Coolant leaking into cylinders. Repair shops said it was a known Ford issue with the 1.5 ecoboost engine. Causes the coolant to leak out and the engine to overheat and the overflow tank to leak.
Incident date: Mar 22, 2026
The defect has left the driver stranded several times and required multiple tows due to the vehicle becoming unsafe and inoperable. On one occasion, the vehicle broke down after a 2.5-hour drive away from home and had to be towed because the overheating and coolant leak issue made it unsafe to continue driving. The vehicle is operated in a region where summer temperatures frequently exceed 115°
... F, which significantly increases the safety risk associated with repeated overheating and roadside disablement. The issue remains unresolved and continues to pose an ongoing safety risk due to sudden overheating and loss of reliable vehicle operation. I am reporting a failure in the integrity of Federal Safety Recall completion and documentation at Kia of Daphne. On 2/6/26, I was issued official paperwork stating a recall part was 'Special Ordered.' On 4/3, I attempted to call for FU in part progress. Informed “recall was done and replaced 2/6, and no part was ordered.” Advisor requested I speak to manager, I declined, request they speak t
... o advisor. No call back until my VM left 4/6 afternoon requesting FU call. On 4/6/26 late afternoon, unprofessional call from another advisor who stated “we don’t see part ordered,” Advisor manager intercepted call, extremely unprofessional, stated he reviewed 4/4 and found nothing to support my statement, yet no FU call was placed to I, nor was any contact ever attempted by Kia. The dealership management proceeded to claim the recall was 'Complete' despite no record of the part arriving or being installed, and other advise “misspoke”. When I presented in person and requested clarification on this discrepancy in documentation/information/last signed document by I based on statement of part is order and will call when arrived to schedule; a male staff member became physically aggressive and volatile. I am concerned that the dealership is inaccurately reporting recall completions to NHTSA/Kia America without actually performing the required repairs or using the specified parts.I am also concerned for the integrity and safe environment of this dealership for customers and staff. I cannot attach the 11 minute video of the encounter, but am happy to provide via email to those who inquire given discretion for other parties.Read more

Complaint Summary: I am filing a complaint regarding a potentially defective A/C compressor that is producing abnormal noise and may indicate premature failure. Description of Issue: My vehicle’s A/C compressor produces a loud, rhythmic “chugging” or knocking noise at idle when the A/C system is off. The noise is excessive and not consistent with normal operation. When the A/C is turned on,
... the noise changes but remains abnormally loud. Based on my experience and comparison to other similar vehicles, this is not normal behavior. A comparable 2024 Subaru WRX does not exhibit this issue, and many other 2022 WRX owners have reported similar symptoms. Safety Concern: This is not just a noise concern—it may indicate internal compressor failure or premature wear. The A/C compressor in this vehicle is part of a continuously driven system (no traditional clutch), meaning it is always engaged via the serpentine belt. If the compressor were to fail internally, it could: •Seize or bind, potentially damaging or breaking the serpentine belt •Lead to sudden loss of power steering assist (if belt-driven) •Cause loss of alternator function (battery charging failure) •Result in engine overheating if the water pump is belt-driven •Leave the vehicle stranded unexpectedly This presents a potential safety risk, especially if failure occurs while driving. Additional Context: Subaru has issued a Technical Service Bulletin (TSB) for a similar A/C compressor issue in another model using a comparable design, which includes replacement procedures for certain serial number ranges. Despite this, Subaru has not acknowledged the issue for the WRX, even though there are numerous owner reports of the same symptoms. Conclusion: The abnormal noise suggests a potential defect that could lead to premature failure and safety-related consequences. I am requesting that this issue be investigated for possible defects in design or manufacturing affecting the A/C compressor in the 2022 SubarRead more

