed towing (11/25), and a faulty tire pressure monitor (12/25). The pattern of recurring failures across unrelated systems at minimal mileage indicates a systemic quality issue with this vehicle.Read more

I am writing to report a serious safety concern involving my 2024 Chevrolet Silverado that resulted in a child being locked inside the vehicle with the key fob present inside the car. On March 25 2026, after parking and turning off the vehicle, I exited the driver’s seat with my key fob inside my purse, which remained in the front of the vehicle. I then walked around to remove my daughter from
... the back seat. During that time, she climbed into the front seat and pressed the interior door lock button, locking all doors. My daughter has autism and is not able to follow instructions or unlock the vehicle on command. As a result, she was locked inside the vehicle with the key fob present inside, and I was unable to gain access from the outside. The vehicle did not prevent the lock from engaging, nor did it allow me to open the door despite the key being inside the vehicle. This situation required emergency assistance (911) to safely remove her from the vehicle. I understand that many vehicles have “lockout prevention” features; however, in this case, the system did not account for a vulnerable occupant inside the vehicle. Allowing the vehicle to be locked from the interior while the key fob is inside—and without any external override—creates a significant child safety risk. I am requesting that Chevrolet review this design behavior as a potential safety issue, specifically: - Why the vehicle allows interior locking when a key fob is detected inside - Why there is no external override to unlock the vehicle when the key is present - Whether a software update or safety feature enhancement can address this risk This is not just a convenience issue—it is a safety hazard that can lead to dangerous outcomes, especially for children or individuals with disabilities. Please confirm receipt of this concern and advise whether this issue has been previously identified or is under review.Read more

The panoramic sunroof gets stuck in the open position and also the motor makes a clicking sound. It is not opened all the way hardly at all, maybe 5 times a year if that. The problem is all over the internet and the dealerships routinely denies any knowledge of it and wants anywhere from 7k to more to fix the entire roof. This is very unprofessional and should not be swept under the rug. Repor

Power Transfer Unit stopped working. Brakes locked up and car would not move. Is not drive-able at this time. Currently at a dealership awaiting part, of which there is NO ETA on. I was just entering a roundabout when the part failed, and I could have been hit as the vehicle fully stopped and could not be moved. There was a grinding noise and no warning prior of this part failing. The problem was

1. My safety system/camera system is no longer working properly. The screen displays either a black/blank screen, an inverted (upside down) image, or an image that appears to be zoomed in to a very small viewing area. The alerts (beeping) does not function consistently when backing up. The horn does not always work. At times the entire electrical display including the sound system will reset witho

Safety is at risk because the video system is in place as a secondary alert system to prevent accident or injury. The horn is a necessary item. If the electrical system should reset during operation this could cause an accident. The rear hatch could cause injury if it does close. 3. These issues have been witnessed by other people, but not in a dealership or service center because these places charge outrageous fees. 4. No inspection done by manufacturer/police/insurance. At time of state regulated inspection all aspects pertinent to that were functional 5.
