Incident Overview: A Continental Control Contact Sport SRS+ tire (DOT 5323) experienced a catastrophic internal structural failure during normal driving on a wet roadway. The tire had approximately 50% tread remaining and was 1–1.5 years old. Failure Description: Inspection indicates internal belt/carcass separation originating within the tire structure, leading to rapid pressure loss and ruptur
... e. No road hazard or external impact was present. Crash Sequence: The failure caused immediate loss of control, vehicle yaw, roadway departure, and multiple impacts (frontal and lateral). The driver airbag deployed, confirming measurable crash force. Injuries: - Aggravation of pre-existing back condition (surgery required) - Hip labral tear and bone spur (surgery required) - Neck and shoulder pain Conclusion: The initiating event was a premature internal structural tire failure, not driver negligence or external hazard.Read more

while operating my 2026 Honda Civic, I experienced a sudden and catastrophic tire blowout with absolutely no prior warning. At no point before or during the incident did my Tire Pressure Monitoring System (TPMS) alert me via the dashboard that my tire pressure was low or compromised in any way. Three critical failures occurred in this incident: 1. TPMS System Failure — My federally mandated TPMS
... system, required under FMVSS 138, failed to detect and alert me of dangerously low tire pressure prior to the blowout. I had zero dashboard warning at any point. 2. Tire Integrity Failure — The tire experienced a catastrophic sidewall blowout on a brand new vehicle. No road hazard, debris, or obstacle was struck prior to the failure. The structural integrity of the tire failed without cause. 3. Vehicle Safety System Communication Failure — The vehicle’s onboard diagnostics failed to detect and communicate a critical safety condition to me as the driver. On a 2026 model year vehicle, multiple redundant systems should have caught and communicated this danger, giving me the opportunity to respond and prevent the incident. These three failures created a dangerous chain of events that left me with no ability to take preventive action. The incident caused significant emotional distress. I have retained the damaged tire as physical evidence and have documented the incident with photographs. I am requesting a full investigation into whether this represents a pattern of TPMS and tire failures in the 2026 Honda Civic, and whether a safety recall is warranted. Evidence retained: Damaged tire, photographs of damage, vehicle in current condition for TPMS inspection.Read more

This is about my tires, follow-up from my previous report ID 11726883 regarding inadequate original tires. Since 1. Tesla was unresponsive to my defective tire issue, 2. Discount Tire insisted on my continuing to drive until all 4 tires reached 4/32" tread depth, and 3. My local tire store, where my Tesla would get towed in the event of a flat while I'm near my home town (since the nearest Tesla
... dealer is >50 miles away), is unable to access the warranty on the original Continental Contact RX tires. He was originally told that they had to be worn to 2/32", I was advised to call Continental Tires Customer Service and chose the option of Warranty claims. I was immediately advised that Continental does not warranty original equipment tires. I was therefore forced to spend significant extra money at my local tire store, which does not get the volume discounts that Discount Tire gets. It is no longer practical for me to use Discount Tire, an hour's drive down the interstate, for my tire needs. It is further impractical for me to drive on tires that do not provide the safety that I need, despite the fact that Discount Tire would give a credit of approximately $50/tire once all 4 reached 4/32". I also now realize that Discount Tire is providing that credit at their own cost, despite the fact that they told me that the Continental with the TE on it (mine has a T2) is eligible for warranty rebate. They scanned my tires with the hand-held scanner and their computer indicated it was rebate-eligible, though the Warranty was for 40,000 miles instead of the 55,000 that the first Discount Tire person told me on the phone. I bought new tires yesterday at 33,002 miles. Bottom line is that both Tesla and Discount Tire are protecting Continental Tires from the unsafe original tires for (at least) the Tesla 2023 Long-range Y. None of the 3 are responsible to the at-risk public who is paying for their profits.Read more

