What component or system failed or malfunctioned, and is it available for inspection upon request? Two components have failed: the infotainment/Apple CarPlay system, including the USB-C connectivity port, and the front collision detection/crash sensor system. The entire A/V unit was replaced on August 1, 2025, yet the infotainment defect persisted. Both systems are currently installed in my vehicl
... e and are available for inspection upon request. How was your safety or the safety of others put at risk? The front collision detection system malfunction directly compromises my ability to rely on the vehicle's built-in safety features to prevent or mitigate crashes. Additionally, the infotainment screen has frozen and reset while the vehicle was in motion. I have also been locked out of my vehicle twice while the engine was running, which created a dangerous situation. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. On July 25, 2025, a technician at Georgetown Infiniti attempted and failed to connect their own phone to the CarPlay system, confirming the defect was in the vehicle and not my personal device. The collision system malfunction has been documented across multiple service visits at both Georgetown Infiniti and Grubbs Infiniti. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes. The vehicle has been inspected and serviced by authorized Infiniti dealerships, specifically Grubbs Infiniti and Georgetown Infiniti, both of which are manufacturer-authorized service centers. Most recently addressed 2/4/20226. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. The infotainment screen displayed warning messages, froze, and reset while driving. The front collision system displayed malfunction warnings on the dashboard.Read more

The sunroof shattered while I was driving. There were no other vehicles around me or on the street. I heard a loud bang and glass rained down on me. The glass is pitched upward like it exploded from the inside. There is no other damage to my car. I had no warning other than the loud bang. When I am not out, my car is in a garage it never sits outside.

I am submitting this complaint to report serious safety, usability, and infrastructure-related issues concerning my Hyundai NEXO hydrogen fuel cell vehicle. The vehicle has become unsafe and practically unusable due to multiple technical problems, lack of service support, and the widespread unavailability of hydrogen fueling infrastructure in the Bay Area of California. These issues collectively p
... resent safety concerns not only for me as the driver but potentially for other Hyundai NEXO owners who may be experiencing similar problems. I purchased the Hyundai NEXO with the expectation that it would provide reliable transportation supported by Hyundai’s service network and the hydrogen fueling infrastructure promoted alongside the vehicle. As a consumer, I was attracted to the NEXO because of its innovative fuel cell technology, environmental benefits, and Hyundai’s commitment to zero-emission vehicles. However, my ownership experience has been extremely difficult and concerning due to both vehicle performance defects and the lack of support required to maintain and operate the vehicle safely. One of the most concerning issues with the vehicle is that the regenerative braking system does not appear to function properly. Regenerative braking is an important component of the NEXO’s efficiency and safety systems. When it does not operate correctly, the vehicle does not slow down in the expected way, and the energy recovery system that is supposed to recharge the vehicle’s systems during deceleration does not function as intended. This raises concerns about both vehicle performance and braking predictability. Another significant issue is that the vehicle’s speed appears to be limited and does not go beyond approximately 60 miles per hour. This limitation occurs even under normal driving conditions where higher speeds are required, such as highway travel. Being unable to exceed approximately 60 mph presents a safetyRead more

Incident 1 (Loss of Propulsion in Traffic) - 12/16/25 The vehicle failed to restart immediately after the auto start-stop system attempted to re-engage the engine. Upon attempting to accelerate, the engine did not restart, resulting in a complete loss of propulsion while in active traffic. The vehicle became temporarily inoperable and unresponsive, creating a hazardous condition with elevated risk
... of rear-end collision and inability to safely clear traffic or intersections. Incident 2 (Failure to Start – Reliability Concern) - 4/1/26 On a separate occasion, the vehicle failed to start entirely upon initial startup (first use of the day). The engine did not turn over as expected, rendering the vehicle inoperable. This incident raises significant concerns about system reliability and dependability, particularly given its occurrence independent of the auto start-stop event. Safety Concern: The first incident demonstrates a critical safety defect involving unexpected loss of propulsion in live traffic conditions. The second incident, while occurring at startup, indicates a broader pattern of failure affecting the vehicle’s ability to reliably start and operate. Together, these incidents suggest a potential defect in the starting/ignition or auto start-stop system that may compromise safe vehicle operation and increase the risk of crash or stranding.Read more

The 12V battery in my 2025 Hyundai Ioniq 5 failed under normal use, causing repeated loss of vehicle function. Voltage measured ~12.3V at rest, dropping to ~12.0V when opening the door, and collapsing to ~11V then ~9V under light load (headlights), resulting in system shutdown. After a failed start attempt, the vehicle experienced a brownout followed by a complete loss of 12V power for approximat
... ely 5 minutes while the battery was replaced. After installing a known-good replacement battery, the vehicle powered up and the instrument cluster displayed “The vehicle is On, Ready” without pressing the start button or providing any operator input. Due to safety concerns, I did not attempt to move the vehicle. Concern: After a complete loss of control power, the vehicle appears capable of entering or reporting a Ready state without explicit operator action. A vehicle should not present or enter a drive-ready state following a power interruption without a new, deliberate user command. This behavior is unpredictable and could present a safety risk if the system were to enable propulsion or mislead the operator about the vehicle’s state. At minimum, it indicates improper state recovery after a power failure.Read more

