2022 Rivian R1T, VIN [XXX] . On March 14, 2026, my service appointment at El Segundo Rivian service center [XXX] ) was deleted from the Rivian app along with all service requests and a timestamped customer message thread documenting my visit that day. Deleted items included active recall remedies FSAM-1795 (seat belt retractor bolts improperly torqued, NHTSA notification mailed ~March 9, 2026) an

Both recalls were issued within 3 weeks of the deletion. This was the third of three consecutive failed appointments — all failed because El Segundo could not provide a promised loaner vehicle (January 21, February 10, March 14, 2026). The only surviving record [XXX] ) was altered by Rivian Service — they inserted the statement that ride height appears to be as expected and does not show variance before any inspection, directly contradicting my written report and a video I submitted through the Rivian app on January 2, 2026 at 9:34am documenting the defect. Rivian also added an all-caps requirement to replicate the issue on a test drive prior to intake despite existing written and video documentation. Rivian has a federal obligation to complete FSAM-1795 and FSAM-1794. Neither has been reinstated as of April 1, 2026. Formal complaint delivered to Rivian CEO via USPS Certified Mail March 18, 2026. Only response was a generic form letter (Inquiry #xxx) stating matter appears resolved. See also separately filed NHTSA complaint re suspension defect, same VIN. 4 out of 5 times driving the car the infotainment screen with get random touches guiding you to unknown menus or system setting or maybe just to a different radio station. Where I think this becomes a legitimate safety concern (asside from changing vehicle settings like security) is when trying to use maps function the random touches will direct the map away from where you are or where your next t
... urn might be. So you basically need to keep hitting re-center just to get you back on course. However most of the time there is an uncomanded touch that is stuck on the screen or possibly a number of touches that prevent a real finger print touch to be sensed ultimately preventing you from re-centering. This causes extended moments where attention is taken away from the road or late/last minute route correction causing unsafe road/driving conditions. The manufacturer only recommends doing a system restart of your infotainment screen which does nothing or complete replacement of the screen which in many cases including my own is not covered by warranty.... the alternative is to disconnect your phone from android auto or apple car play and use it in your hand which again causes distracted driving. If this was something as simple as a volume knob or temperature control not responding I wouldn't even be here but since it always touching different placed and navigating you places you dont want to be i see it as a clear safety issue that should be corrected. I slowly put my foot on the brake and suddenly heard a noise under the vehicle. It sounded like loud grinding. When I pushed on the gas I was suddenly accelerating forward very quickly without my control. This was maybe one or two seconds after I heard the noise.