Four separate incidents but related incidents. Primary concern: vehicle turns off and puts itself in park without notice and cannot be restarted. Two incidents were in traffic at a stop light, and the vehicle could not be started for 12-15 minutes. Two times it occurred while the vehicle had been parked for a short period of time but could not be restarted. March 4, 2026 driver was stopped at a
... light, after about 20 seconds, driver received a dash message “Press brake and push button to start” indicating the vehicle was off, and the vehicle was in park. Driver depressed the brake and the vehicle started. March 7, 2026 driver pulled into a store and when they excited the vehicle, they notice their carplay was still attached to their phone/music. Driver disconnected their phone but notice a dash message “auxiliary connected.” Driver attempted to start the vehicle, but it would not start, however all of the lights, carplay, messages were cycling as if the vehicle was on. When driver attempted to start the vehicle there was no give to the brake and the vehicle would not start. Driver was able to start the vehicle 15 minutes later. March 27, 2026: Driver was stopped at a light and the vehicle turned off and put itself in park. Driver had to contact police t0 block traffic to prevent vehicle from being hit. Vehicle started after 15 minutes. Driver immediately took the vehicle to the dealership. April 2, 2026: Dealership service technician called and said they could not recreate the problem, that all codes were checking out, however they did see a intermittent short in the crank sensor and it was replaced. April 3, Owner pick up the vehicle at 5:30, drove five miles away. Put the car in park to get fuel and the vehicle would not start. My 2024 Subaru Outback has twice become inoperable while stopped at a traffic signal. In both incidents, the engine was off while stopped, but the vehicle remained powered on and would not restart. The vehicle also would not properly power down. The car could not be safely driven or moved and required towing both times. This created a serious safety hazard because the vehicle became disabled in
... active traffic and could not be restarted or properly shut down. After the first incident (on [XXX]), the Subaru dealership replaced the battery. Approximately 4 weeks ([XXX]) later, the exact same failure occurred again. The dealership has now had the vehicle for approximately three weeks and states they cannot replicate the issue. No cause has been identified and no repair has been made. During one of these incidents, my [XXX] child was in the vehicle while we were stranded in traffic. This appears to be a repeat electrical / ignition / operability failure affecting the vehicle’s ability to restart or properly shut down after stopping in traffic. At no point did the door stop or auto-reverse, and there was no noticeable slowing, hesitation, or warning indicating obstruction detection. The door operation sounded normal during closing. The child’s finger remained trapped until an adult manually opened the sliding door to release it. The injury involved a finger amputation involving the distal portion of the finger with an open fracture and nail involvement, requiring emergency room treatment. Emergency department records document a diagnosis of fingertip amputation, with radiology findings noting a soft tissue amputation defect and concern for distal phalanx (tuft) fracture of the affected finger. The air conditioning system on my 2017 Honda Accord Hybrid has failed due to a refrigerant leak. The symptoms began with a complete loss of cooling on the driver’s side while the passenger side remained cool. Eventually, the system failed to provide cool air to the entire cabin. This is a significant safety concern as the failure of the A/C system prevents the climate control from effectively de
... humidifying the cabin air. During humid or rainy conditions, this leads to rapid windshield fogging that cannot be cleared by the defroster, severely impairing driver visibility. The vehicle experienced a complete loss of communication with the ABS control module, resulting in multiple warning lights and loss of ABS and related safety systems. The failure is consistent with Volkswagen Recall 45F2, which describes internal ground and solder failures in the ABS module. Prior to total failure, the vehicle exhibited intermittent warning lights and an ABS fault code (01130 –
... Implausible Signal) in 2021 but was told the car was not under recall by dealer. After the condition progressed, the module became non-communicative and no longer generated fault codes. This is consistent with internal failure preventing communication. The failure affects braking-related safety systems, including ABS and stability control, and creates a potential safety risk, particularly during emergency braking or low-traction conditions. The condition has been confirmed by both a Volkswagen dealership and an independent Volkswagen/Audi specialist. A dealership diagnosis in 2025 once again confirmed that the ABS pump and module had no communication and required full replacement. Independent electrical testing verified that all external wiring, power, and ground inputs were functioning properly, indicating the failure is internal to the ABS module. Despite this, the manufacturer denied recall repair based on the absence of diagnostic fault codes. However, the lack of communication prevents the module from generating fault codes, creating a situation where complete failure does not qualify for recall repair. The failed ABS module is still installed in the vehicle and is available for inspection upon request.Read more

Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? GMC Dealership. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No prior warning. Issue began Jan 31, 2025 (66 days after purchasing the brand new vehicle). Timeline of events: Jan 31, 2026: Horn began sounding unexpectedly and intermittently while driving and parked. Per dealership advice, the horn fuse was removed to allow for safe travel. Feb 5, 2026: First service visit. Service advisor reported a "kinked wire" was found and repaired. Feb 15, 2026: Issue recurred; horn sounded continuously while driving. Fuse was removed again to disable the horn; it has remained inoperable since. Feb 26, 2026: Second service visit. Component: VISIBILITY / SUN VISOR Vehicle: 2023 Toyota Corolla Both the driver-side and passenger-side sun visors in my 2023 Toyota Corolla have failed within approximately two years of normal use. The visors no longer remain in their intended position and instead fall downward unexpectedly. When this occurs, the visor drops directly into the driver’s field of vision, creating a significant safe
... ty hazard by obstructing forward visibility while driving. This condition can happen without warning and requires the driver to physically move the visor out of the way, causing distraction and increasing the risk of an accident. The vehicle has not been modified, damaged, or subjected to abnormal conditions. The visors were used only as intended. Sun exposure is a normal and expected condition for this component, and failure under these circumstances indicates a potential defect in materials or workmanship. Both visors failing in the same manner suggests a systemic issue rather than isolated wear and tear. The vehicle was taken to a Toyota dealership, where I was initially told the repair would be covered under the manufacturer’s warranty. However, the claim was later denied and classified as “wear and tear” due to sun exposure. Given the function of sun visors, this explanation is not reasonable. This issue presents an ongoing safety concern due to impaired driver visibility. I am submitting this complaint so that this defect can be reviewed and tracked for potential broader impact.Read more