I am reporting a safety concern regarding my 2023 Mercedes-Benz EQB 300 (VIN: [XXX] ), currently subject to NHTSA Recall 26V073 involving the high-voltage battery. According to NHTSA records and the manufacturer’s recall notice, this vehicle has an urgent safety recall due to a risk of fire caused by a potential internal short circuit in the high-voltage battery. The defect may result in a vehi
... cle fire while driving or parked. The recall was issued around February 9, 2026, and remains unrepaired with no remedy available. The manufacturer has indicated a permanent repair may not be available until approximately December 2026. I have been instructed to limit charging to 80% and to park the vehicle outside due to fire risk, yet I am still expected to operate the vehicle and transport my family. Additionally, I received third-party notification advising that my vehicle should not use DC fast charging due to this safety risk, significantly limiting normal use. Mercedes-Benz USA offered a monetary payment conditioned on waiving all current and future legal rights while the vehicle remains unrepaired. I have not been provided a guaranteed loaner or replacement vehicle, and the dealership has requested return of the loaner. Due to these limitations, I am incurring and expect additional costs and loss of normal vehicle use, including reduced access to charging benefits and potential fuel or charging expenses. This situation requires continued use of a vehicle with a known fire risk, no remedy, and restricted functionality without reliable alternative transportation. I request this be documented as a safety concern involving the defect, delayed remedy, and limitations on safe and normal vehicle operation. The factory audio/head unit system produces extremely loud, random static/white noise (described as explosive TV snow or screeching) that blasts through all speakers even when the stereo is completely turned off, muted, or not in use. The volume controls and power button have no effect. The only ways to stop it are to pull the audio fuse or turn the entire vehicle off. This occurs intermittently a
... nd unpredictably, often (but not always) worsening in hot weather or after the car sits in the sun. It is loud enough to cause ear pain, startle the driver, and drown out external sounds such as emergency sirens or horns, creating a significant distraction and safety hazard while driving. This is a well-documented issue on 2016–2017 Honda Accords (including Hybrids), widely reported on owner forums, YouTube, and other complaint sites. The failure is internal to the head unit (likely the amplifier circuitry or cooling fan). Honda dealers acknowledge it as a known problem but there is no recall. Temporary workaround is disabling the audio fuse, but this is not a safe long-term solution. I request that NHTSA investigate this as a potential safety defect due to the distraction and loss of audio control.Read more