The original OEM tires on a 2025 Volvo XC90 (purchased new in March 2025) failed prematurely due to what appears to be a manufacturing defect. Tire size: [e.g. 275/45R20]. Brand: [e.g. Pirelli Scorpion]. DOT: 193 7B594P 1924 (manufactured week 19 of 2024). The vehicle's original tires were removed in November 2025 for winter tire installation, at which point the odometer showed 6,808 miles. Th
... e tires were stored indoors in bags and never used again until April 2026, when a professional tire shop (Discount Tire) refused to remount them on the vehicle citing unsafe condition. The tires exhibit severe sidewall bulging, starburst cracking indicating internal structural failure, and at least one tire shows a deep gash with internal steel cords exposed. Three of the four original tires are affected. The fourth original tire was replaced in June 2025 after it failed during normal highway driving - the cause was attributed to a road hazard at the time but is now believed to be the same underlying defect. The replacement tire (a different unit purchased separately) shows no defects whatsoever. The safety risk is significant: three tires with structural sidewall failure were on an active vehicle for the first 6,808 miles of its life, including at highway speeds. One tire experienced an in-service failure. The remaining three deteriorated to the point a professional installer deemed them unsafe to mount. The failed tires are available for inspection. The problem has been confirmed by an independent tire service center (Discount Tire) who refused to install due to safety concerns. The manufacturer has not yet inspected the tires. There were no warning lamps or prior symptoms before the June 2025 in-service failure, and no visible signs of the sidewall damage were observed until the tires were removed from storage in April 2026.Read more

The suspected components involved include the Pirelli tires, as well as the Electronic Stability Program (ESP), traction control system, and 4MATIC all-wheel drive system, which did not appear to effectively intervene. The vehicle spun and was involved in a crash, placing myself, my wife, and my child—who has Autism—at significant risk, along with surrounding motorists traveling at interstate speeds. Despite visible tread remaining, the tires did not perform as expected. Based on warranty expectations of approximately 50,000 miles, the apparent performance failure at only 15,257 miles raises serious safety concerns. The vehicle is currently at a collision repair facility and is available for inspection. The issue has not been reproduced, and a manufacturer inspection is pending. The vehicle has been inspected by law enforcement and insurance representatives. There were no warning lights or alerts related to traction, stability, or tire performance prior to the incident. As a result, the vehicle was rendered undrivable, required towing, and has been under repair for over a month, causing significant inconvenience and financial impact. I am requesting a formal investigation and review for appropriate resolution.Read more

I have replaced several sets of tires with this Tesla Model X 2019. After years of dealing with a front end alignment problem, I finally had the repair done with some assistance from Tesla. When I purchased my vehicle under warranty, I reported problems with steering and pulling to one side, but they repeatedly claimed they couldn't replicate. This was before they allowed customers to drive wit
... h the repaid team. I mention the front-end problems as that has coupled with the rear tires. Inner wear is a consistent problem. I went to Costco today to have them look at tires I bought in November of 2024 and they said the tire was balding and unrepairable. In fact the tires were showing wear on both sides and needed to be replaced. A full set costs $1400.00+ and this is the second time I'm having to replace tires for wearing out quickly. There's a fix online from a company "N2itive". On their website they state, "The N2ITIVE SX-2 adjustable rear camber arms are designed to address the common inner tire wear issues that plague the Tesla Model S and X. These arms are available in the Red and Natural Anodized finishes with a 3-Year Warranty or the Black Anodized finish with Lifetime Warranty and work best when used in conjunction with our TSX-2 Toe Arms." External companies are standing up lines of business based on Tesla's failures. This is a problem and someone is going to get hurt, or worse.Read more

On 03/29/2026 at approximately 11:00 AM, I was driving my 2024 Polestar 2 southbound on [XXX] near [XXX] in the center lane at approximately 80 mph in heavy traffic. Without any prior warning — no vibration, no TPMS alert, no unusual noise, no warning lamps or messages of any kind — the left rear tire suffered a catastrophic tread/belt separation. The tire is a Continental original equipmen