I am submitting a formal safety complaint regarding a defect in my 2020 BMW X3 that has resulted in the failure of a critical emergency safety system. The vehicle displayed an error message indicating an “Emergency Call System Malfunction.” I brought the vehicle to BMW of Murrieta on March 30, 2026, for diagnosis and repair. Upon inspection, the service center determined that the Telematic C
... ontrol Unit (TCU) had sustained water damage. This damage was attributed to water intrusion originating from the vehicle’s shark fin antenna. This is a known issue documented by BMW in internal Service Information Bulletin SIB 65 12 22 (dated October 5, 2022). Despite acknowledgment of this known defect, BMW declined to cover the repair costs for both the antenna-related water intrusion and the resulting damage to the TCU. This failure presents a significant safety hazard. The emergency call system is designed to automatically notify emergency services in the event of a collision. With this system inoperable, occupants may be unable to receive timely emergency assistance, increasing the risk of serious injury or death. Given that BMW is aware of this defect and its potential to disable a critical safety feature, I believe this issue warrants further investigation and should be considered for a mandatory safety recall.Read more

I am submitting this complaint as a follow-up to three prior NHTSA complaints regarding this same vehicle: Complaint #11714848 (submitted 2/1/2026) Complaint #11714852 (submitted 2/1/2026) Complaint #11716040 (submitted 2/5/2026) These prior complaints document a progression of serious electrical, charging, and vehicle control issues that have worsened over time. Initially, the vehicle experien
... ced widespread software and electrical failures affecting core driving and safety systems, including loss of power while driving, unintended system activation, blank instrument displays, and failure of safety-related features such as the SOS emergency system. These issues escalated to include severe charging system failures, inability to reliably charge the vehicle, dramatically reduced range, and intermittent propulsion loss. On February 4, 2026, the vehicle became completely inoperable, displaying multiple critical warnings including “Drive system error,” “Electrical system not working correctly,” and inability to start or move the vehicle. The vehicle was towed to the dealership on February 5, 2026 for repair. I have now been informed by both Volkswagen and the dealership that they have been unable to identify or implement a repair for these issues. Despite this, they plan to return the vehicle to me without performing any substantive repairs. I have been told that diagnostic fault codes have been cleared and that the vehicle is now considered “safe” to drive. However, no root cause has been identified, and no repairs have been made to correct the underlying defects that led to loss of power, system failures, and complete inoperability. The issues previously reported remain unresolved and include: Loss of power while driving Dashboard and infotainment displays going blank while driving Vehicle shifting into park unexpectedly Cruise control activating without driver input Charging system failures preventing reliable charging Repeated warnings indicating unsafe operRead more

In the 2026 Yukons, almost all physical buttons have been replaced by the giant center touchscreen. When starting my vehicle, about a third of the time the screen will not come on, and can’t be turned back on by any means. It will randomly come back on after a few more trips/restarts. This renders several critical systems inoperable. All HVAC does not work, defrosters aren’t available, back
... up cameras and parking assist is not available, and control of any navigation functions or options (ie heated/cooled seats) are not possible. There are no physical buttons for these items, so there are no other options. Voice commands do not function properly when the screen is black, so no option there. After my third visit to the dealership and an open case with GM, they are finally authorizing a screen replacement. We will see if this fixes it. However, my previous explanations from GM pointed to a nebulous future software release, with no date or detail.Read more

While driving in slow, heavy highway traffic, the vehicle suffered a structural failure of the rear crossmember / rear subframe due to severe rust. This is a safety-critical structural component and part of the rear suspension mounting system. When braking, the vehicle swerved hard to the right and crossed into the shoulder. Fortunately, the vehicle was already in the right lane, which likely prev
... ented a more serious incident. The failure created a serious risk of loss of control and crash. After the incident, I took it straight to a mechanic on the way and the condition was identified as a broken rear crossmember caused by advanced corrosion. The failed component is available for inspection.Read more

In cold weather the doors will become unlatched and open. This has happened to all 4 doors, driver side, passenger side, rear driver and rear passenger. We will be driving and the doors will randomly become unlatched and fly open. We have small children and they could have been killed due to this incident. It is extremely unsafe and problematic. We are unbelievably fortunate that our children ha
... ve not been killed due to these incidents. It can happen at any moment and the doors will not close for several hours or days once they become unlatched. Typically when the weather warms up they will close again, which can take days. We have had friends/family allow us to use their garage to park the car so the car will warm up and we can re-close the doors, only to have them fly open again at some random time. This is a very dangerous defect that poses a significant health and safety, especially to children and babies. Additionally it is a liability as it can take several hours/days before the latches will close again. This requires the use of ratchet straps to hold the doors closed as the latches will not close (even if you attempt to reset the latches manually). However, having ratchet straps running from door handle to door handle to keep the doors closed poses other safety concerns. When I reached out to the ford mechanic I was told to replace the door latches as they may be defective. I had the driver side door latch replaced, but unfortunately the problem persists (about twice in the two months since it was replaced).Read more