I was notified of a recall on my from vehicle by Hyundai via email some time in March of 2026. The message said mentioned my vehicle may have a defective/worn purge control check valve that would cause the fuel tank to deform. I assumed it would be a simple swap of the fuel tank and everything would be fine. I was offered a buyback from my Hyundai claims manager over the situation, but I figured since its a simple fix, I'd keep my car and have it fixed. Two weeks went by with no update from the dealership, so I walked in for answers. Turns out, the subframe of my car has been damaged by the expanding fuel tank and they are still waiting for updates from Hyundai on how to proceed as they themselves don't know how to fix my vehicle. My Hyundai claims manager did not mention this when we talked over the phone and he offered a buyback. None of their recall notices mention structural damage anywhere or even as a possibility. The issue appears to involve the vehicle’s electrical control system responsible for power management and system activation while the vehicle is off, possibly including the Smart Power Control Module and related components. It may be arranged for the vehicle to be inspected. The vehicle behaved as though it had been remote started, initiating engine cranking without any user input while turned
... off. If this occurs while parked in a garage or enclosed space, it could lead to dangerous carbon monoxide buildup and pose a serious risk to occupants. Unintended engine activation also presents a risk to nearby people or property. The unintended engine cranking event occurred once and was directly witnessed while I was seated inside the vehicle observing other electrical oddities. The issue has not been reproducible on demand. However, related symptoms including the alarm activating on its own and unexplained electrical activity while the vehicle is off have occurred multiple times. The vehicle has not yet been inspected for this issue by a dealership, manufacturer, or third party. No police or insurance reports have been filed. I plan to take it to the dealership tomorrow (4/4/2026). There were no warning lights or messages before or during the event. Prior symptoms include intermittent alarm activation, display flashing, clicking fuses, and electrical activity while off, beginning before the engine cranking incident.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The specific component has not yet been identified. The issue involves unexplained water intrusion into the vehicle’s floor pan under normal operating conditions. The vehicle is available for inspection upon request. How was your safety or the safety of others put at risk? Water intrusion into the
... cabin creates potential safety risks, including exposure of electrical systems to moisture, possible corrosion of critical components, and degradation of interior air quality (e.g., mold). In September 2025, an authorized Kia dealership performed recall SC284XM (HECU electrical short fire risk) on this 2012 Kia Sorento. During the repair, the dealership disconnected the battery, resetting the PCM/ECU and clearing all OBD-II readiness monitors including the oxygen sensor monitor. Despite driving hundreds of miles afterward, the O2 sensor monitor has not reached a Ready state. A malfu
... nctioning or unmonitored oxygen sensor creates a direct safety risk — the PCM cannot properly regulate the fuel-air mixture, increasing the risk of catalytic converter overheating, engine damage, and potential fire. The vehicle cannot pass California's mandatory smog inspection and the registration has lapsed, forcing the owner to operate an unregistered vehicle or go without transportation entirely. The dealership has verbally confirmed this is a known model-year defect — the root cause is a software condition requiring a PCM reflash, not achievable through normal drive cycles. This is the second occurrence; the identical failure happened in 2022 after a prior battery disconnect on this vehicle. The vehicle is now held at the dealership 11+ days with no update, no loaner, and an ~$460 estimate for repairs caused entirely by Kia's own recall service. A vehicle actively subject to a fire-risk recall (SC284) was returned to an owner in a condition where the engine management system cannot be properly monitored — compounding the original safety risk rather than resolving it. All this an yet a no no-cost remedy has been issued. This vehicle has been subject to five NHTSA recalls: SC200 (engine fire), SC147 (engine bearing), SC284 (HECU fire), SC172 (fuel pipe fire), and brake-shift interlock defect. I am experiencing persistent and unresolved issues with the infotainment and display system in my Toyota Corolla, primarily involving Apple CarPlay overriding the driver display. Even when I manually disable the setting that allows CarPlay/navigation to take over the display behind the steering wheel, the system does not retain this setting. After turning the car off and restarting it, the navigat
... ion/map interface reappears and overrides the driver display again. This behavior is extremely distracting while driving and interferes with my ability to safely view normal vehicle information. It has created multiple situations where I felt at risk of a collision due to unexpected display changes and distraction. In addition, there are ongoing system malfunctions including: •Volume controls becoming unresponsive (the slider does not adjust sound), requiring the system to be restarted to function again. •Intermittent failure of the phone connection/charging port, where the device does not connect properly to the system. A prior software update temporarily addressed one issue, but the core problems persist. I have been attempting to resolve this through Ardmore Toyota dealership in PA for approximately one year without a meaningful fix or access to a qualified technician. This appears to be a recurring software or system defect that affects driver attention and vehicle operation. I am concerned this creates a significant safety risk for myself and others on the road. A suspension grinding noise may indicate issues affecting steering, suspension, or vehicle stability. I am concerned that the vehicle was released without inspection or documentation of a reported safety related condition.Read more

High voltage battery management system generated fault codes BMS_u008 and BMS_f071 during a Supercharger charging session, prohibiting drive mode. Vehicle had been driven normally to the Supercharger with no drive-prohibiting alerts during transit. After connecting to the Supercharger, fault codes manifested. Despite fault condition, vehicle continued charging to 93% state of charge before requiri
... ng towing. This is the second such event — original HV battery also generated BMS fault alerts prohibiting drive mode during a charging session in January 2024 and was replaced with a remanufactured assembly. Third-party firmware alert data shows recurring hardware charging fault alerts (CHG_f018, CHG_f076, customer message: Unable to charge) occurring in January and February 2026, weeks prior to the March 2026 failure. Alert DI_u014 also fired repeatedly on March 18, 2026 indicating insufficient 12V supply contributing to undriveable condition — a separate system from the HV battery. Tesla Service Center has not investigated whether Supercharger charging events contributed to either failure, has not produced Supercharger session logs, and has not attempted software-level fault resolution before recommending full hardware replacement. Tesla's own firmware documentation describes BMS_u008 as potentially self-clearing and BMS_f071 as having a documented clear condition. Remanufactured battery subsequently drained from 88% to 0% over 11 days while unplugged at service center — 6-9% per day versus normal 1-3% per day vampire drain, consistent with active BMS fault causing abnormal parasitic draw.Read more

The truck had been parked in our driveway for approximately 24 hrs when my wife went to the drivers side door to retrieve something that she had left in the center console. After she retrieved what she was looking for she pushed the door closed and the rear window spontaneously and completely shattered. There had been no prior damage whatsoever. The vehicle was towed to our Ford dealership on M
... arch 17th. The service manager at our dealership submitted our warranty claim to Ford and then informed us ~36 hrs later that Ford had not issued any TSBs or SSMs for the 2024 rear window or related parts / functions and therefore would decline to repair the rear window. We have asked them to resubmit the claim because we were not satisfied with Ford declining it. It is now April 2nd and we still have not heard back.Read more
