On Feb 25, 2026 My 2022 Tesla model Y with only 25k miles on it suddenly stopped knowing where it was and almost drove me into a ditch. All cameras stopped working and the gps was stuck in the same wrong location. (Service Mode showed Primary DAS ECU shows "AP IN COREBOOT" and "Status: INIT." Secondary B shows nominal. Features unavailable. GNSS shows 0 satellites in open sky.) Tesla service said
... I was outside of warranty and “There is an internal processor issue with your autopilot system that will prevent updates from installing. The only solution is replacing the computer on April 9, 2026, 12:22,” I had had other computer related issues I brought up during regular service visits but weren’t recorded and I was told software updates would fix such as: delayed rearview camera upon startup, navigation volume resetting, headlights staying on for extended periods after exit, and lockout issues. I asked Tesla to document the specific failure codes which they never sent. They said “Tesla diagnostics are showing the primary Autopilot computer is not booting properly, which is preventing cameras, software updates, and related systems from functioning. Because the computer is not initializing, a firmware reinstall will not complete successfully. At this point, replacement of the computer is the required repair to restore normal functionality.” My car had a safety recall during its service appointment that they could not install software for unless I paid $3200 to replace computer. I had total loss of rearview camera (FMVSS 111 violation) and loss of Active Safety Systems (AEB/FCW), but Tesla refused to repair a federally mandated safety defect if I didn’t pay the full repair cost.Read more

I am filing a safety complaint regarding my Nissan Murano due to a recurring water leak caused by clogged sunroof/moonroof drain tubes. This problem has now happened for the third time, even after prior repairs by an authorized Nissan dealership. Water repeatedly enters the interior of the vehicle, soaking the carpets and interior areas. I am very concerned about mold and mildew developing undern
... eath the carpeting and inside the vehicle because of the trapped moisture. This creates potential health concerns for passengers and affects the safety and air quality inside the car. I am also concerned about possible electrical damage from repeated water intrusion near wiring and electronic components. The ongoing leaks have caused major inconvenience, repair expenses, and concern about the long-term reliability of the vehicle. The first repair was completed by a Nissan dealership at my own expense, yet the same issue returned multiple times afterward. After researching this problem, I found that many other Nissan Murano owners have reported similar recurring drain clogging and water intrusion issues. I believe this may indicate a design defect or inadequate drainage system that should be investigated further. I respectfully request that NHTSA review this issue for potential safety risks related to mold exposure, electrical failures, and recurring water intrusion affecting Nissan Murano vehicles.Read more

I am reporting a serious ongoing issue with my 2019 Honda Civic Sport involving the trunk repeatedly opening on its own both while driving and while parked. The issue is also causing the vehicle alarm to activate unexpectedly. The trunk has opened while the vehicle was in motion, creating a major safety concern. It also randomly opens while parked, sometimes triggering the alarm system for no app
... arent reason. The problem appears to happen intermittently but has become frequent enough that I no longer feel safe leaving the vehicle unattended or driving it normally. I attempted to clean the trunk latch area, but the issue continues. I believe there may be a malfunction involving the trunk latch, actuator, key fob system, electrical wiring, sensor, or alarm system. This situation is dangerous because the trunk can unexpectedly unlatch while driving, potentially blocking visibility or causing damage. The repeated alarm activations and trunk openings while parked are also causing concern about battery drain, security, and reliability. Vehicle Information: 2019 Honda Civic Sport Symptoms: • Trunk opens while driving • Trunk opens while parked • Alarm randomly activates • Trunk may not remain securely latched • Ongoing intermittent electrical or latch-related issue I am requesting that this issue be documented and inspected as soon as possible due to the safety risk involved.Read more

My engine started running rough and, as a result of a repeating PO301 code, the car was towed to Lester Glenn of Toms River. Their diagnostic scan report (codes DTC PO301 & DTC PO303) found the engine assembly has scored cylinder walls due to failed piston rings. The entire engine had to be replaced. The odometer reading was 19,000 miles. Piston rings are designed to last 150,000 plus miles. For t