The tire was factory-installed original equipment and had never been removed or remounted. The entire center tread band delaminated from the tire carcass in a single explosive event. I heard a loud bang and immediately lost control of the vehicle. The rear end spun out and the vehicle crossed two lanes of heavy high-speed highway traffic before coming to rest on the shoulder. CHP responded to the scene and filed a report. I was not struck by any other vehicle. No injuries. No vehicle body damage. However the failure created an extreme risk of death or serious injury to myself and to other motorists in adjacent lanes at highway speed. The failed tire is preserved at the authorized Polestar dealership and is available for inspection upon request. Photographic documentation exists showing textbook belt edge separation: the shoulder tread remains attached on both sides while the entire center section has cleanly separated from the carcass, exposing the inner liner and ContiSilent polyurethane foam layer. The rim is undamaged. The left front tire is from the same production run [XXX] , manufactured one week earlier at the same Hannover plant), making it a sister tire that may carry the same manufacturing defect. These tires are not currently included in the existing Continental recalls, but they are showing the same types of failures described in NHTSA recall 21T‑001 and related Continental bulletins. Specifically, my tires developed cracking/localized tread wear/vibration and v
... isible separation in the tread/sidewall area. This matches the recall descriptions of carcass breaks and belt‑edge separations that can cause sudden air loss or partial/full tread and belt loss, leading to loss of vehicle control and increased risk of a crash. I believe these tires were manufactured with a similar defect and should be investigated and included in the recall.Read more

While traveling at approximately 55 mph under normal road conditions, the rear tire experienced a sudden and catastrophic failure. I felt the tire fail first, followed by an immediate loss of stability. The tire split down the center of the tread without warning. There were no prior symptoms, warning signs, or handling issues before the failure. During the loss of control, I then felt the rear of
... the motorcycle drop further. After the crash, a rear suspension linkage component was found to be cracked at the mounting point. It is UNKNOWN whether the linkage failure occurred before or as a result of the crash sequence, but it was not felt prior to the tire failure. The tire, 180/55ZR17 73W, had been installed in August and had less than 1,500 miles of use. Tire pressure was set to manufacturer-recommended levels prior to operation. The failed tire is available for inspection, and the damaged rear linkage component remains on the motorcycle. The failure created an immediate safety hazard, resulting in a crash and injuries to a passenger. The motorcycle sustained severe damage and is considered a total loss. No prior warnings, messages, or indicators were present before the failure.Read more

The front Continental PremiumContact 6 tires, size 275/35ZR22, DOT [XXX] , developed abnormal and premature tread/shoulder failure. The condition included splitting, tearing, separation along the shoulder area, and exposed internal cords. I first raised concerns about the tire condition at about 20,000 miles. An alignment was performed at 20,142 miles, but the condition later worsened. One of the
... front tires deteriorated while the vehicle was being driven, creating a serious safety risk. The condition was documented with photographs and dashcam evidence. The vehicle was not safely drivable on the affected tire. The full set was later replaced based on BMW’s recommendation, at a cost of $2,358.75. A warranty/defect claim was submitted through the BMW / Dealer Tire / Continental process, but I did not receive a clear manufacturer-level inspection explanation or meaningful response. Tire size was ST 205 / 75 R 15. This tire experienced a significant belt separation failure that I fortunately noticed before departing our campsite. While doing a walk around inspection before departure, I noticed a severe tread deformity in the front passenger side tire on our camper. The tires were cold and the ambient air temperature was in the low 60's. Before removing the tire and wheel fr
... om the trailer, I checked the tire pressure on all 4 tires, and all were within 2 psi of the recommended 65 psi cold pressure, including the failing tire. I removed the wheel and tire assembly and deflated it for safety, and then took it to a local tires shop to replace the tire. They diagnosed it as belt separation and further stated that it would have likely failed with a blowout within a few miles if I hadn't caught it. The tire had an estimated 15,000 miles on it at the time, but had been professionally repaired for a screw puncture approximately 7,000 miles earlier.Read more