The vehicle was continuing to accelerate even after I tried to put it in park. I was worried if the car in front of me moved, that I would keep going. I even pressed the emergency brake button twice and nothing worked. Nothing was working at all. I called the police. They told me to try turning off the car. I pressed the button to turn the car off, but it took several pushes to work. I could hear that the rpms lowered extremely slowly. Once the police came they didn't see anything in the road at all. I am writing because my local Hyundai dealer is failing to make the repair in a timely manner consistent with what my recall notice stated, and is requiring that I make TWO trips to Hyundai in order to get this NECESSARY recall work completed. Meanwhile, if the camera fails, it could put me and others at risk. My service representative, [XXX] , and his service manager showed ZERO concern about my
... time or possible dangers if the rear camera fails, stating that it's Hyundai corporate that made the policy that customers must have their car inspected and photos taken of the rear camera harness assembly during this first visit, BEFORE ordering the parts under my vehicle's specific VIN number. Once they do this and submit it to Hyundai corporate, they say, the parts will be ordered and I will be contacted AGAIN to make yet ANOTHER appointment to have the parts installed. My letter from Hyundai says NOTHING about having to make two visits. It says to schedule an appointment, and the recall work will be done. I actually tried to call the 855 Hyundai number to schedule the work, and that operator said nothing about two visits; she was unable to book the appointment she'd given me, so directed me to the local Hyundai dealer, who also said NOTHING about two appointments being needed to have this recall work done. I am concerned, because i am scheduled to drive out of town in a high pedestrian and vehicle traffic area in two weeks and was doing my due diligence to get the recall work done before the trip. I feel that the two appointments are unnecessary when EVERY 2025 Hyundai Santa Fe with this feature is equipped with the same rear camera and rear camera harness assembly, such that there's no reason to have to FURTHER register it with my VIN, that this could all be done in one appointment. Requiring customers to come for 2 appointments--especially without telling them anywhere in the letter or during 2 phone calls--simply increases risk of injuries if the camera fails. I own a 2003 Toyota Tundra (VIN: [XXX] ) that was subject to Toyota’s frame corrosion recall (Limited Service Campaign A0F). The vehicle was inspected by Toyota in January 2011 and confirmed to have no perforation. In March 2012, Toyota applied a corrosion-resistant compound as part of the recall remedy. Despite this, by May 2013, a structural component (spare tire carrier) failed due to sever
... e rust and detached from the vehicle. This occurred shortly after Toyota performed the corrosion protection treatment. This raises concerns that the recall remedy was not effective in preventing continued structural deterioration. Frame corrosion can impact vehicle safety, especially if critical components weaken or detach. Toyota has declined any further review or assistance, stating the program has expired, despite evidence that the corrosion continued after their repair. I am the original owner of a 2023 Jeep Wrangler Sport with approximately 16,240 miles. The vehicle is completely unmodified. I am reporting a serious safety defect involving the steering and suspension system. When driving at highway speeds (50 mph and above), the vehicle experiences a violent front‑end oscillation whenever it encounters a bump, uneven pavement, or bridge expansion joints. The s
... teering wheel rapidly shakes left to right, and the entire vehicle shudders for 5 to 10 seconds. The oscillation does not stop until I slow the vehicle down to below 45 mph. This issue is reproducible and occurs frequently on rough or uneven road surfaces at highway speeds. During these events, I temporarily lose the ability to maintain proper steering control, creating a dangerous situation for myself, my passengers, and other drivers. This condition is commonly referred to as “Death Wobble,” and it makes the vehicle unsafe to operate. Given the severity of this defect and the safety risk it poses, I am requesting that NHTSA investigate this issue and that Jeep/Stellantis address the defect under the New Vehicle Limited Warranty (3‑year/36,000‑mile basic warranty). The front‑end suspension components—including the track bar, steering damper, ball joints, and related parts—should be inspected and repaired as necessary to eliminate this hazardous oscillation.Read more

The panoramic sunroof on my 2023 GMC Yukon Denali Ultimate XL spontaneously exploded while driving westbound on I-20 on March 13. There was no impact from road debris, no vehicles ahead, no overpass, and no external cause. The failure sounded like an explosion or gunshot. Tempered glass shattered with the sunroof in the closed position, showering the interior with glass fragments. Five occupants w
... ere in the vehicle at the time. No injuries occurred, but the sudden explosive noise and shower of glass created an extreme distraction and safety hazard while operating the vehicle at highway speed. The incident could easily have caused a loss of vehicle control on the interstate. The component that failed is the factory-installed panoramic power sunroof glass, standard equipment on the Denali Ultimate trim. The vehicle was taken to Lugoff Chevrolet for inspection. The dealer confirmed the glass failure and submitted a request to GM for warranty or goodwill coverage. GM offered to cover only 10% of the repair cost, characterizing the failure as an out-of-warranty wear issue despite the spontaneous nature of the defect. There were no warning signs, cracks, chips, warning lamps, or any indication of a problem prior to the failure. The sunroof was closed at the time. The vehicle has been dealer-maintained with no modifications to the sunroof or roof structure. This is a known industry-wide defect. NHTSA has previously investigated spontaneous sunroof shattering and documented over 4,000 complaints. GM has acknowledged awareness of this issue in other GM vehicles and previously issued corrective actions for sunroof glass on the Buick Regal and Chevrolet Cruze. Other GMC Yukon owners have filed NHTSA complaints describing identical failures. The failed sunroof glass and vehicle are available for inspection upon request.Read more