COMPLAINT: Authorized dealer non-compliance with NHTSA Compliance Recall 25V900 (Rearview Camera Software, FMVSS No. 111). Vehicle is a 2019 Audi A8 subject to Recall 25V900, issued by the manufacturer in January 2026. The recall work was scheduled with an authorized Audi dealer in Southern California by telephone. The dealership representative confirmed an appointment for April 17, 2026 at 2:30
... PM and represented that the service would take approximately two hours. The appointment was then confirmed in writing three separate times by SMS from the dealership: twice from its scheduling line on April 15 and April 16, 2026, and once on April 16 at 12:38 PM by a Business Development Manager from the dealership main line, who expressly acknowledged the scheduled service and used it to solicit a vehicle appraisal. On April 17, 2026 the vehicle was presented at the dealership at the scheduled time. The service advisor on duty stated that the vehicle would have to be left until the following Monday and could not be serviced that day. He refused to provide any reason for the change. He refused to identify who had scheduled the appointment. He refused to contact the service director on the owner's behalf. A direct call to the service director at the number on his business card was not answered. The vehicle was returned without service. The recall remains uncompleted. The dealer's conduct effectively imposes an unreasonable burden contrary to the obligations of 49 U.S.C. § 30120 and 49 CFR Part 577. GENERAL GUIDELINES: - Component: rearview camera software (subject of Recall 25V900). - Safety risk: per the recall notice, the software issue may prevent the rearview camera image from displaying, decreasing visibility when reversing. - Confirmation: the recall was identified by the manufacturer; the dealer accepted and confirmed the appointment four times (1 verbal, 3 in writing) before refusing. - No physical inspection has occurred; the recall has not been performed. -Read more

The only way to stop it was to t
... urn off the engine, then turn it back on. The car then began driving normally. I have also had an issue with hesitation felt while accelerating 0-40 mph with study throttle. Upon researching both issues, they should be resolved with TSB calibrations to my car of T-SB-0028-25 and T-SB-0017-24. This is not an error that would come up when doing a diagnostic check on my car's computer per research I have done. I took my car to get the updates at Toyota of Redlands, CA, and they refused to do them unless they could recreate them themselves. I called Toyota to report this, and they said that each Toyota dealership/service is privately owned at their own discretion. I was basically told to return if I experience it again. I do not want to experience this again and they wouldn't take my word on it anyways. I feel like next time I could injure myself or someone else if this glitch happened again. I therefore would like to implore to make this a mandatory preventative TSB calibration for cars that have had this reported/models/year which mine has. This goes for any TSB calibrations should be done preventatively, but especially ones that can cause a potential accident like T-SB-0028-25 and T-SB-0017-24. Here is information on why this would not show up on a diagnostic test: Your Toyota didn't generate an error code because the shudder is a software calibration issue rather than a component failure. The onboard computer (ECU) only triggers a check engine light or throws a Diagnostic Trouble Code (DTC) when sensors detect a hard mechanical or electrical failure.Read more

The vehicle experienced failure of the HVAC heater flap/blend door mechanism on the driver’s side. Specifically, the plastic gear assembly broke at its mounting point, causing the gear to spin freely and preventing the airflow door from opening and functioning properly. As a result, the driver’s side heating and airflow system no longer operated correctly. The failed component is available fo
... r inspection upon request. Because the manufacturer does not sell the small gear component separately, I was forced to obtain a custom 3D-printed replacement part in order to restore operation. This defect created a safety concern because the inability to properly control heat and airflow can affect windshield defrosting and cabin visibility during cold or wet weather conditions. Reduced defrost performance can impair driver visibility and increase the risk of an accident. The problem was confirmed during inspection and repair. Accessing the failed gear required extensive disassembly of the vehicle’s front dashboard area and HVAC system components. The repair process was extremely labor-intensive for what is essentially a small plastic gear failure. Prior to complete failure, the HVAC system began making abnormal clicking/grinding noises and airflow operation became inconsistent. Eventually, the gear mounting point completely failed, causing the actuator gear to spin freely without moving the heater flap. This appears to be a common problem affecting Nissan Rogue vehicles, particularly involving the weak plastic HVAC gear and mounting design. The manufacturer does not offer the individual failed gear as a replacement part, which unnecessarily increases repair complexity and cost.Read more

Around 100,000 miles, my vehicle began experiencing intermittent hesitation, sudden loss of power, and frequent stalling. Prior to the complete failure, the vehicle would occasionally hesitate during acceleration and feel like it was losing engine power while driving. Over time, the problem became more frequent. One day, while driving approximately 50 mph on a side road, the vehicle suddenly stal
... led without warning and completely lost power. I was luckily able to avoid an accident, and no one was injured, but the sudden shutdown created a serious safety hazard and significantly increased the risk of being rear-ended by other vehicles. The vehicle continues to stall frequently and hesitate during operation, making it unsafe to drive. The failed component was confirmed by Nissan. The dealership diagnosed the issue and recommended replacement of the defective part for over $900. However, a Nissan mechanic explained that the repair only involved removing and replacing the common problem, which is the Mass air flow sensor, secured by two screws. I replaced the component myself for approximately $90, and the vehicle temporarily improved. Unfortunately, the stalling, hesitation, and loss of power have returned and now occur constantly. The failed component should still be available for inspection upon request. The issue was confirmed by Nissan personnel, although I completed the repair independently rather than paying the dealership repair cost. Prior to the failure, the vehicle showed symptoms including hesitation, intermittent power loss, and stalling beginning around 100,000 miles. These symptoms worsened over time until the vehicle completely shut down while driving. This appears to be a common issue affecting Nissan vehicles and creates a serious safety concern due to the risk of sudden engine shutdown while driving.Read more

All of a sudden before I even hit the end of the driveway while I had my fo

A lot of black smoke came up out of the steering wheel area where it attached to the dashboard & all of the air bags around the car deployed. Then my car completely turned itself around and the front of my car was now facing the driveway that I had just come out of. This torque of acceleration and braking is why my car spun around 180 degrees. The vehicle had sustained damage to the front end, the hood, front bumper cover, right front head lamp was cracked, and the Toyota emblem was missing. (See attached photos taken at the time of the accident) Since all of the air bags in the whole car had completely deployed, I found that the only way I could get out of the car was to slide myself under the air bag that surrounded the driver side door. The Auto Idle Stop (AIS) system malfunctions during normal driving. When coming to a stop, the AIS shuts off the engine as designed, but the vehicle then fails to restart when the brake is released. The vehicle shifts itself into Park automatically without driver input, either before or at the same time as a dashboard message appears instructing the driver to depress the brake pedal and press the
... start button. When the start button is pressed, the dash resets but the engine does not start. The only way to recover is to fully power the vehicle off and restart it, which is difficult and unsafe at a busy intersection or red light. This puts the driver and others at serious risk. The vehicle becomes immobilized in traffic with no ability to move it out of the path of oncoming or following vehicles, creating a risk of being rear-ended. The unexpected automatic shift into Park is also a vehicle control concern, as the driver does not initiate it. The problem is intermittent. The vehicle has been taken to an authorized dealer twice. On the first visit the dealer kept the vehicle several days with a loaner and attempted to replicate the issue but could not. On the second visit the dealer kept the vehicle one day and again could not reproduce it. A third diagnostic visit is scheduled, during which the service manager intends to drive the vehicle to attempt replication. As of now the failure has not been confirmed or reproduced by the dealer. The vehicle is available for inspection upon request. No warning lamps appear prior to the failure.
